Description
Uretersteinsauginstrument mit einem Schaft
Die Erfindung betrifft ein Uretersteinsauginstrument der im Oberbegriff des Anspruches 1 genannten Art.
Ureteroskope dienen zum Arbeiten im Ureter, dem Körperkanal, der die Niere mit der Blase verbindet. Da dieser Körperkanal sehr eng ist, darf der Durchmesser des Schaftes des Ureteroskopes nicht mehr als maximal 4 - 5 mm betragen.
In dem sich ergebenden sehr beengten Querschnitt des Schaftes muss der Bildleiter einer Optik untergebracht werden. Diese weist am distalen Ende des Schaftes ein Objektiv auf, vom dem aus durch die Länge des Schaftes ein Bildleiter verläuft, der aus hintereinander sitzenden Stablinsen, einem Lichtleitfaserbündel oder einem an eine Videokamera angeschlossenen Videokabel besteht.
Im verbleibenden sehr geringen Querschnitt ist bei bekannten Ureteroskopen ein Kanal angeordnet, der als Arbeitskanal zur Aufnahme von Instrumenten wie z. B. einem Lithotriptor dient, beispielsweise in Form einer Laserfaser, der aber auch als Saugkanal nutzbar ist. Das Ureteroskop ist somit als gattungsgemäßes Uretersteinsauginstrument nutzbar, um mit dem Lithotriptor erzeugte Steinfragmente abzusaugen.
BESTÄTIGUNGSKOPIE
Nachteilig bei den bekannten gattungsgemäßen Uretersteinsauginstrumenten ist das Absaugen von Steinfragmenten durch den Saugkanal, der sehr eng ist, mit einem Querschnitt im Bereich der Größe der Steinfragmente, wodurch es zu Verstopfungen durch sich in dem Saugkanal verkeilende Steinfragmente kommt. Außerdem ist die Größe der absaugbaren Steinfragmente durch den Durchmesser des Saugkanales begrenzt.
Die Aufgabe der vorliegenden Erfindung besteht darin, das gattungsgemäße Uretersteinsauginstrument hinsichtlich der Verstopfungsproblematik und der Beschränkung der absaugbaren Steingröße zu verbessern.
Diese Aufgabe wird mit den Merkmalen des Kennzeichnungsteiles des Anspruches 1 gelöst.
Erfindungsgemäß ist distal vor dem Uretersteinsauginstrument ein Fangbehälter mit zwei Öffnungen angeordnet, wobei an der einen Öffnung der Saugkanal angeschlossen ist. Folglich wird die Flüssigkeit, in der sich diese Operation abspielt, durch die distale Öffnung und durch den Fangbehälter hindurch gesaugt. Abzusaugende Steinfragmente gelangen in den Fangbehälter und können in diesem gehalten und mit diesem abtransportiert werden, ohne dass sie durch den Saugkanal bewegt werden müssen. Der Abtransport des Fangbehälters kann zusammen mit dem Schaft des Uretersteinsauginstrumentes geschehen, das ein Ureteroskop sein kann, so dass für die Ausdehnung des Fangbehälters der volle Querschnitt des Ureteroskopes zur Verfügung steht. Es können auf diese Weise also sehr große Steine oder Steinfragmente entfernt werden.
Der Saugkanal kann durch den in einem konventionellen Ureteroskop vorhandenen Arbeitskanal gebildet sein. Der Fangbehälter kann an dessen distalem Ende vor dem Ureteroskop angeordnet und mit seiner proximalen Öffnung an die dis-
tale Mündung des Arbeitskanales angeschlossen sein. Vorteilhaft sind aber die Merkmale des Anspruches 2 vorgesehen. Dabei ist der Saugkanal in einem Schlauch ausgebildet. Dieser kann unmittelbar als Schaft des Uretersteinsauginstrumentes dienen. Der Schaft kann aber auch im Arbeitskanal eines Ureteroskopes verwendet werden, aus dem er auch herausziehbar ist, so dass der volle Querschnitt des Arbeitskanales für einen Lithotriptor, z. B. eine Laserfaser, zur Verfügung steht.
