Interspinale Prothese (1) mit einem in den Interspinalraum einführbaren Mittelteil (2) mit einer Zentralachse (4), einem inneren Ende (7) und einem äusseren Ende (8) sowie zwei am äusseren Ende (8) entspringenden, radial und diametral zur Zentralachse (4) verlaufenden Fortsätzen (3), welche in den Raum zwischen den Dornfortsätzen zweier benachbarter Wirbelkörper einführbar sind, wobei das Mittelteil (2) an seinem den Fortsätzen (3) abgewandten, inneren Ende (7) eine axiale Vertiefung (5) aufweist, zur Aufnahme eines zur Prothese (1) im wesentlichen symmetrischen Gegenstücks (6).

Description

Interspinale Prothese
Die Erfindung betrifft eine interspinale Prothese gemäss dem Oberbegriff des Patentanspruchs 1 , ein Gegenstück dazu sowie eine daraus kombinierte, mehrteilige interspinale Prothese.
Solche Prothesen dienen als Abstandhalter für zwei benachbarte Wirbelkörper bei einer defekten Bandscheibe, welche den Abstand zwischen den Wirbelkörper sonst verringern würde. Durch den wieder vergrösserten Abstand werden auch die Fazet- ten-Gelenke entlastet.
Aus der WO99/42051 ist eine gattungsgemässe, interspinale Prothese bekannt, welche ein in den Interspinalraum einzuführendes Mittelstück umfasst, aus welchem kranial und kaudal je ein paar Ohren rechts und links vom Mittelstück entspringen, um das Mittelstück im Raum zwischen den Dornfortsätzen (processus spinosus) zweier benachbarter Wirbelkörper - nach erfolgter Implantation - festzuhalten. Nachteilig bei dieser bekannten Prothese ist deren Einstückigkeit, welche die Implantation erschwert, so dass die Entfernung des supraspinalen Ligaments erforderlich ist. Die Entfernung dieses Ligamentes bringt den weiteren Nachteil mit sich, dass die Prothese nach dorsal keinen gesicherten Halt aufweist. Aus diesem Grunde wird in der WO99/42051 vorgeschlagen, die Prothese im interspinalen Mittelstück zu durchbohren, um darin ein Band hindurchzuführen, mit welchem die Prothese an den Dornfortsätzen der benachbarter Wirbelkörper festgebunden werden kann. Dieses Vorgehen ist sehr zeitraubend und kompliziert.
Die obenstehende Diskussion des Standes der Technik erfolgt lediglich zur Erläuterung des Umfeldes der Erfindung und bedeutet nicht, dass der zitierte Stand der Technik zum Zeitpunkt dieser Anmeldung oder ihrer Priorität auch tatsächlich publiziert oder öffentlich bekannt war.
Hier will die Erfindung Abhilfe schaffen. Der Erfindung liegt das Problem zugrunde eine interspinale Prothese zu schaffen, welche unter Beibehaltung des supraspinalen Ligamantes implantiert werden kann, so dass die Operation insgesamt schonender erfolgen kann und die Sicherung des Implantates nach dorsal ohne zusätzliche Mittel gewährleistet ist.
Die Erfindung löst die gestellte Aufgabe mit einer mehrteiligen, interspinalen Prothese, wobei die einzelnen Teile unterschiedliche mechanische Eigenschaften aufweisen können.
Die erfindungsgemässe, interspinale Prothese umfasst im wesentlichen einen in den Interspinalraum einführbaren Mittelteil mit einer Zentralachse und zwei am ausseren Ende entspringenden, radial und diametral zur Zentralachse verlaufenden Fortsätzen, welche in den Raum zwischen den Dornfortsätzen zweier benachbarter Wirbelkörper einführbar sind. Ferner ist am Mittelteil an seinem den Fortsätzen abgewandten, inneren Ende eine axiale Vertiefung angebracht, welche zur Aufnahme eines zur Prothese im wesentlichen symmetrischen Gegenstückes dient.
