Abstract

The invention relates to an occlusion device 1 and a method for producing an occlusion device 1. The occlusion device 1 essentially consists of nitinol, a material with a shape memory function, the basic shape being produced by means of laser-cut form elements 2 from a tube. In the expanded state, the occlusion device 1 has a proximal and a distal retention area 10, 9 as well as a cylindrical waist 13 arranged therebetween. Due to the fact that the lasered form elements 2 can be adapted specifically to the shape properties of the occlusion device 1, it is generally achieved that the discs 11, 12 abut extremely flat against the septum wall and that occlusion or stretching caused by the shunt does not occur in the occlusion device 1 can, as is possible with the usual mesh occluders. In the manufacturing method according to the invention, a laser technique is used in which the final shape of the mold elements 2 is introduced into a corresponding Nitinolrohr by lasered recesses. Subsequently, the final shape of the occlusion device 1 is achieved by a suitable deformation and heat treatment process. Due to the variety of lasing, firmly interconnected form elements 2, the Occlusionsinstrument 1 can be used modified as PFO, ASD, VSD and / or Paravalvulärer leak Occluder.

Description

The present invention relates to an occlusion device, which consists essentially of a combination of lasered mold elements and receives a suitable shape by means of a forming and heat treatment process, with a proximal retention area and a virtually cylindrical ridge between the proximal and the distal retention area, the two retention areas a mostly intravascular intervention on both sides of a shunt to be closed in a septum come to rest, the web passes through the shunt.

Such an occlusion device is suitable for placement in otherwise difficult to reach defect sites in the human heart by minimally invasive treatment methods via the use of catheter technology, such as occlusion of holes (shunts) in the area of the atrial septum between the two atria of the heart (Atria ), the so-called atrial septal defect (ASD), the patent foramen ovale (PFO), but also in an adapted form in the ventricular septum ventricular defect (VSD) and other applications.

Stand der TechnikState of the art

In medical technology, septal defects such as the PFO and ASD defects described at the outset have long been closed by means of a transvenous interventional approach. In the past, various occlusion systems, which essentially consist of a mesh of thin wires or threads, have prevailed.

These self-centering occlusion devices are inserted minimally invasively via a catheter and guide wires into the body of the patient and inserted into the septal defect to be closed. The design is based on the principle that the occlusion device can be tapered to the size of the introducer or catheter used for the intravascular surgical procedure. Such a tapered occluder is then introduced via the catheter into the septal defect to be closed. Thereafter, the Occluder emerges from the catheter, whereupon unfold the self-expanding Schirmchen or retention barriers, which rest on both sides of the septum. The screens in turn contain, for example, fabric inserts made of PET (polyethylene terephthalate) or are spanned by such, whereby the defect or shunt is finally closed. The remaining in the body implants are more or less completely enclosed by endogenous tissue after a few weeks to months.

An example of such a self-centering occluder is the so-called Amplatz occluder with two distal clamps according to the U.S. Patent No. 5,725,552 , It consists of a tubular mesh made of a variety of braided Nitionol wires. This braid is produced in its original form as a round braid which has loose wire ends both at its beginning (or at its proximal side) and at its end (or at its distal side). In the further processing of the round braid then these loose ends must be taken in each case in a sleeve and welded. After this corresponding further processing, both the proximal side and the distal side of the finished occluder each have a protruding sleeve. Dacron patches are sewn into the distal and proximal retention shields and the interposed ridge. Due to the memory effect of the nitinol material used, the two retention screens unfold automatically when leaving the catheter. This is initially done via a balloon-like intermediate stage, wherein the retention screens finally finally occupy a more or less flattened shape on both sides of the septum. The web centers itself during the Aufspannens Schirmchen in the shunt to be closed independently.

Another example of such a self-centering occluder is the Occlutech occluder according to DE10338702 , In contrast to the Amplatz occluder, this occlusion device is made of a ball mesh and only has a clamp at the proximal end. The wires of the braid are made of the same material as the Amplatz occluder; This superelastic metal alloy is made of titanium and nickel and is known by the name Nitinol and is used in a variety of medical devices.

Weitere ähnliche Occluder sind z. B. in WO2007/140797A1 , EP2014239A2 und EP1955661A2 offenbart.Other similar occluders are z. In WO2007 / 140797A1 . EP2014239A2 and EP1955661A2 disclosed.