Der Fangbehälter kann unmittelbar an dem Schlauch befestigt sein und mit diesem in den Arbeitskanal des Ureteroskopes eingeschoben werden. Dazu sind vorteilhaft die Merkmale des Anspruches 3 vorgesehen. Bei flexibler Ausbildung des Fangbehälters kann dieser zusammengefaltet durch den engen Arbeitskanal des Ureteroskopes vorgeschoben werden, um sich anschließend distal vor diesem in die Behälterform zu entfalten. Die Flexibilität sollte sorgfältig gewählt werden, um sowohl dem Schlauch als auch dem Fangbehälter eine gewisse Eigen- steifigkeit zu belassen. Beide benötigen diese, um beim Saugen nicht zusammenzufallen.
Wenn der Fangbehälter sehr weich ist, kann es schwierig sein, seine distale Öffnung in einer Position zu halten, die zum Ansaugen von Steinfragmenten günstig ist. Daher sind vorteilhaft die Merkmale des Anspruches 4 vorgesehen. Dabei kann das Wandmaterial des Fangbehälters sehr weich sein, dennoch wird die distale Öffnung mit einer Stützeinrichtung gehalten, so dass sie offen bleibt und in geeigneter Position steht. Die Stützeinrichtung kann z. B. ein gesondertes Drahtgestell sein, an dem das Wandmaterial des Fangbehälters befestigt ist. Die Versteifung kann in das Wandmaterial des Fangbehälters eingearbeitet sein. Das Wandmaterial des Fangbehälters kann auch selbst von geeigneter Festigkeit sein, um die Stützeinrichtung auszubilden, die vorteilhaft gemäß Anspruch 5 flexibel ist. Damit ist es möglich, die Stützeinrichtung mit größeren Abmessungen als dem Querschnitt des Arbeitskanals auszubilden. Die Stützeinrichtung kann dann
ebenso wie der Fangbehälter in zusammengelegtem Zustand durch den Arbeitskanal geschoben werden, um sich vor diesem dann zu entfalten.
Vorteilhaft gemäß Anspruch 6 ist die distale Öffnung des Fangbehälters verschließbar. Damit wird verhindert, dass in dem Fangbehälter gehaltene Steine wieder herausfallen, was bei den starken Beanspruchungen beim Herausziehen des Fangbehälters zusammen mit dem Ureteroskopes passieren kann.
Um zu verhindern, dass Steine, bzw. Steinfragmente, die in den Fangbehälter gesaugt werden, in den Saugkanal gelangen und diesen dann verstopfen, kann z.B. der Mündungsrand des Saugkanales im Durchmesser etwas verringert werden, so dass nur sehr kleine Steinstücke in den Saugkanal gelangen können, bei denen die Verstopfungsgefahr geringer ist. Allerdings führen solche Maßnahmen zur Verringerung der Saugleistung, was das Ansaugen von Steinstücken erschwert. Vorteilhaft sind daher die Merkmale des Anspruches 7 vorgesehen. Ein Filter im Bereich des Eingangs zum Saugkanal, der z.B. als relativ grobes Gitter mit niedrigem Strömungswiderstand ausgebildet sein kann, hält Steine fern, so dass diese im Fangbehälter bleiben. Mit dieser Maßnahme wird die Verstopfung des Saugkanales äußerst wirksam verhindert.
Vorzugsweise sind gemäss Anspruch 8 der Fangbehälter und der Schaft einzeln oder gemeinsam, vorzugsweise als Baueinheit, als Einmalartikel ausgebildet, wodurch stets hervorragende Sterilität gesichert ist.
Das Uretersteinsauginstrument kann als Schaft nur den Schlauch aufweisen, der auch als steifes Rohr ausgebildet sein kann, um beim vorschieben in den Körper die sichere Steuerung zu ermöglichen. Vorzugsweise ist jedoch gemäss Anspruch 9 das Uretersteinsauginstrument als Ureteroskop ausgebildet. Das ergibt eine Reihe von Vorteilen, z.B. die im Ureteroskop vorgesehene Optik, die das Absaugen unter optischer Kontrolle ermöglicht.
In der Zeichnung ist die Erfindung beispielsweise und schematisch in einer einzigen Fig. 1 dargestellt, die eine Seitenansicht des distalen Endbereiches eines erfindungsgemäßen Uretersteinsauginstrument in Form eines Ureteroskopes zeigt.
