Description
Die Erfindung betrifft ein Implantat zur Suspension der Harn
blase bei Harninkontinenz der Frau.
Die Therapie der Harninkontinenz der Frau zeichnet sich durch
eine Vielzahl unterschiedlicher Behandlungskonzepte aus. Während
leichtere Formen der Streßinkontinenz noch erfolgreich durch
Beckenbodentraining und pharmakologische Therapie behandelt
werden können, so bleibt für die schweren Formen der Harninkon
tinenz nur die operative Therapie. Die Ziele der operativen
Therapie sind es, die anatomisch adäquate und dauerhafte Ver
lagerung des Blasenhalses nach cranio ventral in den abdominalen
Druckbereich zu erreichen und bei einem Descensus genitalis und
Prolaps rekonstruktive Maßnahmen zu ergreifen, um den insuffi
zienten Suspensions- und Suppositionsapparat wieder tragfähig zu
machen.
Etwa 200 Operationsmethoden, Modifikationen und modifizierte
Modifikationen zeigen die Uneinigkeit des therapeutischen Vor
gehens, siehe z. B. N.S. Horbach, "Genuine SUI: best surgical

(September 1994). Angesichts der multifaktoriellen Entstehung
der Streßinkontinenz wählt der Operateur in der Regel die Metho
de aus, die den im individuellen Fall vorliegenden Bedürfnissen
am ehesten gerecht wird. Transvaginale und suprapubische Zugänge
werden ebenso kombiniert wie Gewebsraffungen, Fixationen und
suspendierende Zügel aus Fascienband, Lyodura oder alloplasti
schem Material. Die individuell unterschiedlichen Befundkonstel
lationen erlauben es nicht, einem bestimmten Operationsverfahren
einen generellen Vorzug zu geben. Vielmehr gilt es, aus der
gesamten Breite der zur Verfügung stehenden therapeutischen
Möglichkeiten sorgfältig auszuwählen, abzuwägen und zu entschei
den.
Aus der EP 0 498 912 A1 ist eine intravaginal anzuwendende Vor
richtung zur Behandlung der Harninkontinenz bekannt. Die Vor
richtung ist aufblasbar und besteht im wesentlichen aus einem
Ring mit zwei nach vorne weisenden Vorsprüngen. Sie wird so
plaziert, das die Vorsprünge zu beiden Seiten der Harnröhre
anliegen und das Gewebe dazwischen straffen. Dadurch wird bei
einem plötzlichen Druckanstieg im Unterleib, wie er zum Beispiel
beim Husten auftritt, ein Druck auf die obere Harnröhre ausge
übt, so daß die Kontinenz erhalten bleibt.
Eine weitere intravaginal applizierbare Vorrichtung zur Behand
lung der Harninkontinenz ist in der US 5 386 836 A beschrieben.
Die Vorrichtung besteht aus flexiblem Material und hat ein ring
förmiges Basisteil und zwei nach oben weisende Vorsprünge, mit
deren Hilfe der Blasenhals in angehobener Stellung gehalten
werden kann. Die Vorsprünge können direkt an dem Basisteil ange
ordnet sein oder vom Ende eines zwischenliegenden Teils ausge
hen.
Es ist Aufgabe der Erfindung, ein Implantat zur zuverlässigen
Therapie der Harninkontinenz der Frau zu schaffen, insbesondere
bei extremer Beckenbodenschwäche mit prolabierender anatomischer
Verlagerung der Organe des kleinen Beckens sowie bei Patientin
nen nach mehrfachen erfolglosen Voroperationen mit den üblichen
Techniken.
Diese Aufgabe wird gelöst durch ein Implantat zur Suspension der
Harnblase bei Harninkontinenz der Frau mit den Merkmalen des
Patentanspruchs 1. Vorteilhafte Ausgestaltungen ergeben sich aus
den Unteransprüchen.
Nachdem das erfindungsgemäße Implantat in einem chirurgischen
Eingriff eingesetzt worden ist, wie weiter unten beschrieben,
liegt die Harnblase breitflächig auf dem Implantat auf, wodurch
eine absolut stabile bilaterale Fixation der Harnblase sowohl im
Blasenhals- als auch im Blasenscheitelbereich erreicht wird.
