Abstract

The invention relates to a fixation device for joining bone pieces, which has a carrying body (10) with an internal side (11) and a reverse external side (12), and the carrying body (10) contains openings between the external side (11) and the internal side (12). The characteristic feature of the invention is that the fixation device (1) is formed by the unity of at least two carrying bodies (10, 20) of an average thickness (V) of a maximum of 2 mm, in the case of which, when the fixation device (1) is in use, the external side (12) of the first carrying body (10) and the internal side (21) of the neighbouring second carrying body (20) touch each other at least partly, and at least a part of the openings (13, 14) of the first carrying body (10) and the openings (23, 24) of the second carrying body (20) are situated above each other.

Description

Fixation device for joining bone pieces
The invention relates to a fixation device for joining bone pieces, which has a carrying body with an internal side and a reverse external side, and the carrying body contains openings between the external side and the internal side.
In surgical practice there are numerous widely used solutions for joining bone pieces broken in accidents. In the case of some of these solutions locking plates are used for joining broken bone pieces. Locking plates are rectangular plate-like shaped solids - mostly of a size suiting the size of the bone - with openings. The plates are attached to the broken bone pieces with the help of practically constructed threaded joining elements taken through the openings, in order to rejoin such bone pieces. Such locking plates are described for example in patent specifications no US 4.493.317, US 5.053.036 and US 5.151.103, from publication document no EP 3 18.762. and from publication document no WO 00/531 10.
On the one hand, a basic requirement in connection with the locking plates used is that they should be appropriately thick to provide adequate stability in the case of dynamic and static loads exerted on the bone. On the other hand they must also enable the person doing the operation to adjust the locking plate used to the shape of the bone, to its unique surface, so that the shape of the locking plate is as close to the shape of the bone pieces to be joined as possible, which helps desired union.
However, a significant disadvantage of the currently used locking plates is that they cannot fulfil both requirements at the same time. It is difficult to shape the plates providing appropriate stability, in many cases special tools are required for it, and even then the desired result cannot be reached".
A further disadvantage deriving from the above is that the operation lasts longer, which puts an unjustified extra burden both on the operating surgeon and on the patient, which, in a given case, may result in surgical mistakes or in the development infections or other complications. Our aim with the solution according to the invention was to overcome the deficiencies of the known locking plates and to create a fixation device, which enables simple shaping even by hand besides reaching adequate stability, and so the fixation device can be shaped quickly and safely into a shape, with which the optimal joining of the bones can be realised.
The construction of the fixation device according to the invention was based on the recognition that instead of using one single thick plate, the plate thickness needed for ensuring appropriate stability can also be reached by using several thinner plates placed on top of each other and connected to each other in a special way, and in this way, if the plate-like solids that are thinner and easier to shape than the generally used locking plates are formed other than the ordinary and connected to each other in a way other than the known methods, then during use they form one single co-acting unit, as a result of which a readily- formed fixation device with a great resultant stability, having the desired flexibility within certain limits can be attached to the bone pieces, and by this the task can be solved.
In accordance with the set aim the fixation device according to the invention for joining bone pieces, - which has a carrying body with an internal side and a reverse external side, and the carrying body contains openings between the external side and the internal side, - is constructed in such a way that the fixation device is formed by the unity of at least two carrying bodies of an average base plate of a maximum of 2 mm, in the case of which, when the fixation device is in use, the external side of the first carrying body and the internal side of the neighbouring second carrying body touch each other at least partly, and at least a part of the openings of the first carrying body and the openings of the second carrying body are situated above each other.
A further criterion of the fixation device according to the invention may be that on the interna) side of the carrying bodies there are first positioning pieces and/or on the external side of the carrying bodies there are second positioning pieces, and when the fixation device is in use, the neighbouring carrying bodies are fitted to each other with the help of the first positioning pieces and second positioning pieces co-acting with each other. From the aspect of the fixation device it may be favourable, if at least a part of the carrying bodies has a rectangular plate-like construction and/or at least a part of the carrying bodies is shaped to suit the shape of the broken bone piece and/or the given type of bone fracture.
In the case of a further form of execution of the invention the fixation device is composed of at least three carrying bodies, where at least one of the carrying bodies is corrugated in space, and this carrying body corrugated in space is an intermediate carrying body inserted between the other carrying bodies, or at least one of the carrying bodies has ribs on the internal side and/or on the external side, and this carrying body with ribs is the intermediate carrying body inserted between the other carrying bodies.
In the case of an even further different realisation of the fixation device, at least one of the openings of the carrying bodies situated above each other is suitable for accommodating an angle-stable threaded screw.
In the case of another different form of execution of the invention, at least one of the openings of the carrying bodies situated above each other is suitable for accommodating a threaded connecting element.
In the case of a further different form of execution of the fixation device, among the carrying bodies the external side contacting the broken bone piece has contact zones partly different from the external shape of the bone piece.
The fixation device according to the invention has numerous favourable characteristics. The most important one of these is that due to the carrying bodies having a novel construction and a thickness other than the ordinary thickness of the rigid locking plates, a fixation device is created, which can be shaped easily and quickly, without any special auxiliary device, and has the desired rigidity and flexibility when it is assembled both against static and dynamic effects exerted on the joined bone pieces, as a result of which it can fix the bone pieces to be united in an optimal position during the period of fusion. Consequently the fixation device according to the invention can fulfil both requirements at the same time, which was not possible with the known locking plates used so far. A further advantage is that by pairing carrying bodies of a different thickness, the rigidity of the given fixation device can always be set to result in the most favourable healing in the case of the given bone pieces.
It must also be regarded as an advantage that in the case of using more than two carrying bodies and connecting them in a special way it becomes possible to create a fixation device that facilitates the fusion of the bone pieces and allows micro-movements, which could not be realised at all with the help of locking plates. A further advantage deriving from this favourable effect is that the recovery period is shorter, which may result in saving costs at a social level, and it may also improve the use of capacities in the healthcare system.
