Abstract

An apparatus for use in maintaining the centreline through the femoral head/neck junction and preventing over reaming of the femoral head when carrying out hip resurfacing surgery comprising an axle and a means to lock the axle in a predetermined position in relation to the femoral head/neck junction wherein said axle comprises an elongate member provided with an outwardly extending flange.

Description

DEBRIS RETAINING REAMER
The present invention relates to a device for use in maintaining the centreline and preventing over reaming of the femoral head when carrying out hip resurfacing surgery, together with a device for use in reaming the femoral head that enables bone debris to be collected.
When carrying out hip resurfacing surgery it is essential to accurately ream the femoral head to ensure correct positioning of the prosthetic femoral head component. Over reaming of the femoral head can result in the need to completely replace the femoral head, whilst inaccurate reaming can lead to an ill fitting joint. At present there are devices to check for the correct clearance around the circumference of the femoral head but not over the entire surface of the head. Furthermore, current devices do not counteract the possibility of over reaming, the prevention of which is judged largely by eye. In addition, there is a need to collect bone debris during reaming to prevent contamination of the wound.
The present invention aims to solve the problems of the prior art devices thereby rendering hip resurfacing surgery quicker and safer for the patient.
According to a first aspect the present invention provides an apparatus for use in maintaining the centreline through the femoral head/neck junction and preventing over reaming of the femoral head when carrying out hip resurfacing surgery comprising an axle and a means to lock the axle in a predetermined position in relation to the femoral head/neck junction wherein said axle comprises an elongate member provided with an outwardly extending flange. Preferably the elongate member of the axle is a hollow tube and therefore has an outer diameter and an inner diameter.
Preferably said hollow tube of said axle comprises first and second sections. Preferably said first and second sections of said hollow tube of said axle are of different outer diameter. Preferably the outer diameter of said second section is greater than that of said first section. Preferably the first section of said axle is substantially longer than the second section.
Preferably said flange is a shoulder positioned where said two sections are connected.
Preferably the shoulder is substantially perpendicular to the surfaces of said first and second sections.
Preferably said second section of said hollow tube of said axle further comprises at least one gap extending along at least part of its length and arranged to allow said second section to at least partially splay apart.
Preferably if there is more than one gap the gaps are one or more of, preferably all of, the same dimensions, rectangular, parallel, and spaced equidistantly around the circumference of the second section. Preferably there are three or more, preferably four gaps.
Preferably the locking means comprises a rod of smaller diameter than the inner diameter of said axle. Preferably at least one end of said rod is provided with a portion of larger diameter than at least part of the inner diameter of the second section of said axle. Preferably both ends of the rod are of larger diameter than at least part of the inner diameter of the second section of said axle. Preferably both ends of the rod are substantially ball-shaped.
Preferably if both ends of the rod are of larger diameter than at least part of the inner diameter of said axle, one of the ends is removable.
Preferably the rod is longer than the axle.
Preferably the rod is positioned inside the axle and if only one end of the rod is provided with a portion of larger diameter than at least part of the inner diameter of the second section of the axle, that end of the rod is closer to the second section than the first section, whilst the other end of the rod is closer to the first section than the second section.
This arrangement of the second section of the axle and said at least one end of the rod which is of larger diameter than at least part of the inner diameter of the second section of said axle provides a means of locking the axle inside a predrilled channel representing the centre line through the femoral head/neck bone junction. This is achieved by forcing the surface of the second section to engage the channel tightly by the splaying action of said at least one end of the rod upon the inner surface of the second section when the rod is fed into the second section and pulled enough out of the first section.
Preferably there is a mark around the circumference of the outer surface of the first section of the axle 1.15 cm to 1.35 cm, more preferably 1.20 cm to 1.30 cm, most preferably 1.25 cm from the shoulder.
Preferably the axle and rod are made of stainless steel. In use the axle is positioned inside a predrilled channel representing the centre line through the femoral head/neck bone junction such that the mark is aligned with the surface of the femoral head. The distance between the shoulder and the mark represents the depth of the layer of bone that needs to be removed from the surface of the femoral head during hip resurfacing. The shoulder of the axle prevents over reaming of the femoral head by stopping the reamer travelling any further than the shoulder.
According to a second aspect the present invention provides an apparatus for use in reaming the femoral head when carrying out hip resurfacing surgery comprising a reamer head and a removable cover wherein said reamer head comprises a shaft and a cutting portion, wherein said shaft of said reamer head includes a locking means; and said removable cover comprises a shaft and a shroud, wherein said shroud is connected to said shaft of said removable cover, and wherein said shaft of said removable cover comprises a locking means arranged to engage with said locking means of said shaft of said reamer head, and wherein said shroud covers the reamer head.
