发明背景

据认为大量疾病主要与T-细胞相关或被T-细胞介导。一些疾病如,皮肤T-细胞淋巴瘤,器官移植后的同种异体移植排异,进行性全身硬化症(PSS),肠炎疾病(IBD),类风湿性关节炎(RA),青少年早发性糖尿病(JODM)被认为是由T-细胞介导的。

其它T-细胞介导的现象包括对宿主外来组织的排异。在器官的同种异体移植中,所期望的是阻止对移植器官的排异,或者保持免疫系统的活性,以使身体抵御感染和允许其他正常的身体防卫。移植后的标准治疗手段是有限的,因为使用免疫抑制疗法可引起一种一般性的免疫抑制状态,这种状态导致微生物或偶然的感染,即这种治疗最普通的负面反应。更期望的是具有不引起广泛免疫抑制属性的免疫调节。光除去法已显示出其有效性,Loyola大学的研究者已可以成功地利用光除去法治疗14例心脏排异并对标准免疫抑制药物不应中的13例。在这种情况的变异中,光除去法已成功地被用于一个慢性移植物抗宿主疾病的患者[Rossetti et al.,1995,移植,59:1,pp.149-151]。这种疾病以内源诱发的无免疫活性的宿主为特征,其中免疫活性细胞(骨髓或外周干细胞)在这种条件下被注入患者作为对多种恶性肿瘤和白血病的治疗。这里移植的免疫活性细胞攻击患者(“宿主”),组织来调节免疫活性细胞而不引起进一步广泛的免疫抑制和相应的负作用。
光除去法涉及将外周血白细胞暴露于被紫外A光光激活的8-甲氧基补骨脂素(8-MOP),然后重新注射经处理的白细胞。
血液中的8-甲氧基补骨脂素分子进入白细胞核并插入双链DNA螺旋。在体外过程中,长波紫外光照射UVAR光除去法系统中白细胞富集的血液部分。光激活药物,对UVA能量作出反应,与DNA双螺旋中的胸腺嘧啶碱基连接。这就导致胸腺嘧啶碱基的交叉连接,从而阻止转录过程中的DNA解旋。血浆和改变了的白细胞被重注入患者体内。重注入被光除去法破坏的白细胞的结果是延迟对这些损坏白细胞的免疫攻击,同时另一方面,未修饰的WBC显示同样的细胞表面抗原。
甲氧沙林是在Ammi majus(伞状花科植物)种子中发现的具有光活性的天然物质。它属于补骨脂素或呋喃并香豆素一类的化合物。其化学名称是9-甲氧基-7H-furo[3,2-g][I]-苯并吡喃-7-酮。这个药物的制剂是在10毫升小瓶内的浓度为20mcg/mL的无菌液体。甲氧沙林的药物动力学活性可以在研究者的小册子[在利用体外化学治疗术(ECP)治疗慢性HCV患者时使用或不使用α干扰素的对比研究之研究者手册。1996,6]中得到。体外光化学和不同剂量的UVADEX和紫外光对Beagle狗的毒理学研究也可以在研究者的手册中找到。
UVAR系统
治疗包括三个步骤:1)收集淡黄色表面层(白细胞富集层),2)照射收集的淡黄色表层,和3)重注射处理后的白细胞。收集步骤包括血液抽取、离心和重注射的六个循环。在每个循环中,全血经过离心,在儿科抽取皿中分离。在分离中,血浆(在每一轮中的容量取决于UVAR仪器操作者)和40毫升淡黄色表层在每一轮收集中得到保存。最终,总共240毫升淡黄色表层和300毫升血浆被分离和用于UVA激发。
