Description
- Hintergrund der Erfindung
- 1. Gebiet der Erfindung
- Vorliegende Erfindung bezieht sich auf ein Implantat, um einen Blutfluss direkt zwischen einer Kammer des Herzens und einem Koronargefäß zu leiten. Insbesondere betrifft die Erfindung ein solches Implantat mit einer verbesserten Konstruktion zur Förderung einer geheilten Zellschicht auf einer Innenseite des Implantates.
- 2. Beschreibung des Standes der Technik
- Sowohl das US-Patent Nr. 5,755,682, erteilt am 26. Mai 1998, als auch die noch anhängige US Patentanmeldung Serien Nr. 08/882,397 eingereicht am 25. Juni 1997, mit dem Titel "Verfahren und Gerät zum Ausführen von koronaren chirurgischen Bypasseingriffen" (Englisch: "Method and Apparatus for Performing Coronary Bypass Surgery"), welche auf den vorliegenden Anmelder übertragen und im Namen der Erfinder Mark B. Knudson und William L. Giese eingereicht (veröffentlicht als internationale Anmeldungsveröffentlichung PCT Nr. WO 98/06356) wurden, beschreiben ein Implantat zum Festlegen eines direkten Blutflusskanals von einer Herzkammer zu einem Koronargefäßlumen. Gemäß einer Ausführungsform wird ein L-förmiges Implantat in einer Koronararterie aufgenommen und durch das Myokard geführt, sodass es sich in den linken Ventrikel des Herzens erstreckt. Der Kanal ist starr und ist sowohl während der Systole als auch der Diastole für einen Blutfluss geöffnet, welcher durch den Kanal fließt. Der Kanal dringt in den linken Ventrikel ein, um ein Gewebewachstum und einen Verschluss über eine Öffnung des Kanals hinweg zu verhindern. Das '682 Patent und die '397 Anmeldung beschreiben desweiteren eine Ausführungsform, bei welcher ein Abschnitt des Implantates, welches durch die Herzwand läuft, ein offenes Strukturelement ist, das mit Polyester (z.B. Dakron) ausgekleidet ist. Eine weitere Ausführungsform beschreibt einen Abschnitt des Implantates in einem Koronargefäß als einen ballonexpandierbaren Stent mit offenen Zellen.
- Das US-Patent Nr. 5,429,144 von Wilk, datiert 4. Juli 1995, beschreibt die Lehre von Implantaten, welche durch die Vaskulatur in einem zusammengefalteten Zustand geführt werden und welche expandieren, wenn sie im Myokard platziert werden, sodass sie sich weder in die Koronararterie noch in den linken Ventrikel erstrecken. Die beschriebenen Implantate schließen sich einmal pro Herzzyklus (z.B. während der Diastole in den Ausführungsformen gemäß
7A und7B oder während der Systole in den Ausführungsformen gemäß2A und2B ). Jede dieser beiden Konstruktionen kann mit einem Transplantat ausgekleidet sein. - Die internationale PCT-Anmeldungsveröffentlichung Nr. WO 98/08456, welche den nächstgelegenen Stand der Technik darstellt, beschreibt einen vorstehenden Stent, welcher eine Durchführung vom Herzen zu einem Koronargefäß bildet. Der Stent wird als ein Drahtgitter oder anderes Metall- oder Polymerelement beschrieben und kann selbstexpandierend oder druckexpandierend sein. Die Anmeldung beschreibt, dass der Stent mit einer teilweisen oder vollständigen rohr-förmigen Abdeckung bedeckt sein kann, welche aus einem Material besteht, das Polyester, gewobenen Polyester, Polytetrafluorethylen, expandiertes Polytetrafluorethylen, Polyurethan, Silikon, Polykarbonat, autologes Gewebe und Xenotransplantatgewebe umfasst.
- Ein wichtiges Konstruktionsmerkmal ist die Biokompatibilität. Implantate aus festem Metall werden aus einem Werkstoff (z.B. Titan oder pyrolytischem Kohlenstoff) mit niedrigen Blutpfropf- und Plättchenaktivierungsvorfällen ausgebildet. Obwohl solche Werkstoffe durch den Gebrauch bei einer großen Anzahl Produkte (z.B. Herzventilkomponenten) erprobt sind, ermöglichen diese keine vollständige Heilung. Mit "Heilung" ist das Zellwachstum des Patienten im Verlauf der Zeit über das Implantatmaterial gemeint, sodass das Blut, welches durch das Implantat fließt, nur (oder wenigstens hauptsächlich) den Zellen des Patienten ausgesetzt ist, anstatt einem Fremdkörpermaterial.
