- GEBIET DER ERFINDUNG
- Diese Erfindung betrifft im Allgemeinen Instrumente zur Durchführung chirurgischer Verfahren mit geringeren Eingriffen, und insbesondere Instrumente für chirurgische Verfahren mit geringeren Eingriffen an Herz und großen Blutgefäßen.
- HINTERGRUND DER ERFINDUNG
- Gegenwärtig werden verschiedene Arten an chirurgischen Verfahren durchgeführt, um Erkrankungen des Herzens und der großen Blutgefäße im Bereich des Brustkorbs zu untersuchen, zu diagnostizieren und zu behandeln. Solche Verfahren umfassen die Wiederherstellung und das Austauschen von Mitral-, Aorten- und anderen Herzklappen, die Wiederherstellung defekter Vorhofscheidewände und Ventrikelsepten, Lungenthrombektomie, Behandlung von Aneurysma, elektrophysiologische Kartierung und Ablation des Myocardiums und andere Verfahren, bei denen Vorrichtungen für diese Eingriffe in das Innere des Herzens oder eines großen Blutgefäßes eingeführt werden.
- Unter Verwendung gegenwärtiger Verfahren erfordern zahlreiche dieser Verfahren eine Ganz-Thorakotomie, üblicherweise in Form einer medianen Sternotomie, um Zugang zur Brusthöhle des Patienten zu bekommen. Eine Säge oder ein anderes Schneideinstrument wird verwendet, um das Sternum der Länge nach zu schneiden, wodurch zwei gegenüberliegende Hälften des anterioren oder ventralen Abschnitts des Brustkorbs auseinander gedehnt werden können. Somit wird zur Brusthöhle hin eine große Öffnung geschaffen, durch die das Chirurgenteam das Herz und andere Elemente des Thorax direkt betrachten und bearbeiten können.
- Chirurgische Eingriffe am Herz erfordern im Allgemeinen das Trennen des Herzens und der Koronarblutgefäße vom übrigen Arteriensystem und das Anhalten der Herzfunktionen. Üblicherweise wird das Herz vom Arteriensystem durch Einführen einer externen Aorta-Kreuzklemme mittels Sternotomie und durch Anbringen dieser Klem me an der Aorta zwischen der Arteria anonyma und der Herzkranzöffnung abgetrennt. Herzlähmende Flüssigkeit wird dann in die Koronararterien, entweder direkt in die Koronaröffnung oder durch Durchstechen in den Ansatz der Aorta, injiziert, um die Herzfunktion zum Stillstand zu bringen. In manchen Fällen wird die herzlähmen Flüssigkeit in den Koronarsinus injiziert, um die Perfusion des Myocardiums herabzusetzen. Der Patient wird mit einem kardiopulmonalen Bypass versehen, um die periphere Zirkulation des mit Sauerstoff angereicherten Bluts aufrechtzuerhalten.
- Von besonderem Interesse für die vorliegende Erfindung sind intrakardiale Vorrichtungen zur chirurgischen Behandlung von Herzklappen, besonders der Mitral- und Aortenklappen.
- Laut jüngsten Schätzungen wird in den US-amerikanischen Krankenhäusern jährlich bei mehr als 79.000 Patienten eine Erkrankung der Aorten- oder Mitralklappe diagnostiziert. Mehr als 49.000 Austauschverfahren von Mitralklappen oder Aortenklappen werden jedes Jahr in den Vereinigten Staaten durchgeführt, und dies neben einer erheblichen Zahl an Verfahren zur Wiederherstellung von Herzklappen.
- Verschiedene chirurgische Techniken und Verfahren können eingesetzt werden, um eine erkrankte oder geschädigte Klappe wiederherzustellen, umfassend Annuloplastie (Zusammenziehen des Klappenrings), vierseitige Resektion (Einengen der Klappenblättchen), Kommissurotomie (Durchschneiden der Klappenkommissur, um die Klappenblättchen zu trennen), Verkürzen der Mitralklappen- oder Tricuspidalklappen-Chordae tendonae, Wiederanbringung abgetrennter Mitral- oder Tricuspidalklappen-Chordae tendonae oder papillären Muskelgewebes und Entkalkung von Klappen- und Ringgewebe. Alternativ dazu kann die Klappe durch Exzidieren der Klappenblättchen der natürlichen Klappe und Anbringen einer Austauschklappe an der Klappenstelle ausgetauscht werden, üblicherweise durch Vernähen der Austauschklappe mit dem natürlichen Klappenring. Verschiedene Arten an Austauschklappen werden gegenwärtig eingesetzt, umfassend mechanische und biologische Prothesen, Homotransplantate und Allotransplantate, wie in Bodnar and Frater, Replacement Cardiac Valves, 1–357 (1991) beschrieben wird, das durch Verweis hierin aufgenommen ist. Eine umfassende Erläuterung von Herzklappenerkrankungen und der chirurgischen Behandlung davon kann in Kirklin and Barratt-Boyes, Cardiac Surgery, 323–459 (1986) gefunden werden. Das Dokument WO-A-87/05.489 offenbart eine Herzklappenprothese und ein Instrument zum Einführen eben dieser.