In der bevorzugten Ausführungsform weist das Mittelteil Kupplungsmittel auf, mit welchen das Gegenstück an der Prothese fixierbar ist. Die Kupplungsmittel sind derart beschaffen, dass die Positionen der Fortsätze an der Prothese und die Positionen der Fortsätze am Gegenstück bei fixiertem Gegenstück relativ zueinander bestimmt sind. Vorzugsweise bestehen die Kupplungsmittel aus einem Bajonettverschluss, der einen Anschlag aufweist, so dass bei an der Prothese befestigtem Gegenstück die Fortsätze an Prothese und Gegenstück ihre gewünschte Positionen einnehmen. Anstelle eines Bajonettverschlusses können die beide Teile durch eine Schraub- oder Konusverbindung aneinander fixiert werden, wobei in diesem Fall die Prothese und das Gegenstück vorzugsweise über eine Verdrehsicherung verfügen, so dass das Gegenstück nur in einer bestimmten Position in die axiale Vertiefung an der Prothese einführbar ist. Eine andere Ausgestaltung der Kupplungsmittel besteht in mindestens einem elastisch deformierbaren Nocken, welcher nach dem Zusammenfügen von Prothese und Gegenstück an diesem arretierbar oder einrastbar ist.
In einer anderen bevorzugten Ausführungsform der erfindungsgemässen Prothese umfassen die Kupplungsmittel mindestens einen elastisch deformierbaren Nocken, welcher beim Zusammenfügen von Prothese und Gegenstück elastisch deformierbar ist und nach dem Zusammenfügen von Prothese und Gegenstück in eine komple- mentäre Vertiefung einrastbar ist.
Ein bevorzugte Weiterbildung besteht darin, dass die zur Zentralachse orthogonalen Querschnittsebenen:
a) durch den Mittelteil an seiner engsten Stelle im Bereich des inneren Endes eine Fläche von 50 bis 300 mm2, vorzugsweise von 70 bis 250 mm2 aufweist; und
b) durch die Fortsätze eine Fläche von 70 bis 500 mm2, vorzugsweise 100 bis 450 mm2 aufweist.
Ferner ist die Prothese vorzugsweise aus einem elastischen Material gefertigt, so dass das Mittelteil radial elastisch deformierbar ist. Eine ausreichende radiale, elastische Deformierbarkeit ist durch eine Herstellung der Prothese aus Kunststoff oder durch Ausgestaltung des Mittelteiles mit radial elastisch deformierbaren Zungen erreichbar.
Die Prothese kann aus einem Elastomer, Silikon oder einem Polymer aus der Poly- carbonat-Familie gefertigt werden. Es ist aber auch möglich die Prothese aus einem metallischen Werkstoff zu fertigen, wenn mittels geeigneter mechanischer Vorrichtungen eine Elastizität der Prothese im Bereich des Mittelteils realisiert wird, welcher in den Interspinalraum zu liegen kommt.
In einer anderen Ausführungsform der erfindungsgemässen Prothese ist diese hohl ausgebildet, wobei die Hohlwände kollabierbar ausgestaltet und/oder durch Auffüllung des Hohlraumes expandierbar sein können. Die kollabierbaren Hohlwände weisen den Vorteil auf, dass dadurch eine grössere Deformation der Prothese ermöglicht werden kann, als dies ein elastischer Werkstoff gestatten würde.
Im Bereich des inneren Endes kann das Mittelteil an seiner Aussenfläche glatt oder aufgerauht ausgestaltet sein. Durch die Ausgestaltung der Aussenfläche lässt sich das Anwachsen des Knochens beeinflussen, wobei eine rauhe Aussenfläche das Anwachsen des Knochens an der Prothese fördert, während eine glatte Aussenfläche dieses Anwachsen erschwert oder gar verhindert. Die Oberfläche des Implanta- tes, welche mit dem Knochen in Berührung steht, kann auch durch Einbetten von Hydroxilapatit (HA) geschützt werden.
In der bevorzugten Ausführungsform umfasst das erfindungsgemässe Gegenstück ein inneres Ende, ein äusseres Ende sowie zwei am ausseren Ende entspringende, ebenfalls radial und diametral verlaufende Fortsätze, welche in den der Prothese gegenüberliegenden Raum zwischen den Dornfortsätzen zweier benachbarter Wirbel einsetzbar sind. Zudem ist am Gegenstück gegen das innere Ende gerichtet, ein Zapfen angebracht, welcher in die Vertiefung an der Prothese einführbar ist. Damit lässt sich eine exakte Ausrichtung von Prothese und Gegenstück bei der Implantation erreichen.