Another advantage is that it is faster and simpler to create a fixation device of the desired shape, as a result of which the time of the operation can be reduced as well as the loading of the patient and the operating surgeon. It also results in that the possibility of operative complications reduces or terminates, consequently the recovery process becomes shorter. In the case of the operating surgeon the psychic and physical loading is less, which results in better performance and greater capacities.
It is also advantageous that the carrying bodies forming the fixation device according to the invention can be created in a simple way using the ordinary production technology, by setting the suitable thickness values and appropriately pairing the individual carrying bodies, a fixation device suiting all purposes and having physical parameters that can be varied within a wide range can be constructed.
Below the fixation device according to the invention is described in greater detail in connection with construction examples, on the basis of a drawing. In the drawing figure I is the view a possible form of execution of the carrying body of the fixation device according to the invention, partly in section,
figure 2 is another version of the carrying body of the fixation device according to the invention, partly in section, figure 3 is the side-view of the fixation device according to the invention constructed from carrying bodies, partly in section.
Figure 1 shows a version of the carrying body 10 to be used with the fixation device 1 according to the invention, in the case of which the average thickness "V" of the material of the carrying body 10 made of medical steel plate is 1.3 mm. It can be seen that there are openings 13 and openings 14 between the internal side 1 1 and the external side 12 of the shaped plate-like carrying body 10. Furthermore, on the internal side 1 1 of the carrying body 10 there are first positioning pieces 40, while on the external side 12 there are second positioning pieces 50. Figure 1 also shows that the first positioning pieces 40 and the second positioning pieces 50 are situated on the internal side 11 and external side 12 of the carrying body conforming to each other, so that with their given shape and size they can cooperate with another carrying body 10 of the same shape and size, i.e. in such a way that two carrying bodies 10 can be joined to each other.
In the present case, the first positioning pieces 40 situated on the internal side 1 1 of the carrying body 10 are cavities recessed in the material of the carrying body 10, while the second positioning pieces on the external side 12 of the carrying body 10 are projections extending out from the carrying body 10 and fitting into the cavities on the internal side 1 1. On the internal side 1 1 of the carrying body 10 containing the first positioning pieces 40 there are also contact zones 15 to facilitate the desired fixation of a bone piece 2 - not shown in figure 1 - and the carrying body 10.
Figure 2 shows a carrying body 30, in the case of which the average thickness "V" of the plate forming the basic material is 1 mm, and there are openings 33 and openings 34 between the internal side 31 and the external side 32 of the carrying body 30. Furthermore, the carrying body 30 has ribs 60, which, in the present case, consist of corrugations that run crosswise on the rectangular carrying body 30 and are concave when viewed from the internal side 31 and convex when viewed from the external side 32, and are made by working the material of the carrying body 30 in a pressure tool. Figure 3 shows a fixation device 1 formed by carrying body 10 described above, carrying body 30 and carrying body 20 identical to carrying body 10, when it is joined to a bone piece 2. It can be seen that the openings 13 of carrying body 10, the openings 33 of carrying body 30 and the openings 23 of carrying body 20, and the openings 14 of carrying body 10, the openings 34 of carrying body 30 and the openings 24 of carrying body 20 are situated above each other, and there is an angle-stable threaded screw 70 on the series of openings 13-33-23, and there is a threaded joining element 80 in the group of openings 14-34-24.
The angle-stable threaded screws 70 and/or the threaded joining elements 80 make it possible to attach carrying body 10, carrying body 20 and - in between them - carrying body 30 of the fixation device 1 to the bone piece 2.
In the interest of the easier assembly of carrying body 10, carrying body 30 and carrying body 20, neighbouring carrying body 10 - carrying body 30, and carrying body 30 - carrying body 20 should be oriented with the help of the first positioning pieces 40 and the second positioning pieces 50 before they are attached to the bone piece 2.
Figure 3 also shows that carrying body 30 inserted in between carrying body 10 and carrying body 20 - and having a corrugated form because of the ribs 60 - makes the fixation device 1 attached to the bone piece 2 more flexible, as a result of which it can absorb dynamic effects more favourably.
Obviously, the fixation device can also be assembled differently, other than shown in figure 3. Another possible version is, which consists of two members containing carrying body 10 and identical carrying body 20, but a fixation device 1 consisting of four or even five members, containing carrying body 10, carrying body 20 and carrying body 30 is also possible. Furthermore, the thickness "V" of the individual carrying bodies 10, carrying bodies 20 and carrying bodies 30 can also be different, but it is important to point out that the thickness "V" of the individual elements should not be more than 2 mm, because above this thickness carrying body 10 or carrying body 30 is difficult to form. The thickness "V" below this value, the resultant thickness of the fixation device 1 assembled from carrying bodies 10, carrying bodies 20 and, in a given case, carrying bodies 30, altogether, ensures the desired rigidity and stability, and, in a given case, stability, because the components, due to their smaller thickness "V", are favourably ductile and can be shaped to suit the shape of the surface of the bone pieced 2.
When using the fixation device 1 according to the invention, first for example the internal side 21 of carrying body 20 must be fitted to the bone piece 2 in such a way that the ductile carrying body 20 must be bent to have a shape determined by the shape of the bone piece 2 or a similar shape. Then carrying body 30 and carrying body 10 must be bent to have the desired shape. In the following, the appropriately shaped carrying body 10, carrying body 20 and carrying body 30 can be fitted together in such a way that the internal side 11 of carrying body 10 should contact the external side 32 of cariying body 30, while the internal side 31 of carrying body 30 should contact the external side 22 of carrying body 20. In the interest of appropriate positioning, by joining together the first positioning pieces 40 of carrying body 10 and the second positioning pieces 50 of carrying body 30, and the first positioning pieces 40 of carrying body 30 and the second positioning pieces 50 of carrying body 20, a fixation device 1 can be created, which can be handled as an integral unit representing the entire thickness and bent to assume the shape of the bone piece 2.
The fixation device 1 assembled in advance as above is then placed on the bone piece using the contact zones 25 situated on the internal side 21 of the carrying body 20, the angle-stable threaded screws 70 are taken through openings 13-33-23, while the threaded joining elements 80 are taken through openings 14-34-24 to fix them into the bone piece 2, and in this way the fixation device 1 can be attached to the bone piece 2, as a result of which the elements of the bone piece 2 can be joined together in the desired position until the end of the healing process.
The fixation device according to the invention can be favourably used in all cases, when practically a plate should be used for joining fractured bone pieces. List of references