- Die Mitralklappe, die zwischen dem linken Atrium und dem linken Ventrikel des Herzens angeordnet ist, ist durch die Wand des linken Atriums, das sich normalerweise an der hinteren Seite des Herzens befindet, gegenüber der Seite des Herzens, die durch eine mediane Sternotomie offengelegt wird, äußerst leicht zu erreichen. Um die Mitralklappe über eine Sternotomie zu erreichen, wird daher das Herz gedreht, um das linke Atrium in eine vordere Position zu bringen, die durch die Sternotomie zugänglich wird. Eine Öffnung, oder Arteriotomie, wird dann in die rechte Seite des linken Atriums, das vor den rechten Lungenvenen liegt, geschnitten. Die Arteriotomie wird durch Nähte oder eine Rückziehvorrichtung zurückgezogen, wodurch die Mitralklappe direkt hinter der Arteriotomie freigelegt wird. Eines der zuvor genannten Verfahren kann dann verwendet werden, um die Klappe wiederherzustellen oder auszutauschen.
- Ein anderes Verfahren, um Zugang zur Mitralklappe zu schaffen, kann verwendet werden, wenn eine mediane Sternotomie und/oder Drehmanipulation des Herzens unerwünscht sind. In diesem Verfahren wird ein langer Schnitt in die rechte laterale Seite der Brust, üblicherweise in der Region des fünften Interkostalraums, gesetzt. Eine oder mehrere Rippen können vom Patienten entfernt werden, und andere Rippen in der Nähe des Schnitts können nach außen zurückgespreizt werden, um eine weite Öffnung in die Brusthöhle zu schaffen. Das linke Atrium wird dann an der hinteren Seite des Herzens freigelegt, und eine Arteriotomie wird in der Wand des linken Atriums gebildet, durch die die Mitralklappe zur Wiederherstellung oder zum Austausch erreichbar wird.
- In solchen Verfahren mit Öffnung des Brustkorbs ermöglicht es die durch eine mediane Sternotomie oder rechte Thorakotomie geschaffene, weite Öffnung dem Chirurgen, die Mitralklappe direkt durch die linke Arteriotomie zu sehen und seine oder ihre Hände in der Brusthöhle in großer Nähe zur Außenseite des Herzens zu positionieren, um chirurgische Instrumente zu manipulieren, abgetrenntes Gewebe zu entfernen und/oder eine Austauschklappe durch die Arteriotomie zur Anbringung im Herzen einzuführen. Diese Verfahren und Eingriffe mit Öffnung des Brustkorbs bringen jedoch auch ein schwerwiegendes Trauma, ein signifikantes Risiko von Komplikationen, einen ausgedehnten Krankenhausaufenthalt und eine schmerzhafte Rekonvaleszenzzeit für den Patienten mit sich. Darüber hinaus sind, wenn auch chirurgische Eingriffe an Herzklappen für viele Patienten sehr positive Ergebnisse bringen, zahlreiche Personen, die von einem chirurgischen Eingriff profitieren könnten, nicht in der Lage oder nicht willens, das Trauma und mögliche Risiken aktueller Verfahren auf sich zu nehmen.
- Es besteht daher Bedarf an Vorrichtungen zur Durchführung von Herzklappenwiederherstellung und -austausch sowie anderer Verfahren im und am Herzen und an großen Blutgefäßen, die ein geringeres Trauma, weniger Risiken, kürzere Rekonvaleszenzzeiten und geringere Schmerzen mit sich bringen, als dies bei gegen wärtigen Verfahren der Fall ist. Die Vorrichtungen sollten chirurgische Eingriffe am Herzen oder an großen Blutgefäßen ohne notwendige Ganz-Thorakotomie ermöglichen, vorzugsweise durch kleine Schnitte innerhalb der Interkostalräume des Brustkorbs, ohne die Rippen oder das Sternum des Patienten durchschneiden, entfernen oder signifikant auseinander biegen zu müssen. Insbesondere sollten die Vorrichtungen das Entfernen von Gewebe aus der Brusthöhle sowie das Einführen chirurgischer Instrumente, Vorrichtungen zur Sichtbarmachung, Austauschklappen und dergleichen in die Brusthöhle ermöglichen, um das Wiederherstellen und das Austauschen von Herzklappen zu erleichtern. Vorzugsweise sollten die Vorrichtungen das Austauschen einer Herzklappe durch verschiedene Arten an Prothesen erleichtern, umfassend mechanische und biologische Prothesen, Homotransplantate und Allotransplantate.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die Erfindung stellt eine Vorrichtung zur Durchführung chirurgischer Verfahren mit geringeren Eingriffen innerhalb eines Organs oder Gefäßes bereit, und insbesondere innerhalb des Herzens und der großen Gefäße der Brusthöhle. Die Vorrichtung der Erfindung erleichtert Eingriffe am Herzen oder an Blutgefäßen ohne die Notwendigkeit einer medianen Sternotomie oder einer anderen Form von Ganz-Thorakotomie, wodurch im Wesentlichen das Trauma, Komplikationsrisiken, Rekonvaleszenzzeit und Schmerzen für den Patienten reduziert werden. Durch die Verwendung der Vorrichtung der Erfindung können chirurgische Verfahren durch perkutane Penetration innerhalb der Interkostalräume des Brustkorbs des Patienten durchgeführt werden, ohne eine der Rippen oder das Sternum des Patienten durchzuschneiden, zu entfernen oder signifikant zu versetzen. Die Erfindung erleichtert das Austauschen von Herzklappen durch eine oder zahlreiche verschiedene im Handel erhältliche Austauschklappen, umfassend mechanische Prothesen, Bioprothesen, Homotransplantate und Allotransplantate.
