  • Diese Anmeldung beansprucht die Priorität der am 9. März 2001 beim U.S. Patentamt eingereichten vorläufigen Patentanmeldung Serial Nr. 60/274,843 und der am 26. April 2001 beim U.S. Patentamt eingereichten vorläufigen Patentanmeldung Serial Nr. 60/286,863.
  • Technisches Gebiet
  • Diese Erfindung betrifft allgemein chirurgische Netze zur Verwendung als medizinische Schlinge, wie ein Beckenboden-Ausbesserungsnetz, Verfahren zur Herstellung solcher Netze, Ausrüstungen, die solche Netze einschließen.
  • Hintergrundinformationen
  • Ein chirurgisches prothetisches Netz ist verwendet worden, um Gewebe oder Organe zu behandeln oder zu verstärken, die geschädigt, prolabiert, geschwächt oder auf andere Weise einem Bruch ausgesetzt sind, wie zum Beispiel bei den Leiden Rektozele, Zystozele, Enterozele, Scheidenvorfall und Protozele. Ein Prolaps bezieht sich auf das Abgleiten eines Organs oder Organteils aus seiner normalen Position. Zum Beispiel bezieht sich ein Prolaps des Rektums auf das Vorstülpen der Innenseite des Rektums durch den Anus. Eine Rektozele ist der Prolaps des Rektums in das Perineum. Ein Prolaps des Uterus bezieht sich auf das Fallen des Uterus in die Vagina aufgrund einer Dehnung und Schlaffheit seiner Haltestrukturen. Ein Scheidendachprolaps bezieht sich auf den Prolaps des Kopfendes des Scheidenkanals in Richtung, durch und über den Scheideneingang hinaus. Eine Zystozele (d. h. Vesikozele) ist ein Bruch, der durch die Abwärts- und Rückwärtsverlagerung der Harnblase in Richtung der Vaginalöffnung gebildet wird, wobei er zumeist durch eine Schwächung der Muskulatur während der Entbindung verursacht wird. Jedoch wird jegliche abnormale Absenkung der vorderen Scheidenwand und des Blasengrundes in Ruhe oder unter Beanspruchung als eine Zystozele betrachtet. Eine Enterozele ist ein Bruch der Eingeweide, obwohl der Begriff auch benutzt wird, um sich speziell auf eine Hernienbildung des Beckenperitoneums durch den Douglas-Raum zu beziehen (d. h. posterior vaginal, rektovaginal, Cul-de-Sac- oder Douglas-Raum-Hernie).
  • Chirurgische Netze können auch verwendet werden, um vorübergehend Gewebe aufzuhängen oder Körperorgane zurückzuziehen, z. B. während einer Operation. Zum Beispiel beschreibt das U.S. Patent Nr. 4,973,300 die Verwendung einer Herzschlinge zum Halten des Herzens während einer Operation; und das U.S. Patent Nr. 5,362,294 beschreibt das Zurückziehen von Körperorganen, wie die Gebärmutter oder den Darm, während einer laparoskopischen Operation; das U.S. Patent Nr. 6,102,921 beschreibt die Verwendung einer medizinischen Anastomose-Schlinge zur Verwendung bei der Ausbesserung oder bei der Regeneration von Nerven.
  • Synthetische Netzmaterialien, die als Schlingen zur Behandlung oder Verstärkung von Patientengeweben für diese und viele andere Leiden genutzt werden, können Patienten-Komplikationen verursachen, wie Erosion, was zumindest teilweise auf die scharfen Überstände an den Rändern des Netzes zurückzuführen ist, die während des Herstellungsprozesses oder danach (zum Beispiel, wenn ein Arzt das Material schneidet oder abschert oder das Material auf andere Weise formt) gebildet werden. Diese Überstände können eine irritierende Wirkung haben, die zu einer Erosion führen kann, wenn sie umgebendes Gewebe berühren. Daher existiert ein Bedarf an einer Schlinge, welche die Irritation und Erosion des Gewebes minimiert, welches das von ihr gehaltene Gewebe umgibt.
  • Stressbedingte Harninkontinenz (SUI), die in erster Linie Frauen betrifft, ist ein Leiden, das unter Verwendung von chirurgischen Schlingen erfolgreich behandelt wird. Stressbedingte Harninkontinenz wird im Allgemeinen durch zwei Leiden verursacht, die unabhängig oder in Kombination auftreten können, nämlich intrinsische Sphinkterdefizienz (ISD) und Hypermobilität. Bei der ISD schließt sich die in der Harnröhre angeordnete Harnschließmuskelklappe nicht mehr richtig (passt nicht mehr richtig zusammen), was bewirkt, dass während belastender Tätigkeiten Harn aus der Harnröhre entweicht. Hypermobilität ist ein Leiden, bei dem der Beckenboden gedehnt, geschwächt oder geschädigt ist, was bewirkt, dass sich der Blasenhals und die proximale Harnröhre ansprechend auf eine Zunahme des intraabdominalen Drucks (z. B. aufgrund von Niesen, Husten, Belastung usw.) drehen und absenken, was zu einer nicht ausreichenden Reaktionszeit führt, um ein Schließen der Harnröhre zu fördern, und folglich zu einem Entweichen von Harn und/oder einem Fluss von Harn.
