Abstract
Description
The invention relates to a method for closing vascular malformations, especially cerebral aneurysms, using wire-shaped Embolization elements, which are guided to the site of the Vascular malformation are placed and there due to a mechanical / thermal Pretreatment of a given thrombogenicity and / or cell proliferation adopt a supporting geometric structure.
In most cases, spontaneous subarachnoid hemorrhage is caused by the rupture an aneurysm of the basal brain vessels. The next to the Intensive care acute care required elimination of the aneurysm So far, this has been done almost exclusively through an open neurosurgical intervention. Endovascular treatments are gaining increasing potential equivalent and less invasive alternative in importance. The same applies to the Treatment of arteriovenous vascular malformations. The closure with metal spirals (Coils) inside a guide catheter at the site of the vascular malformation brought, there due to a mechanical / thermal pretreatment adopt the given geometric structure and then use a mechanical, thermal or electrochemical mechanism to be replaced, represents the endovascular procedure is the most promising technique. For this procedure However, both long-term clinical experience and extensive experience are lacking experimental and histopathological studies. So far it was assumed that the Embolization spirals should have a high thrombogenicity in order to to be able to achieve reliable closure. The various attempted this Manufacturers by using different metals as the basic substance (e.g. Platinum, iridium or tungsten). The metal used should be as possible be inert and thrombogenic and have a high X-ray density. An increase thrombogenicity was attempted by covering the spirals with plastic fibers (Nylon, Dacron) to achieve (WO 95/25480-A1). The clinical experience confirmed however, that both methods do not result in sufficient occlusion. in the Clinical trials are increasingly showing that long-term results due to the frequently observed recanalization of the underlying Vascular sagging is unsatisfactory. Significant improvements to the Method to use it as a routine intervention and as an alternative to conventional methods to establish. The same applies in principle outside of the cerebral area to all other areas of induction of such embolization spirals (e.g. in the peripheral vascular area).
One cause of the recurrence is the so-called "compacting" of the spirals due to their given shape (helical spiral turns) have the tendency contract and - supported by the pulsations of the arterial Blood flow - reconfigure. This disadvantage can only be partially overcome compensate for the fact that several spirals are inserted into the aneurysm sac to thereby to achieve a very high packing density. Technically, however, this is often not or only feasible with very high risk for the patient. There is an increased risk perforation of the vessel wall and the dislocation of individual spirals into the lumen of the Carrier vessel. Based on experimental results it could be shown that only an extremely high packing density guarantees the histologically stable closure.
The main cause of recurrence is not a lack of thrombogenicity Embolization spirals can be seen, but rather in the early onset Fibrinolysis, which almost always arises after the application of the spirals Thrombi dissolves within one to two weeks.
On the other hand, the high thrombogenicity also leads to the problem of Flushing the clot from the aneurysm into the carrier vessel and closing it secondary embolic complications (in the cerebral area the stroke).
Zusammenfassend haben die bisherigen Verfahren folgende Nachteile:In summary, the previous methods have the following disadvantages:
Tendency to compact with the consequence of relapse of the disease
Requirement for a high packing density with the consequence of an increased Treatment risks and significantly increased therapy costs
- 3. Risiko sekundär embolischer Komplikationen.3. Risk of secondary embolic complications.
The present invention has for its object a method of the beginning mentioned type to develop in such a way that in the treatment of Vascular malformations, especially of cerebral aneurysms, are both dangerous secondary embolisms are eliminated and spontaneous fibrmolysis is prevented and prevented stable, permanent closure if possible with the repair of the former Vascular wall defect is achieved.
In a method of the type mentioned at the outset, this object is achieved by that the embolization elements one after passing through the guide catheter complex three-dimensional structure with an envelope that is roughly the same corresponds to the anatomical structure of the vascular malformation to be treated and / or that the embolization elements with a biologically active coating be provided.
The method according to the invention allows the prior art avoid the underlying disadvantages and permanently switch off the achieve mentioned vascular malformations. In addition to treating aneurysms of the basal brain vessels, the procedure is suitable. a. also for the closure of Blood vessels that lead to tumors, for the treatment of non-closing Vascular connections between the lungs and heart in newborns (angiomas, pertaining ductus botalli) and for closing fistulas and short-circuits between veins and arteries.