- Gemäß der vorliegenden Erfindung wird eine thorakoskopische Vorrichtung nach Anspruch 1 und eine Prothesenanordnung, umfassend die Vorrichtung und eine Austauschklappe, bereitgestellt. In einer bevorzugten Ausführungsform weisen die Austauschklappe und die Haltevorrichtung zusammen ein Profil mit einer Breite auf, die vorzugsweise geringer ist als die Breite des Interkostalraums. Vorzugsweise weist der Interkostalraum eine Breite von weniger als etwa 20 mm auf. Der Befestigungsabschnitt der Austauschklappe hat üblicherweise einen Außendurchmesser, der größer als die interkostale Breite ist.
- Das Haltemittel der Vorrichtung umfasst einen verlängerten Griff mit einem distalen Ende, das an der Austauschklappe befestigt ist, und einem proximalen Ende, das dem distalen Ende gegenüber liegt. Der Griff ist so ausgebildet, dass er die Austauschklappe in das Herz des Patienten durch den Interkostalraum einführt. Vorzugsweise ist der Griff zumindest etwa 20 cm lang, um das Positionieren der Austauschklappe im Herzen von einem rechten Seitenabschnitt der Brust des Patienten aus zu ermöglichen. Der Griff kann weiters ein Mittel zum Ausklinken der Austauschklappe einschließen, wobei das Ausklinkmittel so ausgebildet ist, dass es vom proximalen Ende des Griffs aus betätigbar ist.
- Der Griff umfasst auch ein Mittel zum Schwenken der Austauschklappe von einer ersten Ausrichtung zur Einführung durch den Interkostalraum zu einer zweiten Ausrichtung zur Anbringung im Herzen des Patienten. Das Schwenkmittel ist so ausgebildet, dass es vom proximalen Ende des Griffs aus betätigbar ist. Auf diese Weise kann die Austauschklappe durch den Interkostalraum mit dem Ende zuerst eingeführt und anschließend um eine Achse, die im Allgemeinen im rechten Winkel zum Griff steht, in eine zur Befestigung im Herzen des Patienten geeignete Ausrichtung geschwenkt werden.
- Vorzugsweise wird die Austauschklappe vorher am Haltemittel befestigt, und diese zwei Teile werden sterilisiert und zusammen in einem sterilen Päckchen verpackt. Auf diese Weise kann das Päckchen im Operationssaal in steriler Umgebung geöffnet werden, und sowohl Klappe als auch Haltevorrichtung sind für den unverzüglichen Einsatz in einem chirurgischen Eingriff bereit.
- Die Erfindung stellt eine Prothesenanordnung zum Anbringen einer Austauschklappe in einer Klappenposition eines Herzens eines Patienten bereit.
- Die Prothesenanordnung umfasst einen verlängerten Griff, der so ausgebildet ist, dass er durch einen Interkostalraum in die Brust des Patienten wie zuvor beschrieben positioniert werden kann. Die Vorrichtung umfasst ein Mittel an einem distalen Ende des Griffs, das eine Austauschklappe ausklinkbar in einer Ausrichtung zur Einführung durch den Interkostalraum hält, und umfasst weiters ein Mittel zum Schwenken der Austauschklappe relativ zum Griff aus einer ersten Ausrichtung zur Einführung durch den Interkostalraum zu einer zweiten Ausrichtung zum Anbringen in der Klappenposition. Die Prothesenanordnung umfasst in einer bevorzugten Aus führungsform weiters ein Mittel am proximalen Ende des Griffs zum Ausklinken der Austauschklappe aus der Haltevorrichtung, nachdem die Prothese positioniert und innerhalb des Herzens befestigt wurde.
- Ein besseres Verständnis der Beschaffenheit und der Vorteile der Erfindung wird durch eingehendes Betrachten der nachstehenden Ausführungen der Beschreibung und der Zeichnungen ermöglicht werden.

- Die Erfindung stellt eine Vorrichtung zur Durchführung chirurgischer Eingriffe innerhalb des Herzens bereit. Während sich die hierin beschriebenen, spezifischen Ausführungsformen der Erfindung auf Wiederherstellung und Austausch von Mitralklappen beziehen, gilt es zu verstehen, dass die Erfindung zur Durchführung einer großen Vielzahl an chirurgischen Verfahren nützlich ist, umfassend die Wiederherstellung und den Austausch von Aorten-, Tricuspidal- oder Pulmonalklappen.