  • Biologische Faktoren, die eine Hypermobilität beeinflussen können, schließen ein: ein schlechter Muskeltonus der Fascia endopelvina (durch Alter oder eingeschränkte Aktivität), Muskeldehnung/Reißen der Fascia endopelvina durch Trauma (z. B. Entbindung), Lösen (Lateraldefekt) von Fascia endopelvina/Arcus tendineus (Muskel/Band), Hormonmangel (Östrogen), in Kombination vorhandene Schäden (Zystozele, Enterozele, Harnröhrenvorfall), sowie Scheidenvorfall. Traditionelle Behandlungsverfahren schließen Blasenhals-Stabilisierungsschlingen ein, bei denen eine Schlinge unter der Harnröhre oder dem Blasenhals platziert wird, um eine Plattform bereitzustellen, die eine Überdehnung verhindert. Eine neu auftauchende alternative Behandlung ist die Platzierung einer Schlinge unter der Mitte der Harnröhre. Eine solche Schlingenplatzierung nutzt das Hypermobilitätsleiden aus, indem sie einen Drehpunkt bereitstellt, um den sich die Harnröhre und der Blasenhals drehen und einen "Harnröhrenknick" liefern werden, um das normale Verschließen der Harnröhre zu unterstützten.
  • Schlingen werden traditionell unter dem Blasenhals platziert, um eine Harnröhrenplattform bereitzustellen, die ein Absenken der Fascia endopelvina begrenzt, während sie eine Kompression für den Harnröhrenschließmuskel liefert, um das Zusammenpassen zu verbessern. Der Ort der Platzierung in der Mitte der Harnröhre liefert einer weniger beweglichen anatomischen Struktur mechanische Stabilität. Blasenhalsschlingen sind am gewünschten Ort traditionell unter Verwendung eines Knochenverankerungsverfahrens befestigt worden. Schlingen unter der Mitte der Harnröhre, die in einem Bereich geringer Mobilität platziert sind, haben die Wirksamkeit eines ankerlosen Ansatzes gezeigt. Sobald man erkannt hat, dass eine minimale Zugkraft, falls überhaupt, notwendig ist, braucht ein Arzt die Schlinge nur durch die Fascia endopelvina gesichert unter der mittleren Harnröhre zu platzieren, um die Schlinge dauerhaft in ihrer Lage zu sichern. Die Schlinge gestattet durch die Netzöffnungen und Netzüberstände eine sofortige Gewebesicherheit, um am Anfang eine Schlingenstabilisierung aufrechtzuerhalten. Während die Heilung stattfindet, bilden das Gewebe der Fascia endopelvina und der Fascia recti neue Gefäße aus und wachsen neu in das gewirkte Muster des Netzes hinein und um dieses herum. Bei diesem Verfahren liefert die Schlinge einen Drehpunkt, um den sich der Beckenboden senken wird (wobei das Hypermobilitätsleiden des Patienten ausgenutzt wird), sowie einen "Knick" in der Harnröhre oder einen höheren Widerstand, um den Harnstrom während Hochbelastungszuständen zu blockieren.
  • Obwohl daher Überstände vorteilhaft zur Behandlung von SUI beitragen können, können sie auch Patientenkomplikationen verursachen, wie Erosion der Vagina oder Harnröhre.
  • Die WO 00/74594 und die DE 703123 offenbaren Schlingen, welche die oben erwähnten Probleme aufweisen.
  • Zusammenfassung der Erfindung
  • Die vorliegende Erfindung betrifft chirurgische Netze oder Schlingen mit einem Abschnitt ohne Überstände (d. h. Überstände sind nicht ausgebildet, geglättet, gerundet oder entfernt), der an einem Teil der Seiten des Netzes angeordnet ist, Verfahren zur Herstellung solcher Netze, medizinische Ausrüstungen, die solche Netze einschließen.
  • Die Vorteile einer solchen Schlinge gemäß der Erfindung schließen eine verminderte Gewebeirritation durch einen Abschnitt ohne Überstände ein, wenn er sich im Kontakt mit Gewebe befindet, wie Harnröhren- und Scheidengewebe, während um den mit Überständen versehenen Teil der Schlinge herum eine schnelle Narbengewebebildung gefördert wird. Die Bildung von Narbengewebe fügt im Allgemeinen Masse hinzu, die das Gewebe zusammendrückt, an dem sie angebracht wird (z. B. die Harnröhre), liefert eine Abstützung, um die Patientenkontinenz zu verbessern, und behindert oder verhindert eine Bewegung der platzierten Schlinge im Anschluss an die Platzierung.