在第一轮收集循环的淡黄色表层收集时开始激发过程的对富含白细胞的血液的激发。收集到的血浆和淡黄色表层和200毫升肝素化的标准盐、200mcgUVADEX(水溶性的8-甲氧基补骨脂素)混合。混合物由1.4毫米的薄层通过PHOTOCEPTOR光激活室,该室被插入PHOTOSETTE的两块UVA灯之间。PHOTOCEPTORUVA灯照射这个UVA-透明的PHOTOCEPTOR室的两侧,允许暴露于紫外A光中180分钟,使得每个淋巴细胞平均暴露1-2J/m2。最终淡黄色表层包含大约总共外周血液单核细胞成分的20%到25%,并且血球容积计值为2.5%到7%。经过光激活阶段30到45分钟以上,液体被重注射到患者体内。
使用这些技术的系统是已知的,籍此可采取体外处理患者血液的技术。例如在美国专利US 4,573,960中,一个患者,Goss,被给以需要光激活的药物,然后抽取患者的血液,分离组分。未处理的组分(红细胞,一部分血浆等)被重返回至患者。然后使患者脱离治疗器械,而分离的组分,如白细胞被暴露于紫外光。光激活之后,处理后的细胞重返患者体内。
在授予Edelson的美国专利US 4321919;4398906和4464166中讨论了用于某种疾病的外部治疗手段,该病中,淋巴细胞如皮肤T-细胞有病理性增长。在这些手段中,患者的血液在化学物质或某抗体的存在下受紫外光激发。紫外光影响到淋巴细胞与化学物质或抗体间的结合,从而阻止淋巴细胞的代谢过程。
大量人病毒可以在单核细胞内感染或复制,或者,感染性病毒颗粒持续存在于单核细胞中。单核细胞可以作为病毒复制的来源或病毒的传播源,或作为感染性病毒颗粒的藏身之所,使其难以被免疫系统消除。不能消灭这些感染病毒源可能会导致慢性疾病的建立。可以感染、复制或存留在单核细胞的病毒包括,但不局限于虫媒病毒、肠病毒、副粘病毒(RSV)、疱疹病毒、巨细胞病毒(CMV)、EB病毒(EBV)、乙型肝炎病毒(HBV)、丙型肝炎病毒(HCV)、丁型肝炎病毒(HDV)、庚型肝炎病毒(HGV)、和逆转录病毒(例如HIV)。
大量人非病毒病原体可以在单核细胞内感染和复制,感染性的非病毒病原体也可以存留在单核细胞内。单核细胞可以作为非病毒病原体复制的来源或非病毒病原体的传播源,或作为非病毒病原体的藏身之所,使其难以被免疫系统消除。不能消灭这些感染性非病毒病原体可能会导致慢性疾病的建立。可以感染、复制或存留在单核细胞的非病毒病原体包括,但不局限于细菌如虫媒细菌,支原体类、和分支杆菌类和寄生虫如疟原虫类和其它虫媒寄生虫。
体外光除去法(ECP)已被成功的应用于治疗HIV感染(美国专利:4960408),而长波紫外光激发的补骨脂素化合物已显示出在体外能使某些病毒失活,如HIV(Quinnan,G,V.et al.,1986转染,26,pp481;Bisaccia,A.et al.,1990,Am.Intern.Med.,113,pp 270;Bisaccia,A.et al.,1991,Ann.Ny Acad.Sci.,636,pp321),流感病毒和单纯疱疹病毒(Redfield,D.C.et al.,1981;Infect.And Immun.,32,pp 1216)。Bisaccia已经对使用ECP作为治疗患有AIDS相关综合征患者手段进行了初步研究。原理是结合使用UVA激发和补骨脂素可以体外损坏HIV病毒,并且重新注入损坏的病毒可引发免疫反应。作者发现ECP导致HIV-Ab产生的增加,CD8(+)白细胞的增加,p24抗原滴度与在3个患者体内无法培养HIV能力的减小。20个患者中的11个在皮肤抗原反应测试中有显著改善。