Durch eine Vierpunkte-Aufhängung mittels der beiden ersten Fort
sätze und der beiden zweiten Fortsätze, die von der Basis der
Grundstruktur des Implantats ausgehen, wird die Harnblase un
abhängig vom Zustand des Beckenbodens wie auf einer Hängematte
vom Implantat bedingt elastisch getragen. So kann ein erneuter
Prolaps oder Descensus auch unter Belastung sicher vermieden
werden. Mit dem erfindungsgemäßen Implantat wird nicht nur der
Blasenausgang, sondern die gesamte Harnblase in eine breite, den
ganzen Blasenboden unterstützende stabile Lagerung mit einbezo
gen. Daraus resultiert keine Erhöhung des Auslaßwiderstandes,
sondern ausschließlich eine streßsichere Entlastung des Sphinc
ter vesicae externus. Eine Obstruktionsgefährdung ist bei regel
rechter Einbringung des Implantates nicht gegeben.
In einer bevorzugten Ausführungsform weist die Grundstruktur des
Implantats mehrere Schichten auf. Dabei kann ein Netz aus Poly
propylen, das nicht resorbierbar ist, auf beiden Seiten mit
einer porösen Beschichtung aus einem resorbierbaren Kompositma
terial aus Polyglactin 910 (einem Copolymer aus Glycolid und
Lactid im Verhältnis 9 : 1) und Polydioxanon versehen sein.
Im Zuge des Abbaus der resorbierbaren Anteile des Implantates im
Körper erfolgt ein bindegewebiger Ersatz dieser Anteile mit
Durchbauung des permanent verbleibenden Netzes. Das Netz ist
daher vor Dislokation gesichert und das umgebende Gewebe gegen
mechanische Reizung bzw. Erosion geschützt. Es resultiert eine
stabile, dauerhafte Suspension der Blase.
Im folgenden wird die Erfindung anhand eines Ausführungsbei
spiels näher beschrieben. Die Zeichnung zeigt in
Fig. 1 eine Aufsicht auf eine Ausführungsform des Implantats.
In Fig. 1 ist eine Ausführungsform eines erfindungsgemäßen
Implantats in Aufsicht dargestellt. Das Implantat besteht aus
einer flächigen, flexiblen Grundstruktur.
Eine Basis 1 ist dreieckähnlich bis länglich oval und hat eine
Längsachse L-L. Im Ausführungsbeispiel ähnelt die Form der Basis
1 eher einem Dreieck mit den Ecken 2, 3 und 4, wobei die Ecke 2
(durch die die Längsachse L-L der Basis 1 verläuft) abgerundet
ist. Die Fläche der Basis beträgt ca. 30-50 cm².
Von der Basis 1 geht in der Nähe der Ecke 2, aber beabstandet
dazu, ein erster Fortsatz 5 aus, der zügelartig gestaltet ist.
Der erste Fortsatz 5 verläuft auf der linken Seite der Längs
achse L-L, und zwar unter einem spitzen Winkel in bezug auf die
Längsachse L-L, der im Ausführungsbeispiel kleiner als 20° ist.
Spiegelsymmetrisch zur Längsachse L-L ist ein weiterer erster
Fortsatz 6 angeordnet, der dieselbe Form hat wie der erste Fort
satz 5.
Von der Ecke 3 der Basis 1 geht ein zweiter Fortsatz 7 aus, der
auf der linken Seite der Längsachse L-L und im wesentlichen
parallel dazu verläuft. Der zweite Fortsatz 7 ist also generell
dem ersten Fortsatz 5 entgegengesetzt gerichtet, d. h. während
sich in Fig. 1 der erste Fortsatz 5 (schräg) nach oben er
streckt, verläuft der zweite Fortsatz 7 nach unten. Ebenso wie
der erste Fortsatz 5 könnte auch der zweite Fortsatz 7 einen
Winkel zur Längsachse L-L der Basis 1 bilden, der von 0° ver
schieden ist. Von der Ecke 4 der Basis 1 geht ein weiterer zwei
ter Fortsatz 8 aus, der in bezug auf die Längsachse L-L spiegel
bildlich zum zweiten Fortsatz 7 verläuft. Die beiden zweiten
Fortsätze 7 und 8 haben die gleichen Maße und sind länger als
die beiden ersten Fortsätze 5 und 6.
Zwischen den beiden ersten Fortsätzen 5 und 6, im Bereich der
Ecke 2, bildet die Basis eine halbrunde oder ovale Ausziehung
von ca. 2-3 cm² Fläche. Der Zweck dieser Ausziehung ist die
Suspension des Blasenhalses und der proximalen Urethra, so daß
nach Einsetzen des Implantats insgesamt eine vollständige Ab
stützung von Blasenboden, Blasenhals und proximaler Urethra
erreicht wird.