  • In einem Aspekt betrifft die Erfindung eine Schlinge zur Verwendung bei einer medizinischen Anwendung. Die Schlinge ist aus einem Netzmaterial hergestellt, das ein erstes und ein zweites Ende einschließt, die entlang einer Längsachse zueinander entgegengesetzt sind (d. h. gegenüber und weg voneinander angeordnet). Das Netzmaterial schließt auch eine erste und eine zweite Seite ein, die zueinander entgegengesetzt sind, wobei sie entlang einer zur Längsachse senkrechten, oder im Wesentlichen senkrechten, Achse durch einen Abstand getrennt sind. Die senkrechte Achse schneidet die Längsachse am Mittelpunkt, oder im Wesentlichen am Mittelpunkt, der senkrechten Achse. Ein Teil der ersten und der zweiten Seite und des ersten und des zweiten Endes des Materials enthält Überstände. Ein Teil der ersten und der zweiten Seite enthält keine Überstände (z. B. Überstände an der ersten und der zweiten Seite sind nicht ausgebildet, geglättet, gerundet oder entfernt), wobei ein Abschnitt ohne Überstände erzeugt wird. Die erste und die zweite Seite können jeweils, zum Beispiel, einen etwa 1 cm bis etwa 5 cm langen, entlang der Längsachse zentrierten Abschnitt ohne Überstände aufweisen.
  • Die Schlinge der Erfindung kann eine Form aufweisen, die für eine medizinische Anwendung geeignet ist; sie kann z. B. rechteckig oder im Wesentlichen rechteckig sein. Alternativ kann die Schlinge achteckig, sechseckig, trapezförmig, elliptisch sein, oder irgendeine andere Form besitzen, die für den vorgesehenen Platzierungsort der Schlinge innerhalb des Körpers geeignet ist.
  • In einem anderen Aspekt betrifft die Erfindung ein Verfahren zur Herstellung einer Schlinge durch unmittelbare Herstellung mit einem Abschnitt ohne Überstände oder durch Glätten, Runden oder Entfernen der Überstände an einem Teil der Schlinge, um einen Abschnitt ohne Überstände zu erzeugen.
  • Das vorgesehene Schlingenmaterial kann aus synthetischen Materialien oder einer Kombination von Säugetiergewebe(n) und synthetischem Material (synthetischen Materialien) stammen. Das Verfahren zur Herstellung der Schlinge kann weiter ein Sterilisieren des Schlingenmaterials entsprechend den auf dem Fachgebiet bekannten Verfahren umfassen, so dass die Schlinge zur Verwendung bei verschiedenen medizinischen Anwendungen geeignet ist, und kann ein Verpacken der Schlinge in einer sterilen Aufnahme einschließen. Das Schlingenmaterial kann in einer Hülle eingeschlossen sein, um die Handhabung der Schlinge zu unterstützen und/oder die Schlinge während der chirurgischen Platzierung zu justieren, oder um zu verhindern, dass sich die Schlinge aufgrund der Handhabung vor der Platzierung innerhalb des Körpers des Patienten dehnt oder missgestaltet wird.
  • Bei einem Verfahren zum Behandeln eines geschädigten Teils des Körpers eines Patienten unter Verwendung einer Schlinge mit einem Abschnitt ohne Überstände wird die Schlinge so im Inneren des Körpers eines Patienten platziert, dass ihre senkrechte Achse im Wesentlichen entlang eines Teils des Körpers des Patienten liegt, wie mittlere Harnröhre, Blase, Rektum, Scheide, Blutgefäß, Nerv, Herz usw.; das Material stützt einen Teil des Körpers des Patienten in einer Weise ab, die das umgebende Gewebe nicht erodiert. Die Schlinge kann unter Verwendung der senkrechten Achse der Schlinge als Führung am geschädigten Teil des Körpers des Patienten zentriert werden. Mit dem gesicherten Schlingenmaterial kann Druck gleichmäßig auf einen Teil des Körpers eines Patienten verteilt werden. Ein chirurgischer Verbinder, wie ein Nahtmaterial, eine Klemme, ein Knochenanker, eine Klammer oder ein anderer geeigneter Verbinder, kann verwendet werden, um die Schlinge an anatomischen Strukturen zu befestigen.
  • Das Schlingenmaterial kann implantiert werden, um weibliche Harninkontinenz entsprechend transvaginalen, transabdominalen oder kombinierten transvaginalen und transabdominalen Methoden zu behandeln. Zum Beispiel kann das Verfahren verwendet werden, um einen Patienten mit SUI zu behandeln, wobei der Abschnitt der Schlinge ohne Überstände benachbart zur mittleren Harnröhre des Patienten platziert wird.