Such and similar braiding occluders usually consist of permanently intersecting wires or threads, with the result that these occluders clearly stretch and bulge both in the PFO slot and ASD defect, and thus a sealing of the defect is only insufficiently effected , The stretching and buckling of such occluders also leads to considerable space losses in the right and left atria, which can adversely affect the pumping power of the heart. In addition, can be on the edges of the arched Occluder thrombi, which can cause strokes and rare infarcts in embolization.

Aufgabenstellungtask

The object of the invention is therefore to provide a functioning Occlusionsinstrument (Occluder), which does not stretch in the defect and bulges, flat ausformt and thus optimally adapted to the antechamber septum. This object of the invention is achieved by an occluder according to claim 1. Advantageous embodiments can be found in the dependent claims.

The present invention according to claim 1 relates to a Nitinolrohr lasered and stent-like in its basic form body, which by repeated application of a suitable Umformungs- and heat treatment process is given a suitable final shape with a rotationally symmetric disc as a proximal retention area, a rotationally symmetrical distal disc and an approximately cylindrical Bridge between the two discs, wherein in the bridge radial terminals are integrated for better anchoring in the defect. The core of the invention is that this specially lasered and shaped occlusion device regresses extremely flat in the defect to its original shape without stretching and buckling, in contrast to the state of the art braiding implants, where in principle wires with the same cross sections are used present invention, the cross-sections and rigidity of the lasered form elements of Occlusionsinstrumentes steadily. In addition, the lasered form elements or struts of Occlusionsinstrumentes are firmly connected to each other and not like the Geflochtsoccluder loose in the braid permanently crossing. As a result of the variance and variety of the lasered basic shapes aimed at in the proximal and distal retention area but also in the region of the waist with the formation into radial clamps, as far as possible for a large number of different defect morphologies, similarly good and optimal properties with respect to a flat end shape (starting shape ) can be achieved.

1 (a-b) 2 shows two schematic side views of the first preferred embodiment of an occlusion device (occluder) according to the invention in the production process. 1 to 4 (a-c) , The occlusion device 1 consists essentially of a solid composite lasered form elements 2 to 2 (a) , where these form elements 2 can be designed arbitrarily, as far as the shape, the cross-section of the form elements 2 as a bridge 5 and the distance or length to the next crossing point 6 , The design of the form elements 2 is oriented exclusively to securing the overall function of the occlusion device 1 to revert back to its original form under the influence of external forces. The form elements 2 can, for example, like in 4 (a) in the distal disc 11 be formed heart or leaf-shaped or as in 29 (a) in the waist 13 with elongated offset shape or any other combinations. The basic material consists of the occlusion device 1 preferably nitinol or another shape memory or memory effect material. The starting point for the production of an occlusion device 1 is a pipe 4 , in which like in 1 (a) by laser corresponding form elements 2 are incorporated and then expanded by a laterally introduced mandrel. A subsequent heat treatment in a salt bath or under hot air stabilizes the intermediate form, wherein the temperatures between 300 and 600 ° C and the holding time in this temperature range from one minute to half an hour. The expansion by means of molded mandrels and subsequent heat treatment can be carried out alternately several times in succession until the final shape is formed stepwise, such as in 1 (b) you can see.

The first preferred embodiment of an occlusion device according to the invention differs substantially from those of the known occlusion device 1 with state of the art, which usually consists of a braid 3 of nitinol wires 15 exist: There are z. B. certain braid main body known, as from 1995 the mesh Occluder 1 from AGA Medical (from 2011 St. Jude) from a cylindrical braid body with two clamps on the Occluderenden, from 2007 by Occlutech GmbH a ball braid with a proximal clamp and PFM 2010 a machine-knitted braid body made of a wire without clamps. Depending on external forces, which on such braids 3 act, shift the crossing points 16 the constantly crossing nitinol wires 15 , which ultimately leads to a certain instability of the occlusion device 1 can lead and is represented by stretching and bulging in the defect; this in turn leads to volume losses in the corresponding heart chambers or atria.