Preferably said cutting portion of said reamer head comprises one or more of, preferably all of, a flat cutting area, a chamfer cutting area, a cylindrical cutting area and an end cutting area. Preferably the end cutting area is a substantially flat ring with an inner edge and an outer edge. Preferably the distance between the inner and outer edges of the end cutting area is approximately 6 mm.
This arrangement of the inner surface of said cutting portion ensures that when reaming is complete the surface of the femoral head will correspond to the inner surface of a prosthetic component. The arrangement of the reamer head and removable cover enables bone debris to be collected in the removable cover when the reamer is in use, preventing such debris from contaminating the wound.
Preferably said shaft of said removable cover is hollow and has an inner diameter that is larger than the outer diameter of said shaft of said reamer head.
Preferably the shroud covers the reamer head between the shaft of the removable cover and the end cutting area. Preferably said shroud comprises a seal arranged such that when said locking means of said shaft of said removable cover engages said locking means of said shaft of said reamer head, said seal contacts said end cutter.
Preferably the locking means of the reamer head and the removable cover comprise a tongue and groove or ball and cage arrangement.
Preferably the cutting portion has grater-like inner surfaces.
Preferably the reamer head is made of metal and the removable cover is made of plastic. Preferably the shroud is manufactured from a transparent plastic.
Preferably the shaft of the reamer head is arranged to be attachable to a motor in order that the reamer head can be automatically spun.
According to a third aspect the present invention provides a combination of an apparatus for use in maintaining the centreline and preventing over reaming when carrying out hip resurfacing surgery as previously described and an apparatus for use in reaming the femoral head when carrying out hip resurfacing surgery as previously described wherein said shaft of said reamer head is hollow and the inner diameter is larger than the outer diameter of at least part of said axle and the shaft of the reamer head is received by the axle.
Preferably the shaft of the reamer head is of a smaller diameter than the shoulder of the axle.
In use the shoulder of the axle prevents over reaming of the femoral head by stopping the reamer travelling any further than the shoulder which is contacted by one or both of the reamer head shaft or the flat cutting area surface.

Figure 1 displays axle 1 having a first section 2 and a second section 3.
The first section 2 of axle 1 has a first end 2a and a second end 2b. The second section 3 of axle 1 has a first end 3a and a second end 3b. The first section 2 is of substantially smaller outer diameter than the second section 3. The first section 2 is substantially longer than the second section 3. The second end 2b of first section 2 is connected coaxially to the first end 3a of second section 3. Where the first section 2 and the second section 3 meet, a shoulder 4 is formed perpendicular to the outer surface of axle 1.
Axle 1 has an inner diameter that tapers from the second end 3b of the second section 3 to the first end 2a of the first section 2.
The second section 3 of axle 1 has four parallel slots 7 such that when viewed from second end 3b of second section 3 the parallel slots resemble a cross (see Figure 2). The first section 2 of axle 1 contains a mark 8 around the circumference of the outer surface of axle 1 approximately 1.25 cm from shoulder 4.
Rod 6 has a first end 6a and a second end 6b. Mounted on the first end 6a of rod 6 is ball 9 which is a substantially ball-shaped removable section. Mounted on the second end 6b of rod 6 is ball 10 which is a substantially ball-shaped non-removable section. The length of rod 6 is greater than the length of axle 1.
The inner diameter of axle 1 is larger than the diameter of rod 6 (excluding balls 9 and 10 of rod 6) throughout their lengths. Both balls 9 and 10 of rod 6 are of larger diameter than the inner diameter of the first end 2a of the first section 2 of axle 1. Ball 9 of rod 6 is of larger diameter than the inner diameter of the second end 3b of the second section 3 of axle 1. Ball 10 of rod 6 is of smaller diameter than the inner diameter of the second end 3b of the second section 3 of axle 1 but as the inner diameter tapers towards the first end 2a of the first section 2 of axle 1 the inner diameter of axle 1 becomes smaller than the diameter of ball 10 of rod 6. The inner diameter of axle 1 becomes smaller than the diameter of ball 10 of rod 6 at a position approximately midway along the second section 3 of axle 1. Figure 3 illustrates reamer head 11 and removable cover 28. Reamer head 11 has a cutting portion 5 and shaft 12. Cutting portion 5 has a flat cutting area 13, a chamfer cutting area 14, a cylindrical cutting area 15 and an end cutting area 16.