另外,据报道在移植外科手术后,在接受针对免疫抑制的光除去法治疗的患者中感染的发生率减少(Meiser,B.M.et al.,1994,移植,
57,pp.563)。然而,对进行移植手术的患者观察的结果并不与光除去法治疗效果相关,因为一般感染事件被记录包括那些接受广泛治疗以防止排斥移植器官的患者。
发明简述
本发明涉及一种利用体外光除去法技术治疗肠紊乱炎症,包括但不限于IBD、Crohn病、和溃疡性结膜炎的方法。另外本发明涉及治疗膀胱炎症疾病,包括但不限于膀胱炎、如间质性膀胱炎。具体地说,利用本发明的方法可治疗IBD、Crohn病、溃疡性结膜炎、间质性膀胱炎的患者。本发明还涉及利用光除去法的方法改变或调制肠紊乱炎症或膀胱炎症患者的单核细胞的功能。本发明治疗的方法还涉及用可被光激活的或光敏性的化合物处理患者的血液,该化合物在被紫外光激发后能够结合到被感染的有核细胞内的核酸上。可被光激活的或光敏性的化合物可通过常规给药方法在体内或体外引入到患者的血液中。
然后利用光除去法对患者的一部分血液进行体外处理,这包括将血液暴露于紫外光,优选的为波长范围在320nm到400nm的长波紫外光,通常称为UVA光。将经光除去法处理后的血液或其一部分重返回患者体内,来调节单核细胞的功能和/或刺激患者的免疫系统产生免疫反应。这种处理会破坏细胞遗传物质,它会象这里公开的一样导致单核细胞功能的改变或调制。
发明详述
本发明旨在利用光除去法治疗有肠炎症紊乱的患者,包括但不限于,IBD、Crohn病、溃疡性结膜炎或膀胱炎症紊乱患者包括但不限于膀胱炎、如间质性膀胱炎。由于并不试图将本发明的范围限于特定的操作理论,而是认为患者的肠炎症紊乱和膀胱炎症紊乱可以通过使用本发明的光除去法治疗来处理。
利用甲氧沙林(光除去法)的体外光化学治疗被认为可引起针对异常T-细胞(对CTCL而言,为癌)的免疫反应。在光除去法过程中,甲氧沙林进入白细胞核,插入双链DNA螺旋。在体外过程中,长波紫外光照射富含白细胞的血液。甲氧沙林在UVA能量的作用下,与DNA双螺旋的胸腺嘧啶碱基结合。这导致胸腺嘧啶碱基的交连,阻止转录过程的DNA的解旋。紫外A光(UVA)破坏异常T-细胞,使得它们更具免疫原性。细胞被光激活后,重注入这些改变了的T-细胞可引起针对携带同样表面抗原的T-细胞的免疫反应[Edelson RL.光除去法:临床相关免疫反应修饰剂。[Review]Ann NY Acad.Sci.191;636:154-64].。其结果是产生针对异常细胞(或是癌克隆或可能是在其表面表达病毒抗原的T-细胞)高度专一性的免疫反应。据估计每个光除去法过程大约处理全部外周血单核细胞5个分隔间的25-50%(连续两天)。
Vowels的工作[Vowels BR,Cassin M,Boufal MH,et al.体外光化学治疗引发单核细胞产生肿瘤坏死因子α的增加:治疗皮肤T-细胞淋巴瘤和全身性硬化的意义。J Invest Dermatol 1992;98:686-92]证实在体外过程中用含8-甲氧基补骨脂素的血浆处理的单核细胞,暴露于紫外A光(光除去法)后,释放肿瘤坏死因子α、IL-1、IL-6可能还有IL-8。一般认为光除去法调节外周血单核细胞/巨噬细胞的活性。