The form elements 2 a first preferred embodiment of an inventive occlusion device 1 in the expanded state in 4 (a-c) are designed via calculations, such as via the finite element method so that the occlusion instrument 1 has sufficient flexibility to be appropriately tapered when used in an intravascular surgical procedure (not explicitly shown). Due to the memory effect of the material has the so-tapered occlusion 1 via a shape memory function, so that the instrument 1 expands automatically after leaving the catheter and resumes a predetermined shape corresponding to the use, namely exactly in the predetermined starting shape, as for example in the side view in FIG 4 (b) , This usually takes place after the occlusion device initially arranged in the catheter 1 was placed at the site to be treated.

As in the figures 4 (a) and 4 (c) shown, the occlusion instrument 1 in the expanded state, a distal retention area 9 , a proximal retention area 10 and in 4 (b) a cylindrical waist 13 on which between the proximal and the distal retention area 10 . 9 is arranged. The two retention areas 9 . 10 serve to occlude a defect or shunt present in a septum. This is done in such a way that those areas 9 . 10 on both sides of the shunt to be closed while the waist 13 through the shunt. The occlusion device according to the invention 1 is a closure system that is delivered to the body of a patient via interventional procedures, such as catheterization and guidewires, and positioned at the designated location.

The occlusion device according to the invention 1 also contains fabric inserts 17 , as well as in 4 (a-c) is apparent. For such fabric inserts PET is usually used, this is a thermoplastic with the chemical name polyethylene terephthalate, which is used as a nonwoven (patch) for use. The fabric inserts 17 are located for additional sealing of the blood stream once each in the discs 11 . 12 , The occlusion device used in the body 1 is then completely enclosed by endogenous tissue after a few weeks or months (endothelialization).

5 (a-c) shows different states of expression of the waist 13 the first preferred embodiment of this occlusion device according to the invention in the final phase of implantation and shortly before the formation of the final shape in the expanded state 4 (A-C) , As a rule, after implantation in the defect or shunt, the occluder will resume its initial shape in the expanded state. For an instrument 1 to 4 (a-c) with narrow cylindrical waist results in a preferred treatment of a PFO defect (persistent foramen ovals), the defects (shunts) in their length (tunnel) differ greatly. That can cause the occluder discs 11 . 12 Drifting apart. To prevent this effect are in the range of the first preferred Occlusionsinstrumentes 1 to 5 (a-c) radial clamps 26 incorporated. So if a stronger septum in the area of the defect, the discs 11 . 12 presses apart, these terminals act 26 counteract this direction of movement and ensure the perfect fit flat final shape of the Occlusionsinstrumentes 1 in the expanded state. 5 (a) shows the waist 13 of the occlusion device 1 in unloaded expanded state and in 5 (b) and 5 (c) increasing loading conditions are shown, if through the stronger septum the occluder discs 11 and 12 be moved apart, which in turn is prevented by the radial clamps 26 counteract the movement (flat final shape). By placing the radial clamps 26 Finally, an even better anchoring of the implant 1 in the defect.

To account for the specific morphology of a PFO defect or shunt or to ensure a secure closure, the distal disc 11 of the occlusion device 1 opposite the proximal disc 12 reduced or as in 6 (a-b) axially displaced laterally, which may be advantageous, for example, if the defect to be occluded placed in the region of the aortic root; also an oval shaped distal disc 11 of the occlusion device 1 is useful. In 8 (a-b) is in addition an axial offset of the distal oval disc 11 of the implant 1 opposite the occlusion device 1 to 7 (a-b) executed.

9 (a-c) takes into account the morphological conditions of large holes in the area of the antechamber septum. In principle, occlusion instruments come here 1 used in which, on average, the diameter of the waist 13 is about as large as the diameter of the defect. The radial clamp made possible by laser technology 26 in the waist 13 functionally gains even greater importance in occluding such atrial septal defects (ASD) with such an ASD occluder 19 , Opposite the PFO occluder 18 to 4 (a-c) There is also an additional patch to seal the blood flow in the waist 13 as in 9 (b) seen. In 10 (a) is the ASD occluder 19 to see in expanded starting position. 10 (b) and 10 (c) illustrate the operating principle of the radial clamps 26 analogous to the above explanations in the 5 (a-c) , Due to the relatively large waist diameter of an ASD occluder 19 opposite a PFO occluder 18 , the number of radial clamps is about 8 to 10 times that of the PFO occluder 18 What can be an advantage for optimal anchoring.

