Description
- Bereich der Erfindung
- Diese Erfindung beschreibt die Abgabe verschiedener Arzneistoff-Kombinationen, entweder systemisch oder lokal, insbesondere aus einem intravasalen Stent, direkt aus Mikroporen im Körper des Stents oder gemischt mit oder gebunden an eine Polymerbeschichtung, die auf den Stent aufgetragen ist, um die Proliferation eines neointimalen Gewebes zu hemmen und dadurch eine Restenose zu verhüten. Diese Erfindung, verabreicht entweder systemisch oder lokal, erleichtert auch das Herbeiführen der Hemmung einer Restenose durch den Stent.
- Hintergrund der Erfindung
- Arteriosklerotische Läsionen, die den koronaren Blutfluß einschränken oder unterbinden, sind die Hauptursache der Sterblichkeit im Zusammenhang mit einer ischämischen Herzerkrankung, die jährlich zu 500.000 bis 600.000 Todesfällen führt. Eine perkutane transluminale koronare Angioplastie (Percutaneous Transluminal Coronary Angioplasty, PTCA) zum Öffnen der verstopften Arterie wurde 1996 an über 550.000 Patienten in den Vereinigten Staaten und mehr als 945.000 Patienten weltweit durchgeführt (Lemaitre et al., 1996). Eine wesentliche Einschränkung dieser Technik ist das Problem eines Post-PTCA-Verschlusses des Gefäßes, sowohl unmittelbar nach der PTCA (akute Okklusion) als auch langfristig (Restenose): 30% der Patienten mit subtotalen Läsionen und 50% der Patienten mit chronischen totalen Läsionen werden nach der Angioplastie eine Restenose entwickeln. Zusätzlich ist die Restenose ein wichtiges Problem bei Patienten, die sich einer Bypaß-Transplantation mit einem Vena saphena-Transplantat unterziehen. Der Mechanismus einer akuten Okklusion scheint etliche Faktoren zu beteiligen und kann aus einem Zurückweichen der Vaskulatur resultieren mit einem sich daraus ergebenden Verschluß der Arterie und/oder Ablagerung von Blutplättchen entlang der beschädigten Länge des neu geöffneten Blutgefäßes, gefolgt durch die Bildung eines Thrombus aus Fibrin und roten Blutkörperchen.
- Eine Restenose nach Angioplastie ist ein eher schrittweise verlaufender Prozeß und umfaßt die anfängliche Bildung einer subkritischen Thrombose mit der Freisetzung von Cell Derived Growth Factors aus adhärenten Blutplättchen mit anschließender Proliferation der glatten Muskelzellen der Intima und einer lokalen Infiltration von Entzündungszellen, was zu einer vasalen Hyperplasie beiträgt. Wichtig ist, zur Kenntnis zu nehmen, daß eine Vielfalt von Prozessen, von denen, einschließlich Thrombose, Zellpoliferation, Zellmigation und Entzündung, jeder zu der Restenoseentwicklung beizutragen scheint.
- In den USA entspricht eine Restenoserate von 30 bis 50% 120.000 bis 200.000 US-Patienten mit dem Risiko, eine Restenose zu erleiden. Wenn nur 80% solcher Patienten eine Wiederholung der Angioplastie wählen (wobei die restlichen 20% ein Koronararterien-Bypaß-Transplantat wählen), kommen zu den Kosten eines Koronararterien-Bypaß-Transplantats für die restlichen 20% Kosten hinzu, sodaß die Gesamtkosten der Restenose leicht Milliarden von Dollar erreichen können. Deshalb könnte eine erfolgreiche Prävention der Restenose nicht nur zu einem erheblichen therapeutischen Vorteil führen, sondern auch zu erheblichen Einsparungen bei der Gesundheitsvorsorge.
- Während der genaue Mechanismus einer Restenose noch unsicher ist, sind die allgemeinen Aspekte der Restenoseentwicklung identifiziert worden:
- 1. In der normalen Arterienwand proliferieren glatte Muskelzellen (Smooth Muscle Cells, SMC) mit einer niedrigen Rate (< 0,1%/Tag). Die SMC in der Gefäßwand treten in Gestalt eines „kontraktilen" Phänotyps auf, der dadurch charakterisiert ist, daß 80 bis 90% des zytoplasmatischen Zellvolumens von dem kontraktilen Apparat eingenommen wird. Das endoplasmatische Retikulum, der Golgi-Apparat und freie Ribosomen sind gering und in der perinukleären Region lokalisiert. Die extrazelluläre Matrix umgibt die SMC und ist reich an heparinartigen Glykosylaminoglykanen, von denen angenommen wird, daß sie dafür verantwortlich sind, daß die SMC den Zustand des kontraktilen Phänotyps beibehalten (Campbell and Campbell, 1985).