- Günstigerweise erleichtert die Erfindung die Durchführung solcher Verfahren durch perkutane Penetration innerhalb der Interkostalräume des Brustkorbs, wodurch sich die Notwendigkeit einer medianen Sternotomie oder anderer Arten von Ganz-Thorakotomie erübrigt.
- Die Bezeichnungen "perkutane interkostale Penetration" und "interkostale Penetration", wie sie hierin verwendet werden, beziehen sich auf eine Penetration in Form eines kurzen Schnitts, einer Inzision, eines Lochs, einer Kanüle, einer Trokarmuffe oder dergleichen durch die Brustwand zwischen zwei nebeneinander liegenden Rippen, worin der Brustkorb des Patienten und das Sternum im Wesentlichen intakt bleiben und die Rippen oder das Sternum weder durchgeschnitten noch entfernt noch signifikant versetzt werden. Sinn dieser Bezeichnungen ist, eine klare Abgrenzung zu einer Ganz-Thorakotomie wie einer medianen Sternotomie zu schaffen, worin das Sternum und/oder eine oder mehrere Rippen durchgeschnitten oder aus dem Brustkorb entfernt werden oder eine oder mehrere Rippen deutlich zurückgebogen werden, um eine weite Öffnung in den Brustkorb zu schaffen. Eine "perkutane interkostale Penetration" kann zu einem Zusammenstoßen oder Überlappen der nebeneinander liegenden Rippen, zwischen denen die Penetration erfolgt, führen, doch die maximale Breite der Penetration, die zur Einführung von Instrumenten, Prothesen und dergleichen in den Brustkorb erforderlich ist, entspricht der Breite des Interkostalraums, der durch zwei nebeneinander liegenden Rippen in ihren natürlichen, im Wesentlichen nicht verbogenen Positionen festgelegt ist. Es gilt zu verstehen, dass eine oder mehrere Rippen geringfügig zurückgezogen oder -gebogen werden können, ohne dadurch den Schutzumfang der Erfindung zu verletzen; dennoch zielt die Erfindung insbesondere darauf ab, Schmerzen, Traumata und Komplikationen zu vermeiden, die durch starke Biegung oder durch Durchtrennen der Rippen in herkömmlichen Verfahren mit Öffnung des Brustkorbs entstehen.

1 veranschaulicht ein System20 zum Klappenaustausch ohne Öffnung des Brustkorbs, das in einem Patienten P auf einem Operationstisch T positioniert ist. Vorzugsweise wird ein Keil oder Klotz W mit einer abgewinkelten oberen Oberfläche mit etwa 20° bis 45° Neigung unter der rechten Seite des Patienten P angeordnet, sodass die rechte Seite des Körpers des Patienten im Vergleich zur linken Seite etwas erhöht ist. Der rechte Arm des Patienten A wird nach unten sich verdrehen gelassen, sodass er auf dem Tisch T aufliegt und dadurch die rechte laterale Seite der Brust des Patienten freilegt. - Das Klappenaustauschsystem
20 umfasst eine Zugangskanüle22 , die perkutan innerhalb eines Interkostalraums zwischen zwei Rippen (strichlierte Linien) an einer rechten lateralen Seite der Brust des Patienten positioniert ist. Zusätzliche thorakoskopische Trokarmuffen24 von herkömmlicher Struktur werden innerhalb von Interkostalräumen in der rechten lateralen Brust oberhalb und unterhalb der Zugangskanüle22 sowie im rechten anterioren (oder ventralen) Abschnitt der Brust positioniert. Ein Endoskop25 von herkömmlicher Struktur wird durch perkutane interkostale Penetration in der Brust des Patienten positioniert, üblicherweise durch eine der Trokarmuffen24 . Das distale Ende des Endoskops25 (strichlierte Linien) wird vorzugsweise so angeordnet, um bei einem Winkel zwischen etwa 30° und 90° in Bezug auf den Stiel des Endoskops25 sehen zu können, um die Sichtbarmachung des Herzens vom rechten Abschnitt der Brusthöhle aus zu erleichtern. Eine Lichtquelle (nicht dargestellt) wird auch am Endoskop25 bereitgestellt, um die Brusthöhle auszuleuchten. Eine Videokamera26 wird am proximalen Ende des Endoskops25 angebracht und wird mit einem Videobildschirm28 verbunden, um das Innere der Brusthöhle darauf sichtbar zu machen. Ein erster Ring zur Führung des Fadens30 wird an einem proximalen Ende der Zugangskanüle22 angebracht. Ein zweiter Führungsring32 wird an einem Trägerstativ34 , das am Tisch T fixiert ist, angebracht. Eine Austauschklappe36 wird am distalen Ende einer Einführungsvorrichtung38 zwischen dem ersten Führungsring30 und dem zweiten Führungsring32 gehalten. Die Einführungsvorrichtung38 erstreckt sich durch den zweiten Führungsring32 und wird vom Trägerstativ34 gestützt. Zusätzliche Instrumente, die in einem Verfahren einzusetzen sind, wie beispielsweise ein Retraktor40 , sowie Schneide-, Näh-, Heftungs-, Atmungs-, Spülungs- und andere Vorrichtungen können durch Zugangskanüle22 , Trokarmuffen24 und/oder kleine perkutane Schnitte in die Interkostalräume des Brustkorbs eingeführt werden. - Nun wird Bezug auf