根据所要求的方法,首先将具光激活活性或光敏性的化合物施用于患肠炎症紊乱或膀胱炎症紊乱的患者血液。具光激活活性或光敏性的化合物可以通过体内给药(口服或静脉内注射)或体外引入患者的部分血液,这些血液是通过使用常规血液抽取技术从患者体内获得的。
根据本发明,具光激活活性或光敏性的化合物暴露于前述波段的电磁辐射,如紫外光,被激活后能结合到核酸上。
接着,已被引入具光激活活性化合物的患者血液的一部分暴露于紫外光下进行光除去法。优选的,光除去法步骤应在体外使用体外光除去仪器进行。根据本发明,光除去法步骤也可以在体内进行。根据本发明,目前优选的体外光除去仪器是由Therakos,Inc.制造的,名为UVAR,对这种仪器的描述可在美国专利4683889中找到。在光除去仪器中将血液暴露于紫外光是本领域普通技术人员的能力范围之内的。
当光除去法步骤在体外进行时,至少有一部分经处理的血液重返回患者体内。优选的,这里描述的治疗方法以大约每周一次到大约每四周一次的间隔重复。根据本发明应用的具光激活活性化合物优选为公知的补骨脂素(呋喃骈香豆精)和补骨脂素衍生物,正如美国专利4321919和5399719中所描述的。替代地,患者的血液可以在一个标准Apheresis型设备中分离,并在一个独立的设备中进行光激活。
根据本发明,优选的可光激活的或光敏的化合物包括但不限于下列:补骨脂素及其衍生物;8-甲氧基补骨脂素;4,5’,8-三甲基补骨脂素;5-甲氧基补骨脂素;4-甲基补骨脂素;4,4-二甲基补骨脂素;4-5’-二甲基补骨脂素;4’-氨甲基-4,5’,8-三甲基补骨脂素;4’-羟甲基-4,5’,-8三甲基补骨脂素;4’,8-甲氧基补骨脂素;和4’-(Ω-氨基-2-氧杂)烷基-4,5’,8-三甲基补骨脂素,包括但不限于4’-(4-氨基-2-氧杂)丁基-4,5’,8-三甲基补骨脂素。根据本发明,最特别优选的光敏化合物是8-甲氧基补骨脂素。
当光敏化合物在体内施用于患者血液时,优选的是口服,但也可以是静脉内注射和/或其它常规给药方式。光敏化合物优选的口服剂量是约0.3到约0.7毫克/千克的范围内,更优选的为大约0.6毫克/千克。
当口服给药时,优选地,光敏化合物应至少在光除去法处理前约1小时到不超过光除去法处理前的大约三小时被给药。如果静脉内注射,相应的时间会短一些。
替代地,光敏化合物给入到患者的血液应在其从患者的身体取出后,在暴露于紫外光之前或同时。光敏化合物可以加入到全部的血液中或其中的一部分中,只要目标血液细胞或血液组分接受到光敏化合物即可。
根据本发明治疗方法中的光除去法处理优选地使用长波紫外光(UVA),波长范围在320到400nm。优选在光除去法处理过程中将血液暴露于紫外光足够长的时间以给血液传递约1-2J/cm2的光。
根据本发明,当光除去法处理在体内进行时,应注意控制最大放射以避免对患者不必要的损伤。计算紫外光最大放射暴露的方法是现有技术中公知的。
首先,在移出血液前或体外给药时从患者体内取血之后,将上述光敏化合物或口服或静脉注射加入到至少一部分供体血液中。优选地,首先利用已知的方法来基本上除去一部分供体血液中的红血球,然后将光活性化合物加入所得的富含白细胞的部分。
在任何情况下,利用紫外光,优选使用UVA以前述的方式对加入光敏化合物的部分血液(或其富含白细胞的部分)进行光激活处理。然后将处理的血液或处理的富含白细胞的部分(就象可能的情况)重返供体。