- 2. Nach Druckexpansion eines intrakoronaren Ballonkatheters während einer Angioplastie werden die glatten Muskelzellen in der Arterienwand verletzt, wodurch eine thrombotische und inflammatorische Antwort ausgelöst wird. Cell Derived Growth Factors, wie etwa Platelet Derived Growth Factor (PDGF), Basic Fibroblast Growth Factor (bFGF), Epidermal Derived Growth Factor (EDGF), Thrombin etc., die aus Blutplättchen (d. h. PDGF), die an der verletzten luminalen Arterienoberfläche anheften, einwandernden Markophagen und/oder Leukozyten oder direkt aus den SMC (d. h. bFGF) freigesetzt werden, rufen eine Proliferation und Migrationsantwort in den medialen SMC hervor. Diese Zellen machen eine phänotypische Änderung vom kontraktilen Phänotyp zu einem „synthetischen" Phänotyp durch, der durch nur wenige kontraktile Filamentbündel, aber erhebliche Mengen an rauhem endoplasmatischem Retikulum, dem Golgi-Apparat und freien Ribosomen gekennzeichnet ist. Die Proliferation/Migration beginnt gewöhnlich innerhalb von ein bis zwei Tagen nach der Verletzung und erreicht bei zwei Tagen in der Media den Gipfel, um danach abzufallen (Campbell and Campbell, 1987; Clowes and Schwartz, 1985).
- 3. Synthetische Tochterzellen wandern zu der Intimaschicht aus glatten Muskelzellen der Arterie und fahren mit der Proliferation fort und beginnen, erhebliche Mengen an extrazellulären Matrixproteinen zu sezernieren. Die Proliferation, Migration und Entzündung hält an, bis sich die beschädigte luminale Endothelschicht regeneriert hat, wobei sich zu dieser Zeit die Proliferation in der Intima verlangsamt, gewöhnlich innerhalb von 7 bis 14 Tagen nach der Verletzung. Die weitere Zunahme der Verdickung der Intima, die während der nächsten 3 bis 6 Monate auftritt, ist vorrangig eher auf eine Zunahme der extrazellulären Matrix als der Zellzahl zurückzuführen. Folglich ist die Migration und Proliferation der SMC eine akute Antwort auf eine Gefäßverletzung, während eine Hyperplasie der Intima eher eine chronische Antwort darstellt (Liu et al., 1989).
- 4. Gleichzeitig mit der lokalen Proliferation und Migration adhärieren Entzündungszellen am Ort der Gefäßverletzung. Innerhalb von 3 bis 7 Tagen nach der Verletzung nehmen die luminal adhärenten Zellen aufgrund einer Migration von Entzündungszellen in die tieferen Schichten der Gefäßwand ab. In Tiermodellen, die entweder eine Ballonverletzung oder eine Stent-Implantation einsetzen, können Entzündungszellen am Ort der Gefäßverletzung mindestens 30 persistieren (Tanaka et al., 1993; Edelman et al., 1998). Entzündungszellen sind deshalb vorhanden und können sowohl zu den akuten als auch den chronischen Phasen der Restenose beitragen.