  • Kurze Beschreibung der Zeichnungen
  • In den Zeichnungen beziehen sich gleiche Bezugszeichen über die verschiedenen Ansichten hinweg allgemein auf dieselben Teile. Die Zeichnungen sind nicht notwendigerweise maßstabsgetreu, sondern veranschaulichen stattdessen die Prinzipien der Erfindung.
  • 1 ist eine Draufsicht auf eine rechteckige Ausführungsform einer Schlinge mit einem Abschnitt ohne Überstände an jeder Seite der senkrechten Achse.

  • Beschreibung
  • Bezug nehmend auf 1, kann eine Schlinge 20 gemäß der vorliegenden Erfindung aus einem oder mehreren Materialien 22 hergestellt sein, und schließt ein erstes Ende 24 und ein zweites Ende 26 ein. Das zweite Ende 26 ist entlang einer Längsachse 28 gegenüber und weg von dem ersten Ende 24 angeordnet. Das Material 22 schließt auch eine erste Seite 30 und eine zweite Seite 32 ein. Die zweite Seite 32 ist entlang einer senkrechten Achse 34 gegenüber und weg von der ersten Seite 30 angeordnet. Die Achse 34 ist senkrecht zu, oder im Wesentlichen senkrecht zu, der Längsachse 28 und schneidet die Längsachse 28 am Mittelpunkt, oder im Wesentlichen am Mittelpunkt, der Achse 28. Die Längsachse 28 der Schlinge 20 kann im Bereich von etwa 2,5 cm bis etwa 45 cm lang sein, während die senkrechte Achse 34 im Bereich von etwa 1,0 cm bis etwa 3,0 cm liegen kann. Die Schlinge ist vorzugsweise 20 bis 30 cm lang und 1 bis 3 cm breit, obwohl in Abhängigkeit von der Größe des Patienten und dem Flächeninhalt des Körperteils, das einer Abstützung bedarf, größere und kleinere Schlingen ins Auge gefasst werden.
  • Die Schlinge 20 und 21 kann rechteckig sein, wie in 1 dargestellt, oder von im Wesentlichen rechteckiger Form (z. B. achteckig). Alternativ kann die Schlinge eine andere Form besitzen (z. B. trapezförmig, sechseckig oder elliptisch), die für ihren vorgesehenen Platzierungsort innerhalb des Körpers geeignet ist. Beispielhafte Formen sind im U.S. Patent Nr. 6,042,534 beschrieben.
  • Die Dicke des Schlingenmaterials 22 kann über das ganze Stück des Materials gleichförmig sein, oder sie kann an einem oder mehreren verschiedenen Orten variieren. Die Dicke des Schlingenmaterials 22 kann im Bereich von etwa 0,02 bis etwa 0,10 cm liegen, wird jedoch typischerweise etwa 0,07 cm betragen und eine gleichförmige Dicke besitzen. Die Materialkonstruktion kann einen Einfluss auf die Materialdicke und -gleichförmigkeit haben; zum Beispiel kann ein Gewebe dickere Bereiche aufweisen, wo sich die Fasern kreuzen.
  • Das Netz kann eine beliebige von einer Reihe von Gewirken, Geweben oder Geflechten aufweisen, wie diejenigen, die in den U.S. Patenten Nr. 5,569,273 ; 5,292,328 ; 5,002,551 ; 4,838,884 ; 4,655,221 ; 4,652,264 ; 4,633,873 ; 4,520,821 ; 4,452,245 ; 4,347,847 ; 4,193,137 ; 5,124,136 ; 3,054,406 und 2,671,444 beschrieben sind.
  • Das Netzmaterial kann aus einem beliebigen von einer Reihe von biokompatiblen Materialien gefertigt werden, wie Nylon, Polyethylen, Polyester, Polypropylen, Fluorpolymeren, Copolymeren davon, Kombinationen davon, oder anderem geeignetem synthetischem Material (anderen geeigneten synthetischen Materialien). Das Material kann zum Beispiel ein synthetisches Material sein, das durch den Körper des Patienten absorbierbar ist. Geeignete absorbierbare synthetische Materialien schließen Polyglykolsäure, Polymilchsäure und andere geeignete absorbierbare synthetische Materialien ein. Das Netzmaterial kann aus einem oder mehreren Garnen gefertigt werden, welche Garne aus einem oder mehreren Materialien hergestellt werden können. Das Netz kann gemäß zahlreichen Fertigungsverfahren hergestellt werden, und kann ausgelegt werden, um eine schnelle Gefäßneubildung im Gewebe und ein schnelles Einwachsen von Gewebe zu erlauben, indem es große Lückenräume aufweist. Zum Beispiel kann jedes Garn des Netzes leere Bereiche zwischen Garnfilamenten aufweisen, und das Fertigungsverfahren kann Spalte erzeugen. Eine beispielhafte Webart ist ein Trikotgewirk mit zwei Garnen pro Nadel, wie in Figur dargestellt. Bei einer bevorzugten Ausführungsform besteht das Netz aus Polypropylen-Monofilamentgarnen.
