- Die vorliegende Erfindung bezieht sich auf ein Implantat, das zwischen zwei sich gegenüber liegenden Wirbelkörpern eingesetzt werden kann, um die Wirbelsäule zu stabilisieren. Im Besonderen dient die vorliegende Erfindung dazu, einen Verbindungskäfig oder -körper zur Verfügung zu stellen, der chirurgisch implantiert wird und benachbarte Wirbelkörper der Wirbelsäule stabilisiert, während dazwischen ein Knochenwachstum möglich ist.
- Hintergrund der Erfindung
- Fusionskäfige oder innerkörperliche Fusionsvorrichtungen stellen Mittel zum Offenhalten der kollabierten Bandscheibenzwischenräume zwischen zwei sich gegenüber liegenden Wirbelkörpern der Bandscheibe zur Verfügung, um die Nerven und/oder das Rückenmark von Druck zu befreien und durch das Einbringen eines Platzhalters oder Platzhaltern in dem Bandscheibenzwischenraum zu erhalten.
- Die gegenwärtig verwendeten Fusionskäfige sind entweder zylindrische oder rechtwinklige Vorrichtungen, die einen externen eingezogenen oder verzahnten Teil zum Verbinden mit den Wirbelkörperendplatten aufweisen, um ein Verrutschen des Käfigs zu verhindern. Derartige Käfige sind grundsätzlich hohl ausgebildet. Sie können mit dem Transplantat gefüllt sein, um das Zusammenwachsen von zwei Wirbelkörpern einzuleiten. Derartige Vorrichtungen stellen ein großes Potential für das Vermeiden von umfangreichen Einschnitten späterer Instrumentation und öffnen die Tür für die minimal invasive Chirurgie.
- Die Kunst des spinalen Implantierens, wie es die Fusionskäfige sind, weist einen hohen Entwicklungsstandard auf. Neueste Entwicklungen führten zu verschiedenen Aspekten des Käfigdesigns. Beispielsweise das US Patent 5,055,104 nach Ray, veröffentlicht am 9. Oktober 1991 stellt ein Verfahren für das chirurgische Implantieren von Gewindefusionskäfigen zwischen benachbarten Lendenwirbeln durch einen älteren Ansatz dar. Die Vorrichtung offenbart darin das Bestehen formstabilen, soliden Helix aus Draht, die hohl ausgebildet ist und Öffnungen zwischen den benachbarten Biegungen des Käfigs aufweist.
- Das US Patent 4,011,602 nach Rybicki und anderen offenbart eine ausziehbare Vorrichtung zum Anlegen an einem Knochengewebe. Die Vorrichtung umfasst einen einstellbaren Mechanismus für das Ausdehnen eines Körperteils gegen das Gewebe, in welches es implantiert ist, um dieses auszubauchen, um bei einer Druck beanspruchenden Belastung geeignet zu sein, um einen geschützten und stabilen Sitz zu erhalten. Jedoch einmal justiert ist die Vorrichtung fest und fixiert genau wie die Vorrichtung, die im oben genannten Ray Patent offenbart ist.
- Das US Patent 5,489,308 nach Kuslich und anderen, veröffentlicht am 6. Februar 1996, stellt ebenfalls ein spinales Implantat zu Verwendung in der spinalen Stabilisation zur Verfügung. Das Implantat umfasst durchgehende Löcher, die so positioniert sind, dass abgespaltene Knochen in das Implantat eindringen können, sobald das Implantat gedreht wird.
- Jedes der vorgenannten Patente, genauso wie viele andere Patente nach dem Stand der Technik, richten sich nach einem alten Ansatz, nämlich Expansion und anschließende Fixierung und Mittel für die automatische Positionierung der Knochensplitter in den Hohlräumen, um eine Verwachsung zu erreichen. Jedoch verbleibt eine Vielzahl von Möglichkeiten mit einer größtmöglichen Wichtigkeit, die nicht durch den Stand der Technik offenbart sind.
- Beispielsweise sind die Fusionsvorrichtungen, die gegenwärtig verwendet werden, zylindrische oder rechtwinklige Vorrichtungen, die groß und unhandlich sind. Die Größe aber ist ein äußerst wichtiger Faktor in der minimal invasiven Chirurgie. Die Vorrichtung wird in den Körper eingebracht, wobei hier eine Kanüle verwendet wird, um die Einschnittgröße und damit die Genesungszeit zu minimieren. Für diesen Vorgang ist es wünschenswert, einen Einschnitt zu verwenden, der so klein wie möglich ist. Eine Kanüle wird verwendet, um das Implantat bis zu dem gewünschten Ziel zu führen. Das Implantat weist dabei eine kleinere Größe auf als der innere Kern der Kanüle, um das Einbringen dadurch zu ermöglichen. Derzeitige Spinalimplantate benötigen einen großen Schnitt aufgrund ihrer großen und unhandlichen Ausgestaltung, die durch ihre Struktur bedingt ist, sind sie ungeeignet für die minimal invasive Chirurgie.