- Zahlreiche Wirkstoffe sind auf vermutete antiproliferative Wirkungen bei einer Restenose untersucht worden, und für einige ist eine Aktivität in experimentellen Tiermodellen gezeigt worden. Einige der Wirkstoffe, für die gezeigt worden ist, daß sie das Ausmaß der Intimahyperplasie in Tiermodellen erfolgreich reduzieren, schließen folgende ein:

Eine antiproliferative Wirkung auf SMC in vitro ist für viele dieser Wirkstoffe, einschließlich Heparin und Heparinkonjugaten, Taxol, Tranilast, Colchicin, ACE-Inhibitoren, Fusionstoxinen, antisense-Oligonukleotiden, Rapamycin und ionisierender Strahlung, gezeigt worden. Demnach können Wirkstoffe mit vielfältigen Mechanismen der SMC-Hemmung einen therapeutischen Nutzen beim Vermindern einer Intima-Hyperplasie aufweisen. - Anders als in Tiermodellen allerdings sind Versuche bei menschlichen Angioplastie-Patienten, eine Restenose durch systemische pharmakologische Mittel zu verhüten, bisher unbefriedigend gewesen. Weder Aspirin-Dipyridamol, Ticlopidin, eine Antikoagulanz-Therapie (akut Heparin, chronisch Warfarin, Hirudin oder Hirulog), ein Thromboxan-Rezeptor-Antagonismus noch Steroide waren zum Verhüten einer Restenose wirksam, obwohl Blutplättchen-Inhibitoren beim Verhüten einer akuten Reokklusion nach der Angioplastie wirksam waren (Mak and Topol, 1997; Lang et al., 1991; Popma et al., 1991). Weiterhin ist das humanisierte monoklonale Antikörperfragment 7E3 gegen den GPIIb/IIIa-Rezeptor von Blutplättchen noch in der Untersuchung, hat jedoch keine aussichtsreichen Ergebnisse für eine Verminderung der Restenose nach Angioplastie und Stent-Implantation gezeigt. Andere Wirkstoffe, die ebenfalls zur Verhütung einer Restenose unbefriedigend waren, schließen die Kalziumkanal-Antagonisten, Prostacyclin-Mimetika, ACE-Inhibitoren, Serotonin-Rezeptor-Anagonisten und antiproliferative Wirkstoffe ein. Diese Wirkstoffe müssen allerdings systemisch verabreicht werden, und das Aufrechterhalten einer therapeutisch wirksamen Dosis ist möglicherweise nicht möglich; antiproliferative (oder einer Restenose entgegenwirkende) Konzentrationen können die bekannten toxischen Konzentrationen dieser Wirkstoffe übersteigen, sodaß Spiegel, die ausreichen, um eine Hemmung der glatten Muskulatur hervorzurufen, möglicherweise nicht erreicht werden ((Mak and Topol, 1997; Lang et al., 1991; Popma et al., 1991).
- Zusätzliche klinische Studien, bei denen die Wirksamkeit von diätetischen Fischöl-Supplementen oder cholesterinsenkenden Wirkstoffen zur Verhütung einer Restenose untersucht worden sind, haben entweder widersprüchliche oder negative Ergebnisse gezeigt, sodaß bisher keine pharmakologischen Wirkstoffe klinisch verfügbar sind, um eine Restenose nach Angioplastie zu verhüten (Mak and Topol, 1997; Franklin and Faxon, 1993; Serruys, P. W. et al., 1993). Jüngste Beobachtungen legen nahe, daß der Antilipid/Antioxidanz-Wirkstoff Probucol zum Verhüten einer Restenose wirksam sein kann, hierfür bedarf es jedoch der Bestätigung (Tardif et al., 1997; Yokoi, et al., 1997).
- Probucol ist derzeit für die Verwendung in den Vereinigten Staaten nicht zugelassen, und eine 30-tägige Vorbehandlungsdauer würde die Verwendung bei einer Not-Angioplastie ausschließen. Außerdem hat sich die Anwendung von ionisierender Strahlung als sehr vielversprechend im Hinblick auf eine Verminderung oder Verhütung einer Restenose nach einer Angioplastie bei Patienten mit Stents erwiesen (Teirstein et al., 1997). Derzeit allerdings sind die wirksamsten Behandlungen einer Restenose die wiederholte Angioplastie, Atherektomie oder Koronararterien-Bypaß-Transplantation, da derzeit keine therapeutischen Wirkstoffe eine Zulassung der US Federal Regulatory Agency (FDA) zur Verwendung zur Verhütung einer Post-Angioplastie-Restenose erhalten haben.
- Anders als eine systemische pharmakologische Therapie haben sich Stents als nützlich erwiesen, eine Restenose teilweise zu verhüten. Stents sind Ballon-expandierbare, Spalten aufweisende Metallröhren (üblicherweise rostfreier Stahl, jedoch nicht darauf beschränkt), die, wenn sie im Lumen einer Koronararterie, die einer Angioplastie unterzogen wurde, expandiert werden, eine strukturelle Stütze für die Arterienwand verfügbar machen. Diese Stütze ist beim Erhalten der Durchgängigkeit des Gefäßlumens hilfreich. In zwei randomisierten klinischen Studien steigerten Stents den angiographischen Erfolg nach PTCA, indem der minimale Lumendurchmesser vergrößert und die Inzidenz einer Restenose nach 6 Monaten herabgesetzt (nicht jedoch aufgehoben) wurde (Serruys et al., 1994; Fischman et al., 1994).
