Abstract
Die Erfindung befasst sich mit der Lösung der klinisch relevanten Problemstellung, die Durchlässigkeit von Biliären- und Pankreasgang-Drainagen zu verbessern. Die vorliegende Erfindung hat zur Aufgabe, Biliäre- und Pankreasgang-Drainagen sowohl für die Früh- als auch Spätphase strömungsmechanisch optimiert auszulegen. Die Erfindung hat damit speziell die Aufgabe, eine strömungsmechanisch optimierte Konstruktion praxisgerecht und handhabungssicher zu realisieren. Die Aufgabe der Erfindung wird durch einen speziell konfigurierten, als zylindrisch ausgebildeten Implantatgrundkörper (Stent) mit einem auf der Außenfläche angebrachten umlaufenden Nutenprofil gelöst. Das Einbringen (Insertion) der Drainage erfolgt minimalinvasiv mit einem Endoskop. Die Erfindung dient der Verhinderung bzw. zumindest der Minimierung von Biofilmbesiedlungen sowie einer ortsadaptierten Strukturkompatibilität (Steifigkeitsanpassung).The invention addresses the solution of the clinically relevant problem of improving the permeability of biliary and pancreatic duct drainage. Die Erfindung befasst sich mit der Lösung der klinisch relevanten Problemstellung, die Durchlässigkeit von Biliären- und Pankreasgang-Drainagen zu verbessern. In Deutschland wird eine Gallensteinprävalenz von etwa 12% angenommen, das heißt etwa 10 Millionen Gallensteinträger leben in Deutschland. Von diesen entwickeln 1/4 bis 1/3 Symptome in Form von Gallenkoliken und/oder Gallenblasenentzündungen. Damit ist eine Zahl von 2,5–3 Millionen anzunehmen. In 15% dieser Fälle wird aufgrund einer Cholezystolithasis operiert und es findet sich im gleichen Fall dann auch eine Choledocholithiasis. Daneben können braune Pigmentsteine direkt im Ductus choledochus entstehen, vor allem bei Infektionen. Bakterielle Hydrolasen spalten biliäre Phospholipide und setzen Bilirubin frei. Diese Prozesse führen dazu, dass die Notwendigkeit besteht, nach einer oder vor einer Operation Gallensteine mittels der endoskopisch retrograden Cholangiopankreatikographie (ERCP) zu extrahieren und bei festsitzendem Stein zunächst einen Stent zu implantieren, um ein Abschwellen der entzündlichen Schleimhaut zu erreichen sowie um den Stein anschließend extrahieren zu können. Erkrankungen wie ein juxtapapilläres Divertikel können ebenfalls die Papille einengen und zu einer Papillotomie führen und eine Stenteinlage erforderlich machen. Weiterhin gibt es benigne Papillenstenosen, die einer Therapie bedürfen. Entweder mit einer Schlitzung oder der Erweiterung durch Drainage, wobei auch mehrere Drainagen gleichzeitig oder nacheinander eingeführt werden, um zu einer definitiven Erweiterung des Ganges zu führen. Ein ganz besonderes Problem sind Pankreaskopf- und Gallengangstumoren, die inoperabel sind und wo einzig die Stentversorgung einen Abfluss der Galle gewährleistet. Vereinzelt kommt es nach Operationen im Bereich von Galle/Leber und Pankreas zu narbigen Stenosen im Verlauf des Ductus choledochus, was eine Drainageversorgung notwendig macht. Seltene Erkrankungen wie das Caroli-Syndrom führen auch zur Notwendigkeit einer Drainageapplikation. Daraus wird offensichtlich, dass Stent-Applikationen in den Choledochus keine seltene Maßnahme sind. Gelegentlich werden auch Stentapplikationen in den Pankreasgang erforderlich, z.B. zur Ableitung von Pankreaspseudozysten oder bei Pankreasgangsteinen.