当前治疗肠炎症紊乱包括化学治疗剂,如氨基水杨酸、皮质甾类、和免疫抑制剂。这些化学治疗剂产生显著的副作用。许多患者在完成一次治疗剂量之前对氨基水杨酸发生副作用。恶心、不适、头痛、和肌痛等副作用经常导致患者的不合作。长期服用皮质甾类的副作用(类库欣综合征的体征、高血压、白内障形成、骨质疏松症和无菌坏死)已经有了详细地记载。另外,尽管皮质甾类对治疗疾病的突发有效,但这些药物阻止复发的能力从没有被证实过。逐渐增大的可发展成癌症、嗜中性白血球减少症和骨髓抑制的危险性也在器官移植受体人群中有详细地记载。
使用本发明方法治疗的肠炎症紊乱的简要描述包括但不局限于:
1.溃疡性结膜炎
溃疡性结膜炎是一种影响结肠的黏膜失调,与显著的病态和致死相关。有相关的肠外表现,包括关节炎,类风湿性脊髓炎、肝炎、葡萄膜炎、坏疽性脓皮病、结节性红斑和硬化性胆道炎。癌会在慢性炎症区发生。溃疡性大肠炎的特殊症状包括痢疾、肠出血、体重减少和异常疼痛和痛性痉挛。该病可能影响任何年龄的人。患此病的儿童不能正常生长和发育。
2.Crohn病
Crohn病以消化道上从嘴到肛门的任意部分的,一块或多块有病灶的,透壁的和不对称的炎症为特征。最常涉及的区域包括回肠末梢和右结肠。患Crohn病的患者中专门涉及结肠的占20%,限于小肠的占15-20%。与Crohn病相关的溃疡呈线形延伸,经常分离袋状正常黏膜,使该病有鹅卵石状的特征表现。
利用本发明方法治疗的特定膀胱炎症紊乱的简要叙述包括但不限于:
1.间质性膀胱炎(IC)
间质性膀胱炎(IC)是一种与IBD类似的慢性严重膀胱失调症,因为它是一种自身免疫疾病,表现为攻击膀胱壁而不是肠壁。象IBD一样,IC在组织上被认定是带有黏膜溃疡和伤疤的膀胱壁炎症渗漏。它导致平滑肌收缩,容尿量减少,尿频,血尿,尿急,夜尿,尿痛,和耻骨弓上骨盆痛。阿密曲替林、羟嗪、二甲基亚砜、chlorpactin和肝磷脂的治疗可能改善病症,但长远地并不改善整个病情。
利用本发明治疗肠炎症紊乱,包括但不限于IBD、Crohn病,治疗膀胱炎症紊乱包括但不限于膀胱炎如IC的结果,是对疾病症状的消除,在这些患者身上反应为疾病活性指数的减少,和/或类固醇计量的显著减少(尤其对于那些类固醇依赖者)和/或避免外科手术(切除受影响组织)和增加无病症间隔。
以下实施例被用来阐述本发明而不应解释为限制本发明。
实施例1
用体外光化学治疗溃疡性结膜炎的患者。
I.患者人群说明
患急性溃疡性大肠炎的患者是这次临床研究的对象。为满足此次研究计划,以下包括/排除标准必须满足:
—患者必须为40公斤
—待分娩的女性患者必须在研究处理前24小时内进行孕尿检查。有效的生育控制必须在研究处理前一个月到计划的末期实行。女患者不能是泌乳的。
—研究前实验值:

●血色素超过6.5mg/dL

II.治疗频率和效果参数的描述
连续进行两天光除去法治疗,持续四周(8次光除去法处理),然后在下个8周内每隔一周进行连续两天的处理(8次额外处理),这样在超过12周进行总共16次光除去法操作。(定作疗程I)。疗程I的目的是为了取得初步有效参数(疾病活性指数[DAI],外科干涉所需评估,估计患者停用类固醇或其它用于控制UC症状的免疫抑制类药物的能力,和评价对内窥镜和组织水平上治疗的反应能力)。为了达到此次方案的目的,一个对治疗成功的反应被定义为:
疾病活性指数(DAI)下降50%,避免外科手术或减少/消除类固醇以及在内窥水平和组织上的改善都被认为有临床意义。
当疗程I完成后,患者在不连续治疗下维持9个月,在接着的疗程内,在研究者的谨慎判断下,用光除去法处理疾病突发(即象溃疡性大肠炎征候[腹泻、出血、和疼痛]增加所证实的)。随后疗程的目的在于计算(1)无疾病间隔,(2)决定病发是否可以被光除去法控制,(3)给停止光除去法治疗提供数据。
III.有效性评价描述
在治疗和随后的疗程内,患者接受以下有效性评价:
A.疾病活性指数:这是一个由Lichtiger等所述的改良的Truelove和Witts标准(表1)。这个标准评价了血肠运动的次数、夜间腹泻存在、不连续性、异常痛、一般健康、和异常的柔和。最小值为0。最大值为21。12或大于12被认为严重范围。显著的改善显示DAI值减少50%。小于4的评分表示临床缓解。

六周评测:患者的疾病活性指数降为20之2,而且大范围改善并被记载为每天腹泻次数降为0-2次,没有粪便不连续的情况。患者的生活质量增长为可能值224之208。记载在病历报告上的患者感觉“很好”并“没有”疼痛,患者报告许多天没有便血。乙状结肠造口术显示直肠黏膜改变,即伴随红斑状的和粒状的变化。目测结肠(有30厘米)全部正常。UC组织学评分从6降为1,UC慢性评分从3降为2,内窥检查评分从2降为1。

概述:42岁男性患者,5年多依赖类固醇控制溃疡性大肠炎病症的历史。
基线评测:参加治疗方案时,血便,结肠内窥镜检查显示黏膜变化从0到40厘米。内窥镜检查结果包括水肿、红斑状和纤维状表现、似小溃疡样侵蚀、(红斑边缘,黄褐色中心凹坑,缺少凸出的边)、模糊/钝形血管。DAI值为3,患者每两天输入20mg强的松.基线生活质量为可能值224的188。
两周评测:DAI仍然是3,但患者的强的松的用量已减为每两天10mg,并没有加重症状。生活质量值为212的188。
实施例2
体外光化学治疗患有Crohn病的患者
I.患者人群描述
患有急性Crohn病的患者是这个临床研究的对象。为满足此次研究计划,以下包括/排除标准必须满足:
A.包含标准
—年龄在18到65岁之间
—根据Malchow的标准和Crohn的活性指数(CDAI)诊断Crohn病
—放射线和/或内窥镜和/或声谱仪定位记录在开始研究前的12个月内
—类固醇依赖疾病必须有每天至少10mg强的松至少三个月的治疗记录来达到症状缓解(缓解被定义为连续两周CDAI<200)。类固醇依赖必须被试图减少类固醇剂量到<10mg情况下的至少一次的复发历史证实。
—在患者被治疗医师完全通知后的,有证明人的情况下书面的或口头的满意
—在开始治疗前,所有育龄妇女必须接受一次第一次治疗前24小时内的有记录的直肠内的β-HCG怀孕检测
—没有已知的对肝磷脂或8-甲氧基补骨脂素的过敏。
B.排除的标准
—患者不能与筛选和治疗步骤合作
—患者由于出血、脓肿和腹膜炎而有可能需要接受外科干预
—患者有临床相关的胃肠狭窄
—患者有肠瘘
—现在或曾经在治疗前的3个月内接触过的环孢菌素-A
—以前没有接触过(3个月内)或3个月内稳定剂量的咪唑硫嘌呤
—有确认的病原体并在过去3个月内接受三次以上用Ciproxin或甲硝哒唑的抗菌治疗,而且每个疗程都在14天以上
—哺乳或怀孕妇女
—患者有严重的并发痛
—心血管不稳定,在光除去法治疗中不能够使得足够量的血液回流