- Ein anderes Problem bei den gegenwärtig verwendeten Implantatkäfig ist das völlige Fehlen des Verständnisses der Beanspruchungsverteilung nach dem Wolf'schen Gesetz und der Fusionsqualität. Die oben zitierten Patente, genauso wie andere, stellen eine feststehende Vorrichtung zur Verfügung, die zwischen den Wirbelkörperendplatten und dem aufgefüllten Transplantationsmaterial eingefügt ist. Es wurde bisher vermutet, dass, wenn das Transplantat genau angeordnet ist, es schon zu einer Verschmelzung kommen wird. Jedoch hängt die Knochenqualität von der Belastung ab. Das Wolf'sche Gesetz vermerkt, dass das Knochenwachstum entlang von Stresslinien erfolgt. Die vorgenannten Käfige des Stands der Technik halten die Wirbelkörper von einander getrennt und agieren mehrheitlich als ein massiver Platzhalter. Daher wird das Transplantationsmaterial innerhalb des Käfigs resorbiert und es wird niemals einer Belastung ausgesetzt sein. Aber ohne eine Belastung wird das Transplantationsmaterial innerhalb des Käfigs keine effektive Qualität der Verschmelzung erreichen. Wenn eine Verschmelzung stattgefunden hat, was schwer genug zu bestimmen ist, am ehesten noch in Metallkäfigen, diagnostiziert durch das verwenden von Röntgenstrahlung oder andere diagnostische Techniken, weist die Qualität der Verschmelzung bestenfalls eine schlechte Qualität auf.
- Zweitens, wie oben bereits erwähnt, ist die Größe des Käfigs eine sehr wichtige Frage. Die zukünftige spinale Chirurgie wird auf einer minimal invasiven, chirurgischen Technik basieren, so wie sie oben diskutiert worden ist. Der Chirurg implantiert den Käfig oder die Kä fige in die Bandscheibe indem er eine Kanüle verwendet. Dies ermöglicht einen kleinen Hauteinschnitt und verringert in dem Weichgewebe Traumata und verringert die Genesungszeit und den Krankenhausaufenthalt. Um den Druck an den Nerven und dem Rückenmark richtig abzubauen, muss der eingefallene Bandscheibenraum geöffnet werden und anschließend in seinem vor dem kollabierten Zustand zurückgeführt und geschlossen werden. Mit den vorgenannten gegenwärtigen Käfigdesigns kann der Bandscheibenraum auf 16 mm geöffnet werden. Derartige Käfige benötigen eine Kanüle von 18 bis 20 mm. Eine derartige Kanüle benötigt einen großen Einschnitt. Es wäre daher wünschenswert ein Mittel zur Verfügung zu stellen, dass es einem 16 mm Käfig ermöglicht mit einer Kanüle, die kleiner als 12 mm oder 14 mm ist.
- Es ist ferner wünschenswert, es dem Käfig zu ermöglichen, die Beanspruchung mit den ihn umgebenden Knochen zu teilen. Eine derartige Dynamik würde es dem Käfig ermöglichen, nicht als erforderlicher Platzhalter zu agieren, sondern es dem Knochen auch zu ermöglichen, Belastungen ausgesetzt zu werden und so die Verschmelzung zu verbessern und so auch den Heilungserfolg und die Verschmelzungsqualität.
- Es wäre des Weiteren wünschenswert den Chirurgen mit einem Mittel zu versehen, das es ihm ermöglicht den Bandscheibenraum leicht zu öffnen. Gegenwärtige Vorrichtungen benötigen eine Instrumentarisierung, um den Bandscheibenraum zu öffnen, diese ist häufig ineffektiv oder schwierig durchzuführen.
- Zusammenfassung der Erfindung
- In Übereinstimmung mit der vorliegenden Erfindung wird ein Fusionsimplantat zur Verfügung gestellt, das zwi schen sich gegenüber liegenden Wirbelkörpern eingefügt werden kann. Das Implantat umfasst einen Körper einschließlich einer inneren Oberfläche, die eine innere hohle Kammer bildet, die darin Transplantationsmaterial aufnehmen kann. Der Körper umfasst des Weiteren Übertragungsmittel für eine Belastung, die die Belastungen von den Wirbelkörpern, die das Implantat umgeben, bis hin zu dem Transplantationsmaterial, das in dem vorgenannten Implantat enthalten ist, übertragen.
- Die vorliegende Erfindung sieht ferner ein Fusionsimplantat vor, das einen Körperteil aufweist, der aus einem hohlen Gehäuse besteht und eine zentrale Längsachse aufweist. Das Gehäuse umfasst wenigstens einen flexiblen Körperteil, um eine Kontraktion und Expansion um die Achse zu ermöglichen.
- Das Fusionsimplantat kann zwischen zwei sich gegenüber liegenden Wirbelkörpern implantiert werden, indem das Implantat in dem zusammengefallenen Bandscheibenraum zwischen zwei benachbarten Wirbelkörpern eingeführt wird. Das Implantat umfasst einen kollabierten Körper. Der Körper wird dann ausgeweitet, um die benachbarten Wirbelkörper auseinander zu spreizen.

