Description
- Die vorliegende Erfindung betrifft eine neue pharmazeutische Form auf der Basis eines therapeutischen Mittels mit einer antitumoralen und antileukämischen Wirkung. Sie betrifft ganz besonders einen neue injizierbare Form, die Produkte aus der Familie der Taxane enthält, wie insbesondere Taxol oder eines seiner Analogen oder Derivate der folgenden allgemeinen Formel (I)
- In der Formel (I) stellt R ein Wasserstoffatom oder einen Acetylrest dar und das Symbol R1 bedeutet einen Rest tert.-Butoxycarbonylamino oder Benzoylamino. Man bevorzugt unter der Gesamtheit dieser Verbindungen die zwei Derivate, in denen R eine Gruppe Acetyl und R1 eine Gruppe Benzoylamino darstellt und diejenigen, worin R ein Wasserstoffatom und R1 einen Rest tert.-Butoxycarbonylamino bedeutet.
- Die erste dieser zwei Verbindungen ist bekannter unter der Bezeichnung Taxol und die zweite bekannter unter der Bezeichnung Taxoter.
- Diese Produkte weisen in vivo eine bemerkenswerte Wirkung bei malignen Tumoren auf, was es ermöglicht hat, sie im Hinblick auf die Behandlung resistenter Erkrankungen mit allen anderen Antikrebs-Therapien zu untersuchen.
- Unglücklicherweise besitzen diese Produkte nur eine geringe Löslichkeit in Wasser und es wurde notwendig, eine Formulierung für die injizierbare Präparation auf der Basis eines oberflächenaktiven Mittels und Ethanol herzustellen. Das Ethanol ist das beste Lösungsmittel, das es gestattet, die Moleküle der Formel (I) aufzulösen.
- Als Beispiel wurde gemäß der Veröffentlichung von Rowinsky, Lorraine, Cazenave und Donehower, erschienen in Journal of the National Cancer Institute, Vol. 82, Nr. 15, S. 1247–1259 vom 1. August 1990 eine erste Lösung hergestellt, genannt "Mutterlösung", die etwa 6 mg/ml Taxol in einer Lösungsmittel-Mischung enthält, die zusammengesetzt ist aus:
- – 50 Vol.-% Ethanol,
- – 50 Vol.-% Cremophor®EL.
- Bei der Injektion wird diese Lösung mit einer Perfusionsflüssigkeit vermischt, die Natriumchlorid oder Dextrose enthält. Um sowohl vom physikalischen als auch vom chemischen Standpunkt her eine stabile Mischung zu erhalten, sprechen die Autoren in diesem Artikel davon, daß man die Konzentration an Wirkstoff in der Perfusionslösung begrenzen muß, und zwar auf Konzentrationen von etwa 0,03 bis 0,6 mg/ml (siehe obige Publikation S. 1251, Spalte 1, 3. Abschnitt).
- So ist es mithin wünschenswert, ausreichende Dosen an Wirkstoff injizieren zu können, und die Mediziner wünschen daher, Konzentrationen an Wirkstoff zwischen etwa 0,3 und 1 mg/ml in der Perfusionsflüssigkeit verabreichen zu können, wobei oberhalb dieser Dosierungen schwer zu beherrschende Phänomene des anaphylaktischen Schocks auftreten, verursacht durch den notwendigen Bestandteil Cremophor® (siehe die Publikation von Rowinsky S. 1250, Spalte 2, letzter Abschnitt). 100
- Um nach dieser Veröffentlichung derartige Konzentrationen (zwischen 0,3 und 1 mg/ml) zu erhalten, ist es erforderlich, Lösungen zu injizieren, die gleichzeitig wie den Wirkstoff Konzentrationen von jeder der folgenden Verbindungen enthalten, Ethanol und vor allem Cremophor® von etwa 8 g pro 100 ml Lösung. Die Behandlung, die oft die Verabreichung von hohen Wirkstoffdosen erfordert, hat bei der Injektion großer Volumina wegen der Tatsache, daß die Konzentration an Wirkstoff in der Lösung relativ gering ist, zur Folge, während der Behandlung zusätzlich zu den anaphylaktischen Erscheinungen auch das Auftreten einer Ethylvergiftung hervorzurufen.
- Es wurde durch den Einsatz der pharmazeutischen Formen der vorliegenden Erfindung herausgefunden, daß man das Cremophor® fortlassen und die Konzentrationen an Ethanol stark herabsetzen kann.
- Deshalb wurde eine Mutterlösung hergestellt, die den Wirkstoff in einer Mischung von Lösungsmitteln enthält, zusammengesetzt aus Ethanol als bestem biokompatiblen Lösungsmittel für die Wirkstoffe aus der Klasse der Taxane und einem oberflächenaktiven Mittel, ausgewählt unter den Polysorbaten, die insbesondere unter der Bezeichnung Tween gehandelt werden.
- Die Mutterlösung wird durch Auflösen des Wirkstoffes in Ethanol und anschließende schrittweise Zugabe des oberflächenaktiven Mittels hergestellt. Auf diese Weise kann man Lösungen herstellen, die 10 bis 100 mg/ml Wirkstoff in einer Mischung enthalten, die ihrerseits etwa 50% oberflächenaktives Mittel umfaßt.