In Fig. 1 ist ein Uretersteinsauginstrument in Form eines Ureteroskopes 1 nur mit dem distalen Endbereich seines Schaftes 2 dargestellt. Der Schaft 2 endet distal mit seiner distalen Stirnfläche 3, in der ein schematisch angedeutetes Objektiv 4 einer Optik liegt, die durch den Schaft 2 hindurch einen Bildleiter 5 aufweist, der das vom Objektiv 4 erfasste Bild zum proximalen Ende des Schaftes 2 überträgt, um dort betrachtet oder angezeigt zu werden. Der Bildleiter 5 kann als Stablinsenanordnung oder als Lichtleitfaserbündel ausgebildet sein. Am Objektiv 4 kann auch eine Kamera angeordnet sein, wobei der Bildleiter 5 als Videokabel ausgebildet ist.
In einem geschnittenen Ausschnittsbereich des Schaftes 2 ist zu sehen, dass dieser von einem Arbeitskanal 6 durchlaufen ist. Dieser ist in üblicher Ausbildung an beiden Enden offen. Im Ausführungsbeispiel nimmt er einen Schlauch 7 mit Lumen 17 auf, der am nicht dargestellten proximalen Ende an eine Saugeinrichtung, z. B. eine Pumpe, anschließbar ist.
Der dargestellte Bereich des Ureteroskopes 1 ist bei bestimmungsgemäßer Verwendung im Ureter bzw. im Nierenbecken angeordnet, also in Flüssigkeit. Diese kann durch den mit seinem Lumen 17 als Saugkanal dienenden Schlauch 7 abgesaugt werden.
Bei herausgezogenem Schlauch 7 kann durch den Arbeitskanal 6 ein nicht dargestellter Lithotriptor eingeführt werden, also ein Instrument zum Zertrümmern von
im Ureter, bzw. im Nierenbecken liegenden Nierensteinen. Die nach der Zertrümmerung verbleibenden Steinfragmente müssen dann entfernt werden.
Der zuvor verwendete Lithotriptor, der durch den Arbeitskanal 6 verlegt war, kann z. B. als Stoßdraht eines Stoßwellenlithotriptors ausgebildet sein oder als Lichtleitfaser eines Laserlithotriptors. Nach gelungener Zertrümmerung eines störenden Steines wird der Lithotriptor aus dem Arbeitskanal 6 herausgezogen und es kann nun der Schlauch 7 bis in die dargestellte Arbeitsstellung eingeschoben werden. Wird nun durch den Schlauch 7 gesaugt, so könnten sich Steinfragmente im Inneren der Schlauches verkeilen und diesen verstopfen. Um das zu verhindern, ist die dargestellt Konstruktion vorgesehen.
Am distalen Rand 8 des Schlauches 7 ist ein Fangbehälter 9 mit dem Rand einer proximalen Öffnung befestigt. Eine zweite distale Öffnung des Fangbehälters 9 ist an einem Ring 10 befestigt, der, von einer Stange 1 1 getragen, im Abstand vor dem Rand 8 des Schlauches 7 gehalten ist.
Bei der dargestellten Ausführungsform mit der Stützeinrichtung 10, 1 1, zum Halten der distalen Öffnung des Fangbehälters 9 kann der Fangbehälter 9 aus sehr leichtem, flexiblem Material bestehen, wie dies in Fig. 1 dargestellt ist. Es sollte aber eine restliche Steifigkeit haben, um nicht beim Saugen zusammenzufallen und den Durchfluss zu verschließen.
Fig. 1 zeigt, dass vier mit gestrichelten Umrissen angedeutete Steinfragmente 12 im Inneren des Behälters 9 liegen. Sie wurden durch die durch den Ring 10 definierte distale Öffnung des Behälters 9 angesaugt. Die Steinfragmente 12 könnten bei starkem Saugen in den Schlauch 7 gesaugt werden, was verhindert werden soll, um dort nicht Verstopfungen zu produzieren. Zu diesem Zweck ist nahe des Randes 8 im Schlauch 7, also im Bereich des Überganges zwischen dem
Schlauch 7 und dem Fangbehälter 9 ein Filter 13 angeordnet, der z. B. als Gitter ausgebildet ist und die Steinfragmente 12 zurückhält.
Nahe dem distalen Ende des Fangbehälters 9, also benachbart zum Ring 10, ist eine Schlinge eines Fadens 14 um den Behälter 9 geschlungen. Der Faden ist bei 15 an der Stange 1 1 befestigt und verläuft nach Umschlingen des Fangbehälters 9 durch eine Führungsöse 16 an der Stange 1 1 und von dort durch den Schlauch 7 nach draußen. Der Operateur kann am äußeren Ende des Fadens 14 ziehen, um die Schlinge zuzuziehen.