Wie die Prothese kann das Gegenstück mit analogen, respektive komplementären Kupplungsmitteln ausgestattet sein. Wiederum können diese Kupplungsmittel einen Bajonettverschluss, elastisch deformierbare Nocken, oder eine Schraub- respektive Konusverbindung umfassen. Ebenfalls kann am Gegenstück eine analoge, respektive komplementäre Verdrehsicherung angebracht sein.
Die bevorzugte Ausführungsform der erfindungsgemässen interspinalen Prothese mit einem Gegenstück zeichnet sich dadurch aus, dass sie im zusammengesetzten Zustand eine zur Zentralachse orthogonal stehende Symmetrieebene aufweist, wobei die Fortsätze der interspinalen Prothese zu denjenigen des Gegenstückes parallel zur Zentralachse betrachtet einen Abstand von mindestens 2 mm, vorzugsweise mindestens 3 mm aufweisen. Der maximale Abstand der Fortsätze der interspinalen Prothese und denjenigen des Gegenstückes beträgt 15 mm, vorzugsweise 12 mm.
Die Erfindung und Weiterbildungen der Erfindung werden im folgenden anhand der teilweise schematischen Darstellungen mehrerer Ausführungsbeispiele noch näher erläutert.

In Fig. 1 ist die interspinale Prothese 1 mit dem Gegenstück 6 in zusammengefügtem Zustand dargestellt. Das Mittelteil 2 der Prothese 1 grenzt dabei mit dem inneren Ende 7 der Prothese 1 an das Gegenstück 6 an. Am ausseren Ende 8 der Prothese 1 sind die beiden Fortsätze 3 senkrecht zur Zentralachse 4 und einander diametral gegenüberliegend angeordnet. Die Fortsätze 3 sind in der hier gezeigten Ausführungsform als Hälften eines ellipsoidartigen Körpers ausgeführt. Die ebenfalls radial und einander diametral gegenüberliegenden Fortsätze 3 des Gegenstückes 6 sind zu einer orthogonal zur Zentralachse 4 stehenden Ebene symmetrisch angeordnet. Als Verdrehsicherung zwischen Prothese 1 und Gegenstück 6 dienen drei radiale, und im parallel zur Zentralachse 4 betrachteten Querschnitt der Prothese 1 symmetrisch angeordnete Nocken 17, welche am Mittelteil 2 am inneren Ende 7 der Prothese 1 vorstehen und in komplementäre Nuten 18 am Gegenstück 6 eingreifen. Koaxial zur Zentralachse 4 umfasst das Mittelteil 2 eine Vertiefung 5, welche vom inneren Ende 7 her in die Prothese 1 bis zu einer Tiefe T eindringt. Das Gegenstück 6 hat einen Zapfen 16, welcher zur Vertiefung 5 komplementär ausgestaltet ist und somit beim Zusammenfügen von Prothese 1 und Gegenstück 6 in die Vertiefung 5 einführbar ist. Ferner umfasst die Prothese 1 einen Fixierbolzen 19 mit einem am ausseren Ende 8 der Prothese 1 zur Anlage bringbaren Bolzenkopf 26. Der Fixierbolzen 19 ist koaxial zur Zentralachse 4 durch die Prothese 1 durchführbar und im Zapfen 16 des Gegenstückes 6 mittels eines Bajonettverschlusses 27 arretierbar, so dass damit die Prothese 1 mit dem Gegenstück 6 lösbar verriegelbar ist. Der Fixierbolzen 19 und das Gegenstück 6 werden von einer zur Zentralachse 4 koaxialen Bohrung 20 durchgehend durchdrungen, so dass die Prothese 1 und das Gegenstück 6 radial kollabierbar sind.
Fig. 2 zeigt eine weitere Ausführungsform der Prothese 1 mit dem Gegenstück 6 in zusammengefügtem Zustand. Die Vertiefung 5 durchdringt die Prothese 1 koaxial vom inneren Ende 7 bis zum ausseren Ende 8. Der Zapfen 16 am Gegenstück 6 wird beim Zusammenfügen von Prothese 1 und Gegenstück 6 in die durchgehende Vertiefung eingeschoben bis das innere Ende 7 der Prothese 1 an den Fortsätzen 3 des Gegenstückes 6 anschlägt. Ferner ist das Gegenstück 6 zwischen dem ausseren Ende 15 und dem inneren Ende 14 mit einer Bohrung 20 durchgehend durchbohrt. Die Kupplungsmittel 11 sind als Schraubverbindung ausgestaltet, wobei die Schraube 21 durch die Vertiefung 5 an der Prothese 1 und durch die Bohrung 20 am Gegenstück 6 vom ausseren Ende 8 der Prothese 1 bis zum ausseren Ende 15 des Gegenstückes 6 durchgeführt wird und mit einer Mutter 22 verriegelbar ist. Zudem ist die Prothese 1 mit einem Hohlraum 12 ausgestaltet, so dass die Hohlraumwände 13 wahlweise kollabierbar oder durch Auffüllung des Hohlraumes 12 mit einem Füllmaterial expandierbar sind.