- Weiterhin scheinen in einer vorläufigen Studie mit Heparin beschichtete Stents den gleichen Vorteil einer Reduktion des Stenosedurchmessers in der Nachfolgezeit zu besitzen, wie dies bei Stents, die nicht mit Heparin beschichtet waren, beobachtet wurde. Eine Heparinbeschichtung scheint auch den zusätzlichen Vorteil zu haben, eine Reduktion der subakuten Thrombose nach einer Stent-Implantation hervorzurufen (Serruys et al., 1996). Folglich ist 1) gezeigt worden, daß eine dauerhafte mechanische Expansion einer Koronararterie mit einer Stenose durch einen Stent einige Maßnahmen einer Restenose-Prävention verfügbar machen, und 2) hat eine Beschichtung von Stents mit Heparin sowohl die Durchführbarkeit als auch die klinische Nützlichkeit einer Abgabe von Arzneimittel lokal am Ort des verletzten Gewebes gezeigt.
- Das Dokument WO 98/36784 offenbart eine implantierbare medizinische Vorrichtung, die in das vasale System oder einen anderen Trakt des menschlichen Körpers eingeführt werden kann. Die Vorrichtung umfaßt eine oder mehrere Beschichtungen, die ein oder mehrere verschiedene bioaktive Materialien einschließen können. Die bioaktive Schicht kann ein entzündungshemmendes Steroid, wie etwa Dexamethason, antiproliferative Wirkstoffe, nicht-steroidale entzündungshemmende Arzneistoffe oder Heparin zur Verwendung als antithrombotischen Wirkstoff einschließen.
- Post-Angioplastie-Restenose ist ein multifaktorieller Prozeß, an dem zahlreiche interaktive Mechanismen beteiligt sind. Dies bedeutet, daß eine wirksame Prävention einer Restenose mit Wirkstoffen, die einen einzigen Wirkungsmechanismus besitzen, möglicherweise nicht durchführbar ist; positive therapeutische Ergebnisse können am besten durch Verabreichung mehrerer Wirkstoffe mit sich unterscheidenden therapeutischen Zielen erreicht werden. Folglich könnte ein potentieller therapeutischer Vorteil in der gemeinsamen Abgabe von Wirkstoffen mit verschiedenen Wirkungsmechanismen, die verschiedene Bestandteile des Restenoseprozesses anzielen, gefunden werden.
- Kurzfassung der Erfindung
- Die vorliegende Erfindung umfaßt einen Ansatz zum Lösen des klinischen Problems einer Restenose, welcher die Verabreichung von Arzneistoff-Kombinationen, entweder lokal oder systemisch, betrifft. Ein Beispiel einer solchen Kombination wäre die Zugabe des entzündungshemmenden Corticosteroids Dexamethason zu einem antiproliferativen Wirkstoff, wie etwa Rapamycin.
- Derartige Kombinationstherapien könnten zu einer therapeutischen Wirkung führen (weniger Proliferation ebenso wie weniger Entzündung, einem Stimulus der Proliferation), die besser ist, als sie mit jedem Wirkstoff alleine wäre. Derartige Wirkstoffe könnten an die Oberfläche eines Stents mittels Inkorporation in entweder eine biologisch abbaubare oder biologisch stabile Polymerbeschichtung an die Oberfläche eines Stents gebunden werden. Alternativ könnten diese Wirkstoffe in einen Stent, der mit Einkerbungen als Reservoir konstruiert ist, enthalten sein. Folglich würde die Abgabe eines antiproliferativen Wirkstoffs und eines entzündungshemmenden Wirkstoffs, die beide in einem Stent enthalten sind, an eine Koronararterie, die während des Angioplastie-Verfahrens verletzt worden ist, den zusätzlichen therapeutischen Vorteil verfügbar machen, der darin besteht, 1) den Grad der lokalen Proliferation glatter Muskelzellen zu begrenzen, 2) einen Stimulus zur Proliferation, d. h. Entzündung, zu reduzieren und dadurch die Wirkung des Stents, die Restenose einzuschränken, steigern.
- Unter einem ersten Aspekt der vorliegenden Erfindung wird ein Katheter zur Abgabe von Arzneistoffen verfügbar gemacht, wie durch den angefügten Anspruch 1 definiert.
- Unter einem zweiten Aspekt der vorliegenden Erfindung, wird ein Stent zur Abgabe von Arzneistoffen verfügbar gemacht, wie durch den angefügten Anspruch 2 definiert.