Due to the geometrical structure of the used given according to the invention Embolization elements with an envelope that approximates the anatomical structure of the vascular malformation to be treated results in a dense wire mesh, that fills the entire inner area of the malformation, so to speak as uniform base and frame for the newly formed tissue. Thereby both thrombogenicity and cell proliferation become essential improved. With the invention it is thus achieved that one of the anatomical structure embolization element in the interior of the vascular malformation Malformation is introduced which is not contracted or deformed and a recanalization by changing the hemodynamics ("breakwater effect") prevented. As with conventional spirals, the application is stretched Form by a commercially available microcatheter. After inserting the preformed Embolization element takes this again due to its inherent elasticity (so-called "Memory Effect") the given complex form and closes it Aneurysm, or impedes the blood flow so significantly that it slows down the flow and turbulence in the lumen creates an intraluminal thrombosis. In contrast to The previously used helix spirals become one by the given structure Reorganization and re-taking of the helix form and the resulting one Recanalization of the aneurysm inside prevented. No longer multiple helicals The risk of perforation is clear when embolization spirals have to be inserted less. The application time and thus the X-ray exposure of the patient is reduced, and also the risk of dislocation of individual spirals avoided.
Der Durchmesser des Grunddrahtes liegt bei 0,01 bis 0,05 mm.The method according to the invention not only induces thrombosis, but rather the growth of fibroblasts and other connective tissue cells stimulated so that the former ostium can also overgrow. Here the shape of the embolization element is chosen so that compacting is avoided. This is the only way to guarantee the final repair of the defect. The shape of the spiral wire, which has a variable length of approx. 2 to 100 cm, typically 5 to 20 cm, depending on the size of the patient to be treated Vascular malformation) takes place by winding on a template or a Cross winding and selective shaping, such as. B. controlled heating in the Metals. The stencils can have different sizes so that the different complex shaped embolization elements in different sizes Are available (envelope diameters from 2 mm to 20 mm, or more). Of the Diameter of the spiral is 0.1 to 0.4 mm, so that an application using the commercially available vascular coaxial microcatheters is possible. The diameter of the Basic wire is 0.01 to 0.05 mm.
When performing appropriate operations, a X-ray examination of the anatomical structure of the vascular malformation first measured and then from different, different sizes Embolization elements selected the appropriate one.
It has been shown that in the most common applications, embolization elements, which have a spherical, conical or ellipsoidal envelope structure, on are most suitable.
An embolization element, its network structure, is expediently used is such that the distance between adjacent wire sections within the Embolization element is less than 1.5 wire diameter.
A further increase in thrombogenicity and cell proliferation can be seen the inventive method by a biologically active coating Reach embolization elements.
Such a biological coating includes the connection and / or the Release of biologically active substances that promote cell growth Development of a stable occlusion of vessels promotes. The biologically active Substances accelerate the initial colonization of the embolization elements Cells (fibroblasts) promote their spread along the Embolization elements and promote cell growth. Furthermore is on Thrombogenic substances are thought to prevent the formation of permanent Enhance the clot in the cavity and prevent acute fibrinolysis. Consequently there is a complete filling of the cavity, which is a prerequisite for is an overgrowth of the ostium with endothelial cells, and thus a permanent one Repair the defect.
In particular, as biologically active substances in the sense of the invention Fibronectin, vibronectin, laminin, albumin, collagens, growth hormones, such as B. Insulin or somatropin, growth factors such as B. Insulin-like Growth factors (IGF-I, IGF-II), epidermal growth factor (EGF), Platelet growth factor (PDGF), fibroplast growth factor (bFGF, aFGF,), transforming growth factor (TGF-β), erythropoietin, Nerve growth factors, brain cell growth factors or endothelial cell growth factor (VEGF), tumor necrosis factors (TNF-α, TNF-β), prostatropins, Prostaglandins, thromboxanes, leukotrienes, immunoglobolins, inerferrons, Interleukins, and / or thrombus-promoting substances such. B. thrombin, Fibrinogen, coagulation factors or prothrombin, are suitable. However, are preferred proliferation-promoting substances, such as fibronectin, are used.
It proves to be particularly advantageous if the Invention on the embolization elements as a carrier material for the biological active substances, an intermediate layer of polymers is first applied. The biologically active substances are created using bivalent bridge molecules (Spacer) covalently bound to functional groups of the polymer surfaces. As Spacers can e.g. B. diisocyanates, dicarboxylic acid chlorides, dicarboxylic acid succinimides, other dicarboxylic acid derivatives or carbodiimides can be used.