- Ein Fusionsimplantat zum Einsetzen zwischen zwei sich gegenüber liegenden Wirbelkörpern ist grundsätzlich als
10 in den Zeichnungen dargestellt. Die einzelnen Ausführungsformen der vorliegenden Erfindung werden durch Strichnummern angezeigt. Diese zeigen gleiche Teile zwischen den einzelnen Ausführungsformen der vorliegenden Erfindung an. - Gewöhnlich umfasst das Implantat
10 einen Körperteil der generell mit12 bezeichnet wird. Der Körperteil12 kann aus verschiedenen Materialien hergestellt werden. Hierfür eignen sich Kompositmaterialien aus zwei oder mehr Metallen, Kunststoffen oder anderen Materialien, die aus dem Stand der Technik gut bekannt sind. derartige Vorrichtungen können entweder gefräst oder gegossen werden, dies hängt davon ab, aus welchem Material das Implantat10 hergestellt ist. Beispielsweise kann das Implantat10 aus einem Metall bestehen, wobei es federnde Eigenschaften, wie sie weiter unten beschrie ben werden, aufweisen kann. Das Metall kann geformt sein und anschließend gefräst werden, um die gewünschte Form und die erforderlichen Toleranzen, wie weiter unten beschrieben, zu erhalten. Wahlweise kann der Körper12 auch aus einem Kunststoffmaterial oder einem Kompositmaterial hergestellt sein. Dieser kann in die jeweils gewünschte Form geschmolzen werden. - Der Körper
12 umfasst eine externe Oberfläche14 und eine interne Oberfläche16 . Die interne Oberfläche16 bestimmt eine innere Kammer18 , die das Transplantationsmaterial (nicht dargestellt) darin aufnimmt. Derartig gestaltete Käfige sind grundsätzlich aus dem Stand der Technik bekannt. Derartige Gestaltungen sind exemplarisch durch die US Patente 5055104, 4011602 und 5489308, wie oben besprochen, offen gelegt. Die externe Oberfläche kann eine offenporige Ummantelung umfassen, wie aus dem Stand der Technik bekannt, um das Wachstum von Knochensubstanz zu ermöglichen. Die externe Oberfläche kann ansonsten strukturiert sein, um die Knochenfixierung zu verbessern. - Um die Möglichkeit einer Verschmelzung zu verbessern, werden die unterschiedlichsten Materialien in die Innenseite der Käfige eingefügt. Jedes ausgewählte Material basiert auf der Erhältlichkeit und der besten Möglichkeit eine gute Verschmelzung zu gewährleisten. Dies umfasst autogenes Transplantat (oder Material, das während des chirurgischen Eingriffs von dem Patienten entnommen worden ist), alogenes Transplantat, wie Knochen, die von einem anderen Patienten entfernt worden sind, das genauso wie alogenes Transplantat bearbeitet wird, das in kleinen Stückchen vorliegt und in einer Vielzahl von Wegen behandelt wird. Mit dem Wunsch nach einer Reduzierung der Verschmelzungszeit können biologisch ak tive Materialien stattdessen verwendet werden. Dies umfasst eine Knochenbehandlung mit morphogenen Proteinen, Kalziumsulfaten und anderen künstlichen Knochenaustauschstoffen.
- Ein besonderes Merkmal der vorliegenden Erfindung ist, dass der Körper
12 Belastungsübertragungsmittel umfasst, um die Belastung von den Wirbelkörpern, die das Implantat10 umgeben zu dem Transplantationsmaterial, das sich innerhalb des Transplantats10 befindet, zu übertragen. Das es ungleichartig den starren Käfigen aus dem bekannten Stand der Technik die mehrheitlich als Platzhalter agieren. Die vorliegende Erfindung sieht eine Konstruktion des Käfigkörpers vor, die sich die Beanspruchung mit den sie umgebenden Wirbelkörpern teilt. Dies wird durch wenigstens einen Teil des Körpers12 erreicht, der flexibel ausgebildet ist und der auch so ausgelegt ist, dass er die Belastung der ihn umgebenden Wirbelkörper verwendet werden kann. Demnach umfasst die vorliegende Erfindung Mittel zur Übertragung der Belastung mit wenigstens einem flexiblen Teil am Körper12 , der entweder Belastungen ablenken kann oder andererseits die Belastung, die durch den umgebenden Wirbelkörper hervorgerufen wird, auf das in ihm enthaltene Transplantationsmaterial innerhalb der inneren hohlen Kammer18 zu übertragen. - Eine erste Ausführungsform der Belastungsübertragungsmittel der vorliegenden Erfindung ist in