- Zusammenfassung der Erfindung
- Gemäß einer bevorzugten Ausführungsform der vorliegenden Erfindung wird ein Transmyokardimplantat zum Festlegen einer Blutflussbahn durch ein Myokard zwischen einer Herzkammer und einem Koronargefäßlumen, welches sich auf der Außenseite des Herzens befindet, beschrieben. Das Implantat umfasst einen Koronarabschnitt, welcher dimensioniert ist, um innerhalb des Gefäßes aufgenommen zu werden. Ein Myokardabschnitt ist dimensioniert, um durch das Myokard in die Herzkammer zu gelangen. Ein Übergangsabschnitt verbindet den Koronar- und den Myokardabschnitt, um Blutfluss vom Myokardabschnitt in den Koronarabschnitt zu leiten. Der Koronarabschnitt und der Myokardabschnitt haben eine offene Konstruktion, um ein Gewebewachstum über eine Wandstärke des Koronarabschnittes und des Myokardabschnittes zu ermöglichen. Der Myokardabschnitt weist ein Mittel zur Steuerung der Koagulationskaskade und Plättchenaktivierung auf, und fördert die Heilung. Eine poröse Auskleidung im Myokardabschnitt wird bereitgestellt, wobei die poröse Auskleidung Poren aufweist, welche kleiner sind als Öffnungen der offenen Konstruktion des Myokardabschnittes. Die poröse Auskleidung deckt wenigstens die offene Konstruktion des Myokardabschnittes ab, jedoch nicht die offene Konstruktion des Koronarabschnittes.

6 ist eine Seitenschnittansicht über ein expandierbares Implantat mit einem Ballonkatheter, der durch einen Myokardabschnitt des Katheters herausnehmbar ist. - Beschreibung der bevorzugenden Ausführungsform
- Mit anfänglichem Verweis auf
1 wird ein Kanal10 in Form einer L-förmigen Röhre gezeigt. Der Kanal10 kann aus Titan oder anderen biokompatiblen Werkstoffen bestehen. Der Werkstoff für den Kanal10 ist vorzugsweise ein radial starrer Werkstoff, um Kontraktionskräften des Myokards zu widerstehen. Gemäß einem nicht begrenzenden Beispiel hat die Röhre einen Außendiameter D0 von ungefähr 3 Millimetern und einen Innendiameter D1 von ungefähr 2,5 Millimetern, womit sich eine Wandstärke von ungefähr 0,25 Millimetern ergibt. - Die Röhre
10 weist einen Koronarabschnitt12 auf, welcher dimensioniert ist, um innerhalb des Lumens eines Koronargefäßes, wie beispielsweise das Lumen80 der Koronararterie82 distal einer Verstopfung81 , wie in1 illustriert, aufgenommen zu werden. Der Kanal10 weist einen Myokardabschnitt14 auf, welcher sich im rechten Winkel zur Achse des Abschnittes12 erstreckt. Der Myokardabschnitt14 ist so dimensioniert, dass er sich von der Koronararterie82 direkt durch das Myokard84 erstreckt und in den linken Ventrikel83 des Patientenherzens vorsteht. - Der Koronarabschnitt
12 hat eine erste Öffnung16 . Der Myokardabschnitt14 hat eine zweite Öffnung18 in Verbindung mit einem Inneren20 des Implantates10 . Somit kann Blut ungehindert durch das Implantat10 zwischen dem linken Ventrikel83 und dem Lumen80 der Koronararterie82 fließen. Blut fließt axial aus der Öffnung16 parallel mit der Achse des Lumens80 . - Die Längsachse des Koronarabschnittes
12 ist mit der Achse des Lumens80 ausgerichtet. Nähte24 befestigen die Arterie82 am Koronarabschnitt12 . Der Proximalabschnitt82a der Koronararterie ist mithilfe von Nähten85 verbunden. - Die Koronar- und Myokardabschnitte
12 ,14 haben eine offene Gitterkonstruktion12a ,14a , welche eine Vielzahl offener Zellen12b ,14b schaffen, die sich über die Wandstärke des Implantates10 erstrecken. Vorzugsweise sind die Koronar- und Myokardabschnitte12 ,14 mit einem Übergangsabschnitt13 in einer 90°-Biegung zwischen den Abschnitten12 ,14 verbunden. Während der Übergangsabschnitt13 eine offene Gitterkonstruktion aufweisen kann, wie Abschnitte12 ,14 , weist der Übergangsabschnitt13 vorzugsweise kleinere offene Flächen in einer solchen offenen Konstruktion auf, oder wird aus einer ununterbrochenen Konstruktion bestehen, wie illustriert. Solch eine Konstruktion erlaubt es, dem Übergangsabschnitt Blutflüsse mit hohen Geschwindigkeiten vom Myokardabschnitt14 zum Koronarabschnitt12 überzuleiten. Eine Gitterkonstruktion mit großen offenen Zellen im Übergangsabschnitt könnte dazu führen das Hochgeschwindigkeitsflüsse Gewebe, welches den Übergangsabschnitt bedeckt (nicht gezeigt), beschädigen. - Ein beliebiger oder sämtliche der Koronarabschnitte