Die in Fig. 3 dargestellte Ausführungsform unterscheidet sich von der oben beschriebenen Ausführungsformen darin, dass der Zapfen 16 am Gegenstück 6 vollständig durch die Vertiefung 5 an der Prothese 1 durchgeführt wird, so dass das innere Ende 14 des Gegenstückes 6 mit dem ausseren Ende 8 der Prothese 1 fluchtet. Ferner weist das Gegenstück 6 mehrere vom inneren Ende 14 bis zum ausseren Ende 15 durchgehende Bohrungen 20 auf, deren Bohrungsachsen parallel zur Zentralachse 4 verlaufen. Durch diese Bohrungen 20 sind Cerclagedrähte 23 führbar, welche zur zur Fixierung der interspinalen Prothese 1 und des Gegenstückes 6 dienen.
Die in Fig. 4 gezeigte Ausführungsform unterscheidet sich von der in Fig. 1 darge- stellten Ausführungsformen nur dadurch, dass die Kupplungsmittel 11 einen Arretierbolzen 28 umfassen, welcher durch die zur Zentralachse 4 koaxiale, die Prothese 1 und das Gegenstück 6 durchdringende Bohrung 20 durchführbar ist. Der Arretierbolzen 28 ist seinem Kopf 29 am ausseren Ende 15 des Gegenstückes 6 zur Anlage bringbar und weist an seiner Spitze radial elastisch deformierbare Nocken 31 auf, welche bei zusammengefügter Prothese 1 und Gegenstück 6 in eine Hinterdrehung 30, deren Durchmesser grösser als der Durchmesser der Bohrung 20 ist, einrasten können, so dass die Prothese 1 und das Gegenstück 6 zusammengehalten werden. Zur Einführung des Arretierbolzens 28 in die Bohrung 20 lassen sich die Nocken 31 mittels axial angeordneter Schlitze 32 senkrecht zur Zentralachse 4 komprimieren, so dass der Arretierbolzen 28 durch die Bohrung 20 durchführbar ist, während im zusammengefügten Zustand die Nocken 31 elastisch zurückfedern und in die Hinterdrehung 30 an der Prothese 1 einrasten. Der Arretierbolzen 28 ist koaxial zur Zentralachse 4 durchgehend durchbohrt, so dass ein Stift 25 durchführbar ist, wodurch ein radiales Einfedern der Nocken 31 verhindert wird.
In Fig. 5 ist eine weitere Ausführungsform der erfindungsgemässen Prothese 1 mit Gegenstück 6 dargestellt. Am ausseren Ende 8 der Prothese 1 sowie am äussern Ende 15 des Gegenstückes 6 sind wiederum die Fortsätze 3 senkrecht zur Zentralachse 4 und einander diametral gegenüberliegend angebracht, wobei die Fortsätze 3 in dieser Ausführungsform parallel zur Zentralachse 4 eine halbkreisförmige Querschnittsfläche aufweisen. Die Vertiefung 5 durchdringt die Prothese 1 vom inneren Ende 7 bis zum ausseren Ende 8 koaxial zur Zentralachse 4. In der Vertiefung 5 ist ein Innengewinde 36 mit einer sehr grossen Gewindesteigung angebracht. Das Gegenstück 6 hat wiederum an das innere Ende 14 angrenzend einen Zapfen 16, welcher ein zum Innengewinde 36 komplementäres Aussengewinde 33 aufweist, so dass die Prothese 1 und das Gegenstück 6 mittels dieser Gewindeverbindung aneinander lösbar befestigbar sind. Zwischen dem Zapfen 16 und den Fortsätzen 3 ist am Gegenstück 6 eine erste, sägezahnartige Verzahnung 34 angebracht, welche mit einer komplementären, zweiten Verzahnung 35 am inneren Ende 7 der Prothese 1 beim Zusammenfügen der Prothese 1 mit dem Gegenstück 6 in Eingriff bringbar ist, so dass durch die asymmetrische Ausgestaltung der Verzahnungen 34;35 eine Sicherung gegen ein unbeabsichtigtes Lösen von Prothese 1 und Gegenstück 6 gebildet wird.