Defekte bzw. Eine derartig bevorzugte Ausführungsform eines Occlusionsinstrumentes 1 bezeichnen wir erfindungsgemäß als Flexiblen VSD-Occluder 28.Defects or shunts in the region of the large chambers of the heart, so-called ventricular septal defects (VSD defects) lead to a dramatic deterioration in the function of the cardiovascular system. In this case, it may be advantageous if a further preferred embodiment of an occlusion device according to the invention 1 to 9 (a-c) is modified in the way that the distal disc 11 slightly reduced in diameter axially offset to the waist 13 and proximal disc 12 , where the number of patches is once each in the slices 11 . 12 and waist 13 remains unchanged (illustration in 11 (a-b) ). Such a preferred embodiment of an occlusion device 1 we call according to the invention as a flexible VSD Occluder 28 ,

12 (a) and 12 (b) show a schematic side view in 12 (a) and an associated plan view in FIG 12 (b) Another preferred embodiment of the invention flexible VSD Occluders 28 to 11 (b) , with respect to this plan view, the top view in 12 (b) in addition to the left offset of the distal disc 11 indicates a repeated offset upwards.

In 13 (a-b) we find another preferred embodiment of the flexible VSD occluder 11 (b) , this time the proximal disc 12 with the associated ball coupling was shifted to the left and in the form of increase in F 14 (b) with an additional offset of the proximal disc 12 up.

Further preferred embodiments of occlusion devices according to the invention 1 in the expanded state arise when in the 11 to 14 the inside fabric inserts 17 to be replaced by a patch envelope (stocking) completely surrounding the occluder 31 , Such a patch case 31 may be advantageous to better seal the defect site. Due to the increased demands on the elastic behavior, the previously used PET material has to be replaced by suitable biocompatible elastomers.

Elastomers are plastics whose glass transition point is below the operating temperature and which can be stretched in a non-destructive elastic and tensile manner up to an extension of several 100% of their original length under tensile and compressive load and after forming their original shape. In medicine, biocompatible elastomers are widely used, e.g. As polysiloxane elastomers for the production of blood vessels, for eye implants, for the reconstruction of the female breast or flexible joints for fingers, toes or wrist. Conventional elastomers such as crosslinked natural or synthetic rubber but also siloxane elastomers show a polymer network structure in which the main chains are covalently crosslinked and can accordingly not be melted and processed only consuming. In contrast, thermoplastic elastomers (TPE) contain reversible physical crosslinking points (eg hydrogen bonds or crystallites), which split or melt on heating without the macromolecules decomposing and thus making the elastomers thermally processable. This makes biocompatible TPE particularly suitable as a tailor-made material for medical applications. TPEs consist of "hard" domains embedded in a "soft" matrix; above the glass transition temperature or melting temperature of the hard segments thermoplastic processing is possible. TPE are copolymers with at least two types of building blocks that allow tailor-made synthesis of elastomeric materials. The most important thermoplastic elastomers are polyolefin elastomers, thermoplastic polyurethane (PUR) elastomers (polyester or polyether PUR) and thermoplastic polyester elastomers (eg, multibutylene oxy terephthalate). In the PUR-based TPE, for example, oligomeric polyether or oligomeric aliphatic polyester diols form the soft matrix. These soft segments can be linked with commercial diisocyanates, forming the hard urethane or allophanate building blocks. Due to the chain length of the soft segments and the nature of the diisocyanate component, the Shore hardness, flexibility and modulus of elasticity of the TPE can be systematically varied. For the preparation of polyurethanes for medical applications, 4,4'-diphenylmethane diisocyanate (MDI) is usually used, the linear or branched OH-terminated polyesters or polyethers being reacted with an excess of diisocyanate. It is also possible to incorporate OH-terminated oligoolefins, oligodienes or siloxanes in a thermoplastic polyurethane network.

An inventive flexible VSD occluder 28 with fully enclosing patch sleeve 31 is shown as a variant embodiment; in 15 analogous to 11 with internal patches; in 16 analogous to 12 , in 17 analogous to 13 and 18 analogous to 14 ,

19 (a) shows a schematic side view of another preferred embodiment of an occlusion device according to the invention 1 in the expanded state and in 19 (b) in the plan view, in both representations still without Force from the outside. In contrast acts in 19 (c) a lateral force from the right onto the distal disc (in practice a lateral pressure from one side of the ventricle outer wall). For such a universal flexible VSD occluder 29 the entire distal disc moves 11 slightly to the left over the original occluder edge 19 (b) out. In 20 (b) and 20 (c) are different load cases of the VSD occluder after 19 shown. However, it is always true that the distal disc 11 not rejuvenated in lateral movement to the left in diameter.