12 , Übergangsabschnitte13 und Myokardabschnitte14 können in einer endgültigen Größe als starre Einheiten ausgebildet sein, oder sie könnten auch mit geringen Diametern ausgebildet sein, welche danach zu voller Größe expandieren. Z.B. illustriert3 einen Koronarabschnitt12' , welcher sich vom Übergangsabschnitt13' auf ein offenes Ende16' mit vermindertem Diameter verjüngt. Die Verjüngung erlaubt ein leichtes Einführen in eine Koronararterie. Nach einem solchen Einführen kann der verjüngte Koronarabschnitt12' zu voller Größe expandiert werden, wie durch die gestrichelten Linien in3 illustriert wird. Eine solche Expansion kann mithilfe von Kathetern mit Ballonspitzen ausgeführt werden, wie dies für Stentangioplastiken üblich ist. Ein zusammengefaltetes und danach expandiertes Implantat10 , bei welchem sämtliche Abschnitte12 ,13 , und14 expandiert sind, erlauben die Anwendung als ein perkutan eingesetztes Implantat. Die vorliegenden Zeichnungen illustrieren ein momentan bevorzugtes chirurgisch eingesetztes Implantat. Bei der chirurgischen Anwendung ist die Arterie82 mit einer Ligatur versehen. Das Implantat10 ist durch das Epikard und Myokard auf einer Seite der Arterie82 geführt. -
5 illustriert einen Ballon100 , welcher in einem verjüngten Koronarabschnitt12 positioniert ist. Eine Leitung102 vom Ballon100 ist durch eine Öffnung113' im Übergangsabschnitt13' geführt. Die Öffnung113' kann mithilfe eines Verschlusses115' verschlossen werden, nachdem der Ballon100 und die Leitung102 durch die Öffnung113' zurückgezogen sind. Als Alternative kann die Ballonleitung bei einem Übergangsabschnitt13'' mit offener Zellkonstruktion (4 ) durch die Öffnungen des Übergangsabschnittes113'' geführt werden.6 illustriert, wie die Leitung102 durch eine Öffnung18 des Myokardabschnittes geführt wird. Die Leitung102 kann von außerhalb des Herzens nach Oben gezogen werden, um den Ballon100 zu entfernen. Als Alternative kann die Leitung102 durch einen Katheter (nicht gezeigt) am angrenzenden Ende18 in den linken Ventrikel gezogen werden. - Sowohl bei perkutanen als auch chirurgischen Implantaten erlaubt ein flexibler Übergangsabschnitt
13 (wie es mit einer Stent-Gitterkonstruktion erreicht werden würde) eine relative Gelenkbewegung zwischen den Koronar- und Myokardabschnitten12 ,14 , um sicherzustellen, dass der Koronarabschnitt axial mit dem Lumen80 ausgerichtet ist. Fehlen solche Gelenkverbindungen, kann eine solche axiale Ausrichtung dadurch erreicht werden, dass die Position des Myokardabschnittes14 genau gesteuert wird, sodass der Koronarabschnitt12 mit dem Lumen80 nach der Implantierung axial ausgerichtet ist. - Die offene Zellkonstruktion der Koronar- und Myokardabschnitte
12 ,14 erlaubt ein Gewebewachstum durch die offenen Zellen12c ,14c nach der Implantierung. Der Heilungsprozess im Koronarabschnitt12 ist der gleiche wie bei koronaren Stents. Endothelgefäßzellen wachsen über das Strukturmaterial12a des Abschnittes12 und bedecken es. - In Abschnitt
14 wird Myokardgewebe, falls es nicht gehindert wird, durch die Zellen14c wachsen. Des Weiteren ist das Myokard hochgradig thrombogen. Deswegen kann ein unkontrollierter Kontakt zwischen dem Myokard82 und dem Inneren20 des Implantates eine Thrombose des Implantates10 zur Folge haben. Des Weiteren glaubt man, dass das Epikard (d.h. die äußere Schicht des Myokards) eine größere Dichte an Myokard-Wachstumszellen aufweist, welches zur Heilung beiträgt. - Um das Wachstum im Myokardabschnitt
14 zu steuern, wird eine Auskleidung30 im Myokardabschnitt14 angeordnet. Die Auskleidung30 besteht aus einem beliebigen porösen Material um Gewebewachstum entgegen zu nehmen und ist vorzugsweise ein Polyesterstoff (z.B. Dakron). Die poröse Auskleidung30 weist Zwischenräume auf, die kleiner sind als die offenen Zellen12c ,14c . Die Auskleidung30 wird auf der Innenseite des Myokardabschnittes14 gezeigt, könnte jedoch auch oder als Alternative die Außenseite umgeben. - Die Auskleidung
