Description
Die Erfindung betrifft ein Verfahren zur Herstellung radioaktiver Stents
und ihre Verwendung zur Restenoseprophylaxe.
Radioaktive Stents sind Stand der Technik (EP 0433011 A1, WO 94/26205, US 5176617).
Stents sind Endoprothesen, die die Offenhaltung gangartiger Strukturen in Körpern von
Menschen oder Tieren ermöglichen (z. B. Gefäß-, Ösophagus-, Trachea-,
Gallengangstent). Sie werden als palliative Maßnahme bei Verengungen durch Verschluß
(z. B. Atherosklerose) oder Druck von außen (z. B. bei Tumoren) verwendet. Radioaktive
Stents werden beispielsweise nach gefäßchirurgischen Eingriffen nach gefäßchirurgischen
oder interventionell radiologischen Eingriffen (z. B. Ballonangioplastie) zur
Restenoseprophylaxe eingesetzt. Derartige radioaktive Stents können beispielsweise durch
Aktivierung eines nichtradioaktiven Stents mittels Bestrahlung mit Protonen oder
Deuteronen aus einem Zyklotron hergestellt werden (WO 94/26205).
Es besteht nun das Problem, daß einerseits am Ort der Anwendung der Stents in der Regel
kein Zyklotron verfügbar ist um eine Aktivierung der Stents vorzunehmen und
andererseits der aktivierte Stent aufgrund der z. T. kurzen Halbwertszeit der aktivierten
Isotope und aus Strahlenschutzgründen nicht beliebig lager- und transportierbar ist.
Aus der US-Patentschrift 5,176,617 ist ein Stent bekannt, der aus einem nichtradioaktiven
Grundkörper besteht und an seiner Oberfläche mit einem radioaktiven Isotop beschichtet ist. Es
wird nicht im Detail offenbart, wie ein solcher Stent hergestellt werden kann.
Aufgabe der vorliegenden Erfindung ist es daher Stents zur Verfügung zu stellen, die
unabhängig von einem Zyklotron aktiviert werden können. Insbesondere ist es Aufgabe
der Erfindung Stents zur Verfügung zu stellen, die unabhängig von einem Zyklotron mit
einem vorher ausgewählten radioaktiven Isotop beschichtet werden können.
Die oben geschilderte Aufgabe wird erfindungsgemäß durch eine chemische Abscheidung
des radioaktiven Isotops auf dem Stent gelöst.
Hierzu wird der ausgewählte Stent in eine Lösung getaucht, die das radioaktive Isotop
enthält. Das radioaktive Isotop wird dann chemisch auf dem Stent abgeschieden. In
Abhängigkeit von den ausgewählten Materialen des Stents einerseits und des
abzuscheidenden radioaktiven Isotops andererseits kommen zwei Möglichkeiten der
Abscheidung in Betracht:
Bei der chemischen Reduktion wird der Lösung, die das radioaktive Isotop in
gelöster Form sowie den Stent enthält, ein Reduktionsmittel (z. B. SnCl2, KBH4,
Dimethylboran, Formaldehyd, Natriumhypophosphit) zugesetzt.
Übersicht:

Nach 1 Minute bis 10 Stunden wird der Stent aus der jeweiligen Lösung genommen
und gewaschen. Der Stent ist oberflächlich mit dem radioaktiven Isotop beschichtet.
Auf diese Weise können zum Beispiel Radioisotope der Elemente Ag, Au, Bi, Co,
Cr, Cu, Fe, Gd, Hg, Ho, In, Ir, Lu, Mn, Ni, P, Pb, Pd, Pm, Pt, Re, Rh, Ru, Sc, Sm,
Tb, Tc oder Y auf metallischen Stents (z. B. Stahl, Nitinol) abgeschieden werden.
Bei der chemischen Fällung wird der Lösung, die das radioaktive Isotop in
gelöster Form sowie den Stent enthält, ein Fällungsmittel (z. B. Oxalsäure,
Phosphorsäure oder deren Salze oder Na2CO3) zugesetzt.
Auf diese Weise können zum Beispiel Radioisotope der Elemente Ag, Au, Bi, Co,
Cr, Cu, Fe, Gd, Hg, Ho, In, Ir, Lu, Mn, Ni, Pb, Pd, Pm, Pt, Re, Rh, Ru, Sc, Sm, Tb,
Tc oder Y auf metallischen Stents (z. B. Stahl, Nitinol) abgeschieden werden.
Die Erfindung betrifft daher ein Verfahren zur Herstellung radioaktiver Stents, das
dadurch gekennzeichnet ist, daß ein nichtradioaktiver Stent in eine Lösung, die mindestens
ein radioaktives Isotop in ionischer Form enthält, getaucht wird und das Isotop dann
chemisch auf dem Stent abgeschieden wird.
Im Rahmen der vorliegenden Anmeldung sollen für radioaktive Isotope die Schreibweisen
nnX und X-nn (X: Elementsymbol, nn: Massenzahl) als synonym gelten (Beispiel: 110Ag
entspricht Ag-110).
Die oben beschriebenen Verfahren werden im allgemeinen bei Temperaturen von 0-100°C
durchgeführt. Bei der Beschichtung des Stents können in Abhängigkeit von dem
abgeschiedenen Isotop auch nichtwäßrige Lösemittel eingesetzt werden. Bei Einsatz eines
nichtwäßrigen Lösemittels soll dieses vor der Implantation entfernt werden
Die Stents können auch mit zwei oder mehr verschiedenen Isotopen beschichtet werden.
Insbesondere ist es möglich kurz- und langlebige Isotope gemeinsam auf einem Stent
aufzutragen (beispielsweise 55Co mit 55Fe oder 99Mo mit 57Co).
Die nötigen Arbeitsgänge zur Durchführung der oben prinzipiell beschriebenen Verfahren
sind dem Fachmann bekannt. Spezielle Ausführungsformen sind detailliert in den
Beispielen beschrieben.
Die nach dem erfindungsgemäßen Verfahren hergestellten Stents lösen die eingangs
beschriebene Aufgabe. Stents können durch die offenbarten Verfahren problemlos und
exakt dosiert radioaktiv markiert werden. Die erfindungsgemäßen Stents sind
physiologisch gut verträglich. Wie im Tiermodell gezeigt werden konnte, wird die
Restenose nach Ballondenudation durch Implantation der erfindungsgemäßen Stents
signifikant inhibiert.
Der besondere Vorteil der erfindungsgemäßen Verfahren ist, daß der Mediziner vor Ort
einen (nichtradioaktiven) Stent nach seinen Bedürfnissen auswählen und den ausgewählen
Stent dann durch das beschriebene Verfahren aktivieren kann. Die wenigen dazu nötigen
Stoffe und Lösungen können entsprechend vorbereitet angeliefert werden, so daß der
entsprechende Mediziner nur noch den unbeschichteten Stent in der vorgegebenen
Reihenfolge in die einzelnen Lösungen tauchen muß.
Die folgenden Beispiele sollen den Erfindungsgegenstand erläutern ohne ihn auf diese
beschränken zu wollen.
Ein Wiktor-Stent (22.85 mg, Modell 6570, Medtronic) wird mit 2 ml gesättigter
Natriumoxalat-Lösung überschichtet. Man setzt 37 MBq Yttrium-90-trichlorid-Lösung
hinzu und erhitzt für 30 min auf 60°C. Anschließend wird der Stent entnommen und 3× mit
5 ml 0.9%iger Natriumchlorid-Lösung gewaschen. Der so markierte Wiktor-Stent trägt
eine Aktivität von 0.88 MBq Y-90.
Ein Strecker-Stent (6,51 mg, SS/5-4, Boston Scientific) wird mit 726 µl Natriumper
technetat-Lösung (231,9 MBq) überschichtet. Man gibt 100 µl Zinn-(II)-chlorid-dihydrat
lösung (5 mg SnCl.2H2O/1 ml 0,01 M HCl) hinzu, stellt das Reaktionsgemisch für 5 min
in ein Ultraschall-Bad und inkubiert abschließend 25 min bei Raumtemperatur. Der Stent
wird getrocknet und 3× für 15 min mit 726 µl 0,9%iger Natriumchlorid-Lösung
gewaschen. Abschließend überschichtet man erneut mit 726 µl 0,9%iger Natriumchlorid-
Lösung und stellt das Reaktionsgemisch für 5 min in ein Ultraschall-Bad. Der getrocknete
Strecker-Stent trägt eine Aktivität von 1,1 MBq-Tc-99m/6,51 mg (≅ 29,7 µCi/6,51 mg ≅
4,6 µCi/1 mg).
Ein Strecker-Stent (6,60 mg, SS/5-4, Boston Scientific) wird mit 736 µl Natrium
perrhenat-Lösung (240,2 MBq) überschichtet. Man gibt 100 µl Zinn-(II)-chlorid-dihydrat-
Lösung (5 mg SnCl2.2H2O/1 ml 0,01 M HCl) hinzu, stellt das Reaktionsgemisch für 5
min in ein Ultraschall-Bad und inkubiert abschließend 25 min bei Raumtemperatur. Der
Stent wird getrocknet und 3× für 15 min mit 736 µl 0,9%iger Natriumchlorid-Lösung
gewaschen. Abschließend überschichtet man erneut mit 736 µl 0,9%iger Natriumchlorid-
Lösung und stellt das Reaktionsgemisch für 5 min in ein Ultraschall-Bad. Der getrocknete
Strecker-Stent trägt eine Aktivität von 1,0 MBq-Re-186/6,6 mg.

