Abstract

本发明涉及一种治疗人体皮肤疾病的组合物,和该组合物的给药方法和制备方法。
技术背景
皮肤病在幼年、青年和成年时期是一种常见的问题。皮肤病可包括,例如,痤疮、炎性疾病如特应性湿疹或酒渣鼻。
寻常痤疮是一种特征在于毛囊皮脂腺炎症的囊性疾病,例如黑头粉刺、丘疹、脓疱、囊肿和小结节。主要地作为一种青春期疾病(并且常常引起情绪苦恼),痤疮是内生的并且是多种因素影响造成的。痤疮发展中的主要进行性因素包括小囊上皮的角化过度,皮脂生成增多和痤疮丙酸杆菌的增殖。这些因素首先导致小囊衬层的角化过度,包括角蛋白和皮脂以及脓疱性痤疮(P.痤疮)代谢的游离脂肪酸副产物的滞留,这可以造成炎性痤疮丘疹和脓疱。

Fulton报导了局部抗生素红霉素在痤疮治疗中的首先使用(Fulton,J.E.Jr.和Pablo,G.痤疮的局部抗菌疗法.红霉素家族的研究.Arch.Dermatol.110:83-86,1974)。这些有效剂的局部给药具有减小副作用的优点,特别是那些系统性药物作用引起的副作用,例如恶心、胃肠不适、腹泻和阴道酵母菌过度生长。
林可霉素抗生素已经应用在痤疮的局部治疗中(U.S.专利号3,969,516)。Upjohn Co.(现为Pharmacia & Upjohn)出售的林可霉素有Cleocin T溶液、凝胶、洗剂和纱布。Cleocin T凝胶是对Cleocin T溶液的改进,因为没有使用醇并且易于涂敷在面部皮肤上治疗痤疮。这种凝胶是基于carbomer 934,NF。
特应性皮炎是一种多基因病,它具有遗传缺陷(predelection)并受到外界因素的强烈影响。这种疾病会感染婴儿、儿童、青年和成年人并且在本质是变应性的。分布是对称的,典型地包括面部、颈部和弯曲区域。特应性皮炎是慢性、复发且通常瘙痒的。局部治疗经常包括局皮质类固醇类,例如地奈德、戊酸氢化可的松、氟轻松、曲安奈德、戊酸倍他米松、氢化可的松丁酸酯、丙酸卤倍他索(halobetasol propionate)、二丙酸倍他米松、丙酸氯倍他索、双醋二氟松、丙酸氟替卡松、布地奈德等。
酒渣鼻是鼻子、面部和皮肤其它发红区域的一种慢性炎性疹。这种疾病在中年妇女中常见且特征在于红斑、丘疹、脓疱、毛细管扩张和皮脂腺增大。其病因学原因完全不清楚;然而血管舒缩不稳定性和绝经是易感因素。在炎性脓疱囊中常常可以发现生物毛囊脂螨,它在这种疾病中可能起作用。治疗包括局部使用甲硝唑和口服四环类抗生素。
发明概述

本发明提供一种新的局部用凝胶或洗剂给药体系用于治疗皮肤病,特别是寻常痤疮。该体系的一个独特方面是使用了提供凝胶原料的聚合材料,其具有非常低的粘度但它在化妆学上优雅且有助于使可倾倒组合物易于流过滴管尖的给药过程。组合物

