Die Erfindung betrifft eine Humeruskomponente für eine Schulterprothese, mit einem kugelförmigen Ende und einem Schaft an dem gegenüberliegenden Ende davon zur Aufnahme in dem Markkanal des Oberarms zur Befestigung der Humeruskomponente an dem Oberarm und einer Schulter zwischen dem kugelförmigen Ende und dem Schaft.
Eine der mit größtem Erfolg angewendeten Schulterersetzungen ist die Stanmore-Prothese, die in der US-PS 3 694 820 beschrieben ist. Dabei handelt es sich um eine Prothese aus einer Kugelgelenkverbindung mit einer passenden Einrichtung zum Befestigen der beiden Komponenten an einem Knochen.
Aufgabe der Erfindung ist es, eine Humeruskomponente zur Verwendung mit einer solchen Prothese zu schaffen.
Diese Aufgabe wird durch eine Humeruskomponente der eingangs beschriebenen Art gelöst, die gemäß der Erfindung dadurch gekennzeichnet

ist, daß die Schulter von der Kugel des kugelförmigen Endes einen Abstand aufweist und einen zirkulären Abschlußrand mit einer Mehrzahl von Verbindungslöchern in seiner Peripherie aufweist.
Durch die Löcher können die Suturen (Fäden) zum Beschleunigen der Befestigung von Sehnen am.Knochen hindurchgeführt werden. Die Schulter erstreckt sich im wesentlichen unter einem rechten Winkel zu dem Krümmungszentrum der Kugel, und der Hals oder Schaft der Prothese ist langgestreckt. Die Stanmore-Prothese ist ursprünglich dazu ausgebildet worden, die Rotormanschette (die verbundenen Sehnen aus den Subscapularis vorn und den Infraspinatus und Teresminormuskeln hinten) zu halten. Bei Gelenkentzündung oder ähnlichem bewirkt die Rotormanschette häufig, daß die Tuberositasan dem Acromion anstößt und so eine Bewegung beschränkt. Mit der neuen Humeruskomponente gemäß der Erfindung werden die Rotormanschette und die Tuberositasentfernt, wodurch eine freie Bewegung der Schulter möglich wird. Mit den durch die Löcher hindurchgehenden Suturen können die Sehnen von der Rotormanschette an dem Knochen befestigt werden.
Weitere Merkmale und Zweckmäßigkeiten der Erfindung ergeben sich aus der Beschreibung von Ausführungsbeispielen. Von den Figuren zeigen:
Fig. 1 eine Vorderansicht, teilweise im Schnitt, einer Ganzschulterprothesenausführung mit der Humerusprothesenkomponente gemäß der Erfindung;
Fig. 2 eine Teilansicht ähnlich der in Figur 1 von einer bekannten Stanmore-Prothese;
Fig. 3 eine Vorderansicht, aus der das Entfernen der Tuberositas des· Humerus und das Befestigen der Rotor- oder Drehmanschette an dem Knochen mit Suturen durch Löcher in der Humeruskomponente gezeigt ist; und

Fig. 4 eine auseinandergezogene Darstellung einer Ganzschulterprothesenausführung gemäß der Erfindung.
In Figur 1 ist eine Prothesenhumeruskoroponente 8 in Verwendung mit einem Standart-Stanmore-Kugelgelenk 6 gezeigt. Die Humeruskomponente 8 besteht aus einem kugelförmigen Ende 10 mit einem passenden Krümmungsradius und einer solcher. Form, daß sie in die Fassung des Kugelgelenks 6 hineinpaßt. Das kugelförmige Ende 10 wird von einem Schaft 12 gehalten, der in einer nach außen gedrehten kreisförmigen Schulter 14 endet. Die Schulter 14 hat von der Kugel 10 einen Abstand, der ungefähr gleich dem Durchmesser der Kugel 10 ist. Der Schaft 12 ist somit wesentlich länger als der Schaft bei der bekannten Humeruskomponente, die in Figur 2 gezeigt ist. Darüberhinaus ist die Schulter 14 annähernd im rechten Winkel zur Hauptachse von Schaft und Kugel im Gegensatz zur bekannten Ausführung angeordnet, bei der die Ebene der Schulte.scheibe einen Winkel mit der Achse bildet. Der wesentlichste Unterschied besteht aber darin, daß die Schulter 14 eine Mehrzahl von Löchern 16 an ihrem Abschlußrand besitzt. Die Funktion dieser Löcher wird im folgenden noch beschrieben. Die Humeruskomponentenprothese erfordert das Entfernen der Tuberositas 20 des Humerus.
Bei Verwendung der Humeruskomponentenprothese 8 wird der Kopf des
im
Oberarms 17 in einem/wesentlichen rechten Winkel zum Oberarm abgeschnitten. Das ist ein Unterschied zum Stand der Technik, bei dem die Tuberositas 20 nicht herausgeschnitten wird, die häufig auf dem Acromion anstößt und die Bewegung, des Schultergelenks begrenzt, wenn die Prothese eingesetzt wird. Ein Schaftteil 18 der Komponente 8 wird dann in bekannter Weise mit dem Markkanal des Humerus 17 verklebt bzw. verbunden. Dann werden durch die Löcher 16 Fäden (Suturen) hindurchgeführt, um die Sehnen an der richtigen Stelle anzuordnen und die Sehnen dann fest an dem Knochen zu halten.
In Figur 3 ist ein Verfahren zur Befestigung der Subscapularissehnen 26 gezeigt. In einer ähnlichen Weise werden auch die Infraspinatus und Teresminorsehnen an der Rückseite des Oberarms befestigt, was jedoch nicht gezeigt ist.

1. Humeruskomponente für eine Schulterprothese, mit einem kugelförmigen Ende und einem Schaft an dem gegenüberliegenden Ende davon zur Aufnahme in dem Markkanal des Oberarms zur Befestigung der Humeruskomponente an dem Oberarm, und einer Schulter zwischen dem kugelförmigen Ende und dem Schaft, dadurch gekennzeichnet, daß die Schulter (14) von der Kugel des kugelförmigen Endes (10) einen Abstand aufweist und einen zirkulären Abschlußrand mit einer Mehrzahl von Verbindungslöchern (16) in seiner Peripherie aufweist.
2. Humeruskomponente nach Anspruch 1, dadurch gekennzeichnet, daß der Abstand der Schulter (14) von der Kugel des kugelförmigen Endes (10) im wesentlichen gleich dem Durchmesser der Kugel ist.
3. Humeruskomponente nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Schulter (14) sich im wesentlichen in einem rechten Winkel zur Achse von Kugel (10) und dem die Kugel (10) mit der Schulter (14) verbindenden Schaft (12) erstreckt.