  • Absorbierbare synthetische Materialien können ebenfalls zur Verwendung gemäß der Erfindung geeignet sein. Solche absorbierbaren synthetischen Materialien schließen zum Beispiel Polyglykolsäure (PGA), Polymilchsäure (PLA) und andere verfügbare absorbierbare synthetische Materialien ein. Ein geeignetes PGA-Material ist unter der Handelsbezeichnung DEXON von TYCO erhältlich. Andere geeignete polymere und nicht-polymere synthetische Materialien können gemäß der Erfindung verwendet werden. Beispielhafte Materialien, wie oben angegeben, findet man in den U.S. Patenten Nr. 6,090,116 ; 5,569,273 ; 5,292,328 ; 4,633,873 ; 4,452,245 ; 4,347,847 ; 3,124,136 ; 3,054,406 und 2,671,444 sowie bei C.B. Inglesia et al. (1997) Int. Urogynecol. J. 8: 105–115.
  • Alternativ kann das Schlingenmaterial 22 aus einer Kombination von Säugetiergewebe(n) und synthetischem Material (synthetischen Materialien) stammen. Die Quelle von Säugetiergewebe kann zum Beispiel menschliches Gewebe, Leichengewebe oder durch Gewebeengineering hergestelltes menschliches Gewebe sein. Das Säugetiergewebe kann alternativ von einer tierischen Quelle stammen, wie Schweine-, Schaf-, Rinder- und Pferde-Gewebequellen. Solche Kombinationen können auch Materialien einschließen, die sowohl synthetische Polymere und tierische Zellen enthalten, welche behandelt werden, so dass das Kollagen oder andere normalerweise antigene Fasern der Tierzellen vernetzt werden. Bei einer Ausführungsform umfasst mindestens ein Teil des Netzteils der Schlinge, der das Gewebe des Patienten berührt, ein synthetisches Material, was eine Glattheit der Überstände erforderlich macht.
  • Die Überstände (d. h. scharfe Vorsprünge oder ausgefranste Ränder) 40, die sich bilden, wenn das Material 22 geschnitten, kleingeschnitten, zerrissen, durchgescheuert oder auf andere Weise hergestellt wird, können entlang von jeglichem Rand des Materials 22 angeordnet sein. Die Überstände 40 sind im Allgemeinen nützlich, um ein Gewebewachstum in das Material 22 anzuregen. Jedoch hat man gefunden, das einige Überstände 40 das benachbarte Gewebe erodieren können, wenn die Schlinge 20 in den Körper eines Patienten eingesetzt wird. Dementsprechend ist ein Teil der an den Seiten 30 und 32 angeordneten Überstände 40 (z. B. bei einigen Ausführungsformen bis innerhalb von etwa 1 cm bis etwa 5 cm von jeder Seite der senkrechten Achse 34) nicht ausgebildet, geglättet, gerundet oder entfernt, um einen Abschnitt 42 ohne Überstände zu bilden. Indem man diese zu Reizungen führenden Vorsprünge entfernt, die sich in enger Nähe der Harnröhre und der vorderen Scheidenwand befinden werden, werden die Erosionswirkungen verringert.
  • Mit fortgesetzter Bezugnahme auf 1, ist bei einer Version der Schlinge eine Linie 36 entlang der senkrechten Achse 34 einer rechteckigen Schlinge 20 angeordnet. Die Linie 36 kann zum Beispiel durch Aufbringen von chirurgischer Tinte entlang der senkrechten Achse 34 des Materials 22 gebildet werden. Vorzugsweise schneidet sich der ungefähre Mittelpunkt der Abschnitte 42 ohne Überstände der Seiten 30 und 32 mit der Linie 36. Daher kann die Linie 36 als visuelle Führung verwendet werden, um die Abschnitte 42 ohne Überstände gleichmäßig mit demjenigen Teil des Körpers des Patienten auszurichten, zu dessen Abstützung die Schlinge 20 verwendet wird.