2 genommen. In2 wird die Zugangskanüle22 innerhalb eines Interkostalraums I in der rechten lateralen Seite der Brust, vorzugsweise im dritten, vierten, fünften oder sechsten Interkostalraum zwischen nebeneinanderliegenden Rippen R, positioniert. Zusätzliche Trokarmuffen24A ,24B werden innerhalb von Interkostalräumen oberhalb und unterhalb der Zugangskanüle22 in der rechten lateralen Seite der Brust positioniert. Die Zugangskanüle22 und die Trokarmuffen24A ,24B werden so positioniert, dass Instrumente42 , die durch diese eingeführt werden, in Richtung der rechten Seite des linken Atriums des Herzens H ausgerichtet werden können. Eine Trokarmuffe24C wird in einem Interkostalraum in der rechten anterioren Seite der Brust so positioniert, dass das Endoskop25 eingeführt werden kann, um die Brusthöhle und das Herz H einsichtig zu machen, ohne dabei mit Instrumenten, die durch die Zugangskanüle22 oder die Trokarmuffen24A ,24B eingeführt wurden, in Berührung zu kommen. Eine zusätzliche Trokarmuffe24D wird in einen Interkostalraum in der anterioren Seite der Brust genau zur Rechten des Sternums und vor der rechten lateralen Seite des Herzens H positioniert. - Fachleuten wird verständlich sein, dass es in manchen Fällen wünschenswert sein kann, manche oder alle Trokarmuffen
24 und/oder die Zugangskanüle22 zu entfernen und Instrumente direkt durch kleine, perkutane interkostale Inzisionen in der Brust einzuführen. Günstigerweise ist, anders als bei laparoskopischen, arthroskopischen und anderen endoskopischen Verfahren, unter Verwendung des Verfahrens der Erfindung keine Dehnung der Brust erforderlich, sodass ein Austreten von Dehnungsflüssigkeit durch perkutane Penetrationen keine Problematik darstellt. So können entweder thorakoskopische Trokarmuffen ohne Flüssigkeitsdichtungen oder perkutane Inzisionen zur Einführung von Instrumenten in die Brusthöhle eingesetzt werden. Trokarmuffen sind dennoch im Allgemeinen bevorzugt, um einen offenen Durchgang zur Brusthöhle zu ermöglichen, um benachbartes Gewebe vor Verletzungen zu schützen, die durch Kontakt mit Instrumenten entstehen könnten, und um Schäden an Instrumenten, Endoskopen, Austauschklappen und dergleichen zu vermeiden, wenn diese in die Brusthöhle eingeführt werden. - In den
4D –4A wird die Zugangskanüle22 im Detail beschrieben. Die Zugangskanüle22 umfasst einen Körper44 mit einem proximalen Ende46 , einem distalen Ende48 und einem Durchgang50 , der sich zwischen diesen zwei Enden erstreckt. Der Körper44 ist so ausgebildet, dass er in einen Interkostalraum I passt, ohne daneben liegende Rippen R merklich zu biegen, und üblicherweise eine Breite von weniger als etwa 20 mm aufweist. Der Durchgang50 ist so ausgebildet, dass das Durchführen der Austauschklappe36 dadurch erleichtert wird. Die Austauschklappe36 kann zahlreiche verschiedene Konfigurationen aufweisen, muss jedoch einen Durchmesser haben, der zumindest gleich groß ist wie jener der natürlichen Herzklappe des Patienten, einen Durchmesser, der üblicherweise größer ist als die Breite der Interkostalräume in der Brusthöhle. Um ein Durchschneiden oder Zurückziehen der Rippen des Patienten zu vermeiden, wird daher die Austauschklappe36 durch den Durchgang50 der Zugangskanüle22 mit dem Ende zuerst durchgeführt, wie nachstehend noch genauer beschrieben wird. Um solche Einführungen von Austauschklappen36 aufzunehmen, weist der Durchgang50 üblicherweise eine Querschnittsbreite w von etwa 12 mm bis 20 mm und eine Querschnittshöhe h auf, die etwas größer als die Querschnittsbreite w ist, üblicherweise das 2–6fache der Querschnittsbreite misst und vorzugsweise im Bereich von 25 mm bis 50 mm liegt. Der Durchgang50 kann verschiedene Querschnittsformen, umfassend oval, rechteckig, die Form einer Aschenbahn und dergleichen, aufweisen. Dies nimmt zahlreiche verschiedene Austauschherzklappen auf, umfassend mechanische und biologische Prothesen sowie Homotransplantat- und Allotransplantat-Gewebsklappen. Es wird jedoch verstanden werden, dass bestimmte Austauschklappen zusammenklappbar sind oder eine ausreichend kleine Größe aufweisen, sodass der Durchgang50 in der Zugangskanüle22 einen runden oder quadratischen Querschnitt aufweisen kann und stets das Durchführen der Austauschklappe durch den Durchgang gestattet. Dennoch ist eine Querschnittsform, in der die Höhe größer als die Breite ist, immer noch von Vorteil, da sie mehr Bewegungsfreiheit beim Manipulieren der Austauschklappe und anderer Instrumente, die durch den Durchgang50 eingeführt werden, lässt. - In