- Genaue Beschreibung der Zeichnungen

- Vielfach-Arzneimittel-Therapie, kombiniert mit einem Stent
- Wie zuvor dargelegt, ist die Implantation eines koronaren Stents in Verbindung mit einer Ballon-Angioplastie hochwirksam beim Behandeln eines akuten Gefäßverschlusses und kann das Risiko einer Restenose reduzieren. Intravasale Ultraschall-Untersuchungen (Mintz et al., 1996) legen nahe, daß eine koronare Stent-Implantation eine Gefäßkonstriktion wirksam verhütet, und daß der größte Anteil des späten Lumenverlusts nach einer Stent-Implantation auf ein Plaque-Wachstum zurückzuführen ist, wahrscheinlich verbunden mit einer Neointima-Hyperplasie. Der späte Lumenverlust nach einer Stent-Implantation ist beinahe zweimal höher als der, der nach einer herkömmlichen Ballon-Angioplastie beobachtet wird. Folglich kann in dem Ausmaß, in dem Stents mindestens einen Teil des Restenoseprozesses verhüten, eine Kombination von Wirkstoffen, die eine Entzündung und Proliferation verhütet oder eine Proliferation durch eine Vielzahl von Mechanismen verhütet, in Kombination mit einem Stent die wirksamste Behandlung einer Post-Angioplastie-Restenose verfügbar machen. In dieser Hinsicht ist ein Stent in Verbindung mit einer systemischen Behandlung mit den oben vorgeschlagenen Arzneistoff-Kombinationen oder einer lokalen Abgabe solcher Arzneistoff- Kombinationen eine attraktive Behandlung. Sowohl die systemische als auch lokale Abgabe einer Vielzahl von Arzneistoffen aus einem Stent weist die folgenden Vorteile auf:
- 1. Verhütung eines Zurückweichens des Gefäßes und Umgestalten durch die Wirkung des Stents als Gerüst;
- 2. Verhütung einer Vielzahl von Bestandteilen der Neointima-Hyperplasie, der vasalen Antwort auf die Verletzung.
- Eine lokale Verabreichung von Arzneistoff-Kombinationen an Koronararterien mit einem implantierten Stent könnte einen zusätzlichen therapeutischen Vorteil haben:
- 1. es wären höhere Gewebekonzentrationen erreichbar als dies bei einer systemischen Verabreichung auftreten würde;
- 2. eine reduzierte systemische Toxizität; und
- 3. eine einzige Behandlung und die Einfachheit der Verabreichung.
- Ein zusätzlicher Vorteil einer Arzneistoff-Kombinationstherapie kann darin bestehen, daß die Dosis jedes der therapeutischen Bestandteile reduziert und dadurch deren Toxizität einschränkt wird, während noch immer eine Reduktion der Restenose erreicht wird. Die Kombinationstherapie ist deshalb eine Mittel, um das therapeutische Verhältnis von Wirksamkeit zu Toxizität eines Anti-Restenose-Wirkstoffes zu verbessern.
- Wie aus den begleitenden
1 bis4 ersichtlich ist, ist es möglich, gegenwärtig hergestellte Stents zu modifizieren, um eine angemessene Arzneistoffabgabe verfügbar zu machen. Wie aus den1a ,2a und3a ersichtlich, kann jede Stent-Abstützung10 ,20 ,30 modifiziert werden, um ein bestimmtes Reservoir11 ,21 ,31 aufzuweisen. Jedes dieser Reservoirs kann offen oder geschlossen sein, wie gewünscht. Diese Reservoirs können den Arzneistoff, der abgegeben werden soll, enthalten. Die4 zeigt einen Stent40 mit einem Reservoir45 , das an der Spitze eines flexiblen Verbindungsstücks geschaffen ist. Selbstverständlich ist beabsichtigt, daß dieses Reservoir45 nützlich ist, um jeden Arzneistoff an einem speziellen Punkt der Flexibilität des Stents abzugeben. Dementsprechend kann dieses Konzept für Stents vom Typ der „zweiten Generation" nützlich sein. Verfahren zum Beschichten solcher Stents werden beispielsweise im DokumentUS 6,273,913 , angemeldet am 16. April 1998, und im Dokument EP-A-1127582, angemeldet am 25. Februar 2000, beschrieben. - Bei jeder der vorangehenden Vorrichtungen ist es allerdings zweckmäßig, daß die Arzneistoffdosis mit genügend Spezifität und einer ausreichenden Konzentration verabreicht wird, um eine wirksame Dosierung in dem Läsionsgebiet verfügbar zu machen. In dieser Hinsicht muß die Reservoirgröße in den Stent-Abstützungen eine Größe von ungefähr 0,1 mm bis ungefähr 1 mm Tiefe und 7 mm bis 15 mm Länge einhalten oder eine ausreichende Größe, um mindestens eine therapeutische Menge des Arzneistoffs aufzunehmen. Dann sollte es möglich sein, die Arzneistoffdosierung an dem gewünschten Ort und in der gewünschten Menge geeignet zu verabreichen.