Claims

Patentansprüche
1. Interspinale Prothese (1 ) mit einem in den Interspinalraum einführbaren Mittelteil (2) mit einer Zentralachse (4), einem inneren Ende (7) und einem ausseren Ende (8) sowie zwei am ausseren Ende (8) entspringenden, radial und diametral zur Zentralachse (4) verlaufenden Fortsätzen (3), welche in den Raum zwischen den Dornfortsätzen zweier benachbarter Wirbelkörper einführbar sind, dadurch gekennzeichnet, dass das Mittelteil (2) an seinem den Fortsätzen (3) abgewandten, inneren Ende (7) eine axiale Vertiefung (5) aufweist, zur Aufnahme eines zur Prothese (1) im wesentlichen symmetrischen Gegenstücks (6).
2. Prothese (1) nach Anspruch 1 , dadurch gekennzeichnet, dass das Mittelteil (2) Kupplungsmittel (11) aufweist, mit welchen das Gegenstück (6) daran fixierbar ist.
3. Prothese (1) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die zur Zentralachse (4) orthogonale Querschnittsebene (9) durch den Mittelteil (2) an seiner engsten Stelle (10) im Bereich des inneren Endes (7) eine Fläche von 50 bis 300 mm2, vorzugsweise von 70 bis 250 mm2 aufweist.
4. Prothese (1 ) nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die zur Zentralachse (4) orthogonale Querschnittsebene durch die Fortsätze (3) eine Fläche von 70 bis 500 mm2, vorzugsweise von 100 bis 450 mm2 aufweist.
5. Prothese (1) nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass sie aus einem elastischen Material gefertigt ist, das eine elastische Deformation des Durchmessers des Mittelteils (2) an seiner engsten Stelle (10) um 10 - 60 %, vorzugsweise von 15 bis 50 % relativ zum unbelasteten Durchmesser erlaubt.
6. Prothese (1) nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass sie aus einem Kunststoff mit der Shorehärte 65A - 90A besteht.
7. Prothese (1 ) nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass sie aus einem metallischen Werkstoff besteht und das Mittelteil (2) radial elastisch deformierbare Zungen umfasst.
8. Prothese (1) nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass sie hohl ausgebildet ist, so dass ihre Hohlraumwände (13) wahlweise kollabierbar oder durch Auffüllung des Hohlraums (12) mit einem Füllmaterial expandierbar sind.
9. Prothese (1) nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass der Mittelteil (2) im Bereich seines inneren Endes (7) an seiner Aussenfläche glatt ausgebildet ist.
10. Prothese (1) nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass der Mittelteil (2) im Bereich seines inneren Endes (7) an seiner Aussenfläche aufgerauht ist.
11. Prothese (1) nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass mindestens auf einem Teil ihrer Oberfläche Hydroxilapatit (HA) in das Prothesenmaterial eingebettet ist.
12. Prothese (1) nach einem der Ansprüche 2 bis 11 , dadurch gekennzeichnet, dass die Kupplungsmittel (11) einen Bajonettverschluss (27) umfassen.
13. Prothese (1) nach einem der Ansprüche 2 bis 11, dadurch gekennzeichnet, dass die Kupplungsmittel (11 ) elastisch deformierbare Nocken (31 ) umfassen, welche nach dem Zusammenfügen von Prothese (1) und Gegenstück (6) an diesem arretierbar sind.
14. Prothese (1) nach Anspruch 13, dadurch gekennzeichnet, dass mindestens die Kupplungsmittel (11 ) koaxial zur Zentralachse (4) mit einer Bohrung (20) versehen sind und dass die Prothese (1) einen Stift umfasst, welcher in die Bohrung (20) einführbar ist.
15. Prothese (1) nach einem der Ansprüche 2 bis 11, dadurch gekennzeichnet, dass die Kupplungsmittel (11 ) eine Konusverbindung umfassen.
16. Prothese (1) nach einem der Ansprüche 2 bis 11 , dadurch gekennzeichnet, dass die Kupplungsmittel (11 ) eine Schraubverbindung umfassen.