″维生素A类″是与视黄酸有关的溶角质药物并且一般包括化学实体例如视黄醇及其酯,并且与天然衍生物及结构相关合成类似物密切有关。这包括,例如视黄醇、视黄醛、维生素A酸(全反式维生素A酸)、异维生素A酸、阿达帕林(6-[3-(1-金刚烷基)-4-甲氧基苯基]-2-萘甲酸)等。其中,优选维生素A酸。
通常,局部用皮质类固醇是氢化可的松(也称作皮质醇)的结构修饰物且具有局部抗炎活性的化合物。代表实例包括″Goodman &Gilman’s The Pharmacological Basis of Therapeutics,″第8版(McGraw-Hill,Inc.)(1993)第1575页表65-1中所述的那些。具体地,局部用皮质类固醇包括本申请‘技术背景”部分中提及的那些。优选的皮质类固醇,当用在单一的活性成份时,包括双醋酸二氟拉松、地萘德、丙酸氟替卡松、丙酸卤倍他索或布地奈德。当用在单一活性成分时首选丙酸卤倍他索。
非甾类抗炎药(NSAID)是当局部涂敷时通过抑制前列腺素合成或通过其它作用机理阻止炎症反应的化合物。在上文所述的Goodman和Gilman中可以发现代表性实例。代表性实例包括布洛芬、消炎痛、双氯芬酸和萘普生及其盐。优选双氯芬酸。
抗炎咪唑是阻止局部炎症反应的咪唑类化合物。甲硝唑是适合于本发明的代表性咪唑化合物。
在组合物的详述中,在此给出的数量范围是那些组合物产生功能结果的量。所以,这些的范围一般引入术语″约″来指出范围中的某种灵活性,即所给出下限和上限数量范围的±10%或更小。
如上所述,所述的活性成份可以单独存在或合用。例如,局部用抗生素,如磷酸克林霉素,可以与局部用皮质类固醇合用。当主要为在面部区域涂敷以治疗痤疮设计制剂时,优选将抗生素(例如磷酸克林霉素)与一种低效皮质类固醇合用,例如地奈德、戊酸氢化可的松、氟轻松、丁酸氢化可的松或曲安奈德。局部用抗生素也可以与维生素A类合用,例如磷酸克林霉素和维生素A酸或阿达帕林合用。
本发明的组合物应包括聚合材料,其应当以足够使组合物的粘度达到不超过约15,000cP,优选100-约12,000,和更优选约300-约10,000的水平的量存在。粘度是在室温(20-25℃)下用Brookfield粘度计模型DV-I+,#27锭子(spindle)以每分钟12转数(rpm)测定。如果测量的粘度小于4,000cP,应当用#21锭子代替#27。通过使粘度维持在约15,000cP以下,可以获得更多的吸引人的化妆特性并且通过改进的流动和倾倒性更加容易精确涂敷。
现已发现特别适用于本发明组合物中的聚合物是轻度交联聚丙烯酸聚合物,其购自B.F.Goodrich的商品名CARBOPOL。它们类属于丙烯酸聚合物类。CARBOPOL聚合物类是基于聚丙烯酸结构的亲水性聚合物。适用于本发明中的轻度交联聚合物包括CARBOPOL 910,941,971和981和CARBOPOL ETD 2050。
CARBOPOL 941或981对于本发明来说特别有价值,因为基于CARBOPOL 941或981的凝胶的粘度比其浓度低。这个特征是该聚合物结构在中和水体系中低水平的交联的结果。与此对照的是,具有高水平的交联的聚丙烯酸聚合物,例如CARBOPOL 980或974P,在同等的浓度下生成粘度较高的凝胶。
在pH7.5下CARBOPOL 941或981的0.5%溶液具有4,000-11,000cP的粘度测量(Brookfield粘度计在20rpm),可与CARBOPOL 940或980的同等0.5%溶液的40,000-60,000cP的粘度测量比较(参见:B.F.Goodrich Product Guide,Bulletin 2)。
轻度交联聚丙烯酸聚合物,例如CARBOPOL 941和981的这种低水平粘度特征为本发明的组合物提供两个优越性。与由高度交联聚合物制成的粘度相当的凝胶相比,由这些由轻度交联聚合物之一制成的凝胶可以提供更良好的肤感和光滑性。其次,低粘度凝胶可以通过滴管或滴注型分配器非常精确地给药,与之相比,稠凝胶的市售产品无法精确给药。
CARBOPOL 941 NF树脂和其共溶剂聚合替代品,CARBOPOL 981NF树脂,提供低粘度的永久乳剂和混悬剂。用这些树脂制成的凝胶具有优异的透明度。在离子体系中,它们可以比大多数其它CARBOPOL树脂性能优良并在溶剂体系中以低于1.5%的浓度下工作得更好。所述聚合物购自B.F.Goodrich Specialty Chemicals,9911Brecksville Road,Cleveland,OH 44414-3247。
CARBOPOL树脂是丙烯酸与聚链烯醚或二乙烯基二醇交联的聚合物。所述聚合物直径平均约0.2微米的初级微粒的絮凝粉末。该絮凝粉末附聚,通过Coulter计数器测定平均为2-7微米。一旦形成,这些附聚物无法再分解为初级微粒。
各初级微粒可以被看作是通过交联相互连接的聚合物链的网状结构。没有交联,初级微粒将是盘绕的但化学上没有键合的线性聚合物链的集合。它们在水中最多可以溶胀至其初始体积的1000倍(并且是其初始直径的10倍)形成凝胶,尤其是当暴露在大于约4-6的pH环境下时。由于这些聚合物的pKa是6.0±0.5,聚合物主链上的羧基电离,造成负电性微粒之间的排斥,其增加了所述聚合物的溶胀。此类的高度交联聚合物不能溶解在水中,而是通过形成均质分散体来形成凝胶。
粉末形式时CARBOPOL树脂的玻璃化转变温度是105℃(221°F)。然而,当该树脂与水接触时玻璃化转变温度急剧下降。所述聚合物链开始呈旋涡状且回旋的半径变得越来越大。肉眼观察,这种现象本身表现为溶胀。
本发明的含水组合物将任选地包含水可溶混的溶剂和防腐剂。如果需要应存在水可溶混的溶剂(即共溶剂)以协助溶解活性成份。所述共溶剂可以是单一组分或混合物。实例包括那些与水可混溶的溶剂,例如乙醇、丙二醇、甘油、聚乙二醇400等。某些水可混溶的溶剂,例如甘油或丙二醇也可为所述组合物增加有益的保温性。药物进入皮肤的释放和渗透作用可以通过水可混溶共溶剂组合物来改进。
适用于所述组合物的防腐剂是协助保证组合物稳定和/或防止细菌生长的物质。因此,防腐剂可以是一种或多种抗氧剂、螯合剂、抗菌剂等。适当的防腐剂包括对羟基苯甲酸甲酯、对羟基苯甲酸丁酯、在羟基苯甲酸丙酯、苄醇、山梨酸、imidurea、硫汞撒、没食子酸丙酯、BHA、BHT、柠檬酸、乙二胺四乙酸二钠等。另一种任选添加剂是香料。通常,它仅仅以痕量存在且对组合物的功能没有影响。
特别适合于痤疮治疗的一种优选的组合物应具有约3-9、优选约4-7、和首选约5-6的pH。因此,该组合物也根据需要含有pH调节剂,其水平是使pH调节至预期范围。此类试剂包括多种药学可接受的有机或无机碱,例如,氢氧化钠和tromethamine。pH选择应部分依赖于组合物所选活性成份的pH耐受限。实施例给出了某些化合物和适合组合物的pH值的指导。