1 dargestellt. Der Körper umfasst einen oberen Teil20 und einen unteren Teil22 , um in Wirkverbindung mit den oberen und den unteren Enden der Wirbelkörperplatten zu treten, nachdem die Vorrichtung angrenzend hierzu eingebracht worden ist. Der obere und untere Teil20 ,22 sind ablenkbar und können daher eine Belastung von den angrenzenden Wirbelkörpern zu dem Transplantationsmaterial, das innerhalb der hohlen Kammer18 liegt, ablenken. Der obere und der untere Teil20 ,22 sind federartig ausgebildet und zwar in einer axialen Richtung relativ zu der Längsachse24 , die durch die hohle innere Kammer18 bestimmt wird und die ein Mittel zur nach außen führenden Flexation des Käfigs bildet. Der Körper12 kann eine einheitliche Dicke aufweisen oder er kann aber auch unterschiedliche Dicken an wenigstens einem Ort aufweisen. Der Körper kann relativ dünne Seitenwandteile26 ,28 aufweisen, die relativ zu dem Rest des Körpers12 flexibel ausgebildet sind. Die Körperseitenteile26 ,28 sind in der1 als sich gegenüber liegende Körperteile dargestellt. Derartige Körperseitenteile26 ,28 können ebenfalls eine Belastung ablenken, sobald eine Belastung auf den Körper12 und den ihn umgebenden Wirbelkörper auftritt und so die Belastung in die inneren hohle Kammer18 und das darin enthaltene Transplantationsmaterial abzuleiten. So kann die Wandstärke über den gesamten Käfig wie gewünscht sich ändern. - Das Implantat
10 der vorliegenden Erfindung kann so hergestellt sein, dass es den Fachmann mit einer Serie von Implantaten versorgt, die eine wechselnde Wandstärke aufweisen, nebenbei den Fachmann noch mit einer wechselnden Größe und einer wechselnden Flexibilität versorgen. Der obere und der untere Teil20 ,22 oder die Seitenteile26 ,28 oder eine Kombination hiervon kann auf das Gewicht des Patienten abgestimmt werden, indem die Wandstärke auf den Patienten abstimmbar ist. Größere Patienten, die eine größere Belastung auf ein Implantat ausüben, können mit einem Implantat10 versehen werden, das dickere Seitenteile aufweist, das weiterhin flexible Eigenschaften aufweist und für die Be lastung angepasst ist. Gleichermaßen gibt es Patienten mit einem kleineren Körperbau, diese haben auch ein geringeres Körpergewicht, diese können mit einem Implantat10 versehen werden, das eine dünnere Körperwandstärke aufweist und die eine Biegsamkeit unter einer geringeren Belastung erlaubt. - Die
2 bis5 veranschaulichen unterschiedliche Unterarten der vorliegenden Erfindung, wobei der Körper12' bis12'''' einen zusammenziehbaren Körperteil umfasst. Mehr noch als in der Ausführungsform nach2 dargestellt, umfasst der Körper12' ein erstes und ein zweites offenes Ende30 ,32 , die die Längsachse24' dazwischen definiert. Ein Teilstück des Körpers12' erstreckt sich über die Längsachse24' und zwischen den offenen Enden30 ,32 . In diesem Ausführungsbeispiel werden die Mittel zur Übertragung der Belastung für die Kontraktion und Expansion des Körpers12 entlang der Längsachse24 geführt. Eine derartige Kontraktion des Körpers12 wird ebenfalls durch das Implantat10 zur Verfügung gestellt, so dass es sich zu einer kleineren Größe zusammenziehen kann, so dass es von der chirurgischen Seite her durch eine möglichst klein gehaltene Kanüle, wie es im Detail weiter unten beschrieben wird, eingebracht werden kann. - Bezug nehmend auf die Struktur, wie sie in
2 dargestellt ist, besteht der Körper12' in seiner Gesamtheit aus einem flexiblen Material, das eine federgleiche Kontraktion und Expansion des Körpers12 um die Längsachse24' erlaubt. Alternativ kann ein Teil des Körpers12' aus einem flexiblen Material bestehen, das relativ biegsam zu dem verbleibenden Körperteil12' ist, so wie es oben angesprochen worden ist, im Bezug auf die erste Ausführungsform. Entscheiden ist, dass wenigstens ein Teil des Körpers12' flexibel ausgebildet ist, um eine Kontraktion und Expansion des Körpers12' um die Längsachse24' , wie unten beschrieben wird, zu ermöglichen. - Wie oben angegeben, sind derartig flexible Materialien aus dem Stand der Technik gut bekannt, und können federartige Materialien aus Metall und Metallkompositen sein aber auch aus Kunststoff und Kunststoffkompositen bestehen.