30 hat ein oberes Ende32 , welches mithilfe geeigneter Mittel (z.B. Nähte, nicht gezeigt) an das obere Ende des Myokardabschnittes14 befestigt ist. Ein unteres Ende34 ist über die Öffnung des Myokardabschnittes14 gefaltet und mithilfe von Nähten36 an das Äußere des Abschnittes14 befestigt. Der Myokardabschnitt14 ist so dimensioniert, dass er nur mit dem übergefalteten Auskleidungsmaterial, welches dem Inneren des linken Ventrikels83 freigelegt ist, hervorsteht. - Die Auskleidung
30 wirkt als ein poröses Substrat, in welches Gewebe einwachsen kann. Um Thrombosen zu verhindern, ist die Auskleidung30 mit einem Mittel zum Steuern der Koagulationskaskade und Plättchenaktivierung und Adhäsion imprägniert. Ein Beispiel für solch ein Mittel ist Heparin, könnte jedoch auch ein beliebiges anderes Anti-Koagulierungsmittel oder Anti-Plättchenmittel sein. Auch ein Mittel, wie z.B. ein grundlegender Fibroblastwachstumsfaktor könnte verwendet werden, um die Heilung zu beschleunigen. - Das Mittel ermöglicht es, dass Strukturzellen auf der Auskleidung, dank einer Begrenzung der Thrombenbildung, welche das Implantat im ungesteuerten Fall verschließen würde, wachsen. Dank der offenen Konstruktion können die strukturellen Heilungszellen des Epikards auf der Auskleidung wachsen. Danach können Endothelzellen auf den Strukturzellen wachsen.
- Folglich fördert die beschriebene Struktur einen Heilungsprozess in drei Schritten:
- 1. Medikamentenmittel steuern die Heilung dadurch, dass Koagulation und Plättchenaktivierung minimiert werden, welches ansonsten durch Mittel vom Myokard stimuliert werden würde; und
- 2. Strukturzellen wachsen in und auf die Auskleidung
30 , welche jetzt mit dem Thrombus ausgekleidet ist, um die Heilung zu beginnen, und um ein Gefäßbett zu bilden; und - 3. Endothelzellen wachsen über die Strukturzellen.
- Im Übergangsabschnitt
13 erlaubt eine offene Zellstruktur ein Gewebewachstum wie beim Koronarabschnitt12 . Solch ein Wachstum kann auch bei der festen Konstruktion vorkommen. Als Alternative kann sich die Auskleidung30 in den Übergangsabschnitt13 erstrecken. Zusätzlich kann die offene Zellstruktur im Übergangsabschnitt13 eine Bewegung zwischen dem Koronarabschnitt und dem Myokardabschnitt erlauben. Solch eine Struktur wird in4 gezeigt. Der offene Übergangsabschnitt13'' wird durch eine Spule13a'' zwischen dem Koronarabschnitt12'' und dem Myokardabschnitt14'' gebildet. Diese Struktur erlaubt eine Biegung am Übergangsabschnitt. Als ein Resultat kann der Koronarabschnitt axial in der Arterie ausgerichtet werden, ohne zuerst den Myokardabschnitt genau zu positionieren. - Während die vorliegende Erfindung anhand einer bevorzugenden Ausführungsform beschrieben wurde, versteht man, dass sich Änderungen und Äquivalente für den Fachmann auf dem Gebiet ergeben, welche die Vorteile der vorliegenden Erfindung haben. Es ist beabsichtigt, dass solche Modifikationen innerhalb des Umfanges der nachfolgend beigefügten Ansprüche einbegriffen sind. Z.B. kann die Auskleidung
30 viele Konstruktionen verwenden, welche PTFE, expandiertes PTFE, Polyurethan, Polypropylen oder andere biologisch kompatible Abdeckmaterialien oder natürliches Gewebe umfassen. Des Weiteren kann eine Restenose des Koronarabschnittes12 mithilfe von radioaktiver Therapie verhindert werden (z.B. kann der Koronarabschnitt mit einem Betastrahler mit kurzer Halbwertszeit versehen werden). Die Auskleidung30 kann auch entweder aus einem resorbierbaren oder nicht-resorbierbaren Material bestehen. Genmanipulierte Zellen können umgewandelt werden, um Antikoagulationsmittel und andere Mittel, welche das Blut flüssig halten, abzusondern (wie z.B. Gewebeplasminogenaktivator und glatte Muskelzellen, welche abgeändert werden, um Salpetersäure abzuäußern).