- Beispiel 1
- Um die Fähigkeit einer Arzneistoff-Kombination, die Zellproliferation zu verhüten, zu bestimmen, wurden menschliche glatte Muskelzellen (Clonetics, Walkersville, MD) mit einer Dichte von 10.000 Zellen/Vertiefung in jede Vertiefung von 24-Loch-Platten ausgesät und in Wachstumsmedium, das Heparin, EGF (Epidermal Growth Factor), FGF (Fibroblast Growth Factor) und Serum enthielt, kultiviert. Nach 24 Stunden wurde das Wachstumsmedium gewechselt und frisches Medium, das verschiedene Konzentrationen von Testwirkstoffen (0,01–10 mg/ml) enthielt, wurde zu den Vertiefungen gegeben, wobei Dreifachbestimmungen vorgesehen waren. Das Medium wurde durch frisches Medium (plus Testwirkstoffe) nach drei Tagen ersetzt. Am Tag 5 wurden die Zellen durch Trypsin/EDTA abgelöst und unter Verwendung eines Hämacytometer gezählt. Die Lebensfähigkeit der Zellen wurde durch Trypanblau-Ausschluß bestimmt.
- Tabelle 1 macht das prozentuale Kontrollwachstum von menschlichen glatten Muskelzellen bei den verschiedenen getesteten Konzentrationen des entzündungshemmenden Wirkstoffs Dexamethason entweder ohne oder in Gegenwart von zwei Konzentrationen des antiproliferativen/antiimmunen Wirkstoffs Rapamycin verfügbar. Dexamethason erzeugte eine konzentrationsabhängige Abnahme der Proliferation glatter Muskelzellen in diesem Modellsystem. Der IC50-Wert (diejenige Konzentration, die erforderlich ist, um eine Verringerung der Proliferation auf 50% der Kontrollzellzahl hervorzurufen) für die Hemmung glatter Muskelzellen mit Dexamethason alleine, bestimmt aus Tabelle 1, beträgt 5 μg/ml. Es wurde festgestellt, daß die Zugabe von 0,2 μg/ml Rapamycin zum Inkubationsmedium die IC50-Bestimmung von Dexamethason auf 0,05 μg/ml reduzierte. Die Zugabe einer größeren Konzentration von Rapamycin (2 μg/ml) reduzierte die IC50-Bestimmung für Dexamethason weiter auf weniger als 0,01 μg/ml.
- Demnach war in dem Maße, in dem die Rapamycin-Konzentration im Inkubationsmedium erhöht wurde, weniger Dexamethason erforderlich, um eine 50%ige Hemmung des Zellwachstums hervorzurufen. Da bei den eingesetzten Mengen von Rapamycin eine 50%ige Hemmung des Zellwachstums nicht erreicht wurde, zeigt Tabelle 1, daß Konzentrationen sowohl von Rapamycin als auch Dexamethason unterhalb ihrer entsprechenden IC50-Mengen kombiniert werden können, um eine Wirkung auf das Zellwachstum auszuüben, die größer ist als bei jedem Wirkstoff einzeln. Eine solche Arzneistoffkombination kann therapeutisch zur Hemmung der intimalen glatten Muskelzellproliferation, welche die Stent-Implantation begleitet, nützlich sein. Während die Wirksamkeit bei diesen geringeren Dosen beibehalten werden konnte, könnten die Toxizitäten, die mit jedem dieser Wirkstoffe verbunden sind, verbessert werden.
- Die folgenden Beispiele werden verwendet, um die verschiedenen Zusammensetzungen von medizinischen Stent-Beschichtungen, die ein oder mehrere Arzneistoffe enthalten, zu zeigen. Diese werden in Tabelle 2 zusammengefaßt.
- Beispiel 2
- Dieses Beispiel beschreibt die Herstellung einer Grundbeschichtung, die Rapamycin enthält.
