Description
Die Erfindung betrifft einen Saugring für Augenoperationen
gemäß dem Oberbegriff des Anspruchs 1, wie er aus der
DE 37 07 004 A1 bekannt ist.
Für Operationen am menschlichen Auge benutzt man zur
Stabilisierung der Form des Auges Ringe, die z. B. aufgenäht
werden (z. B. Flieringaring), aber auch Hohlringe mit zum
Auge offenem Profil, wie sie z. B. bei der Technik
refraktiver Chirurgie zur Anwendung kommen (Barraquer-
Ringe). Diese weisen im Profil rechtwinkligen Querschnitt
auf, wobei die offenen Schenkel des rechten Winkels am Auge
angelegt werden und zwischen dem rechten Winkel des Ringes
und dem Auge einen Hohlraum einschließen, der zur Erzeugung
eines Unterdrucks dient. Durch diesen Unterdruck wird das
Auge fixiert, aber auch der Augeninnendruck erhöht. Für
Augenoperationen, deren Techniken einen normalen Augendruck
verlangen, sind solche Ringe nicht anwendbar. Es handelt
sich um alle augeneröffnenden Techniken, bei denen die
Erzeugung eines Unterdrucks im Ring bei der Eröffnung des
Auges zu einem Vortreten von Iris und Linse und Glaskörper
führen würde. Diese augeneröffnenden Operationen sind vor
allen Dingen die Operation des Grauen Stars und die
perforierende Keratoplastik, d. h. die Übertragung einer
Hornhautscheibe von einem Spenderauge auf das Auge des
Patienten. Für alle diese augenöffnenden Operationen ist
eine ringförmige, den Augapfel nicht deformierende und nicht
druckerhöhende Fixierung wünschenswert.
Aufgabe der Erfindung ist es daher, einen Saugring für
Augenoperationen zu schaffen, mit dem das Auge sicher
fixierbar ist, ohne beim Fixieren den Augapfel zu deformieren
und den Augeninnendruck zu erhöhen.
Die Aufgabe wird mit den im Anspruch 1 angegebenen Merkmalen
gelöst.
Es handelt sich um einen Saugring, der konzentrisch zur
Hornhaut dem Auge aufgesetzt wird und mit Hilfe einer
externen Saugpumpe über einen Hohlgriff ans Auge angesaugt
wird. Der Saugring besteht aus Stahl- oder festen
Kunststoffteilen. Die Vermeidung der Erhöhung des
Augendrucks wird dadurch erreicht, daß das Hohlprofil, d. h.
die äußere Ringwand des Saugrings zum Auge hin durch ein dem
Radius des Auges entsprechendes Innenteil abgedeckt wird,
wie dies auch aus der DE 37 07 004 A1 bekannt ist. Dadurch,
daß die Augenwand wegen des Innenteils des Saugrings nicht
deformiert werden kann, ändert sich auch der Augendruck
nicht.
Dieses Innenteil liegt mit Einzelsegmenten an der
zylinderförmigen und an der kreisförmigen Innenfläche der
Ringinnenwand unter Berührungsschluß an. Das Innenteil wird
an der äußeren Ringwand fixiert durch Bolzen oder
vorzugsweise Schrauben, die die äußere Ringwand durchsetzen
und in eines der Einzelsegmente einmünden.
Der so gestaltete Fixationsring erlaubt dem Chirurgen die
Vorzeichnung oder Eröffnung des Auges, wobei das Messer
entlang der Innenbegrenzung des Ringes geführt werden kann.
Diese innere Öffnung kann durch Einlagerung konzentrischer
Scheiben verkleinert werden. Solche Scheiben können drehbar
gelagert sein und Aussparungen für eine Messeraufnahme
haben. Auch die Anlagekante für das Messer kann in
beliebiger Weise gestaltet werden, so daß eine Messerführung
in beliebigem Winkel möglich wird.
Es ist weiterhin möglich, die Innenöffnung so zu gestalten,
daß sie z. B. durch eine schwalbenschwanzartige Führung
zur Aufnahme des Fußes für ein Rundschneidemesser dient.
Ein Ausführungsbeispiel der Erfindung ist in der Zeichnung
dargestellt und wird wie folgt erläutert:
Es zeigt
Fig. 1 einen Schnitt in der Vorderansicht des Saugrings (1) mit
seinem Innenteil (2), wobei die Saugkanäle (3) durch
Einzelsegmente bzw. Stege (4) unterbrochen sind.