—用已知具有光敏活性药物的治疗
—静脉取血不方便的患者
—携带HIV,Hbs抗原或HCV阳性的患者
—具有以下实验值的患者
●血球蛋白少于10.5g/Dl
●血小板少于100×109/L
●WBC<4000×109/L
●血清胆红素>3mg/dL
●血清肌氨酸酐>2mg/dL
●PT<60%,PTT>50sec
II.治疗频率和有效性参数的描述
A.治疗频率和有效性评价
初期0
在研究的初期,满足内含标准的患者,或那些在将来观察中能满足的患者被挑选出来。患者疾病类固醇依赖过程的回顾性记录必须保证。
每个患者的CDAI在研究开始前六周被确定。在下个六周,患者在持续剂量下被稳定,该剂量是保证Crohn病症状不活跃(定义为CDAI<200持续两周)的最小剂量。患者在维持剂量必须大于每天10mg强的松时进入研究。
对患者进一步的监测发生在研究开始前的一周内(周-1)。

—以前外科史
—以前对Crohn病的治疗
—根据内含标准定义的类固醇依赖证据的记录
—病史和身体状况
—实验室检测(有差别的CBC、ESR、CRP、1-酸醣蛋白、血清电解液、肌氨酸酐、BUN、SGOT、SGPT、胆红素、γ-GT、血清葡萄糖、PTT、PT、和尿分析)

—三倍可渗透性试验:
蔗糖、乳果糖、甘露醇
—生活质量指标
初期1
满足包含标准的患者在六个月的初期被引入研究。将来疾病活动的记录和累积类固醇剂量必须保证。
在初期,根据确定的类固醇戒除计划连续地尝试进行减少类固醇剂量。重复以上尝试减少类固醇的剂量到根据相应计划维持剂量以下。维持剂量在实际治疗期被一直减少。
根据CDAI的测量,患者的临床活性被用作决定每个类固醇戒除计划是否减量的标准。
类固醇减量在疗程1的第一天开始。2周内通过用接下来的较低水平的标准剂量系列(例如,50毫克、37.5毫克、25毫克、15毫克、10毫克、5毫克)改变最初的类固醇剂量来减少剂量.在接下来的两周内,试着减少剂量至这个水平。如果临床症状减缓能持续(在起始水平CDAI>60或第二周>200水平上没有增长)的话,类固醇剂量以一个额外的剂量水平减少4周,就象上面说明的一样。
主疾病的复发(在起始值CDAI>60或两周后>200水平上增加)通过在发炎前恢复类固醇剂量到剂量序列的下一个较高水平两周来治疗。如果以这个剂量水平不能达到临床症状缓解(CDAI<200持续两周),那么将类固醇剂量增至每天50毫克持续一周,接着按照标准类固醇戒除计划减少类固醇剂量。然而,类固醇剂量仅仅减少到最近一次观察到临床症状缓解时的剂量。这个剂量有可能不同于维持剂量,而称为“缓解稳定剂量”,这个剂量然后再根据类固醇戒除计划。在疗程末期的维持剂量不同于研究初期的维持剂量的可能性是不能排除的。
患者Crohn病的临床过程在开始光除去法处理期前的两周要维持稳定。
在开始光除去法处理期前的两周,患者被给予Crohn病日志,以记录与他们疾病相关的症状。
在初期,备用于分化CBC、化学、CRP和1-酸糖蛋白的血液样品每四周取一次。
治疗期
光除去法治疗连续进行两天作为六个月中的一个月基础。
在每月一次的探访的第一天治疗前,进行以下检测:
—检查临床状态
—CDAI
—记录Crohn病的肠外症状
—记录类固醇剂量和其它伴随药物
—实验室检测:血样的分化CBC、化学、CRP、和1-酸糖蛋白
—评价负效应
—生活质量指数