- Es wurden Stents mit Parylene CTM unter Verwendung eines Dampfabscheideverfahrens, das durch die Hersteller des Parylene-Beschichtungsinstruments (SCS Madison, Wisconsin) zur Verfügung gestellt wird, beschichtet. Der Stent wird gewogen und dann für die Beschichtung eingespannt. Während der Stent gedreht wird, wird eine Lösung aus 1,75 mg/ml Poly(ethylen-co-Vinylacetat) (PEVA), 1,75 mg/ml Polybutylmethacrylat und 1,5 mg/ml Rapamycin, gelöst in Tetrahydrofuran, aufgesprüht. Der beschichtete Stent wird aus dem Sprühstrahl entfernt und an der Luft trocknen gelassen. Nach einer abschließenden Wägung wird die Menge der Beschichtung auf dem Stent bestimmt.
- Beispiel 3
- Dieses Beispiel beschreibt die Herstellung einer Grundbeschichtung, die Dexamethason enthält.

- Dieses Beispiel beschreibt eine Stent-Beschichtung, die aus einer Grundbeschichtung, die Rapamycin und Dexamethason enthält, und einem arzneistofffreien Sperrüberzug besteht.
- Ein Stent wird wie in Beispiel 4 beschichtet. Nachdem die Beschichtung sorgfältig getrocknet worden ist, wird eine Lösung aus 2,5 mg/ml Polybutylmethacrylat, gelöst in Tetrahydrofuran, aufgesprüht. Sie wird dann luftgetrocknet, um ein Endgewicht des Überzugs von 150 μg aufzuweisen.
- Beispiel 6
- Dieses Beispiel beschreibt eine Stent-Beschichtung, die aus einer Grundlage, die Rapamycin enthält, und einer Überschichtung mit Dexamethason besteht.
- Ein Stent wird wie in Beispiel 2 beschichtet. Es wird eine Lösung aus 1,75 mg/ml Poly(ethylen-co-Vinylacetat) (PEVA), 1,75 mg/ml Polybutylmethacrylat und 1,5 mg/ml Dexamethason, gelöst in Tetrahydrofuran, aufgesprüht. Der beschichtete Stent wird aus dem Sprühstrahl entfernt und an der Luft trocknen gelassen. Das Endgewicht jeder Schicht beträgt üblicherweise 250 μg für ein Gesamtgewicht der Beschichtung von 500 μg.
- Beispiel 7
- Dieses Beispiel beschreibt eine Stent-Beschichtung, die aus einer Grundlage, die Dexamethason enthält, und einer Überschichtung mit Rapamycin besteht.
- Ein Stent wird wie in Beispiel 3 beschichtet. Eine Lösung aus 1,75 mg/ml Polyethylen-co-Vinylacetat) (PEVA), 1,75 mg/ml Polybutylmethacrylat und 1,5 mg/ml Rapamycin, gelöst in Tetrahydrofuran, wird aufgesprüht. Der beschichtete Stent wird aus dem Sprühstrahl entfernt und an der Luft trocknen gelassen. Das Endgewicht jeder Schicht beträgt üblicherweise 250 μg für ein Gesamtgewicht der Beschichtung von 500 μg.
- Die folgenden Beispiele beschreiben das Verfahren und die Ergebnisse des Überprüfens der in vitro-Freisetzung von Rapamycin und Dexamethason aus einem beschichteten Stent.
- Beispiel 8
- Dieses Beispiel beschreibt das Verfahren zum Ausführen der in vitro-Freisetzung von Rapamycin und Dexamethason aus einem beschichteten Stent.
- Jeder Stent wurde in 2,5 ml Freisetzungsmedium (wäßriges Ethanol, 25 Vol.-%, bei Raumtemperatur), das in einem 13 × 100 mm Kulturröhrchen mit einem Schraubdeckel enthalten war, plaziert. Das Röhrchen wurde in einem Wasserbad (INNOVATM 3100, New Brunswick Scientific) bei 200 Upm geschüttelt, während Umgebungsbedingungen aufrecht erhalten wurden. Nach einem vorgegebenen Zeitintervall (das von 15 min bis ein Tag reichte) wurden die Röhrchen aus dem Schüttler genommen und die entsprechenden Stents vorsichtig in ein 2,5 ml-Aliquot frisches Freisetzungsmedium überführt. Das neue Röhrchen wurde in den Schüttler plaziert und das Schütteln wieder aufgenommen. Eine Probe wurde aus dem Aliquot, das zuvor den Stent enthalten hatte, entfernt und in ein HPLC-Vial zur Bestimmung des Gehalts an Rapamycin und Dexamethason durch HPLC plaziert.
