Description

The invention relates to a tissue extraction endoscopy instrument according to the Preamble of claim 1, which ver for example for a liver biopsy is reversible and into a treatment instrument insertion channel of an Endo skops can be inserted and removed from it.

Fig. 8 zeigt ein freies Endes einer sog. Fig. 8 shows a free end of a so-called "guillotine" tissue removal needle, which che usable as a tissue extraction endoscopy instrument. A sharp needle end 11 is provided at the free end of a rod-shaped needle shaft 10 . Furthermore, a tissue receiving recess 12 is provided in the region of the needle end 11 , viewed from the side, for receiving a cut-out tissue sample.

On the umbilical shaft 10 , an outer sleeve 20 is axially movably fitted, while an annular blade 21 for cutting out tissue is provided on the inner peripheral side of the free end of this outer sleeve 20 , which enters the tissue receiving recess 12 .

FIGS. 9 and 10 show states in which removal endoscopy instrument using the tissue biopsy tissue is removed. According to FIG. 9, the outer sleeve is retracted slightly element 20 after penetration of the tissue removal endoscopy in a fabric 100 so that a target region of the tissue enters the Gewebeaufnahmeausnehmung 12 100. Referring to FIG. 10, the outer sleeve is temporarily out 20 for cutting tissue samples 101 from tissue 100, wherein the tissue sample in the Gewebeaufnahmeaus recess 12 occurs.

If the tissue extraction endoscopy instrument is used in the manner described above, a tissue sample which is larger than the tissue receiving recess 12 cannot be removed. In addition, in the practical application according to FIG. 11, there is a space between the tissue receiving recess 12 and the tissue sample 101 . If the tissue sample 101 is cut out with the outer sleeve 20 advanced, it is pressed through the outer sleeve 20 and raised. For these reasons, the tissue sample 101 taken is slightly smaller than the size of the tissue receiving recess 12 .

The invention has for its object a tissue extraction endoscopy to specify instrument with which a sufficient amount of egg ner tissue sample can be taken without the instrument size increases.

This task is the one with a tissue extraction endoscopy instrument gangs mentioned by the features of the characterizing part of the patent claim 1 solved.

Weiterbildungen der Erfindung sind Gegenstand von Unteransprüchen.Developments of the invention are the subject of dependent claims.

An exemplary embodiment of the present invention is described below with reference to attached drawings explained.

Figs. 1 and 2 show the overall configuration of a tissue sampling-Endos copy instrument. Fig. 1 shows a state in which a needle shaft 10 is connected to egg nem suction instrument 50 , wherein an outer sleeve 20 is moved forward in a given frontmost position, while Fig. 2 shows a state in which the outer sleeve 20 to the operating side is withdrawn. The suction instrument 50 is not shown in FIG. 2.

At the free end of the needle shaft 10 there is a sharp needle end 11 with a forward-pointing sharp tip without a hole. Immediately behind the needle end 11 , a tissue receiving recess 12 is seen from the side of the needle shaft 10 .

A suction channel 13 connected to the tissue receiving recess 12 extends from the rear end of the tissue receiving recess 12 over the entire length of the needle shaft 10 . This means that the webeaufnahmeausnehmung behind the Ge part of the needle shaft 10 situated has the shape of a tube.

The outer sleeve 20 is axially movably fitted onto the needle shaft 10 and covers almost the entire length thereof. At the free end of the outer sleeve 20 , a forwardly bevelled annular blade 21 is provided on its inner circumferential side.

The outer sleeve 20 is slightly shorter than the needle shaft 10 (for example by about 2-10 cm). At the operating end of the outer sleeve 20 , a manually operable locking screw 23 is provided so that the end thereof can be brought into contact with the outer circumferential surface of the needle shaft 10 .

By tightening the locking screw 23 , the outer sleeve 20 can therefore be fixed relative to the needle shaft 10 , while it can be moved relative to the needle shaft 20 when loosening this locking screw 23 . On the operating side of the needle shaft 10 and the outer sleeve 20 finger supports 14 and 22 are seen before, whereby an axial movement is facilitated.