In der Nähe der Enden der ersten Fortsätze 5 und 6 und der zwei
ten Fortsätze 7 und 8 können Löcher 9 vorgesehen sein, die beim
Einsetzen und Fixieren (Festnähen) des Implantats von Hilfe sein
können.
Im Ausführungsbeispiel hat das Implantat drei Schichten. Die
mittlere Schicht besteht aus einem nicht resorbierbaren, ge
schmeidigen Netz von 0,7 mm Dicke, das aus monofilen Polypropy
lenfäden geknüpft ist. Dieses Material verliert im Körper seine
physikalischen Eigenschaften langfristig nicht und ist unemp
findlich gegen pH-Wert-Schwankungen. Es ist elastisch und uni
direktional dehnbar, um Formänderungen zu ermöglichen, wie sie
z. B. bei Druck auf das Abdomen, durch den Füllungszustand der
Blase oder bei Miktion gegeben sind.
Auf beiden Seiten des Polypropylennetzes ist eine poröse Be
schichtung aus einem resorbierbaren Kompositmaterial aufge
bracht, das aus Polyglactin 910 und Polydioxanon besteht, siehe
zum Beispiel DE 38 30 005 C1. Im Ausführungsbeispiel werden
diese äußeren Schichten nach Implantation innerhalb von ca. 120
Tagen rückstandsfrei resorbiert.
Der Grundgedanke dieses Materialverbundes ist es, die bindege
webskonduktiven Eigenschaften der Schichten aus dem Kompositma
terial, die vorzugsweise als Vlies-Schichten gestaltet sind, mit
der dauerhaften Stabilität des Polypropylennetzes zu kombinie
ren. Tierexperimentielle Befunde zeigen, daß die oberflächliche
Vliesschicht innerhalb der ersten drei Wochen mit zahlreichem
fibrohistiozytärem Gewebe durchbaut wird. Insbesondere Filamente
aus Polyglactin 910 dienen als Leitschiene für die Ausbildung
gerichteter kollagener Bindegewebsfasern, die im Unterschied zu
Narbengewebe keinerlei Schrumpfungstendenz zeigen. Mit fortge
schrittener Resorption der äußeren Schichten wird das Netz aus
Polypropylen durch lockeres kollagenes Bindegewebe durchbaut und
von diesem umgeben. Diese Einheilungsvorgänge sichern das Im
plantat vor Dislokation und schützen das umgebende Gewebe gegen
mechanische Reizung bzw. Erosion, bei fester und dauerhafter
Suspension der Harnblase. Die gute Gewebsverträglichkeit der
verwendeten Materialien ist durch breite klinische Anwendung
bestätigt.
Die in vitro gemessenen Berstdrücke des Implantats liegen weit
über den physiologisch unter Belastung am Menschen auftretenden
Kräften. Das Implantat ist unidirektional um ca. ein Drittel
seiner Ausgangslänge dehnbar.
Das in Fig. 1 dargestellte Implantat hat eine Basis 1 mit drei
eckähnlicher Form. Abweichungen hiervon sind möglich. Wie schon
erwähnt, müssen die ersten Fortsätze 5 und 6 auch nicht genau
entgegengesetzt zu den zweiten Fortsätzen 7 bzw. 8 verlaufen.
Die genaue Form des Implantats kann auf die anatomischen Gege
benheiten der Patientin abgestimmt sein.
Auch der beschriebene Schichtenaufbau des Implantats und die
Materialauswahl dienen nur als Beispiel. Andere gewebeverträgli
che Materialien können ebenso zum Einsatz kommen.
Im folgenden wird eine Möglichkeit beschrieben, wie das erfin
dungsgemäße Implantat in einem chirurgischen Eingriff eingesetzt
werden kann.