3 ist zu sehen, dass, nachdem alle Fäden198 aus der Brusthöhle wieder herausgezogen und in die Schlitze62 des Führungsrings30 gelegt wurden, die Fäden an der Austauschklappe36 angebracht werden, die durch die Einführungsvorrichtung in Position gehalten wird. Die Austauschklappe36 kann von jeder beliebigen Art im Handel erhältlicher Prothesen sein, umfassend mechanische und biologische Prothesen, mit oder ohne Gerüst, wie in Bodnar and Frater, Replacment Cardiac Valves, 4–7, und in Jamieson, "Modern Cardiac Valve Devices-Bioprostheses and Mechanical Prostheses: State of the Art", J. Card. Surg. 8, 89–98 (1993) beschrieben wird. Mechanische Klappen können vom Kugelventiltyp, wie die Starr-Edwards-Klappe (Baxter Healthcare Corp., Edwards CVS Div., Irvine, CA), vom Kippscheibentyp, wie die Medtronic-Hall-Klappe (Medtronic, Inc. Minneapolis, MN), die Björk-Shiley-Monostrut-Klappe (Shiley, Inc., Irvine, CA), die Omniscience®-Klappe (Omniscience Medical Inc., Grove Heights, MN) sowie vom Zweiflügeltyp, wie die St.-Jude-Medical-Klappe (St. Jude Medical, Inc., St. Paul, MN), die Baxter-Duromedics-Klappe (Baxter Healthcare Corp., Edwards CVS Div., Irvine, CA), die Carbomedics-Klappe (Carbomedics, Inc., Austin, TX), oder die Sorin-Klappe (Sorin Biomedica, Saluggia, Italien), sein. Bioprothesen-Klappen können porcine Aortenklappen wie die Nancock-II-Bioprthese (Medtronic, Inc., Minneapolis, MN), die Carpentier-Edwards-supraringförmige Bioprothese (Baxter Healthcare Corp., Edwards CVS Div., Irvine, CA), die Carpentier-Edwards- gerüstlose Bioprothese (Baxter Healthcare Corp., Edwards CVS Div., Irvine, CA), die St.-Jude-Bioimplant- Bioprothese (St. Jude Medical, Inc., St. Paul, MN) oder die Medtronic Intact®-Bioprothese (Medtronic, Inc., Minneapolis, MN) sowie perikardiale Klappen wie die Mitroflow-Bioprothese (Mitroflow International, Inc., Richmond, British Columbia, Kanada) oder die Carpentier-Edwards- perikardiale Bioprothesen (Baxter Healthcare Corp., Edwards CVS Div., Irvine, CA) sein. Die Erfindung erleichtert auch Klappenaustausch mit Homotransplantaten und Allotransplantaten sowie mit einer Vielzahl an Austauschklappen, die hier nicht spezifisch aufgezählt werden. - In einer beispielhaften Ausführungsform erleichtert die Erfindung das Austauschen einer Mitralklappe eines Patienten durch eine mechanische, Zweiflügel-Austausch- klappe, wie beispielsweise durch die St.-Jude-Medical-Klappe, wie sie in den
5A –5C veranschaulicht wird. In dieser Ausführungsform umfasst die Austauschklappe36 einen ringförmigen Rahmen218 und ein Paar an Blättchen220 , die schwenkbar am Rahmen218 angebracht sind. In der offenen Konfiguration, die in den5A –5B dargestellt ist, stehen die Blättchen220 fast parallel zueinander, woraus eine Durchflussöffnung222 entsteht, durch die Blut in Pfeilrichtung224 fließen kann. Wird Flüssigkeitsdruck auf die Innenseiten226 der Blättchen220 ausgeübt, so schwenken die Blättchen220 zu einer geschlossenen Konfiguration und blockieren somit die Durchflussöffnung222 . Ein Nähring228 ist am Rahmen218 angeordnet, an dem Fäden198 zum Befestigen der Austauschklappe36 im Herzen angebracht werden können. - Wie in den
5B –5C dargestellt, kann die Austauschklappe36 auf der Einführungsvorrichtung38 zur Einführung in das Herz durch den Durchgang50 der Zugangskanüle22 montiert werden. Die Austauschklappe36 kann je nach Größe der auszutauschenden Mitralklappe verschiedene Größen aufweisen. Der Außendurchmesser des Nährings228 beläuft sich üblicherweise auf 19 mm bis 35 mm, was bei den meisten erwachsenen Patienten größer ist als die Breite des dritten, vierten, fünften oder sechsten Interkostalraums, die im Bereich von 15 mm bis 20 mm liegt. Die Höhe der Austauschklappe36 andererseits ist geringer als die Breite dieser Interkostalräume und beläuft sich üblicherweise auf etwa 8 mm bis 15 mm. Daher ist der Durchgang50 so ausgebildet, dass die Austauschklappe36 in einer Ausrichtung mit dem Ende zuerst durchgeführt werden kann, wie in5C gezeigt wird. - Nun wird die Einführungsvorrichtung