  • Ein jegliches Verfahren, das die Überstände 40 glätten, abrunden oder entfernen wird, um ihre scharfen Kanten zu beseitigen, ist geeignet. Zum Beispiel können die Überstände 40 durch Abbrennen oder Abschmelzen mit Wärme geglättet werden. Ein solches Erwärmungsverfahren verursacht ein Schmelzen der scharfen Überstände 40 bis zurück zu dem gewebten Knoten 44, wobei ein Abschnitt 42 ohne Überstände gebildet wird, wie am besten in 2 dargestellt. Der Abschnitt 42 ohne Überstände kann an beiden Seiten 30 und 32 angeordnet sein, wobei er zum Beispiel etwa 1 bis 4 cm an jeder Seite der senkrechten Achse 34 einnimmt. Die Überstände können zum Beispiel entlang eines 5, 6, 7, 8, 9 oder 10 cm langen Teils der Seite des Netzmaterials entfernt werden.
  • Ein beispielhaftes Verfahren zur Herstellung einer Schlinge 20 der Erfindung aus einem Material 22 schließt zum Beispiel die Herstellung eines Schlingenmaterials 22 und die Ausbildung eines Abschnitts 42 ohne Überstände an einem Teil eines Materials 22 an Seiten 30 und 32 benachbart zur senkrechten Achse 34 ein. Die Schlinge 20 kann gebildet werden, indem man ein größeres Stück Schlingenmaterial 22 zuschneidet. Die Überstände 40 an einem Teil von jeder Seite 30 und 32 werden nicht gebildet, geglättet, gerundet oder entfernt (z. B. bis zu den gewebten Knoten), um einen Abschnitt 42 ohne Überstände zu bilden. Der Abschnitt 42 ohne Überstände kann einen Ausschnitt der Seiten 30 und 42 überspannen, der eine Länge von bis zu etwa 4 cm aufweist, jedoch gewöhnlich mindestens etwa 1 cm, und der Ausschnitt ist vorzugsweise an der senkrechten Achse 34 zentriert. Bei einer alternativen Ausführungsform kann der Abschnitt 42 ohne Überstände einen Ausschnitt der Seiten 30 und 32 überspannen, der eine Länge von 5, 6, 7, 8, 9 oder 10 cm aufweist. Bei einer Version des Verfahrens werden die Überstände 40 geglättet, gerundet oder entfernt, indem man die Überstände einer Wärmequelle aussetzt (d. h. durch Kontakt oder indem man die Wärmequelle in enge Nachbarschaft zu den Überständen bringt). Bei einem alternativen Verfahren kann eine geradlinige Messerschneide verwendet werden, die auf eine ausreichende Temperatur erwärmt wird, um gleichzeitig zu schneiden und die Überstände 40 zu glätten.
  • Die Schlinge 20 kann von einer Hülle oder Umhüllung umgeben oder darin eingeschlossen sein, wie in der gemeinsam mit der jetzigen Anmeldung eingereichten U.S. Patentanmeldung mit dem Titel "System for Implanting an Implantat" beschrieben. Die gemeinsam eingereichte Anmeldung enthält auch Verfahren zum Einbringen von Schlingen, die in einer Umhüllung eingeschlossen sind.
  • Bezug nehmend auf 1, kann die Schlinge 20 vor dem Implantieren im Körper eines Patienten in einem vorgeschriebenen Arzneimittel vorgetränkt werden. Beispielhafte Arzneimittel schließen allgemein Neomycin, Sulfa-Arzneimittel, Bakteriostatika und Antibiotika ein. Bei einigen Ausführungsformen setzen das hydrophile Material, das Arzneimittel oder beide, wenn sie in Kombination verwendet werden, das Arzneimittel in Patientengewebe beim Kontakt frei. Die Arzneimittel, die an die Patientengewebeoberflächen abgegeben werden, wenn man sich Zugang verschafft und die Schlinge 20 einsetzt, sind daher beim Kontakt mit dem Gewebe des Patienten während des Implantierens der chirurgischen Vorrichtung aktiv.
  • Alternativ wird die Schlinge 20 aus einem nicht-benetzbaren Material hergestellt, wie einem Polypropylen, Polyethylen, Polyester, Polytetrafluorethylen, TYVEK®, Mylar® oder Copolymeren davon. Polytetrafluorethylen, das zur Verwendung gemäß der vorliegenden Erfindung geeignet ist, ist von DuPont (Wilmington, Delaware, unter der Handelsbezeichnung TEFLON®) erhältlich. Solche nicht-benetzbaren Materialien nehmen keine Flüssigkeiten auf, zum Beispiel Arzneimittel in Lösung. Um es zu ermöglichen, dass sich Arzneimittel an diese nicht-benetzbaren Materialoberflächen binden oder von diesen absorbiert werden, kann die Schlinge 20 mit einer Substanz behandelt werden, die benetzbar ist, wie zum Beispiel eine benetzbare Beschichtungszusammensetzung. Die benetzbare Beschichtungszusammensetzung kann eine synthetische Beschichtung (z. B. Polyvinylpyrrolidon oder PVP), eine natürliche Beschichtung (z. B. Kollagen) oder ein physikalisch absorbierendes Material (z. B. ein Zellulose oder offenzelliges Polyurethan umfassender Schwamm) sein. Die benetzbare Beschichtungszusammensetzung kann hydrophil sein, so dass sie hydrophile Arzneimittel aufnimmt oder absorbiert. Geeignete hydrophile Beschichtungen können wasserlöslich sein und schließen zum Beispiel Hydroplus (Boston Scientific Corp., Natick, MA), Hydropass (Boston Scientific Corp., Natick, MA), Hyoscyaminsulfat, das unter der Handelsbezeichnung CYTOSPAZ von Polymedica (Woburn, MA) erhältlich ist, und Ketrodac Frometamin, das unter der Handelsbezeichnung Toradol von Roche Pharmaceuticals (Nutley, NJ) erhältlich ist. Hydrophile Arzneimittel, die gemäß der Erfindung verwendet werden können, schließen zum Beispiel Oxybutyninchlorid, Lidocain, Ketorolac und Hyoscyaminsulfat ein.