Auf der Umfangsfläche am Bedienungsende des Nadelschaftes 10 sind Skalen (eine Markierung) 15 vorgesehen, so daß eine Bedienungsperson die Stellung der Bedienungsseite der Außenhülse 20 in Bezug auf die Skalen 15 beobachten kann, wodurch es möglich wird, die Stellung des freien Endes der Außenhülse 20 in Be zug auf den Nadelschaft 10 zu erkennen (d. h. zu erkennen, daß die Außenhülse 20 in einer vorgegebenen Stellung gemäß Fig. 1 steht und wieweit sie sich aus der vorgegebenen Stellung bewegt hat).Scales (a mark) 15 are provided on the peripheral surface at the operating end of the needle shaft 10 so that an operator can observe the position of the operating side of the outer sleeve 20 with respect to the scales 15 , thereby making it possible to determine the position of the free end of the outer sleeve 20 in relation to the needle shaft 10 to be recognized (ie to recognize that the outer sleeve 20 is in a predetermined position according to FIG. 1 and how far it has moved from the predetermined position).

A scale part, which indicates that the outer sleeve 20 is in the predetermined position, may be longer than other scale parts (may be provided, for example, over the entire circumference) or may differ in color from these other scale parts. In addition, a marking 15 A can also be provided, which indicates that the needle end 11 is received by the outer sleeve 20 .

At the operating end of the needle shaft 10 , a connection 16 , which is formed in one piece with the finger rest 14 , is provided, which can be detachably fastened to the suction device 50 . This connection 16 is connected to the suction channel 13 so that the suction instrument 50 , such as a fuel ze, can be connected to this, to take tissue in the tissue receiving recess 12 at the free end of the needle shaft 10 .

The tissue extraction endoscopy instrument according to this exemplary embodiment is inserted into a treatment instrument insertion channel of an endoscope, for example an ultrasound endoscope, and removed therefrom. The instrument can be inserted into the treatment instrument insertion channel in a state in which it is guided through a guide tube, for example a tetrafluoroethylene plastic tube, around the treatment instrument insertion channel during insertion and removal against damage by the To protect needle end 11 .

FIGS. 3 to 6 show cases in which the biopsy tissue Ausführungsbei described above is taken game, for example, the liver or the pancreas using the endoscopic instrument according to Gewebeentnahme-. Referring to FIG. 3, the free end of the outer sleeve 20 na he at the free end of the needle shaft 10 so that the Gewebeentnahmeausneh mung is closed by the outer sleeve 20 12. In this state, the locking screw 23 is tightened to fix the outer sleeve 20 relative to the needle shaft 10 , after which the needle end 11 is inserted into the tissue 100 .

If the tissue removal recess 12 is inserted at a predetermined depth, the locking screw 23 is loosened and the outer sleeve 20 is withdrawn slightly towards the operating side in order to expose the tissue reception recess 12 , as is shown in FIG. 4. The tissue 10 therefore enters the Ge tissue receiving recess 12 .

The suction instrument 15 is then actuated to guide the suctioned tissue from the tissue receiving recess 12 through the suction channel 13 . A portion of the tissue 100 , which becomes the tissue sample 101 , is therefore fully sucked into the Ge tissue receiving recess and into the interior of the intake duct 13 beyond the sen inlet, as shown in Fig. 5.

The outer sleeve 20 is then moved forward into the original position in order to cut the tissue sample 101 with the sheet 21 from the tissue 100 and to insert it into the tissue receiving recess 12 . As shown in FIG. 7, a tissue sample 101 can be removed in a simple manner, the volume of which is larger than that of the tissue receiving recess 12 .

Through the intake duct provided according to the invention, which through the Na shaft runs and reaches the inside of the tissue receiving recess, it becomes possible to take a tissue sample that is completely in the Ge weave receiving recess and the interior of the intake duct via its on let it be sucked out. There can therefore be a sufficient amount of tissue can be removed without enlarging the instrument. Because of the needle shaft Pipe shape also increases its flexibility, which increases its Introduction into the treatment introduction channel of an endoscope is facilitated.