38 unter Verweis auf die6 –8 beschrieben. Die Einführungsvorrichtung38 umfasst einen Stiel230 mit einem distalen Ende232 , einem proximalen Ende234 und einem inneren Lumen236 , das dazwischen liegt. Die Länge des Stiels230 wird so ausgewählt, dass sie das Anbringen der Austauschklappe36 in der Mitralklappenposition innerhalb des Herzens des Patienten von außerhalb der Brusthöhle des Patienten aus ermöglicht, und beläuft sich üb licherweise auf zumindest etwa 20 cm, und vorzugsweise auf etwa 25 cm bis 35 cm. Ein Griff238 ist am proximalen Ende234 angebracht, und ein drehbarer Knauf240 ist wiederum am Griff238 montiert, um die Austauschklappe36 in Bezug auf den Stiel230 schwenken zu können. Ein Ziehring242 erstreckt sich proximal vom drehbaren Knauf240 , um die Austauschklappe36 von der Einführungsvorrichtung38 zu lösen. Wie in den8A –8B am besten ersichtlich ist, erstreckt sich die Stößelstange244 durch das innere Lumen236 und ist an ihrem distalen Ende248 an ein Schwenkmittel250 gekoppelt, das innerhalb eines Schlitzes252 am distalen Ende232 des Stiels230 schwenkbar montiert ist. Ein Schaft254 erstreckt sich distal vom Schwenkmittel250 und weist ein Gewinde oder andere Mittel zum Fixieren an eine Klappenhaltevorrichtung255 für die Austauschklappe36 auf. Der Knauf240 ist an einem Gewindeschaft256 fixiert, der in einer Gewindebohrung258 im Griff238 aufgenommen ist, wobei sich die Drehung des Knaufs240 auf den Gewindeschaft256 distal oder proximal, je nach Richtung der Drehung, überträgt. Die Stößelstange244 weist ein proximales Ende260 auf, das in ein distales Ende262 des Gewindeschafts256 eingreift. Eine Feder264 spannt die Stößelstange244 in einer proximalen Richtung gegen das distale Ende262 vor. Auf diese Weise zieht oder drückt die Drehung des Knaufs240 an der oder auf die Stößelstange244 , wodurch das Schwenkmittel250 so geschwenkt wird, dass sich der Schaft254 entweder distal oder lateral erstreckt. - In den
7A –7G ist ersichtlich, dass die Klappenhaltevorrichtung255 einen stationären Arm266 , der am Schaft254 befestigt ist, und einen beweglichen Arm268 umfasst, der beweglich auf den stationären Arm266 montiert ist. Jeder dieser Arme266 ,268 weist einen ringförmigen Kanal270 auf, der so ausgebildet ist, dass er in den Rahmen218 der Austauschklappe36 innerhalb des Durchflusskanals222 (5A ) eingreift. Die Arme266 ,268 sind weiters zur Einführung durch den Durchgang50 der Zugangskanüle22 dimensioniert und ausgebildet, wenn die Austauschklappe36 in den Kanälen270 gehalten wird. Ist sie an den Schaft254 an der Einführungsvorrichtung38 befestigt, wie in7A veranschaulicht wird, kann die Klappenhaltevorrichtung255 durch Drehen des Knaufs240 in Pfeilrichtung272 geschwenkt werden. So kann die Austauschklappe36 , die von der Haltevorrichtung255 gehalten wird, durch den Durchgang50 in der Zugangskanüle22 mit dem Ende zuerst eingeführt werden und dann etwa um 90° in eine zum Anbringen in Mitralklappenposition innerhalb des Herzens H geeignete Ausrichtung geschwenkt werden. - Um das Ausklinken aus der Haltevorrichtung
255 der Austauschklappe36 aus einer Position außerhalb des Körpers des Patienten zu erleichtern, wird ein Drahtzug274 an den beweglichen Arm268 durch beispielsweise einen Ankerball276 , der innerhalb einer Öffnung278 vorhanden ist, gekoppelt (siehe8A ). Der Drahtzug274 erstreckt sich durch ein inneres Lumen (nicht dargestellt) in der Stößelstange244 und ist ihrem proximalen Ende280 am Ziehring242 befestigt. Eine Feder282 innerhalb der Öffnung284 im Knauf240 spannt den Ziehring242 in einer distalen Richtung vor. So schwenkt ein Ziehen am Ziehring242 den beweglichen Arm268 , wie in7C gezeigt wird, wodurch die Austauschklappe36 aus den Kanälen270 entfernt werden kann. Der Ankerball276 und/oder der Ziehring242 können so ausgebildet sein, dass sie vom Drahtzug244 entfernt werden können, wodurch die Klappenhaltevorrichtung255 aus der Einführungsvorrichtung38 durch Entkoppeln des Arms 266 vom Schaft254 entfernt werden kann. - Um die Austauschklappe