Further preferred embodiments of occlusion devices according to the invention 1 in the expanded state arise when in 19 (see also 20 ) the three layers of patches 17 be replaced by a corresponding patch sleeve 31 as in below 22 ( 20 ) is shown.

Gleichermaßen ist ein solcher erfindungsgemäßer Universeller Flexibler VSD-Occluder Soft 30 herstellbar, bei welchem sich nach 24(b) und 24(c) die proximale Scheibe 12 von rechts von der Herzwand her eindrückt, ohne dass der linke Rand der proximalen Scheibe 12 sich über den gegenüberliegenden Rand bewegtFurther flexibilization measures can be used to form corresponding shaped elements via an adapted laser structure 2 in the area of the waist 13 and in the area of both discs 11 . 12 be constructively adjusted so that a lateral pressure on the discs 11 . 12 causes that as in 23 (b) and 23 (c) represented the distal disc 11 Pressed under the force of the right side of the right, without affecting the distal disc 11 moved beyond the left edge. Such a Universal Flexible VSD-Occluder Soft 30 For example, it can be used effectively with small defects near the heart valve. Likewise, such an inventive Universal Flexible VSD Occluder is Soft 30 producible, in which after 24 (b) and 24 (c) the proximal disc 12 From the right of the heart wall presses in, without the left edge of the proximal disc 12 moves over the opposite edge

Ein erfindungsgemäßer Universeller Flexibler VSD-Occluder Soft 30 kann ebenso mit vollständig umschließender Patch-Hülle 31 ausgeführt werden, ansonsten sind 26 wie 23, 27 wie 24 und sind 28 wie 25. 25 (a) shows a schematic side view of another preferred embodiment of an occlusion device according to the invention 1 in the expanded state, with the flexible properties of Occluder 1 to 23 and 24 in 25 (b) at the same time, the discs are standing 11 . 12 simultaneously under the action of the right and taper off the edge, without the discs 11 . 12 move out on the opposite side. The load case in 25 (c) , with large lateral pressure from the right in both the distal disc 11 as well as in the proximal disc 12 corresponds approximately to the conditions in the case of a large defect in the ventricular septum in the area of the apex of the heart. In such a Universal Flexible VSD Occluder Soft 30 is after 25 (d) a load case possible, with the distal disc 11 and the proximal disc 12 be moved in opposite directions. An inventive Universal Flexible VSD Occluder Soft 30 Also available with fully enclosed patch sleeve 31 are executed, otherwise are 26 as 23 . 27 as 24 and are 28 as 25 ,

In 29 (a) One finds an illustration of a detailed technical solution for the Universal Flexible VSD Occluder Soft according to the invention 30 in the area of the waist 13 , The waist 13 is twisted by previous specific laser processing. Such twisted form elements 32 have an outer shape as in a conventional multi-pass isometric thread with a large pitch. If now in the transitions to the occluder discs 11 . 12 one twist each 33 is realized in opposite directions, the Occluderscheiben can move as in the 23 to 28 was described and in 29 (b) with an opposite direction of movement of the discs 11 . 12 , Such an occluder 1 to 29 is a schematic side view in the expanded state without force with patch sleeve (stocking) 31 in 30 (a) and under force 30 (b) shown in detail.

31 (a) shows a schematic side view of another preferred embodiment of an occlusion device according to the invention 1 in the expanded state and 30 (b) an associated plan view. The occluder discs 11 . 12 are elongated oval and the waist between them 34 has a corrugated structure. Under these conditions, it is possible that as in 32 (b) in the plan view 24 It is shown that the occlusion device 1 can bend laterally "banana-shaped"; This case occurs when from the right center in the area of the waist 34 and at the discs 11 . 12 From the left outside at the same time forces meet in opposite directions. A technically simple and meaningful use of such Occluders after 31 and 32 results in the application as Paravalvularer leak Occluder 21 After a patient an artificial biomechanical or mechanical heart valve was surgically or interventionally used to replace his natural heart valve and resulting in the posterior there leaks in the edge area.