  • In ähnlicher Weise kann eine hydrophobe Beschichtung auf einer oder mehreren Oberflächen der Schlinge 20 verwendet werden. Geeignete hydrophobe Beschichtungen schließen ein, sind jedoch nicht beschränkt auf hydrophobe Beschichtungen, die gemäß der Erfindung verwendet werden können schließen Polytetrafluorethylen, Silikon und Pyrelen ein. Solche hydrophoben Beschichtungen können in Verbindung mit hydrophoben Arzneimitteln verwendet werden und absorbieren diese. Geeignete hydrophobe Arzneimittel schließen ein, sind jedoch nicht beschränkt auf geeignete hydrophobe Arzneimittel schließen ein Ibuprofen, Ketoprofen, Diclofenac und Lidocain in hydrophiler Form. Dort, wo die Bindung zwischen diesen Beschichtungen und Arzneimitteln schwach ist, wird das Arzneimittel, das absorbiert ist, leicht freigesetzt, um an die Oberflächen abgegeben zu werden, die es berührt. Alternativ kann eine stärkere Bindungsaffinität für eine langsamere zeitgesteuerte Freisetzung des Arzneimittels sorgen.
  • Dort, wo die auf die Oberfläche der Schlinge 20 aufgebrachte Beschichtung eine ionische Ladung besitzt, werden sich Arzneimittel, die eine komplementäre Ladung umfassen, an die Beschichtung binden, wenn die Beschichtung und das Arzneimittel einander ausgesetzt werden. Die Stärke von jeglicher Bindung wird einen Einfluss darauf haben, wie leicht das Arzneimittel aus der Oberfläche der Schlinge 20 freigesetzt wird. Dort, wo die ionische Bindung zwischen der Beschichtung und dem Arzneimittel schwach ist, wird das Arzneimittel leichter freigesetzt. Bei Ausführungsformen, wo eine schnelle Arzneimittelfreisetzung wünschenswert ist, sollte eine kovalente Bindung zwischen der Oberflächenbeschichtung und dem Arzneimittel vermieden werden.
  • Alternativ kann die Schlinge 20 mit einer hydrophilen Beschichtung 75 beschichtet werden. Die mit einer hydrophilen Beschichtung 75 beschichtete Schlinge 20 kann kurz vor der Operation in eine Lösung getaucht werden, die ein hydrophiles Arzneimittel enthält. Bei einer anderen Ausführungsform werden die hydrophile Beschichtung und das hydrophile Arzneimittel vermischt, um eine einzige Beschichtung zu bilden. Diese Beschichtung kann auf der Oberfläche der Schlinge 20 angeordnet werden.
  • Verfahren zum Einbringen und Einsetzen der Schlinge, z. B. um weibliche stressbedingte Inkontinenz zu behandeln, schließen ein, sind jedoch nicht beschränkt auf transvaginale, transabdominale (perkutane) und kombinierte transvaginale und transabdominale Methoden.
  • Veränderungen, Abwandlungen und andere Ausführungen dessen, was hier beschrieben ist, werden dem Durchschnittsfachmann auf dem Gebiet einfallen, ohne den Umfang der Erfindung zu verlassen.
  • Dementsprechend ist die Erfindung nicht auf die vorangehende beispielhafte Beschreibung begrenzt.

Claims (20)

  1. Schlinge (20) zur Verwendung bei einer medizinischen Anwendung, umfassend: (a) ein erstes Ende (24) und ein zweites Ende (26), wobei das zweite Ende entlang einer Längsachse (28) entgegengesetzt zu und weg von dem ersten Ende angeordnet ist; und (b) eine erste Seite (30) und eine zweite Seite (32), die sich zwischen dem ersten und dem zweiten Ende der Schlinge erstrecken, und wobei jede Seite einen Abschnitt (40) mit Überständen und einen Abschnitt (42) ohne Überstände aufweist.