Ein Wiktor-Stent (22,92 mg, Modell 6570, Medtronic) wird mit 2,56 ml Natrium
pertechnetat-Lösung (911,5 MBq) überschichtet. Man gibt 256 µl Zinn-(II)-chlorid
dihydrat-Lösung (5 mg SnCl2.2H2O/1 ml 0,01 M HCl) hinzu, stellt das Reaktionsge
misch für 5 min in ein Ultraschall-Bad und inkubiert abschließend 25 min bei Raum
temperatur. Der Stent wird getrocknet und 3× für 15 min mit 2,56 ml 0,9%iger Natrium
chlorid-Lösung gewaschen. Abschließend überschichtet man erneut mit 2,56 ml 0,9%iger
Natriumchlorid-Lösung und stellt das Reaktionsgemisch für 5 min in ein Ultraschall-Bad.
Der getrocknete Wiktor-Stent tragt eine Aktivität von 5,9 MBq-Tc-99m/22,92 mg.
(≅ 159,5 µCi/22,92 mg ≅ 6,9 µCi/1 mg).

Ein Wiktor-Stent (22,31 mg, Modell 6570, Medtronic) wird mit 2,5 ml Natriumperrhenat-
Lösung (884,1 MBq) überschichtet. Man gibt 249 µl Zinn-(II)-chlorid-dihydrat-Lösung (5
mg SnCl2.H2O/1 ml 0,01 M HCl) hinzu, stellt das Reaktionsgemisch für 5 min in ein
Ultraschall-Bad und inkubiert abschließend 25 min bei Raumtemperatur. Der Stent wird
getrocknet und 3× für 15 min mit 2,5 ml 0,9%iger Natriumchlorid-Lösung gewaschen.
Abschließend überschichtet man erneut mit 2,5 ml 0,9%iger Natriumchlorid-Lösung und
stellt das Reaktionsgemisch für 5 min in ein Ultraschall-Bad. Der getrocknete Wiktor-
Stent trägt eine Aktivität von 5,2 MBq-Re-186/22,31 mg.
(≅ 140,5 µCi/22,31 mg ≅ 6,3 µCi/1 mg).