Der Fangbehälter 9 kann dadurch an beiden Enden verschlossen werden, nämlich am einen Ende durch den Filter 13, am anderen Ende durch die Schlinge des Fadens 14. Nun kann der Fangbehälter 9 auch grob bewegt werden, ohne dass Steinfragmente 12 aus ihm entweichen können.
Der Fangbehälter 9 und auch der Ring 10 der Stützeinrichtung sind elastisch flexibel ausgebildet und können so weit zusammengelegt, bzw. -gefaltet werden, dass sie beim Einführen durch den Arbeitskanal 6 geschoben werden zu können, vor dem sie sich dann in die dargestellte Position entfalten können. Ebenso können der Fangbehälter 9 und der Ring 10 auch in rückwärtiger Richtung, also in proximaler Richtung, wieder durch den Arbeitskanal 6 gezogen werden. Auf diese Weise können kleinere Steine mit dem Fangbehälter 9 durch den Arbeitskanal 6 nach draußen befördert werden. Sind größere Steinfragmente oder viele Steinfragmente im Fangbehälter 9 angeordnet, wie dies die Fig. 1 zeigt, dann wird der Fangbehälter 9 zusammen mit dem gesamten Ureteroskop 1 herausgezogen, um die Steine zu entfernen.
Der Fangbehälter 9 kann anders als dargestellt ausgebildet sein, z. B. sich trichterförmig vom Rand 8 bis zum Ring 10 erweiternd aus relativ steifem, selbsthal-
tendem Material, so dass dieses Material selbst die Stützeinrichtung ausbildet. Eine gesonderte Stange 1 1 und ein gesonderter Ring 10 können dann entfallen.
Der Fangbehälter 9 kann auch unmittelbar am distalen Rand 8 des Arbeitskanales 6 befestigt sein, so dass der gesonderte Schlauch 7 entfällt.
Umgekehrt kann auch auf den Schaft 2 des Ureteroskopes 1 verzichtet und der Schlauch 7 selbst als Schaft verwendet werden.
Der Fangbehälter 9 kann aus geschlossener Folie ausgebildet sein, oder z. B. auch aus gelochtem Material, sofern die Löcher den gewünschten proximalen Strömungsfluss von der distalen zur proximalen Öffnung des Fangbehälters 9 nicht zu stark stören, wodurch das Ansaugen von Steinen behindert werden könnte.
Der Schlauch 7 und der Fangbehälter 9 sind bevorzugt als Einmalartikel ausgebildet und werden steril verschweisst hergestellt.
Claims
1. Uretersteinsauginstrument mit einem Schaft (7, 2), der von einem an eine Saugeinrichtung anschließbaren Saugkanal (17, 6) durchlaufen ist, dadurch gekennzeichnet, dass distal vom Schaft (7, 2) ein Fangbehälter (9) mit einer distalen und einer proximalen Öffnung angeordnet ist, von denen die proximale Öffnung an das distale Ende (8) des Saugkanales (17, 6) angeschlossen ist.
2. Uretersteinsauginstrument nach Anspruch 1, dadurch gekennzeichnet, dass der Schaft als Schlauch (7) ausgebildet ist.
3. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Fangbehälter (9) flexibel ausgebildet ist.
4. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die distale Öffnung des Fangbehälters (9) mittels einer Stützeinrichtung (10, 1 1) gehalten ist.
5. Uretersteinsauginstrument nach Anspruch 4, dadurch gekennzeichnet, dass die Stützeinrichtung (10, 11) flexibel ausgebildet ist.
6. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Fangbehälter (9) im Bereich seiner distalen Öffnung verschließbar ausgebildet ist.
7. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass im Bereich der proximalen Öffnung des Fangbehälters (9) ein den Zugang zum Saugkanal (17, 6) versperrender Filter (13) angeordnet ist.
8. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Fangbehälter (9) und/oder der Schaft (7) als Einmalartikel ausgebildet sind.
9. Uretersteinsauginstrument nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Uretersteinsauginstrument als Ureteroskop (1) ausgebildet und der Saugkanal (17, 6) im Arbeitskanal (6) des Ureteroskops (1) angeordnet ist.