Das Innenstück (2) wird durch Schrauben oder Bolzen oder
Nieten (5) mit der Außenwand befestigt.
Fig. 2 zeigt eine Draufsicht in den Ring von unten.
Fig. 3 zeigt das Innenteil (2) isoliert von der äußeren Ringwand
(1) im Schnitt.
Fig. 4 zeigt die Draufsicht auf das Innenteil (2) mit Stegen (4).
Fig. 5 zeigt eine perspektivische Ansicht des Innenrings mit seinen
Einzelsegmenten bzw. Stegen.
Fig. 1 zeigt den Querschnitt eines metallischen Saugrings
zur Operation am menschlichen Auge, dessen dem Auge
zugewandte äußere Ringwand (1) ein annähernd
dreiecksförmiges Innenteil (2) mit Hilfe von
Befestigungsmitteln (5) hält, wobei dieses Innenteil (2) an
der zylinderförmigen und an der kreisringförmigen
Innenfläche der äußeren Ringwand anliegende Einzelsegmente
(4) aufweist, die zwischen dem Innenteil (2) und der äußeren
Ringwand (1) ringsum laufende Kanäle (3) freilassen. Die dem
Mittelpunkt des Saugrings (1) zugewandte Fläche des
Innenteils (2) ist dem Radius des Auges entsprechend von der
Dreiecksform abweichend geformt. Zur besseren Fixation der
äußeren Ringwand dient ein Konturversatz an der unteren und
oberen Kante der äußeren Ringwand (1).
Fig. 1 zeigt ferner, daß die kommunizierenden Kanäle (3)
zwischen äußerer Ringwand (1) und Innenteil (2) in den
Hohlgriff (6) münden, der etwa radial abstehend lösbar
befestigt ist, mittels dem der Saugring plaziert wird und
mittels dem nach der Fixierung des Auges das Auge bewegt
werden kann.
Fig. 2 zeigt eine Draufsicht in den Saugring von unten,
wobei die Öffnung der Kanäle (3) zwischen den Stegen (4)
erkennbar ist.
Fig. 3 zeigt das durch Lösen der Befestigungsmittel (5)
isolierte Innenteil (2) im Schnitt. Die an der Mantelfläche
und der dazu senkrechten Stirnfläche angeordneten
Einzelsegmente (4) sind sichtbar.
Fig. 4 zeigt die Draufsicht auf das Innenteil (2) von oben,
wobei die Anordnung der Einzelsegmente (4) erkennbar ist.
Claims (3)
1. Saugring zur Fixation des menschlichen Auges bei
Operationen, bestehend aus einer äußeren Ringwand (1) mit
rechtwinkligem Profil, die eine von der Achse des Rings
weggerichtete Bohrung zur Befestigung eines Hohlgriffs (6)
zum Anschluß an einen externen Vakuumerzeuger hat, wobei die
äußere Ringwand (1) ein am Auge anlegbares, ringförmiges
Innenteil (2) trägt, dessen Querschnitt im wesentlichen ein
rechtwinkliges Dreieck darstellt, dadurch gekennzeichnet,
daß das Innenteil (2) sowohl an seiner Mantelfläche als auch
an seiner dazu senkrechten Stirnfläche zirkulär
aufeinanderfolgende, voneinander beabstandete Einzelsegmente
(4) trägt, die an der zylinderförmigen und an der
kreisringförmigen Innenfläche der Ringwand (1) anliegen und
zwischen der Ringwand (1) und dem Innenteil (2) Kanäle (3)
bilden, die untereinander und mit der von der Achse des
Rings weggerichteten, in den Hohlgriff (6) mündenden Bohrung
zur Erzielung eines Unterdrucks kommunizieren und wobei das
Innenteil (2) durch die Ringwand (1) durchsetzende
Befestigungsmittel (5) mit der äußeren Ringwand (1)
verbunden ist.
2. Saugring nach Anspruch 1, dadurch gekennzeichnet, daß die
Befestigungsmittel (5) lösbar sind und die Stirnfläche der
Ringwand (1) durchsetzen.
3. Saugring nach Anspruch 1 oder 2, gekennzeichnet dadurch,
daß der Handgriff (6) lösbar befestigt ist.