- Der Gegenstand der vorliegenden Erfindung ist der folgende: Das in der Veröffentlichung des Journal of National Cancer Institute beschriebene Cremophor® wurde durch ein Polysorbat ersetzt. Wenn man nämlich eine injizierbare Lösung im klinischen Bereich verwendet, die Ethanol und als oberflächenaktives Mittel Polysorbat 80 anstelle von Cremophor® enthält, so stellt man fest, daß die anaphylaktischen Reaktionen im Verhältnis zur Anwendung der gleichen, mit Cremophor® hergestellten Lösung stark verringert sind. Außer diesem beträchtlichen Vorteil kommt hinzu, daß sich in überaus erstaunlicher Weise herausgestellt hat, daß in den Fläschchen der Mutterlösung die Konzentration an Wirkstoff 15 mg/ml erreichen kann. Die Perfusionslösung enthält nach der Verdünnung dieser Fläschchen eine Menge an Ethanol und eine Menge an oberflächenaktivem Mittel, fast etwas weniger von beiden.
- Die ausgehend von den vorstehenden Mutterlösungen hergestellten Perfusionen, die vorteilhafterweise eine Konzentration an Wirkstoff von beispielsweise 1 mg/ml aufweisen, enthalten weniger als 50 ml/l Polysorbat und Ethanol, was eine Verringerung von etwa 40% darstellt, bezogen auf die Perfusionen des Standes der Technik.
- Diese Perfusionen sind vom physikalischen Standpunkt her stabil, das heißt, daß man keine Erscheinung eines Niederschlages vor etwa 8 Stunden beobachtet.
- Die Perfusionen von Taxol oder Taxoter werden anschließend dem Menschen mit einem vorbestimmten Durchsatz injiziert, in Abhängigkeit von der Menge an Wirkstoff, die man zu verabreichen wünscht. Mit diesen Lösungen beobachtet man keine Erscheinungen eines anaphylaktischen Schocks, der mit den Lösungen des Standes der Technik jedoch zu verzeichnen ist.
- Die Erfindung wird noch vollständiger mit Hilfe der folgenden Beispiele beschrieben, die jedoch nicht als Einschränkung der Erfindung betrachtet werden sollen.
- BEISPIELE GEMÄSS DER ERFINDUNG
- BEISPIEL 1
- Man löst 0,450 g Taxoter in 15 ml Ethanol. Dann vervollständigt man mit Polysorbat 80 bis auf 30 ml, um eine Lösung mit einem Gehalt von 15 mg/ml Taxoter zu erreichen. Die physikochemische Stabilität dieser Lösung ist zufriedenstellend.
- Diese Lösung enthält nach Vermischen mit einer Lösung von Glucose (5%) in der Weise, daß man eine Endkonzentration von 1 mg/ml erhält, etwa 33 ml/l Polysorbat 80 und 33 ml/l Ethanol.
- Die Perfusion ist mehr als 21 Stunden lang stabil. Man versteht unter physikalischer Stabilität die Tatsache, dass während dieser Periode keine Ausfällungserscheinungen zu beobachten sind.
- BEISPIEL 2
- Man wiederholt das gleiche Beispiel mit einer Ausgangskonzentration von 10 mg/ml Taxoter. Die Ergebnisse sind in der Tabelle 1 angegeben.
- VERGLEICHSBEISPIEL GEMÄSS STAND DER TECHNIK
- Man löst 0,180 g Taxol in 15 ml Ethanol. Dann vervollständigt man mit Cremophor®, um 30 ml einer Lösung zu erhalten, die 6 mg/ml Taxol enthält.
- Diese Lösung wird in der gleichen Perfusionslösung wie oben verdünnt, und zwar in einem Verhältnis von 1 mg/ml, so daß die Perfusionslösung 87,7 ml/l Cremophor® und 87,7 ml/l Ethanol enthält.
- Die Perfusionslösung ist mehr als 21 Stunden lang stabil.
- BEISPIEL GEMÄSS DER ERFINDUNG MIT TAXOL
- Man wiederholt das gleiche Beispiel wie das des Standes der Technik, indem man das Cremophor® durch Polysorbat 80 ersetzt. Die erhaltenen Ergebnisse der Stabilität sind in der Tabelle 1 angegeben.
- BEISPIEL 3
- Man löst 65 g Taxoter in 2083 ml Ethanol. Dann stellt man durch Zugabe von 2083 ml Polysorbat 80 auf 4147 ml ein. Anschließend homogenisiert man durch mechanisches Rühren und filtriert über ein Filter mit der Porenweite von 0,2 μm. Man erhält eine Lösung mit etwa 15 mg/ml Taxoter.
- Diese Lösung, die nach Verdünnen auf einen Gehalt an Taxoter von 1 mg/ml in einer Perfusionstasche 5 Dextrose enthält, ist mindestens 96 Stunden lang stabil.
Claims (6)
- Neue Zusammensetzungen auf der Basis von Produkten der Taxan-Klasse in Lösung einer Mischung von Ethanol und Polysorbat.
- Zusammensetzungen nach Anspruch 2, dadurch gekennzeichnet, daß in der Verbindung der Formel (I) R eine Wasserstoffgruppe und R1 einen Rest tert.-Butoxycarbonylamino darstellt.
- Zusammensetzungen nach Anspruch 2, dadurch gekennzeichnet, daß in der Verbindung der Formel (I) R eine Gruppe Acetyl und R1 einen Rest Benzoylamino darstellt.
- Zusammensetzungen nach irgendeinem der Ansprüche 2 bis 4, dadurch gekennzeichnet, daß sie zwischen 6 und 15 mg/ml Verbindung der Formel (I) enthalten.
- Perfusion, dadurch gekennzeichnet, daß sie etwa 1 mg/ml Verbindung der Formel (I) und daß sie weniger als 35 ml/l Ethanol und weniger als 35 ml/l Polysorbat enthält.