  2. Schlinge nach Anspruch 1, bei welcher der Abschnitt ohne Überstände der ersten Seite und der zweiten Seite 3 cm lang ist und in Bezug zu dem ersten und dem zweiten Ende ungefähr zentriert ist.
  3. Schlinge nach Anspruch 1, bei welcher der Abschnitt ohne Überstände der ersten Seite und der zweiten Seite 1 cm lang ist und in Bezug zu dem ersten und dem zweiten Ende ungefähr zentriert ist.
  4. Schlinge nach Anspruch 1, bei welcher der Abschnitt ohne Überstände der ersten Seite und der zweiten Seite 0,5 cm lang ist und in Bezug zu dem ersten und dem zweiten Ende ungefähr zentriert ist.
  5. Schlinge nach einem vorangehenden Anspruch, bei welcher der Abschnitt ohne Überstände wärmegeglättet ist.
  6. Schlinge nach einem vorangehenden Anspruch, umfassend eine sichtbare Linie (36), die sich zwischen der ersten und der zweiten Seite erstreckt und als visuelle Führung zwischen dem ersten und dem zweiten Ende angeordnet ist, und wobei die Linie optional eine chirurgische Tinte umfasst.
  7. Schlinge nach einem vorangehenden Anspruch, bei welcher: (a) die Schlinge ein synthetisches Material (22) umfasst; und optional das synthetische Material mindestens eines von Nylon, Polyethylen, Polyester, Polypropylen, Fluorpolymeren oder einem Copolymer davon ist; oder (b) die Schlinge eine im Wesentlichen rechteckige Form besitzt.
  8. Schlinge nach einem vorangehenden Anspruch, bei welcher die Schlinge mindestens teilweise durch eine Hülle umschlossen ist.
  9. Verfahren zur Herstellung einer Schlinge, umfassend: Bereitstellen eines Materials in einer für eine medizinische Anwendung geeigneten Form, wobei das Material einschließt (a) ein erstes Ende und ein zweites Ende, wobei das zweite Ende entlang einer Längsachse entgegengesetzt zu und weg von dem ersten Ende angeordnet ist, (b) eine erste Seite und eine zweite Seite, die sich zwischen dem ersten und dem zweiten Ende erstrecken, und (c) Überstände, die aus der ersten und der zweiten Seite ragen; und Glätten, Runden oder Entfernen der Überstände entlang eines Abschnitts der ersten Seite und der zweiten Seite, um an jeder Seite einen Abschnitt ohne Überstände und einen Abschnitt mit Überständen zu bilden.
  10. Verfahren nach Anspruch 9, bei dem der Glättungsschritt durch Wärmebehandlung erzielt wird.
  11. Schlinge nach einem der Ansprüche 1 bis 8, bei welcher die Abschnitte ohne Überstände der ersten und der zweiten Seite im Wesentlichen entgegengesetzt zueinander angeordnet sind.
  12. Schlinge nach Anspruch 11, bei welcher der Abschnitt ohne Überstände von sowohl der ersten und der zweiten Seite ungefähr bei einer Mitte der Schlinge angeordnet ist.
  13. Schlinge nach einem der Ansprüche 1 bis 8, 11 oder 12, bei welcher die zweite Seite um einen Abstand entlang einer Achse (34), die im Wesentlichen senkrecht zur Längsachse ist, entgegengesetzt zu und weg von der ersten Seite angeordnet ist.
  14. Schlinge nach einem der Ansprüche 1 bis 8 oder 11 bis 13, bei welcher die Schlinge ein bioabsorbierbares Material umfasst.
  15. Schlinge nach Anspruch 14, bei welcher das bioabsorbierbare Material Polyglykolsäure oder Polymilchsäure ist.
  16. Schlinge nach einem der Ansprüche 1 bis 8 oder 11 bis 15, bei welcher die Schlinge vorgetränkt ist.
  17. Schlinge nach einem der Ansprüche 1 bis 8 oder 11 bis 16, bei welcher die Schlinge weiter ein Arzneimittel umfasst.
  18. Schlinge nach einem der Ansprüche 1 bis 8 oder 11 bis 15, bei welcher die Schlinge weiter eine benetzbare Beschichtung, eine hydrophobe Beschichtung, eine hydrophile Beschichtung oder eine Beschichtung mit einer ionischen Ladung umfasst.
  19. Schlinge nach einem der Ansprüche 1 bis 8 oder 11 bis 18, bei welcher die Schlinge weiter eine Arzneimittel enthaltende Beschichtung umfasst.
  20. Verfahren nach Anspruch 9 oder Anspruch 10, weiter umfassend Vortränken der Schlinge in einem Arzneimittel.