- Der Körper
12' umfasst sich gegenüber liegende beabstandete Kanten34 ,36 , die sich entlang der Gesamtlänge des Körpers12' erstrecken. Die Kombination von einem flexiblen Körper12' und dem im Wesentlichen zähe Gestalt aufweisenden Querschnitt des Körpers12' mit den daran umfassenden beabstandeten Kanten34 ,36 ermöglichen es dem Körper12 sich einzurollen nach der Art vergleichbar mit einem eingerollten Blatt Papier. Der wirksame Querschnittsdurchmesser der Vorrichtung kann leicht von 16 mm bis zu einer Größe von 8 oder 10 mm zusammengezogen werden. Das Implantat kann so durch eine Kanüle mit einem Durchmesser von 12 mm oder 14 mm eingeführt werden. Diese Abnahme des Querschnittsdurchmessers ist signifikant für die Reduzierung der chirurgischen Wunden von einer Größe, die eine plastische Chirurgie zur Behandlung von Narben nötig macht, zu einer Größe, die in keiner Vernarbung resultiert. - Das Langloch, das durch den Raum zwischen den Kanten
34 ,36 definiert ist, kann in der Kombination mit der Gestalt des Körpers12 in Übereinstimmung der Verwendung variieren. Beispielsweise ist in2 ein Ausführungsbeispiel dargestellt, wobei der Körper12' eine zylindrische Gestaltung aufweist und die Kanten34 ,36 den gleichen Abstand entlang ihrer Länge aufweisen und so ein gerades Langloch bilden. Das gerade Langloch bildet für den zylindrischen Körper12 eine Kontraktion mit gleichem Abstand, dabei unterstützt die zylindrische Ausführungsform des Körperteils während der Kontraktion des Körpers12' entlang der Längsachse24 . - Eine alternative Ausführungsform ist in
3 dargestellt, wobei das Körperteil12'' eine konische Ausgestaltung aufweist. Die konische Ausgestaltung wird bevorzugt, um die natürliche Wirbelsäulenanatomie wieder herzustellen. Die natürliche Wirbelsäulenanatomie resultiert üblicherweise durch Veränderungen der Winkel in den Zwischenräumen zwischen den Wirbelkörpern. Dies geschieht im Besonderen im Bereich der Lendenwirbel. Der Bereich der Lendenwirbe hat eine natürliche Krümmung. Das resultiert aus den Bandscheibenzwischenräumen der Oberflächen der Wirbelkörper, die nicht parallel zueinander sind. Ein konischer Käfig kann die natürliche Anatomie der Wirbelsäule wieder herstellen, indem er einen Winkel zwischen den sich gegenüber liegenden äußeren Oberflächen14' des Körpers12'' , die in Kontakt mit den Wirbelkörperendplatten stehen, wieder herstellt. Die Kanten34' ,36' unterscheiden sich von einander entlang der Länge des Körpers12'' . Beim Kollabieren des Körpers12'' bis die Kanten34' ,36' sich berühren wird der konische Käfig zu einem Zylinder, dadurch wird es möglich ihn durch eine schmale Kanüle einzuführen, ähnlich wie es bei den zylindrischen Käfigen nach den1 und2 möglich ist. - Ein weiters Ausführungsbeispiel der vorliegenden Erfindung ist in
4 dargestellt, wobei ein zylindrischer Körperteil12''' dargestellt ist in Kombination mit einem gewinkelten Langloch30 , das durch die Kanten34'' ,36'' definiert ist. Diese Ausführungsform veran schaulicht die Variabilität der vorliegenden Erfindung zur Verwendung in den unterschiedlichsten Situationen. - Ebenso zeigt die
5 einen im Wesentlichen zylindrisch gestalteten Körperteil14'''' , umfassend mit vier parallel verlaufenden Kanten34''' ,36''' . Jedoch umfasst der Körperteil14'''' im Wesentlichen flache sich gegenüber liegende Seitenteile38 ,40 . Der Körperteil14'''' bildet so im Wesentlichen eine rechtwinklige Form aus. Derartige Formen werden bevorzugt, um ein Rollen oder Rotieren dieser Vorrichtung innerhalb des Bandscheibenraumes zu verhindern. Die Vorrichtung10'''' kann auch in einer mehr rechteckigen Ausgestaltung konstruiert sein. Dadurch kann sie den Raum effizienter ausfüllen nach den Vorrichtungen des Standes der Technik hätte man hierfür zwei derartige Vorrichtungen gebraucht. Beispielsweise das US Patent 5,192,327 nach Brantigan offenbart ein rechteckiges Implantat für Wirbelkörper, hierfür müssen mehrere Implantate verwendet werden, um das gewünschte Resultat zu erhalten. Die vorliegende Erfindung benötigt ein einzelnes Implantat in verschiedenen Ausführungsformen für die Implantation eines einzelnen Implantats anstelle von mehreren Implantaten. - Wie in
