Abstract

技术领域
本公开涉及一种标本取出装置。更具体地,本公开涉及一种用于微创手术操作中的标本取出装置。
背景技术
腹腔镜和内窥镜手术操作是微创操作,其中借助经由相对较小的入口或进入开口(例如,由身体的自然通道而限定的开口、由组织穿刺器具(如套管针)生成的开口等)插入身体中的细长器具,在体内执行操作。
微创操作经常用于从身体内部部分地或全部地移除身体组织或器官,例如,肾切除术、胆囊切除术、十二指肠切除术、回肠切除术、空肠切除术和其他此类操作。在这些操作中,通常需要经由皮肤中的进入开口或插管来移除受感染的组织或器官。便于这种操作的多种捕集装置(entrapment device)(如标本取出袋或标本取出囊)在本技术领域中为公知的。
标本取出袋通常包括防刺穿材料层、防潮材料层和拉线。在某些实例中,标本取出袋可以配有用于切碎或以其他方式处理置于标本取出袋中的组织的碎分器。使用时,将标本取出袋置于体腔内。将身体组织或器官置于标本取出袋内。然后,经由皮肤切口从体腔中拉出标本取出袋。
然而,在某些实例中,置于公知的标本取出袋中的身体组织或器官可能会在标本取出袋的底部积聚并膨胀(例如,在将较长的组织置于取出袋的示例中),因而导致有时难以从相对较小的进入开口移除标本取出袋。此外,标本取出袋中可能滞留有空气,且因而可能会增加标本取出袋的总体积;正如所能想到的这也可能导致难以从相对较小的进入开口移除标本取出袋。而且,为了从进入开口移除公知的标本取出袋,使用者通常对单一点(即拉线连结标本取出囊的点)施加拉力,因而存在从此点撕裂标本取出袋的可能性。
如上所述,仍存在对于一种改进型标本取出装置的需求,其便于在微创手术操作中组织的移除。
发明内容
本公开提供了一种标本取出囊(specimen retrieval pouch),其配置为与一个或多个用于将组织置于标本取出囊内的手术器具结合使用。所述标本取出囊包括闭合底部和开放上部。所述标本取出囊包括可操作地联接至内壁并且沿其长度方向延伸的细长管。所述细长管具有沿其长度方向设置的多个开口。缝合线能够在所述标本取出囊的近端处被操作(accessible)且置于所述细长管内,并且延伸经过所述细长管的远端。所述缝合线能够在所述细长管内相对于所述标本取出囊运动。所述缝合线的远端具有针或结,所述针或结用于连结至组织并随后在拉动所述缝合线穿过所述细长管时将组织拖到所述标本取出囊内。一个或多个加强突出物可操作地布置在所述标本取出囊上。所述一个或多个加强突出物配置为供使用者抓取,以便在拉动所述缝合线穿过所述细长管时提供相反的拉力。

具体实施方式
现在将参照附图对本公开的实施例进行详细描述,其中在每个附图中相似的附图标记表示相同或相应的元件。在本公开中,术语“远端”是指器具的最远离使用者的部分,而术语“近端”是指器具的最靠近使用者的部分。在下面描述中,不对公知的功能或结构进行详细描述,以免不必要的细节影响本公开的清晰性。
正如参照本公开在此所使用的,术语“腹腔镜”和“内窥镜”可互换使用,且指代具有用于插入插管或皮肤上小切口的相对狭窄操作部的器械。它们也指微创手术操作。应当认为,本公开适用于任何手术操作,其中与身体内部的接口限于相对较小切口,且无论是否使用在微创手术操作中所用的插管。
参照图1A至图1B,示出了根据本公开的一个实施例的包含标本取出囊组件4(囊4)的标本取出装置或施放器10。虽然囊4可以从患者处的开口插入并随后取出,而不使用标本取出装置10,但是为了说明的目的,此处仍描述了囊4从标本取出装置10中展开的情况。