Claims (13)
- Transmyokardimplantat (
10 ) zum Festlegen einer direkten Blutflussbahn von einem linken Ventrikel (83 ) zu einem Koronargefäß (82 ), das Implantat (10 ) umfassend: einen Koronarabschnitt (12 ), welcher dimensioniert ist, um innerhalb des Gefäßes (82 ) aufgenommen zu werden; einen Myokardabschnitt (14 ), welcher dimensioniert ist, um durch das Myokard in den linken Ventrikel (83 ) zu gelangen; einen Übergangsabschnitt (13 ), welcher den Koronarabschnitt (12 ) mit dem Myokardabschnitt (14 ) verbindet; wobei wenigstens der Koronarabschnitt (12 ) und der Myokardabschnitt (14 ) eine offene Konstruktion (12a ,14a ) aufweisen, um ein Gewebewachstum über eine Wandstärke des Koronarabschnittes (12 ) und des Myokardabschnittes (14 ) zu ermöglichen; eine poröse Auskleidung (30 ) im Myokardabschnitt (14 ), wobei die poröse Auskleidung Poren umfasst, welche kleiner sind, als Öffnungen (14b ) der offenen Konstruktion (14a ) des Myokardabschnittes (14 ); dadurch gekennzeichnet, dass wenigstens der Myokardabschnitt (14 ) ein Mittel beinhaltet, welches eine Koagulationskaskade und Plättchenaktivierung steuert, und wobei die poröse Auskleidung (30 ) wenigstens die offene Konstruktion (14a ) des Myokardabschnittes (14 ), aber nicht die offene Konstruktion des Koronarabschnittes (12 ) abdeckt. - Implantat (
10 ) gemäß Anspruch 1, welches des Weiteren ein Mittel zur Heilungsbegünstigung umfasst. - Implantat (
10 ) gemäß Anspruch 1, wobei die poröse Auskleidung (30 ) aus einem Polyestergewebe hergestellt ist. - Implantat (
10 ) gemäß Anspruch 1, wobei die poröse Auskleidung (30 ) das Mittel beinhaltet. - Implantat (
10 ) gemäß Anspruch 1, wobei das Mittel Heparin ist. - Implantat (
10 ) gemäß Anspruch 1, wobei das Mittel eine Antikoagulanssubstanz ist. - Implantat (
10 ) gemäß Anspruch 1, wobei das Mittel eine Anti-Plättchen Substanz ist. - Implantat (
10 ) gemäß Anspruch 2, wobei das Mittel zur Heilungsbegünstigung ein Wachstumsfaktor ist. - Implantat (
10 ) gemäß Anspruch 1, wobei der Koronarabschnitt (12 ) von einem ersten Diameter (D0) auf einen zweiten vergrößerten Diameter (D1) erweiterbar ist. - Implantat (
10 ) gemäß Anspruch 1, wobei der Myokardabschnitt (14 ) von einem ersten Diameter auf einen zweiten vergrößerten Diameter erweiterbar ist. - Implantat (
10 ) gemäß Anspruch 1, wobei der Übergangsabschnitt (13 ) eine Bewegung zwischen dem Koronarabschnitt (12 ) und dem Myokardabschnitt (14 ) gestattet. - Implantat (
10 ) gemäß Anspruch 1, wobei die poröse Auskleidung (30 ) auf dem inneren Teil der offenen Konstruktion (14a ) des Myokardabschnittes (14 ) hergestellt ist. - Implantat (
10 ) gemäß Anspruch 1, wobei der Übergangsabschnitt (13 ) einen festen Aufbau hat.