2 dargestellt, umfasst der Körper12' einen Kontraktionslimitierenden Mechanismus, um die Kontraktion des Körpers12' über seine Längsachse24' zu begrenzen. Der Kontraktion limitierende Mechanismus kann einen Lippenteil44 umfassen, der sich von beiden Kanten34 ,36 her erstreckt (die Lippe wird dargestellt in ausgezogener Form von der Kante36 in2 ). Wie die2 weiterhin zeigt, berührt und liegt die Kante34 gegen den Lippenteil44 während der Kontraktion der Körpers12' über seine Längsachse24' an. Dabei wird die Kontraktion des Körpers12' in seiner Längsachse24' begrenzt. - Der Lippenteil
44 wirkt dabei als ein Limitierungsmechanismus für die Anzahl des Gewichtes der durch den Körper12' zu tragen ist und überträgt diese Kraft auf das im Inneren liegende Transplantationsmaterial. Der Betrag der Kontraktion des Körpers12' über seine Längsachse24' hängt von der Form der Belastung ab, die von den ihn umgebenden Enden der Wirbelkörperplatten übertragen werden. Dies wird kontrolliert durch den Lippenteil44 . Der Lippenteil44 ermöglicht es dem Käfig die Last zu teilen bis ein gewisser Betrag erreicht ist, um dann als ein Platzhalter zu fungieren, sofern das Transplantationsmaterial nicht völlig abgesichert ist oder von einer schlechten Qualität ist. Mit anderen Worten eine Belastung wird durch den Körper12' übertragen insofern der Körper zwischen den sich spannenden Raum zwischen den Kanten34 ,36 sich zusammenzieht und in dem zusammengezogenen Zustand die Kanten34 ,36 entweder mit der Kante34 oder mit der Lippe44 in Kontakt stehen. Die Lippe44 stellt eine größere Oberfläche zur Verfügung, um den Kontakt zwischen den Kanten34 ,36 zu halten. Sobald genügend Belastung auf den Körper12' einwirkt, so dass die Kanten34 ,36 in Kontakt stehen, wird ein Mehr an Belastung in den Körper12' eingeleitet, sofern dieser weitere flexible Teile, wie detailliert betrachtet in der Ausführungsform nach1 , vorhanden sind. In diesem maximalen zusammengezogenen Zustand agiert der Körper12' nur noch als ein Platzhalter und nicht länger als ein Überträger von Belastungen. Wie auch immer, der Platzhalter ermöglicht einen genügend großen Raum, um angrenzende Nerven und Rückenmarkteile zu schützen. - Jeder der Körper
12 bis12'''' umfasst Mittel, um das Implantat10 bis10'''' zwischen zwei sich gegenüber liegenden Wirbelkörpern zu fixieren. Jede der Ausführungsformen beinhaltet gezahnte Profilierungen in Form von Fixierungsmitteln46 . Derartige Fixierungsmittel können in Übereinstimmung mit verschiedenen Gestaltungen oder Designs, wie sie im Stand der Technik zu finden sind, ausgeführt sein. Derartige Fixierungsmittel können heligal, gerade, angulär oder keilförmig ausgeformt sein, aber auch jede andere Ausgestaltung im Design, wie sie nach dem Stand der Technik bekannt ist, kann hierfür verwendet werden. - Jede der zusammenziehbaren Ausführungsformen der vorliegenden Erfindung sind mit Raum bildenden Löchern
48 versehen, wobei jedes Paar von Löchern über dem Längsschlitz, wie in den2 bis5 gezeigt, geneigt sind. Die Löcher dienen zum Verbinden mit Instrumenten (nicht dargestellt) einschließlich Zungen oder anderen Instrumenten, die dazu dienen können, den Körper12' bis12'''' zusammenzuziehen, um ihn in eine Kanüle einzuführen. Gleichermaßen kann eine Rückhalteplatte (nicht dargestellt) verwendet werden, wenn eine Kontraktion und Expansion für die Implantation erforderlich ist und diese nicht für die Langzeitverwendung gewünscht wird. Die Anlage kann an eine Furche angeheftet werden, Fäden oder Zähne können nach der Käfigeinführung und Expansion so das Implantat weiterhin warten. - Die vorliegende Erfindung kann aus einem Memory Metall bestehen, wie beispielsweise Nitinol. Derartige Metalle können zu einer gewünschten Form gebogen werden, wie beispielsweise die gebogenen Körper
12' bis12'''' , in einen zusammengezogenen Zustand und halten so den zusammengezogenen Zustand aufrecht. Einmal implantiert kann ein Instrument verwendet werden, um einen elektrischen Strom, Hitze oder Kälte zu erzeugen, um den Körper in seine ursprünglich gemerkte Form im expandierten Zustand zurückzuführen. Auf diese Weise kann das Implantat in einem kollabierten oder im Wesentlichen kollabierten Zwischenraum zwischen zwei Wirbelkörpern der Bandscheibe eingeführt werden. Die Anregung, wie etwa Hitze, elektrischer Strom oder Kälte, kann dem Implantat zugeführt werden, wobei das Implantat sich in seiner expandierten Form zurückentwickeln wird und dabei den Bandscheibenraum auseinander drückt und dabei bewirkt, dass die gequetschten Nerven oder dergleichen wieder freigegeben werden. Das Implantat wird dann den Raum offen halten und bewirken, dass der Patient wieder entlassen werden kann. - Bei der Verwendung des Implantats, das direkt eingeführt werden muss, ohne die Einführung über eine Kanüle, so ist ein Langloch nicht mehr nötig. Das Implantat wird durch einen Einschnitt und Rückzug über die Seite geneigt, so wie das Implantat verkauft wird. So wie der richtige Einschnitt gemacht worden ist und im Wesentlichen das Bandscheibenmaterial entfernt worden ist, um Platz für den Käfig oder die Käfige, wenn zwei schmalere Käfige gewünscht werden anstelle eines einzelnen Käfigs, wird das Implantat in den Raum eingeführt, so dass die Zähne