组合物的优选配方应不含有防腐剂或与市售的原料相比含有更低水平的防腐剂。这很重要,因为组合物中防腐剂的存在可以造成皮肤的刺激或变应性反应。减少组合物中的皮肤刺激性或变应性反应的可能性可以提供更良好的产品。对于含有磷酸克林霉素的组合物,主要的产品是Cleocin T凝胶。它是一种澄清的粘性凝胶,试验证明它不如本发明的由更加轻度交联聚合物制成的低粘物质那样更容易接受。通过将凝胶的粘度控制在低水平,可以使它精确地从透明塑料挤压瓶而不是软膏管中分配出来。优点有二个。一个优点是通过使用减小的孔嘴可以达到精确的剂量控制并改进上市产品。此外,试验证明低粘物质在化妆上更加优雅,因此可以带来更经常的使用。
在制备本发明的组合物时,采用制药学领域中的通用制剂技术。参见,例如,Remington:The Science和Practice of Pharmacy,第19版,Mack Publishing Company(1995)。具体制剂的制备可以参见实施例。
为了制备具有两种活性成份的凝胶,其中一种活性成份悬浮而另一种活性成份溶解,首先将不溶性活性成份加入到水可溶混的组分,或者一部分的含有表面活性剂的水中使其分散。单独地,将另一种活性成份和任何其它防腐剂组分溶解在纯水中。将胶凝剂分散在水溶液中同时适当搅拌。随后将第一活性成份的分散体加入到凝胶中并充分混合成掺混物。最后,加入pH调节剂调节pH至预定范围。当两种活性成份均溶解时凝胶的制备相似,但仅仅改变第一步。首先将具有较低水溶性的活性成份加入到溶剂、溶剂混合物或水中。混合至溶解。单独地,溶解其它活性成份和任意的防腐成份于纯水中,使胶凝剂分散在水溶液中同时适当搅拌。随后将第一活性成份的溶液加入到凝胶中并充分混合成掺混物。最后,加入中和剂调节pH至预定范围。
对于抗生素(例如磷酸克林霉素)与维生素A类(例如维生素A酸)的组合,有三种制剂途径可以适用于本发明的组合物:1)水凝胶,由轻度交联丙烯酸聚合物胶凝剂生成,并且磷酸克林霉素被溶解而维生素A酸悬浮;2)水包油乳剂,其中磷酸克林霉素溶解在用轻度交联丙烯酸聚合物胶凝剂增稠的水中并且维生素A酸溶解在内部液体油相中;和3)由水和水可溶混的有机溶剂组成的溶液,其中磷酸克林霉素和维生素A酸均已溶解。
下列组合物是本发明所用组合物类型的代表。
当所述的组合物含有一种抗生素时,例如磷酸克林霉素,该组合物具有约4-7的pH并且含有;
(a)约0.5%-2.0%w/w磷酸克林霉素,
(b)约0.1%-0.4%w/w的所述聚合物,