Shaft 12 of reamer head 11 is an elongate hollow tube and has an inner diameter, a first end 17, a second end 18 and a locking means 19 midway between first end 17 and second end 18. The inner diameter of shaft 12 of reamer head 11 is arranged to be larger than the outer diameter of the first section 2 of axle 1, larger than the diameter of the first end 9 of rod 6, and smaller than the outer diameter of the second section 3 of axle 1.
The second end 18 of shaft 12 of reamer head 11 is connected perpendicularly around its outer circumference, to the inner edge 20 of flat cutting area 13 which extends away from shaft 12 and is a substantially flat ring with a grater-type surface and an outer edge 21. Outer edge 21 of flat cutting area 13 is connected to cylindrical cutting area 15 which is positioned perpendicular to the flat cutting area 13 via chamfer cutter 14 to create a funnel-like shape. The diameter of cylindrical cutting area 15 is larger than the diameter of flat cutting area 13. Chamfer cutting area 14 is a ring of frustoconical shape with a grater-type surface. Cylindrical cutting area 15 is a cylinder with a grater-type surface and an outer edge 25. Outer edge 25 of cylindrical cutting area 15 is connected perpendicularly around its circumference, to the inner edge 26 of end cutting area 16 which extends outwardly from cylindrical cutting area 15. End cutting area 16 is a substantially flat ring with a grater-type surface, an inner edge 26 and an outer edge 27 (see Figure 4) . The distance between the inner edge 26 and outer edge 27 of end cutting area 16 is approximately 6 mm. Removable cover 28 comprises a shaft 29 and a shroud 30. Shaft 29 of removable cover 28 is a hollow tube and has an inner diameter that is larger than the outer diameter of shaft 12 of reamer head 11. Shaft 29 of removable cover 28 has a first end 31, a second end 32 and a locking means 33 arranged to engage the locking means 19 of shaft 12 of reamer head 11. Shroud 30 of removable cover 28 comprises a funnel-like shape with a first end 34 and a second end 35. The diameter of the first end 34 of shroud 30 is smaller than the diameter of the second end 35 of shroud 30. The first end 34 of shroud 30 is connected to the second end 32 of shaft 29 of removable cover 28. The second end 35 of shroud 30 has a seal 36 around its circumference.
Shroud 30 is arranged such that when the locking means 33 of shaft 29 of removable cover 28 engages the locking means 19 of shaft 12 of reamer head 11 , shroud 30 covers the section of the reamer head 11 extending between the second end 32 of shaft 29 of removable cover 28 and the outer edge 27 of end cutting area 16, with seal 36 contacting the outer edge 27 of end cutting area 16.
In use, rod 6 is placed inside axle 1 by removing the first end 9 of rod 6, feeding rod 6 into axle 1 via the second end 3b of second section 3 of axle 1 far enough to be able to then replace the first end 9 of rod 6. The rod 6 and axle 1 are then placed, via the second end 10 and the second end 3b of second section 3 respectively, inside a predrilled channel 37 representing the centre line through the femoral head/neck bone junction (see Figure 5). Axle 1 is then positioned such that mark 8 is aligned with the surface of the femoral head 38 and rod 6 is pulled towards the first end 2a of the first section 2 of axle 1 until the second section 3 of axle 1 is forced, by ball-shaped second end 10 of rod 6, to engage channel 37 tightly. The reamer head 11 is engaged with the removable cover 28 to form a debris retaining reamer which is connected via the first end 17 of shaft 12 of reamer head 11 to a motor arranged to spin reamer head 11. The debris retaining reamer is then placed over axle 1 and rod 6 and the femoral head 38 is reamed using axle 1 as the centre for the reaming operation. The femoral head 38 is reamed until the surface of flat cutting 13 of reamer head 11 contacts the surface of shoulder 4 of axle 1. The femoral head should then be ready for a prosthetic component to be fitted having the same dimensions as the surface of the femoral head and the channel, and the inner surface of cutting portion 5.
Typically, for an adult male, the end cutting area 16 will have an inner diameter of 46 mm and an outer diameter of 60 mm.

Claims

1. An apparatus for use in maintaining the centreline through the femoral head/neck junction and preventing over reaming of the femoral head when carrying out hip resurfacing surgery comprising an axle and a means to lock the axle in a predetermined position in relation to the femoral head/neck junction wherein said axle comprises an elongate member provided with an outwardly extending flange.
2. An apparatus according to claim 1 wherein the elongate member of the axle is a hollow tube comprising an outer diameter and an inner diameter.