36 auf der Haltevorrichtung255 festzuhalten, wenn die Haltevorrichtung255 nicht an der Einführungsvorrichtung38 befestigt ist, ist ein Paar an Löchern286 in Arm266 in Übereinstimmung mit einem entsprechenden Paar an Löchern288 in Arm268 vorhanden. Ist die Austauschklappe36 auf der Haltevorrichtung255 platziert worden, kann ein Faden (nicht dargestellt) durch die Löcher286 ,288 gezogen werden, um ein Schwenken des Arms268 zu unterbinden und dadurch die Austauschklappe36 an der Haltevorrichtung255 zu halten. Wenn die Haltevorrichtung255 an der Einführungsvorrichtung38 erst einmal befestigt wurde, kann der Faden entfernt werden, wodurch ein Schwenken des Arms268 in Reaktion auf ein Drehen des Knaufs240 ermöglicht wird. - Häufig wird es wünschenswert sein, dass die Klappenhaltevorrichtung
255 und die Austauschklappe36 bereits vor Anwendung zusammen angeordnet, sterilisiert und in einem sterilen Päckchen zusammen verpackt werden. So können nach dem Öffnen des sterilen Päckchens im Operationssaal die Austauschklappe36 und die Haltevorrichtung255 sofort für chirurgische Zwecke eingesetzt werden. Weiters kann es wünschenswert sein, dass die Einführungsvorrichtung38 mit der Austauschklappe36 sterilisiert und in dasselbe sterile Päckchen mit verpackt wird. In solchen Fällen kann die Haltevorrichtung255 in die Einführungsvorrichtung38 integriert und von ihr nicht abnehmbar fixiert werden, wobei die Austauschklappe36 auf die Arme 266, 268 am distalen Ende der Einführungsvorrichtung38 innerhalb des sterilen Päckchens montiert wird. Alternativ dazu kann die Einführungsvorrichtung38 eine wieder verwendbare Vorrichtung sein, die zum Zeitpunkt der Durchführung des chirurgischen Eingriffs im Operationssaal an der Haltevorrichtung255 und der Austauschklappe36 befestigt wird.
Claims (9)
- Prothesenanordnung (
38 ) für den Austausch einer Herzklappe ohne Öffnung des Brustkorbs, wobei die Prothesenanordnung Folgendes umfasst: eine Austauschklappe (36 ); und eine Haltevorrichtung (255 ), umfassend einen verlängerten Griff (230 ) mit einem distalen Ende (232 ), das an der Austauschklappe (36 ) angebracht ist, und einem proximalen Ende (234 ), das dem distalen Ende (232 ) gegenüber liegt, dadurch gekennzeichnet, dass der Griff (230 ) Mittel (250 ) zum Schwenken der Austauschklappe (36 ) von einer ersten Ausrichtung zur Einführung durch einen Rippenzwischenraum (I) in den Brustkorb des Patienten mit dem Ende zuerst zu einer zweiten Ausrichtung zur Anbringung im Herzen (H) des Patienten umfasst, wobei das Schwenkmittel (250 ) so ausgebildet ist, dass es vom proximalen Ende (234 ) des Griffs (230 ) aus betätigbar ist. - Prothesenanordnung nach Anspruch 1, worin die Klappenprothese einen Befestigungsring (
228 ) mit einer Achse aufweist und die Haltevorrichtung (255 ) so ausgebildet ist, dass sie am Griff (230 ) derart befestigbar ist, dass die Längsachse des Griffs etwa im rechten Winkel zur Achse des Befestigungsrings steht, wenn sich die Klappenprothese in ihrer zweiten Ausrichtung befindet. - Prothesenanordnung nach Anspruch 1 oder 2, worin der Griff (
230 ) eine Länge von zumindest 20 cm aufweist. - Prothesenanordnung nach einem der Ansprüche 1 bis 3, worin die Klappenprothese (
36 ) eine mechanische Klappenprothese ist. - Prothesenanordnung nach einem der vorangegangenen Ansprüche, die außerdem einen Ausklinkmechanismus umfasst, der so ausgebildet ist, dass er die Klappenprothese (
36 ) von der Haltevorrichtung (255 ) löst. - Prothesenanordnung nach einem der Ansprüche 1 bis 5, worin die Klappenprothese (
36 ) innerhalb eines Winkels von etwa 90°C schwenkbar ist. - Prothesenanordnung nach einem der vorangegangenen Ansprüche, worin die Klappenprothese (
36 ) einen Außendurchmesser von zumindest 20 mm aufweist. - Prothesenanordnung nach einem der Ansprüche 1 bis 7, worin die Klappe (
36 ) und die Haltevorrichtung (255 ) in einem sterilen Päckchen verpackt sind. - Prothesenanordnung nach einem der Ansprüche 1 bis 8, worin die Austauschklappe (
36 ) einen ringförmigen Befestigungsabschnitt und einen beweglichen Klappenabschnitt, der an den Befestigungsabschnitt gekoppelt ist, aufweist.