一种其中抗生素是磷酸克林霉素且维生素A类是维生素A酸的凝胶组合物可以含有
(a)(i)约0.5%-约2.0%w/w磷酸克林霉素,和
(ii)约0.01%-约0.05%w/w维生素A酸;

本发明的另一方面是一种治疗人体中的皮肤病的方法,该方法包括将一种组合物以足以改善该疾病的量和时间施用到患有所述疾病的对象皮肤的感染区域,其中该组合物是本发明申请所述的组合物。优选地,所述的组合物在治疗期间每天给药1次。根据患者的改善情况,治疗可以在少于1周至2个月或更长时间内。改善的进程可以由患者或主治医生来监测。
可以用本发明的组合物治疗的皮肤病包括寻常痤疮、酒渣鼻和多种炎性病症,包括特应性皮炎。这些病症的讨论可以参见MerckManual。例如,寻常痤疮是一种感染毛囊和皮脂腺的炎性疾病。病灶常常位于面部,但颈部、胸部、背上部和肩也可以被感染。
对象皮肤的被感染区域可以是机体中存在皮肤的任何部位。组合物的足以改善皮肤病的用量和给药时间将取决于对象和皮肤病症。通常,从挤压瓶的滴管嘴或眼药滴管挤出足够量到被感染区域并且轻轻涂抹到皮肤内。经常地,需要不超过数滴来涂敷在被感染区域。制造的制品

本实施例提供本发明的一种可倾倒凝胶组合物。步骤a-f所述的方法可以制备表1的组合物。组合物称作″Clindagel″。已经提交了一个指定Clindagel作为商标的申请。

实施例IIIClindagel和Cleocin-T凝胶的比较
本实施例提供临床数据证明,本发明的组合物与已知市售组合物相比具有优越性。
在寻常痤疮中进行多中心调查-盲法临床试验来比较本发明的组合物(参见实施例I)每天一次Clindagel和每天2次(按照制造商说明)Cleocin-T凝胶(参见实施例II)。三百二十四名患者,半数一组,治疗长达12周。调查者是″盲目的″,在评估患者痤疮的状况前她/他不知道患者使用的是何种治疗。

试验制品(凝胶赋形剂)通过盲法识别码来鉴定,由此防止试验对象了解所涂敷试验制品的特性。各试验对包括试验制品L对R,其是指分别在面部的左侧和右侧使用。指定L和R的试验制品可以改变从而使各试验制品在R和L试验位置并通过涂敷顺序被随机评估。
所述的对象在性别上平衡。该群体的平均年龄是34岁,年龄范围在25-44岁之间。

实施例VII实施例VI的制剂A和B中的维生素A酸的化学稳定性的评价
本实施例提供试验数据证明本发明的两种组合物中的维生素A酸在加速试验条件下的稳定性。
已知维生素A酸相对不稳定,所以,在12周加速稳定性研究中评价这些组合制剂的化学稳定性。将凝胶包装在安瓿玻璃瓶内,各瓶8克,并且储藏在40℃下。使用用于维生素A酸霜剂(USP 24,1684页)的方法在初始和第2、4和12周进行高效液体色谱测定。发现两种组合物在这种加速试验中保持其效价。表XlI概括了化学稳定性结果。

通过用刮铲将维生素A酸粉末和没食子酸丙酯(抗氧剂以延缓维生素A酸的氧化损失)刮到CleocinT凝胶(实施例II)中来制得维生素A酸0.025%和克林霉素1%的联合凝胶制剂。定量配方如表XIII所示。