Von einem Pfannenstielschnitt aus wird das Spatium retropubicum
dargestellt. Nach Lösen des Blasenscheitels vom Peritoneum bis
zur Scheide wird die Blase vom Scheidendach abpräpariert, und
zwar proximal gut ca. zwei fingerbreit bis zum Blasenhals und
dort nur noch ca. ein fingerbreit. Die lateral verlaufenden
Gefäße und der Harnleiter sind sorgfältig zu schonen. Dieses
präparative Vorgehen kann durch präoperatives Unterspritzen der
Vagina mit Kochsalzlösung allein oder durch Zusatz von Suprare
nin 1 : 200 000 erleichtert werden. Es empfiehlt sich, die Scheide
auszutamponieren. Bei liegendem Dauerkatheter gelingt die Präpa
ration mit einem Overholt und einer Schere zwischen Blasenhin
terwand und Scheide nach distal und lateral, so daß sich die
Overholt-Spitze beidseitig paraurethral im Spatium retropubicum
darstellen läßt.
Von retrosymphysär zu beiden Seiten des Urethra/Blasenhalswin
kels wird jeweils ein kräftiger Führungsfaden mit dem Overholt
zwischen Blase und Scheide eingezogen. An diesen Führungsfäden
lassen sich die beiden zweiten Fortsätze 7, 8 oder vorderen
(proximalen) Zügel des Implantats zwischen Scheide und Blase
nach retrosymphysär einziehen und exakt plazieren. Die breite
Basis 1 des Implantats kommt zwischen Blase und Scheide zu lie
gen. Auf eine ausreichend weit unter den Blasenhals reichende
Lage des alloplastischen Implantats ist zu achten, um eine be
stehende Blasenhalsinsuffizienz zu beheben oder eine solche
nicht entstehen zu lassen. Die beiden ersten Fortsätze 5, 6 oder
hinteren (distalen), retropubischen Zügel werden zu beiden Sei
ten der Urethra vorbeigeführt und hinter den beiden Schambeinä
sten an dem Ligamentum pubicum superior fixiert.
Die Blasenhinterwand vom Blasenhals bis zum Blasenscheitel ruht
breitflächig auf dem Implantat. Beim Einbringen des Implantats
ist darauf zu achten, daß die oft sehr dünne Blasenwand nicht
mit urethralwärts geschoben wird, sondern nach abdominal hin
angespannt ist. Knautscht das Blasengewebe im Blasenhalsbereich,
kann hier später ein Prolabieren dieses überschüssigen Blasen
wandanteils resultieren.
Zur Vermeidung einer Dislokation ist dennoch eine ausreichende
Anzahl von feinen monofilen Fixationsnähten (z. B. mit Fadenstär
ke 4/0) zu empfehlen. Es bietet sich an, die Scheide durch Ein
zelknopfnähte zu beiden Seiten am Implantat anzuheften und damit
die Vagina in die Suspension mit einzubeziehen.
Vor Befestigung der ersten Fortsätze 5, 6 wird die Harnblase mit
ca. 300 ml Kochsalzlösung aufgefüllt, um sicherzustellen, daß
der Harnblase genügend Raum zur Ausdehnung und zur Erhaltung
einer ausreichenden Blasenkapazität verbleibt. Die beiden zwei
ten Fortsätze 7, 8 des Implantats werden rechts und links durch
den Muskulus rectus hindurchgezogen und über diesen kreuzweise
adaptiert.
Ein zu starkes Anziehen der zweiten Fortsätze 7, 8 birgt die
Gefahr, den retropubischen Raum und damit die Ausdehnungsmög
lichkeit der Harnblase zu sehr einzuschränken.
Zur Vermeidung einer Infektion ist von einer Blaseneröffnung
oder einer suprapubischen Harnableitung möglichst abzusehen.
Die Verwendung des erfindungsgemäßen Implantats versteht sich
nicht als Universalmethode, sondern sie stellt insbesondere ein
zuverlässiges Verfahren in Extremsituationen eines Descensus
vesicae bei generalisierter Beckenbodenschwäche und -rarifizie
rung der Gewebstextur dar. Die Harnblasensuspension mit dem
Implantat eignet sich besonders bei ausgeprägten Rezidiven, auch
nach mehreren vorausgegangenen Eingriffen. Neben einer schnellen
postoperativen Mobilisation ist auch dann eine rasche berufliche
Integration gewährleistet, wenn durch schweres Heben der Becken
boden hoher Belastung ausgesetzt wird. Bei extrem adipösen Pa
tientinnen mit vertikalem und rotatorischem Descensus wäre eine
Verwendung des erfindungsgemäßen Implantats als Primäreingriff
denkbar. Der wesentliche Vorteil liegt in der breitflächigen,
absolut stabilen, bilateralen Fixation der Harnblase sowohl im
Blasenhalsbereich als auch im Blasenscheitelbereich. Unabhängig
vom Zustand des Beckenbodens wird die Harnblase wie auf einer
Hängematte vom Implantat bedingt elastisch getragen. Ein erneu
ter Prolaps oder Descensus kann auch bei Belastung sicher ver
mieden werden. Da mit dem Implantat nicht nur der Blasenausgang,
sondern die ganze Harnblase in eine breite, den ganzen Blasenbo
den unterstützende, feste Lagerung mit einbezogen ist, wird
durch die Suspension des Implantats nicht eine Erhöhung des
Auslaßwiderstandes angestrebt, sondern ausschließlich eine
streßsichere Entlastung des Sphincter vesicae externus. Eine
Obstruktionsgefährdung ist bei regelrechter Einbringung des
Implantates nicht gegeben.