According to a further feature of the invention, the biologically active Substances also in a degenerable polymer layer, e.g. B. from polylactides, Polyesters or polyamino acids. You will then be with progressive degradation of the polymer layer is continuously released.
Preferred polymer interlayers are preferred for the purposes of the invention substituted poly-p-xylylene used by CVD polymerization (CVD: chemical vapor deposition) from e.g. B. amino, hydroxy, carboxy, (hydroxyl) alkylene, Chlorine or trifluoroacetyl-p-cyclophanes can be generated by the Gorham process. The p-cyclophanes become more than 650 ° C at reduced pressure and temperatures split and polymerize at temperatures below 200 ° C on the surface of the embolization element. This procedure offers with regard to a application according to the invention numerous advantages, such as the uniform Coating from the gas phase, dispensing with the use of solvents, Polymerization initiators or additives, an effective use of the used Monomer amounts and the possibility of targeted adjustment of Surface parameters.
Furthermore, the application of the polymer intermediate layer can be carried out by Plasma polymerization of olefins with the connection of biologically active Substances suitable functional groups, such as. B. allylamine, allylic cabbage, Butenols, butylamines, acrylic acid, acrylic acid derivatives, acrylates, Hydroxymethyl acrylate. Also ethene, propene, ethyne, propyne, acetone - typically as a mixture with oxygen or sulfur dioxide - are to be generated of such a polymer intermediate layer.
Furthermore, it has been shown that the polymer intermediate layer can also be coated with polymers, such as. B. polyurethanes, polyolefins, polyesters or polysaccharides can be done from the liquid phase. Then the polymer interlayer activated by argon plasma treatment. By subsequent irradiation of the activated surface with an excimer lamp or an excimer laser Graft copolymers with hydrogels, such as polyhydroxymethyl acrylate, polyacrylate, Polyethylene oxide or poly-4- (acryloyloxy) butyl hydrogen glutarate. To the terminally functional groups become the biologically active substances bound.
According to the invention, an embolization spiral made of metal can also be used Polymer thread can be used, the z. B. from polytetrafluoroethylene, polyamides, Polyesters, polyolefins, polyurethanes or polycarbonates can be made and typically with radiopaque substances such as B. powdered tantalum, powdered Tungsten, barium sulfate, bismuth oxide, carbonate or sulfate is added. In this Traps can be suitable functional ones for binding bioactive substances Groups - provided they are not already on the surface of the polymer interlayer are present - by applying one of the functionalized ones Polymer layers or by methods of production established for polymers functional groups, such as B. plasma etching, radiation or wet chemical Modifications are made.

Figs. 1 to 3 show the treatment of a cerebral aneurysm. According to Fig. 1, x out and monitored, first a guide catheter (1) for an embolization element (2) into the damaged blood vessel (3) into the interior of the aneurysm out (4). The wire-shaped embolization element ( 2 ) is then pushed in the guide catheter into the region of the malformation. An embolization element ( 2 ) consists of a metal or a metal alloy which is free of surface oxidation at room temperature, body temperature and during heating or cooling, e.g. B. platinum or platinum-iridium alloys. Designs made of other metals or alloys, consisting e.g. B. from tungsten, tantalum, iridium, gold, niobium, rhodium, osmium, palladium, nickel-titanium alloys or stainless steels, however, are also possible in principle.
The embolization element ( 2 ) / ( 5 ) is mechanically / thermally pretreated in such a way ("memory effect") that after passing through the guide catheter it adopts a specific, predetermined three-dimensional, geometric structure with an envelope which is approximately the same as the anatomical structure of the treating vascular malformation corresponds (see Figs. 2 and 3).
The resulting three-dimensional network ( 5 ), which evenly fills the vascular malformation, promotes cell growth considerably and thus contributes significantly to the desired, fast and stable occlusion of the aneurysm. The desired success is significantly increased if, according to the invention, the embolization element ( 2 ) / ( 5 ) is additionally provided with a biologically active coating.
In addition to the advantages already mentioned, the coating of the embolization element leads to a reduction in the frictional resistance when the elongated embolization element ( 2 ) is inserted through the guide catheter ( 1 ), which considerably simplifies the operation procedure. Of course, the application of a biologically active coating, consisting of an intermediate polymer layer and biologically active substances, is not bound to the geometry of the embolization element. Conventional embolization elements can also be activated with similar success with the proposed biologically active coating.