简言之,标本取出装置10包括手柄组件6,其包括固定接合在一起的手柄部8和11(图1A)。细长管或轴12从手柄组件6延伸,并设计为具有插入穿过用于内窥镜或腹腔镜操作的套管针插管的尺寸(图1A)。在某些实施例中,可将轴12配置为能够进行关节式运动。驱动杆21在轴12内沿其长度延伸,并可操作地联接至指圈14形式的致动件(图1A)。以适合从标本取出装置10展开囊4的合适方法将驱动杆21的远端可操作地联接至囊4。指圈14配置为接合使用者的手指并配置为在轴12内轴向地平移或运动驱动杆21。拉环16通过任何合适的联接方法(如打结、粘合等)可操作地联接至缝合线18的近端,并配置为便于拉动缝合线18(图1A)。例如,参见图1A并结合图1B,在示出的实施例中,通过一种或多种合适的联接方法(如压配合或摩擦配合)将拉环16可拆卸地联接至指圈14。
现在参照图2至图4,其中示出了囊4。囊4可以由能够形成柔性膜的任何合适的生物相容性材料制造。更具体地,囊4由基本透明的聚合物材料形成的一片或多片柔性薄膜或薄片而构成。在示出的实施例中,囊4由借助一种或多种合适的接合方法(如粘合、焊接等)接合在一起的两片塑料材料而构成。在示出的实施例中,两片塑料材料焊接在一起以分别形成内壁30和外壁32(图2至图4)。在示出的实施例中,内壁30具有纹理以形成多个流道36(图2和图3),配置所述流道36以允许空气在置于囊4中的组织“T”周围流动,以便防止囊4中和组织“T”附近出现大面积滞留气体。癌细胞基本不能渗透穿过囊4。囊4的比例与器官的捕集或移除的需要相称。在某些实例中,可以设计囊4的尺寸并由合适的材料制造囊4,以允许器官组织的处理(如切碎或分割),例如减小其体积以便于从体腔中取回。
囊4具有大体管状或细长配置,其由加宽、开放的上部或口部26和锥形、闭合的下部28而限定(图2至图4)。在某些实施例中,口部26可以是可开放的且可闭合的。也就是说,拉线或其他合适的装置可操作地联接至口部26并配置为在其致动时(即拉动拉线时)将口部闭合;当待切除的组织“T”已病变时,该实施例特别有用。
继续参照图2至图4,细长管34包括相应的开放的近端35和开放的远端37。开放的近端35和开放的远端37配置为允许缝合线40在细长管34内自由运动。另外,远端37的尺寸能够防止将缝合线40的远端44经过开放的远端37完全拉入细长管34内。
细长管34沿囊4的长度延伸(图2至图4)。细长管34通过一种或多种合适的联接方法(如焊接、粘合、嵌件成型等)可操作地联接至内壁30。在示出的实施例中,在沿内壁30的各间隔位置(如图2至图4中剖面线“w”所示)处焊接细长管34。按照这种方式沿囊4的内壁30焊接细长管34,有助于在将组织“T”拖到囊4内时排空囊4中的空气并为囊4提供结构支撑。
细长管34可以由任何合适的生物相容性材料制成,其包括但不限于塑料、金属、金属合金。在示出的实施例中,细长管34由塑料制成。在示出的实施例中,细长管34由如下塑料制成,所述塑料使得囊4能够插入并且随后从手术取出装置10中展开。例如,在一个具体实施例中,用于制造细长管34的塑料使得囊4能够卷起(或折叠)并装载或放置到标本取出装置10的细长轴12内。在囊4展开时,用于制造细长管34的塑料使得包含细长管34的囊4能够打开(或展开)。
沿细长管34的长度方向排列多个适当配置的开口38(图2至图4)。配置开口38以在将组织“T”拖到囊4内时允许空气通过其逸出,以便于将组织“T”置于囊4内。为此,可以沿细长管34的长度方向径向地排列开口38,以形成大体螺旋的配置(图2至图4)或其他配置。随着组织“T”进入囊4,空气从开口38排出并传送到囊4的近端,因而使得组织“T”能够完全进入囊4,而不会受到形成在囊4中和组织“T”周围的气穴的阻碍。