Claims

1. Fixation device for joining bone pieces, which has a carrying body (10) with an internal side (1 1) and a reverse external side (12), and the carrying body (10) contains openings between the external side (11) and the internal side (12), characterised by that the fixation device ( 1) is formed by the unity of at least two carrying bodies (10, 20) of an average thickness (V) of a maximum of 2 mm, in the case of which, when the fixation device (1) is in use, the external side (12) of the first carrying body (10) and the internal side (21) of the neighbouring second carrying body (20) touch each other at least partly, and at least a part of the openings (13, 14) of the first carrying body (10) and the openings (23, 24) of the second carrying body (20) are situated above each other.
2. Fixation device as in claim 1 , characterised by that on the internal side (11, 21) of the carrying bodies (10, 20) there are first positioning pieces (40) and/or on the external side (12, 22) of the carrying bodies (10, 20) there are second positioning pieces (50), and when the fixation device ( 1 ) is in use, the neighbouring carrying bodies (10, 20) are fitted to each other with the help of the first positioning pieces (40) and second positioning pieces (50) co-acting with each other.
3. Fixation device as in claim 1 or 2, characterised by that at least a part of the carrying bodies (10, 20) has a rectangular plate-like construction.
4. Fixation device as in any of claims 1-3, characterised by that at least a part of the carrying bodies (10, 20) is shaped to suit the shape of the broken bone piece and/or the given type of bone fracture.
5. Fixation device as in any of claims 1-4, characterised by that the fixation device (1) is composed of at least three carrying bodies (10, 20, 30).
6. Fixation device as in claim 5, characterised by that at least one of the carrying bodies (10, 20, 30) is corrugated in space, and this carrying body (30) corrugated in space is an intermediate carrying body (30) inserted between the other carrying bodies (10, 20).
7. Fixation device as in claim 5, characterised by that at least one of the carrying bodies (10, 20, 30) has ribs (60) on the internal side (31) and/or on the external side (32), and this carrying body (30) with ribs is the intermediate carrying body (30) inserted between the other carrying bodies (10, 20).
8. Fixation device as in any of claims 1-7, characterised by that at least one of the openings (13, 23, 33) of the carrying bodies (10, 20, 30) situated above each other is suitable for accommodating an angle-stable threaded screw (70).
9. Fixation device as in any of claims 1-7, characterised by that at least one of the openings (13, 23, 33) of the carrying bodies (10, 20, 30) situated above each other is suitable for accommodating a threaded connecting element (80).
10. Fixation device as in any of claims 1-9, characterised by that among the carrying bodies ( 10, 20, 30) the external side (12, 22) contacting the broken bone piece (2) has contact zones (15, 25) partly different from the external shape of the bone piece (2).