46 bis46'''' und andere Oberflächen auf dem Implantat, das Implantat10 bis10'''' in einer geeigneten Position festhalten. - Um eine kleinere Kanüle zu verwenden oder die Möglichkeit des Implantats voll auszuschöpfen, wird der Bandscheibenraum geöffnet, dafür hat das Implantat ein Langloch, so wie es in den
2 bis5 beansprucht ist. Das Implantat ist zusammengezogen und bereit in eine Kanüle eingelegt zu werden, um in den Bandscheibezwischenraum, wie es oben beschrieben worden ist, platziert zu werden. Sobald die Einlage aus der Kanüle in den Bandscheibenzwischenraum eingeführt ist, federt das Implantat natürlich in seine originale Form zurück oder es wird induziert bis es seine originale Form über die Stimulierung wieder erreicht hat. Dies geschieht im Besonderen bei Memory Metallen, wie sie oben besprochen worden sind. - Mit Vorrichtungen nach dem Stand der Technik verfügt das Implantat über Transplantationsmaterial, um eine Verschmelzung zu induzieren. Durch die Möglichkeit, dass sich das Implantat zusammen ziehen und auseinander ziehen kann und eine Belastung durch die Wirbelkörperendplatten an den Körpern
12 bis12'''' des Implantats10 bis10'''' gelangt, kann das Transplantationsmaterial dadurch eine Last von den Wirbelkörperendplatten zu dem eingebetteten Transplantationsmaterial weiterleiten. Die Vorrichtung zieht die Vorteile von dem angewandten Wolfschen Gesetz und stellt gleichzeitig eine ausgezeichnete Verschmelzungsqualität zur Verfügung. - Im Rückblick auf das oben Gesagte, kann die vorliegende Erfindung als ein Implantat das entweder als ein Platzhalter der Belastungen von angrenzenden Wirbelkörperendplatten zu dem Transplantationsmaterial enthaltenden Implantat verwendet werden oder auch als ein zusammenziehbares oder auseinanderziehbares Implantat.
- Die Erfindung wurde in einer anschaulichen Art und Weise beschrieben und die Terminologie ist so zu verstehen, dass sie die Natur der Wörter der Beschreibung wiedergeben und nicht als eine Limitierung gedacht sind.
- Es ist offenkundig, dass viele Modifikationen und Variationen der vorliegenden Erfindung möglich sind, in Hinblick auf die oben genannte Lehre. Es ist deshalb verständlich, dass die Reichweite der anhängigen Ansprüche der Erfindung möglicherweise anders in der Praxis ausgeführt wird als diese im Wesentlichen hier beschrieben worden sind.
Claims (14)
- Ein Fusionsimplantat (
10 ) zum Einsetzen zwischen zwei sich gegenüberliegenden Wirbelkörpern, wobei das Implantat (10 ) umfasst: einen Körper (12 ) einschließlich einer inneren Oberfläche (16 ), die eine innere hohle Kammer (18 ) bildet, die darin Transplantationsmaterial aufnehmen kann, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) Belastungsübertragungsmittel aufweist, die die Belastungen von den Wirbelkörpern, die das vorgenannte Implantat (10 ) umgeben hin zu dem Transplantationsmaterial, das in dem vorgenannten Implantat (10 ) enthalten ist, übertragen, der vorgenannte Körper (12 ) optional Fixierungsmittel (46 ) enthält, um das vorgenannte Implantat (10 ) zwischen zwei sich gegenüberliegenden Wirbelkörpern zu fixieren; wobei das Implantat geeignet ist, um den Zwischenraum zwischen zwei Wirbelkörpern aufrecht zu erhalten. - Ein Implantat (
10 ) nach Anspruch 1, dadurch gekennzeichnet, dass obere und untere Teile (20 ,22 ) vorgesehen sind, um die oberen und unteren Wirbelkörper zu verbinden, wobei die besagten oberen und unteren Teile (20 ,22 ) ablenkbar ausgebildet sind und die vorgenannten Belastungsübertragungsmittel definieren. - Ein Implantat (
10 ) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) ein erstes und ein zweites offenes Ende (30 ,32 ) aufweist, wobei zwischen den Enden eine Längsachse (24 ) definiert ist und sich über die Länge des vorgenannten Körpers (12 ) erstreckt und zwischen den vorgenannten offenen Enden (30 ,32 ) das vorge nannte Belastungsübertragungsmittel zur Kontraktion und Expansion des vorgenannten Körpers (12 ) über die vorgenannte Längsachse (24 ) entlang der gesamten Länge zur Verfügung gestellt ist. - Ein Implantat (