Der Wiktor-Stent (Modell 6570, Medtronic) wurde wie in Beispiel 4 beschrieben, mit
Tc-99m beschichtet. Einem narkotisierten (Rompun/Ketavet 1 : 2) weißen Neuseeländer
Kaninchen (3,2 kg Körpergewicht) wurde die A-femoralis freigelegt. Über eine 5 F-
Schleuse wurde der markierte Wiktor-Stent in das Gefäß eingeführt und durch Inflation
des Ballonkatheters in der infrarenalen Aorta fixiert. Anschließend wurde der Katheter
entfernt und sowohl die A. femoralis als auch die Wunde vernäht. Über einen Zeitraum
von 8 h nach Applikation des Stents wurden Ganzkörperszintigramme mit Hilfe einer
handelsüblichen Gammakamera angefertigt. 5 h nach Applikation des Stents wurde ein
Szintigramm angefertigt. Aktivität konnte nur im Bereich des sich in der infrarenalen
Aorta des Tieres befindlichen Stents lokalisiert werden. Während des gesamten Unter
suchungszeitraumes wurde keine detektierbare Aktivität vom Stent abgespült. Nach 8 h
wurde das Kaninchen getötet, der Stent entnommen und die Aktivität im Gammazähler
gemessen. Die am Stent haftende Aktivität war, unter Berücksichtigung des radioaktiven
Zerfalls von 99mTc in 99Tc, genauso hoch wie zu Versuchsbeginn.
In eine Zementationszelle (Abb. 2a) wird ein Strecker-Stent (1993 mg) in eine alkalische
Kupfersulfat/Kalium-Natriumtartratlösung mit einer Aktivität von 47,3 MBq gegeben.
Nach Zusatz von Formaldehydlösung erfolgt die Abscheidung von elementaren Kupfer.
Die aktive Lösung wird entfernt und der Stent wird 4 mal mit physiologischer
Kochsalzlösung gewaschen. Er zeigt eine Aktivität von 1,63 MBq.

In eine Zementationszelle (Abb. 2b) wird ein Nitinol-Stent (496 mg) in eine Lösung
bestehend aus Kalium-Goldcyanid (K[99Au(CN)4]) mit einer Aktivität von 137,8 MBq,
Kaliumcyanid und Kaliumhydroxid gegeben. Nach Erwärmung auf 75°C, wird
Kaliumborhydrid zugegeben und 3 Minuten gerührt. Nach 4 Minuten wird die Lösung
abgelassen und der Stent wird 4 mal mit physiologischer Kochsalzlösung gewaschen.
Seine Aktivität beträgt 1,31 MBq.

In eine Zementationszelle wird ein Strecker-Stent (997 mg) in eine Lösung bestehend aus
Natrium-Silbercyanid (NaAg(CN)2) mit einer Aktivität von 40 MBq/mg Stent, Natrium
cyanid, Natriumhydroxid sowie gegeben. Nach Erwärmen auf 55°C wird Dimethylboran
zugegeben. Man rührt 4 Minuten bei 55°C, läßt dann die Lösung ab, wäscht 4 mal mit
physiologischer Kochsalzlösung den Stent und bestimmt die Aktivität. Sie beträgt 1,34
MBq.

In eine Zementationszelle (Abb. 2a) wird ein Strecker-Stent (1996 mg) in eine Lösung aus
Palladiumchlorid, Salzsäure, Ammoniak und Ammonchlorid gegeben. Die Lösung hat eine
Temperatur von 55°C und wird gerührt. In die Lösung werden 9 mg
Natriumhypophosphit-Monohydrat, das eine Aktivität von 36,4 MBq hat, eingerührt. Auf
dem Stent scheidet sich eine Palladium-Phosphor-Legierung ab, die eine Aktivität von
1,31 MBq hat.