3. An apparatus according to claim 2 wherein said hollow tube of said axle comprises first and second sections.
4. An apparatus according to claim 3 wherein said first and second sections of said hollow tube of said axle are of different outer diameter.
5. An apparatus according to claim 4 wherein the outer diameter of said second section is greater than that of said first section.
6. An apparatus according to any of claims 3 to 5 wherein the first section of said axle is substantially longer than the second section.
7. An apparatus according to any of claims 3 to 6 wherein said flange is a shoulder positioned where said two sections are connected.
8. An apparatus according to claim 7 wherein the shoulder is substantially perpendicular to the surfaces of said first and second sections.
9. An apparatus according to either claims 7 or claim 8 wherein there is a mark around the circumference of the outer surface of the first section of the axle 1.15 cm to 1.35 cm from the shoulder.
10. An apparatus according to any of claims 3 to 9 wherein said second section of said hollow tube of said axle further comprises at least one gap extending along at least part of its length and arranged to allow said second section to at least partially splay apart.
11. An apparatus according to any of claims 2 to 10 wherein the locking means comprises a rod of smaller diameter than the inner diameter of said axle.
12. An apparatus according to claim 11 wherein at least one end of said rod is provided with a portion of larger diameter than at least part of the inner diameter of the second section of said axle.
13. An apparatus according to claim 12 wherein both ends of the rod are of larger diameter than at least part of the inner diameter of said axle and one of the ends is removable.
14. An apparatus according to any of claims 11 to 13 wherein the rod is longer than the axle.
15. An apparatus for use in reaming the femoral head when carrying out hip resurfacing surgery comprising a reamer head and a removable cover wherein said reamer head comprises a shaft and a cutting portion, said removable cover comprises a shaft and a shroud, said shroud is connected to said shaft of said removable cover, and wherein said shroud covers the reamer head.
16. An apparatus according to claim 15 wherein said shaft of said reamer head includes a locking means and wherein said shaft of said removable cover comprises a locking means arranged to engage with said locking means of said shaft of said reamer head.
17. An apparatus according to claim 15 or claim 16 wherein said cutting portion of said reamer head comprises one or more of a flat cutting area, a chamfer cutting area, a cylindrical cutting area and an end cutting area.
18. An apparatus according to claim 17 wherein the end cutting area is a substantially flat ring with an inner edge and an outer edge.
19. An apparatus according to claim 18 wherein the distance between the inner and outer edges of the end cutting area is approximately 6 mm.
20. An apparatus according to claim 19 wherein said shaft of said removable cover is hollow and has an inner diameter that is larger than the outer diameter of said shaft of said reamer head.
21. An apparatus according to any of claims 17 to 20 wherein the shroud covers the reamer head between the shaft of the removable cover and the end cutting area.
22. An apparatus according to any of claims 17 to 21, as they depend from claim 18, wherein said shroud comprises a seal arranged such that when said locking means of said shaft of said removable cover engages said locking means of said shaft of said reamer head, said seal contacts said end cutter.
23. An apparatus according to claim 16 or any of claims 17 to 22, as they depend from claim 16, wherein the locking means of the reamer head and the removable cover comprise a tongue and groove or ball and cage arrangement.
24. An apparatus according to any of claims 15 to 23 wherein the cutting portion has grater-like inner surfaces.
25. An apparatus according to any of claims 15 to 24 wherein the reamer head is made of metal and the removable cover is made of plastic.
26. An apparatus according to any of claims 15 to 25 wherein the shroud is manufactured from a transparent plastic.
27. An apparatus according to any of claims 15 to 26 wherein the shaft of the reamer head is attachable to a motor in order that the reamer head can be automatically spun.
28. An apparatus for use in reaming the femoral head when carrying out hip resurfacing surgery comprising a reamer head wherein the reamer head comprises a shaft and a cutting portion, wherein said cutting portion of said reamer head comprises a flat cutting area, a chamfer cutting area, a cylindrical cutting area and an end cutting area.
29. A combination of an apparatus for use in maintaining the centreline and preventing over reaming when carrying out hip resurfacing surgery according to any of claims 1 to 14 and an apparatus for use in reaming the femoral head when carrying out hip resurfacing surgery according to any of claims 15 to 28 wherein said shaft of said reamer head is hollow and the inner diameter is larger than the outer diameter of at least part of said axle and the shaft of the reamer head is received by the axle.
30. A combination according to claim 29 wherein the shaft of the reamer head is of a smaller diameter than the shoulder of the axle.