10 ) nach Anspruch 3, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) aus flexiblem Material besteht, das eine federartige Kontraktion und Expansion des vorgenannten Körpers (12 ) über die vorgenannte Längsachse (24 ) ermöglicht. - Ein Implantat (
10 ) nach Anspruch 3 oder Anspruch 4, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) sich gegenüber beabstandete Kanten (34 ,36 ) aufweist, die sich an der vorgenannten Länge des vorgenannten Körpers (12 ) ausdehnen, die vorgenannten beabstandeten Kanten (34 ,36 ) sind typischerweise in einem gleichen Abstand zueinander entlang der vorgenannten Länge des vorgenannten Körpers (12 ) beabstandet oder sie divergieren voneinander entlang der vorgenannten Länge des vorgenannten Körpers (12 ). - Ein Implantat (
10 ) nach Anspruch 5, dadurch gekennzeichnet, dass Kontraktion begrenzende Mittel vorgesehen sind, um die Kontraktion des vorgenannten Körpers (12 ) über die vorgenannte Längsachse (24 ) einzuschränken. - Ein Implantat (
10 ) nach Anspruch 5 oder 6, dadurch gekennzeichnet, dass wenigstens eine der vorgenannten Kanten (34 ,35 ) ein Lippenteil (44 ) aufweist, das sich an die Kante anschließt und die vorgenannte andere Kante der vorgenannten Kanten (34 ,36 ) in der Lage ist, sich gegen das vorge nannte Lippenteil (44 ) zusammenzuziehen, während sich der vorgenannte Körper (12 ) über die vorgenannte Längsachse (24 ) zusammenzieht und dabei fähig ist, die Kontraktion des vorgenannten Körpers (12 ) über die vorgenannte Längsachse (24 ) zu beschränken. - Ein Implantat (
10 ) nach einem der Ansprüche 3 bis 7, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) aus einem Memory Metall besteht, welches bei Anwendung eines Reizes aus seinem ursprünglichen Zustand in einen zusammengezogenen Zustand biegbar ist und fähig ist wieder in den vorgenannten ursprünglichen Zustand zurückzukehren. - Ein Implantat (
10 ) nach einem der Ansprüche 1 bis 4 oder nach dem Anspruch 8 soweit er von dem Anspruch 3 oder 4 abhängig ist, dadurch gekennzeichnet, dass der Körper Fixierungsmittel umfasst, um das vorgenannte Implantat mit den sich gegenüberliegenden Wirbelkörpern zu fixieren, wobei die vorgenannten Fixierungsmittel eingezogene Teile (46 ) umfassen, die sich an der entgegen gesetzten äußeren Oberfläche des besagten Körpers (12 ) befinden, ferner umfasst der Körper entgegen gesetzte flexible Seitenwände (26 ,28 ) zwischen denen sich die vorgenannten eingezogenen Teile (46 ) befinden und die vorgenannten Belastungsübertragungsmittel definieren. - Ein Implantat (
10 ) nach Anspruch 1, dadurch gekennzeichnet, dass der Körper (12 ) hohle Schalen umfasst, die eine zentrale Längsachse (24 ) aufweisen, die vorgenannten Schalen wenigstens einen flexiblen Körperteil umfassen, um eine Kontraktion und Expansion entlang der vorgenannten Achse (24 ) zu ermöglichen. - Ein Implantat (
10 ) nach Anspruch 10, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) ein flexibles Material umfasst, was eine federgleiche Kontraktion und Expansion des vorgenannten Körpers (12 ) entlang der vorgenannten Längsachse (24 ) ermöglicht. - Ein Implantat (
10 ) nach Anspruch 11, dadurch gekennzeichnet, dass ein Bereich des vorgenannten Körpers (12 ) Öffnungen aufweist, die es erlauben, dass sich das Transplantationsmaterial innerhalb des vorgenannten Körpers (12 ) mit Knochensubstanz außerhalb des vorgenannten Körpers (12 ) verbindet und sich an dem vorgenannten Implantat (10 ) einfügt. - Ein Implantat (
10 ) nach einem der Ansprüche 1 bis 12, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) kollabierbar und ausdehnbar ist, wobei der vorgenannte Körper (12 ) in der Lage ist, sich automatisch auszudehnen und von einem eingefügten Instrument zu lösen, wie etwa beim Weiten des Bandscheibenraumes zwischen sich gegenüberliegenden Wirbelkörperendplatten. - Ein Implantat (
10 ) nach einem der Ansprüche 1 bis 13, dadurch gekennzeichnet, dass der vorgenannte Körper (12 ) eine externe Oberfläche (14 ) umfasst, wobei die externe Oberfläche (14 ) eine poröse Ummantelung aufweist, die es der Knochensubstanz ermöglicht, daran einzuwachsen und/oder eine strukturierte externe Oberfläche für eine verbesserte Knochensubstanzfixierung zu bilden.