—渗透检测(仅在第0和第6月)
—结肠窥镜活组织检查是任选的
在治疗的第—天开始类固醇减量。根据标准类固醇剂量戒除计划,4周内的剂量减少相当于5毫克的强的松。Crohn病的复发并且CDAI值超过450导致研究结束。

实施例3
体外光化学治疗间质性膀胱炎患者
I.患者人群说明
患急性间质性膀胱炎的患者是本次临床研究的对象。为满足此次研究计划,以下包括/排除标准必须满足:
A.包含标准
—年龄在18到65之间
—在患者被治疗医师完全通知后的,有证明人的情况下书面的或口头的满意
—在开始治疗前,所有育龄妇女必须接受一次第一次治疗前24小时内的有记录的直肠内的β-HCG怀孕检测。另外,在研究中她们必须同意接受足够形式的避孕。
—没有已知的对肝磷脂或8-甲氧基补骨脂素的过敏。
—患者必须在膀胱充满时有疼痛感,而在空着时有舒缓感
—并且—
患者必须有耻骨弓上、骨盆的、尿道的、阴道的或会阴的疼痛
—并且—
患者每天排尿频率超过5次和每夜夜尿频率超过2次
—并且—
在膀胱镜检查有水扩张后,患者有聚成密集的小簇。
—或—
Hunner溃疡。
B.排除标准
—患者不能与筛选和治疗步骤合作
—患者有良性或恶性膀胱瘤
—患者有放射、细菌、或环磷酰胺诱发的膀胱炎
—患阴道炎的患者
—患者患有症状的尿道的憩室
—患者患有子宫、子宫颈、阴道的或尿道癌
—患者患有活跃疱疹
—患者有膀胱或尿道低端结石
—膀胱炎症状被抗菌素、尿抗菌素、和尿镇痛剂缓解
—症状持续不超过12个月
—患者有由排尿引起的非自发的膀胱收缩
在膀胱容量为400ml时,没有感到尿急
—哺乳或怀孕妇女
—患者有严重的并发症
—心血管不稳定,在光除去法治疗中不能够使得足够量的血液回流
—用已知具有光敏活性药物的治疗
—静脉取血不方便的患者
—携带HIV,Hbs抗原或HCV阳性的患者

II.治疗频率与有效性的说明
光除去法以每周连续两天,进行四周(8次光除去法处理),然后以2每两周连续两天,进行8周(另外8光除去法),这样总共在12周进行16次光除去法。经过12周,光除去法减量至每月一次或更多,这依赖于患者的长期反映。治疗可以被重新开始来治疗疾病复发。
对本发明治疗方法结果的期望是疼痛、血尿和排尿频率的显著减少。患者需要治疗IC的伴随药物的品种和数量减少,导致整个生活质量的提高和疾病的减轻。
Claims (8)
1.体外处理肠紊乱炎症或膀胱炎症患者血液的方法,包括以下步骤:
A.将具有光激活活性或者光敏性的化合物体外加入所述患者血液中;
B.将步骤A的血液暴露于可激活所述光激活活性或者光敏性化合物的光下足够时间。
2.权利要求1的方法,其中所述具有光激活活性或者光敏性的化合物为补骨脂素或补骨脂素衍生物。
3.权利要求2的方法,其中所述具有光激活活性或者光敏性的化合物是8-甲氧基补骨脂素。
4.权利要求1的方法,其中所述可激活光激活活性或者光敏性化合物的光为紫外光。
5.权利要求4的方法,其中所述紫外光是波长范围为320到400纳米的长波紫外光。
6.权利要求1的方法,其中血液暴露于光中的时间足以给血液传递1-2J/cm2的光。
7.权利要求1的方法,其中步骤A中在加入光激活活性或者光敏性的化合物之前先处理患者血液,以除去血液中的红血球。
8.权利要求1的方法,其中所述肠紊乱炎症是肠炎疾病、Crohn病、和溃疡性结膜炎,所述膀胱炎症是间质性膀胱炎。