- Beispiel 9
- Dieses Beispiel beschreibt das Verfahren zum Analysieren von Rapamycin im Freisetzungsmedium.
- Das HPLC-System, das zum Analysieren der Probe verwendet wurde, war ein Waters Alliance mit einem PDA 996. Dieses System ist mit einem Fotodioden-Array-Detektor ausgestattet.
- Jeder Probe wurden 20 μl entnommen und an einer C18-Umkehrphasensäule (Waters SymmetryTM Column: 4,6 mm × 100 mm RP18, 3,5 μm mit einer passenden Vorsäule) unter Verwendung einer mobilen Phase, die aus Acetonitril/Methanol/Wasser (38 : 34 : 28 Vol./Vol.) bestand, bei einer Flußrate von 1,2 ml/min analysiert. Die Säule wurde während der Analyse bei 60°C gelagert. Unter diesen Analysebedingungen wies Rapamycin eine Retentionszeit von 4,75 ± 0,1 min auf. Die Konzentration wurde aus einer Standardkurve aus der Konzentration gegen die Antwort (Bereich unter der Kurve), die mit Rapamycin-Standards im Bereich von 50 ng/ml bis 50 μg/ml aufgenommen worden war, bestimmt.
- Die Ergebnisse des Überprüfens der Rapamycin-Freisetzung aus den beschichteten Stents, wie oben beschrieben, werden in den
5 ,7 und9 gezeigt. - Beispiel 10
- Dieses Beispiel beschreibt das Verfahren zum Analysieren von Dexamethason im Freisetzungsmedium.
- Das HPLC-System, das zum Analysieren der Probe verwendet wurde, war ein Shimadzu Class-VP Chromatography Laboratory-System. Dieses System ist mit einem Fotodioden-Array-Detektor ausgestattet. Jeder Probe wurden 20 μl entnommen und an einer C18-Umkehrphasensäule (Waters SymmetryTM Column: 4,6 mm × 100 mm RP18, 3,5 μm) analysiert. Eine isokratische mobile Phase, die aus Methanol/Wasser (55 : 45 Vol/.Vol.) bestand, aufgegeben bei einer Flußrate von 0,8 ml/min, wurde in den ersten 6,5 min der Analyse verwendet, gefolgt von 100% Methanol für 2 min; letzteres diente dazu, die Entfernung von Rapamycin, das auf der Säule zurückgehalten wird, sicherzustellen. Die Säule wurde während der Analyse bei 25°C gelagert. Unter diesen Analysebedingungen wies Dexamethason eine Retentionszeit von 5,9 ± 0,1 min auf. Die Konzentration wurde aus einer Standardkurve aus der Konzentration gegen die Antwort (Bereich unter der Kurve), die mit Dexamethason-Standards im Bereich von 40 ng/ml bis 4,0 ng/ml aufgenommen worden war, bestimmt.
- Die Ergebnisse des Überprüfens der Dexamethason-Freisetzung aus den beschichteten Stents, wie oben beschrieben, werden in den
6 ,8 und9 gezeigt. - Diese und andere Konzepte werden hier offenbart. Dem Leser wird offensichtlich sein, daß Modifikationen des Stents oder der angewendeten Arzneistoffdosierung möglich sind. In je dem Fall sollte jedoch zu erkennen sein, daß jede mögliche Modifikation in den Umfang der Erfindung fällt, welche anhand der beigefügten Ansprüchen und ihren Äquivalenten zu verwirklichen ist.
- Die Begriffe „Rapamycin", „Taxol", „Angiopeptin", „Symmetry" und „Hirulog" werden hierdurch als Markennamen anerkannt.
Claims (3)
- Katheter zur Abgabe von Wirkstoffen an ein Blutgefäßlumen eines Patienten, wobei eine therapeutische Dosiermenge der Kombination aus Rapamycin und Dexamethason auf diesen Katheter geschichtet ist.
- Stent zur Abgabe von Wirkstoffen an ein Blutgefäßlumen eines Patienten, wobei eine therapeutische Dosiermenge von Rapamycin und Dexamethason auf den Stent geschichtet ist.
- Stent nach Anspruch 2, wobei der Stent eine Vielzahl von Stützen umfaßt, wobei die Stützen in dem Lumen des Patienten expandierbar sind, wobei mindestens eine der Stützen ein Resevoir darin enthält.