4周内在5℃、40℃和50℃下评价。实施例VI,制剂A和实施例VIII的物理稳定性。稳定性评估基于在初始、2周和4周时描述的细致物理检查。研究结束时,进行显微镜检查来检测维生素A酸的沉淀和结晶生长。下面所概括的数据(表XIV)表明,改进的市售制剂CleocinT凝胶与本发明的组合物实施例VI(制剂A)相比在物理上不稳定。

g.在高速搅拌下,依次将步骤″e″油相和步骤″f″药物相加入到步骤″d″水相并充分混合。
h.冷却该乳剂同时连续搅拌。
i.加入tromethamine溶液并搅拌形成均匀乳剂。冷却至室温同时连续搅拌。
实施例XI
本实施例提供一种本发明的可倾倒性凝胶组合物,该凝胶含有皮质类固醇。该制剂适用于治疗炎性皮肤病例如特应性皮炎。

e.用桨式混合器或者优选用旋筒-定子型的均化器将微粉化的丙酸卤倍他索分散在步骤″d″中。
f.用桨式混合物将步骤″e″加入到步骤″c″至药物原料均匀分散。
g.将tromethamine溶解在是其重量10倍的纯水中。搅拌的同时,用tromethamine溶液调节pH并且增稠该凝胶。连续增量加入直至pH达到约6.5。
h.加入水达到100%的批量大小并且用桨式混合器混合直至均匀。
实施例XII
本实施例提供另一种本发明的可倾倒凝胶组合物。该制剂含有甲硝唑用于局部涂敷在被感染的皮肤区域,例如酒渣鼻。

Claims (47)
1.用于治疗人体对象中的皮肤病具有约3-约9的pH且粘度小于约15,000cP的组合物,该组合物基本上具有下列组成:
(a)治疗有效量的至少一种治疗所述疾病的化合物,
(b)与所述混合物相容的药学可接受的、轻度交联聚丙烯酸聚合物,
(c)调节pH的药学可接受的碱,
(d)任选地水可溶混的溶剂,
(e)任选地防腐剂,
(f)任选地至少一种表面活性剂,其单独或与油相结合,以及
(g)水。
2.权利要求1的组合物,其中所述化合物是抗生素、咪唑、维生素A类、皮质类固醇,或非甾类抗炎药(NSAID)。
3.权利要求2的组合物,其中所述化合物是单用或与皮质类固醇或维生素A类合用的抗生素。
4.权利要求3的组合物,其中所述的化合物是抗生素单用。
5.权利要求4的组合物,其中所述的抗生素是磷酸克林霉素。
6.权利要求5的具有约4.0-7.0的pH的组合物,该组合物基本上具有下列组成
(a)约0.5%-2.0%w/w磷酸克林霉素,
(b)约0.1%-0.4%w/w的所述聚合物,
(c)调节pH的碱,
(d)约15.0%-25.0%w/w的水可溶混的溶剂,
(e)小于约0.2%w/w的防腐剂,和
(f)适量加入至纯水至100%w/w。
7.权利要求6的组合物,其中部分(d)的水可溶混的溶剂是以约3.0%-10.0%w/w的量存在。

(e)约0.1-0.15%w/w对羟基苯甲酸甲酯,和
(g)适量加入至纯水至100%w/w。
9.权利要求6的具有约5.0-6.0的pH的组合物,该组合物基本上具有下列组成:
(a)约1.0-1.5%w/w磷酸克林霉素,
(b)约0.2%w/w的所述聚合物,
(c)调节pH的碱,
(d)约5.0%w/w聚乙二醇400,
(e)约0.1-0.15%w/w对羟基苯甲酸甲酯,和
(g)适量加入至纯水至100%w/w。
10.权利要求3的组合物,其中所述的抗生素与皮质类固醇组合,该抗生素是磷酸克林霉素,并且所述的皮质类固醇是地奈德、戊酸氢化可的松、氟轻松、丁酸氢化可的松或曲安奈德。

(a)(i)约0.5%-约2.0%w/w磷酸克林霉素,和
(ii)约0.01%-约0.05%w/w维生素A酸;
(b)约0.1%-约0.5%w/w的所述聚合物;
(c)调节pH的碱;
(d)约10%-约30%w/w的水可溶混溶剂;
(e)小于约0.2%的防腐剂;和
(g)适量加入至纯水至100%w/w。
13.权利要求11的具有约5-6的pH的组合物,该组合物是一种基本上具有下列组成的洗剂