17. Prothese (1) nach einem der Ansprüche 13 bis 16, dadurch gekennzeichnet, dass die Prothese (1 ) ein in einem Gegenstück (6) aufnehmbare Verdrehsicherung umfasst.
18. Prothese (1) nach Anspruch 1 , dadurch gekennzeichnet, dass die axiale Vertiefung (5) des Mittelteils (2) als durchgehende, das gesamte Mittelteil durchdringende Bohrung ausgebildet ist.
19. Prothese (1) nach einem der Ansprüche 1 bis 18, dadurch gekennzeichnet, dass die parallel zur Zentralachse (4) betrachteten Querschnittsflächen der Fortsätze (3) Halbkreisflächen sind.
20. Gegenstück (6) für eine Prothese (1) nach einem der Ansprüche 1 bis 19, dadurch gekennzeichnet, dass
A) das Gegenstück (6) ein inneres Ende (14), ein äusseres Ende (15);
B) zwei am ausseren Ende (15) entspringende, radial und diametral zur Zentralachse (4) verlaufende Fortsätze (3), welche in den Raum zwischen den Dornfortsätzen zweier benachbarter Wirbel einführbar sind, und
C) gegen das innere Ende (14) gerichtet, einen Zapfen (16) umfasst, welcher in die Vertiefung (5) an der Prothese (1) einführbar ist.
21. Gegenstück (6) nach Anspruch 20, dadurch gekennzeichnet, dass der Zapfen (16) Kupplungsmittel (11 ) umfasst, mit welchen die Prothese (1) daran fixierbar ist.
22. Gegenstück (6) nach Anspruch 21 , dadurch gekennzeichnet, dass die Kupplungsmittel (11 ) einen Bajonettverschluss umfassen.
23. Gegenstück (6) nach Anspruch 21, dadurch gekennzeichnet, dass die Kupp- lungsmittel (11) elastisch deformierbare Nocken umfassen, welche nach dem Zusammenfügen von Gegenstück (6) und Prothese (1) an diesem arretierbar sind.
24. Gegenstück (6) nach Anspruch 21 , dadurch gekennzeichnet, dass die Kupplungsmittel (11) eine Konusverbindung umfassen.
25. Gegenstück (6) nach Anspruch 21 , dadurch gekennzeichnet, dass die Kupplungsmittel (11) eine Schraubverbindung umfassen.
26. Gegenstück (6) nach einem der Ansprüche 22 bis 24, dadurch gekennzeichnet, dass die Prothese (1) ein in einem Gegenstück (6) aufnehmbare Verdrehsicherung umfasst.
27. Gegenstück (6) nach einem der Ansprüche 20 bis 26, dadurch gekennzeichnet, dass die parallel zur Zentralachse (4) betrachtete Querschnittsfläche eines Fortsatzes (3) Halbkreisflächen sind.
28. Interspinale Prothese (1 ) mit einem Gegenstück (6), dadurch gekennzeichnet, dass sie im zusammengesetzten Zustand eine zur Zentralachse (4) orthogonal stehende Symmetrieebene aufweist.
29. Interspinale Prothese (1) nach einem der Ansprüche 1 bis 19 mit einem Gegenstück (6) nach einem der Ansprüche 20 bis 26.
30. Interspinale Prothese (1) mit einem Gegenstück (6) nach Anspruch 28 oder 29, dadurch gekennzeichnet, dass die Fortsätze (3) der interspinalen Prothese (1) zu denjenigen des Gegenstückes (6) einen Abstand von mindestens 2 mm, vorzugsweise mindestens 3 mm aufweisen.
31. Interspinale Prothese (1) mit einem Gegenstück (6) nach einem der Ansprüche 28 bis 30, dadurch gekennzeichnet, dass die Fortsätze (3) der interspinalen Prothese (1) zu denjenigen des Gegenstückes (6) einen Abstand von höchstens 15 mm, vorzugsweise höchstens 12 mm aufweisen.
32. Interspinale Prothese (1) mit einem Gegenstück (6) nach einem der Ansprüche 28 bis 31 , dadurch gekennzeichnet, dass die interspinale Prothese (1 ) eine erste Verdrehsicherung und das Gegenstück (6) eine zur ersten Verdrehsicherung komplementäre, zweite Verdrehsicherung umfasst, so dass die Fortsätze (3) der interspinalen Prothese (1) und die Fortsätze (3) des Gegenstückes (6) in eine zueinander fluchtende Position bringbar sind.