Claims (12)
1. Implantat zur Suspension der Harnblase bei Harninkontinenz
der Frau, mit einer flächigen, flexiblen Grundstruktur, bei
der von einer dreieckähnlichen bis länglich ovalen Basis
(1) zwei erste Fortsätze (5, 6) und zwei zweite Fortsätze
(7, 8) ausgehen, wobei die beiden ersten Fortsätze (5, 6)
auf gegenüberliegenden Seiten der Längsachse (L-L) der
Basis (1) verlaufen und wobei die beiden zweiten Fortsätze
(7, 8), generell entgegengesetzt zu den beiden ersten Fort
sätzen (5, 6), auf gegenüberliegenden Seiten der Längsachse
(L-L) der Basis (1) verlaufen.
2. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die
Grundstruktur symmetrisch zur Längsachse (L-L) der Basis
(1) ausgestaltet ist.
3. Implantat nach Anspruch 1 oder 2, dadurch gekennzeichnet,
daß die beiden ersten Fortsätze (5, 6) und die beiden zwei
ten Fortsätze (7, 8) zügelartig sind.
4. Implantat nach einem der vorhergehenden Ansprüche, dadurch
gekennzeichnet, daß die beiden ersten Fortsätze (5, 6)
kürzer sind als die beiden zweiten Fortsätze (7, 8).
5. Implantat nach einem der vorhergehenden Ansprüche, dadurch
gekennzeichnet, daß die Basis (1) dreieckähnlich ist und
die beiden ersten Fortsätze (5, 6) in der Nähe der Ecke
(2), durch die die Längsachse (L-L) der Basis (1) verläuft,
aber beabstandet dazu, von der Basis (1) ausgehen, so daß
zwischen den beiden ersten Fortsätzen (5, 6) im Bereich der
Ecke (2) eine halbrunde bis ovale Ausziehung zur Abstützung
des Blasenhalses und der proximalen Urethra ausgebildet
ist.
6. Implantat nach Anspruch 5, dadurch gekennzeichnet, daß die
beiden ersten Fortsätze (5, 6) unter einem spitzen Winkel,
der kleiner ist als 20°, zur Längsachse (L-L) der Basis (1)
verlaufen und daß die beiden zweiten Fortsätze (7, 8), aus
gehend von den beiden übrigen Ecken (3, 4) der Basis (1),
im wesentlichen parallel zur Längsachse (L-L) der Basis (1)
verlaufen.
7. Implantat nach einem der vorhergehenden Ansprüche, dadurch
gekennzeichnet, daß die Grundstruktur ein nicht resorbier
bares Material aufweist.
8. Implantat nach Anspruch 7, dadurch gekennzeichnet, daß das
nicht resorbierbare Material Polypropylen ist.
9. Implantat nach einem der vorhergehenden Ansprüche, dadurch
gekennzeichnet, daß die Grundstruktur ein resorbierbares
Material aufweist.
10. Implantat nach Anspruch 9, dadurch gekennzeichnet, daß das
resorbierbare Material ein Gemisch aus Polyglactin 910 und
Polydioxanon ist.
11. Implantat nach einem der vorhergehenden Ansprüche, dadurch
gekennzeichnet, daß die Grundstruktur mehrere Schichten
aufweist.
12. Implantat nach Anspruch 11, dadurch gekennzeichnet, daß die
Grundstruktur dreischichtig aufgebaut ist, wobei ein Netz
aus Polypropylen auf beiden Seiten mit einer porösen Be
schichtung aus einem Kompositmaterial aus Polyglactin 910
und Polydioxanon versehen ist.