In eine Zementationszelle (Abb. 2b) wird ein Edelstahl-Stent (498 mg) in eine Lösung aus
Palladiumchlorid, Salzsäure, Ammoniak und Ammonchlorid gegeben. Die Lösung hat eine
Temperatur von 55°C und wird gerührt. In die Lösung werden 6 mg
Natriumhypophosphit-Monohydrat, das eine Aktivität von 37,8 MBq hat, eingerührt. Auf
dem Stent scheidet sich eine Palladium-Phosphorlegierung ab, die eine Aktivität von 1,16
MBq hat.

In eine Zementationszelle (Abb. 2b) wird ein Nitinol-Stent (96 mg) in eine Lösung aus
Palladiumchlorid, Salzsäure, Ammoniak und Ammonchlorid gebracht. Die Lösung hat
eine Temperatur von 55°C und wird gerührt. In die Lösung werden 3 mg
Natriumhypophosphit-Monohydrat, das eine Aktivität von 39,4 MBq hat, eingerührt. Auf
dem Stent scheidet sich eine Palladium-Phosphorlegierung ab, die eine Aktivität von 1,37
MBq hat.

In einer Galvanisierungszelle (Abb. 1) wird ein Edelstahl-Stent (1992 mg) in eine auf 50°C
erwärmte Lösung von Phosphorsäure mit einer 32P-Aktivität von 41,4 MBq gebracht.
Man schaltet den Stent als Anode und elektrolysiert 2 min bei 2 V. Dann läßt man die
Lösung ab, spült den Stent 4 mal mit physiologischer Kochsalzlösung und mißt die
Aktivität des Stents. Sie beträgt 0,93 MBq.

Claims (9)
1. Verfahren zur Herstellung radioaktiver Stents, dadurch gekennzeichnet, daß ein
nichtradioaktiver Stent in eine Lösung, die mindestens ein radioaktives Isotop in
ionischer Form enthält, getaucht wird und das Isotop dann chemisch auf dem Stent
abgeschieden wird.
2. Verfahren zur Herstellung radioaktiver Stents gemäß Anspruch 1, dadurch
gekennzeichnet, daß das Isotop reduktiv auf dem Stent abgeschieden wird.
3. Verfahren zur Herstellung radioaktiver Stents gemäß Anspruch 2, dadurch
gekennzeichnet, daß als Reduktionsmittel SnCl2, KBH4, Dimethylboran,
Formaldehyd oder Natriumhypophosphit verwendet wird.
4. Verfahren zur Herstellung radioaktiver Stents gemäß Anspruch 1, dadurch
gekennzeichnet, daß das Isotop durch chemische Fällung auf dem Stent
abgeschieden wird.
5. Verfahren zur Herstellung radioaktiver Stents gemäß Anspruch 4, dadurch
gekennzeichnet, daß als Fällungsmittel Oxalsäure, Oxalate, Phosphorsäure,
Phosphate oder Na2CO3 verwendet wird.
6. Verfahren zur Herstellung radioaktiver Stents gemäß einem der Ansprüche 1-5,
dadurch gekennzeichnet, daß das radioaktive Isotop ein Isotop des Elementes Ag,
Au, Bi, Co, Cr, Cu, Fe, Gd, Hg, Ho, In, Ir, Lu, Mn, Ni, P, Pb, Pd, Pm, Pt, Re, Rh,
Ru, Sc, Sm, Tb, Tc oder Y ist.
7. Verfahren zur Herstellung radioaktiver Stents durch chemische Fällung gemäß
Anspruch 4, dadurch gekennzeichnet daß, die Metallstentoberfläche in einer
radioaktiven Phosporsäurelösung anodisch oxidiert wird.
8. Verfahren zur Herstellung radioaktiver Stents gemäß einem der Ansprüche 1-7,
dadurch gekennzeichnet, daß mehrere Isotope oberflächlich auf dem Stent
abgeschieden werden.
9. Verwendung radioaktiver Stents, die nach einem der Verfahren nach einem der
Ansprüche 1-8 hergestellt sind, zur Herstellung eines Implantates für die
Restenoseprophylaxe.