再次参照图2至图4,缝合线40(或其他合适的装置)经由细长管34可操作地联接至囊4。特别地,当拉动拉环16时,缝合线40能够在细长管34内相对于囊4从用于连结至组织“T”的第一位置(图2)运动到用于将组织“T”拖到囊4内的后续位置(例如,参见图3和图4)。
缝合线40配置为通过一种或多种合适的固定方法固定至组织“T”,以将组织“T”拖或拉到囊4中。在如图2至图4所示的一个具体实施例中,针42(或其他合适的装置,例如:弹簧夹、夹钳、勾子等)(经由一种或多种合适的方法)可操作地联接至缝合线40的远端44。可选择地,例如参见图5,远端44可以包括适当配置的圈“L”。在此实施中,圈“L”可操作地连结至组织“T”。当需要切除的组织“T”已病变(例如癌变组织)时,不能刺穿组织“T”,即为了减少感染健康组织的可能性,这时该实施例特别有用。
一个或多个加强突出物46(图2至图4中示出了一个加强突出物46)可操作地布置在囊4上,并设置为在拉动缝合线40穿过细长管34时相反的施加到缝合线40的力。特别地,配置加强突出物46以便使用者抓取,从而在拉动缝合线40穿过细长管34时提供相反的拉力。例如,当拉动缝合线40穿过细长管34时,使用者可以利用抓紧器(或类似装置)抓取加强突出物46。在图2至图4所示的实施例中,加强突出物包括设置为便于抓取(即勾住)加强突出物46的孔48。加强突出物46可操作地布置在囊4的闭合底部28附近的外壁32上。
加强突出物46可以由任何合适的生物相容性材料制成,其包括但不限于塑料、金属、金属合金等。在示出的实施例中,加强突出物46由塑料制成,所述塑料使得囊4能够插入并且随后从标本取出装置10中展开。例如,在一个具体实施例中,用于制造加强突出物46的塑料使得囊4能够卷起(或折叠)并装载或放置到标本取出装置10的细长轴12内。在囊4展开时,用于制造加强突出物46的塑料使得包括加强突出物46的囊4能够打开(或展开)。
在如图5所示的一个具体实施例中,加强突出物46可以替换为两个加强翼或加强段46a和46b。加强段46a和46b起到了类似于加强突出物46的功能。此外,当将组织“T”拖到囊4中时,加强段46a和46b为囊4提供附加支撑。而且,加强段46a和46b是柔性的并能够抵靠囊4的外壁32而彼此齐平布置。可以理解的是,在不使用标本取出装置10的情况下,当经由进入开口定位囊4时,该设计非常有用。在图5所示的实施例中,每个加强段46a和46b限定了相应的各行孔48a和48b。孔48a和48b为可操作的以实现与孔48相同的功能。
在使用中,利用标本取出装置10插入患者的进入开口,以在患者的体腔内定位囊4。随后,囊4从标本取出装置10中展开并打开(图2)。在一个具体的手术方案中,可以利用抓紧器来帮助打开囊4。在打开后的配置中,可以利用抓紧器通过针46或圈“L”将远端44连结至组织标本“T”,例如肠内组织。之后,通过拉环16拉动缝合线40穿过细长管34,以将组织“T”拖到囊4内(图2至图4)。随着组织“T”被拖到囊4内,空气从孔38和/或流道36排出。具体地,滞留在囊4中的空气经由流道36导向至孔38,并传送到囊4的近端。使用者可以抓取加强突出物46(或在某些实例中,一个或两个加强段46a和46b),以便于将组织“T”拉到囊4内。
当组织“T”定位到闭合下部28附近时,使用者可以释放加强突出物46(或加强突出物46a和46b)并拉动缝合线40,以移除其中包含组织“T”的囊4。在拉动缝合线40时,开放远端37将缝合线40的远端44及其连结的组织“T”保持在囊4内。也就是说,配置开放远端37以阻止缝合线40的远端44(以及与之相联的针或圈“L”)由此通过并进入细长管34。
当囊4邻近进入开口时,使用者可以抓取加强突出物46(或加强片46a和46b)以经由进入开口拉出囊4。可选择地,使用者能够简单地使用缝合线40经由进入开口拉出囊4。
其中放置有缝合线40的细长管34的独特配置能够克服通过与公知标本取出囊相关联的前述缺陷。也就是说,其中放置有缝合线40的细长管34使得使用者能够从闭合下部28将组织“T”拉到囊4中,因而减少或排除了组织“T”在囊4内积聚和膨胀的可能性。而且,孔38和/或流道36能够减少或去除囊4内滞留的空气。进一步,加强突出物46(或突出物46a和46b)提供了从进入开口移出囊4的附加的和/或可选的方法;当囊4中包含大组织段或器官时,其具有优势。也就是说,采用通过加强突出物46(或突出物46a和46b)而非拉动缝合线40来拉动囊4的方法将包含大组织“T”的囊4移出,降低了在拉动囊4穿过进入开口时,囊4沿细长管34撕裂或破裂的可能性。
根据前述内容并参照各个附图,本领域的技术人员能够认识到还可以在不偏离本公开的范围的情况下对本公开进行某些修改。例如,如上所述有益的是,为囊4提供能够闭合和打开的顶部26。在该实例中,标本取出装置10可以装配有配置为可操作地联接至囊4的拉线(未示出)。在本技术领域中包括拉线的标本取出装置是公知的。在1994年9月16日提交的托维(Tovey)等人共同拥有的第5,647,372号美国专利中,公开了一种可以与囊4结合使用的此类标本取出装置,该专利的全部内容通过引用合并于此。在该实例中,可以理解的是,为了使囊4起到本文所述的功能,可能需要对囊4进行某些修改。例如,囊4的开放顶部可能需要穿孔或开槽。

Claims (20)

1.一种标本取出囊,包括:
闭合底部和开放上部,所述标本取出囊包括可操作地联接至内壁并且沿其长度方向延伸的细长管,所述细长管具有沿其长度方向排列的多个开口;
缝合线,其能够在所述标本取出囊的近端处被操作且置于所述细长管内,并且延伸经过所述细长管的远端,所述缝合线能够在所述细长管内相对于所述标本取出囊运动,所述缝合线的远端具有针或圈,所述针或圈用于连结至组织,并随后在拉动所述缝合线穿过所述细长管时,将组织拖到所述标本取出囊内;以及
至少一个加强突出物,其可操作地布置在所述标本取出囊的外壁上,配置所述至少一个加强突出物以供使用者抓取,以便在拉动所述缝合线穿过所述细长管时提供相反的拉力。
2.根据权利要求1所述的标本取出囊,其中,所述标本取出囊由接合在一起以形成所述内壁和外壁的至少两片塑料构成。
3.根据权利要求2所述的标本取出囊,其中,所述至少两片塑料通过焊接或粘合接合在一起。
4.根据权利要求1所述的标本取出囊,其中,至少所述内壁具有纹理。
5.根据权利要求1所述的标本取出囊,其中,所述标本取出囊的开放上部较宽,并朝向所述闭合底部变窄。
6.根据权利要求1所述的标本取出囊,其中,所述开口配置为在将组织拖到所述标本取出囊内时允许空气由此逸出,以便于将所述组织置于所述标本取出囊内。
7.根据权利要求1所述的标本取出囊,其中,所述开口沿所述细长管的长度方向径向地排列,以形成大体螺旋的配置。

10.根据权利要求1所述的标本取出囊,其中,所述至少一个加强突出物包括至少一个限定为贯穿的开口,以便于供使用者抓取。
11.根据权利要求10所述的标本取出囊,其中,所述至少一个加强突出物沿所述标本取出囊的长度方向延伸,并限定多个贯穿的开口。
12.一种标本取出装置,包括:
施放器;
标本取出囊,其可操作地布置在所述施放器内并从能够从所述施放器展开,所述标本取出囊限定有穿过其中的纵轴线,并包括闭合底部和开放上部,所述标本取出囊包括沿其长度方向延伸的细长管,所述细长管具有沿其长度方向排列的多个开口;以及

14.根据权利要求11所述的标本取出装置,其中,所述开口配置为在将组织拖到所述标本取出囊内时允许空气由此逸出,以便于将所述组织置于所述标本取出囊内。
