背景技术
本发明主要涉及医疗植入物,更具体地涉及一种可植入的连接口(access port)装置和用于使这一连接口装置固定在组织上的固定系统。
对患者起治疗作用的医疗植入物是公知的。这些装置包括心房脉冲产生器、血管连接口、注射端口(例如与胃束带系统一起使用)和胃调步装置。所述植入物需要被固定在通常位于皮下的适当位置以正确地工作。最好植入所述装置的方法是迅速、简单并高效,并要求切口尽可能小。
发明内容
本发明涉及一种系统,所述系统包括一个可植入的连接口,例如但不限于,用于使可膨胀胃束带膨胀和缩小的可植入的连接口。通常,所述系统包括被配置成能例如通过流体导管连接至胃束带可膨胀部分的连接口。与胃束带一起使用的连接口是公知的,并在例如2004年9月15日提交的美国专利申请No.10/562,964;2005年1月21日提交的美国专利申请No.10/562954;2006年6月22日提交的美国专利申请11/472,902;2006年5月31日提交的美国专利申请No.11/444,702;以及,2006年9月29日提交的美国专利申请No.11/540,177中有所描述,这些专利申请的每一篇的全部公开内容都通过此具体引用的方式纳入本文中。
在本发明的一个方面,提供了一种用于使连接口固定于身体组织的系统。
所述系统主要包括连接口组件,所述连接口组件包括连接口和大体上包含所述连接口的连接口外壳。所述连接口可被构造以容纳、接收和允许流体在所述连接口组件和患者之间,或者进入患者体内另一植入装置例如胃束带中的流通。
例如,所述连接口包括底部、侧壁和针可穿透的隔膜。所述针可穿透的隔膜与所述底部相分隔,位于与所述底部基本平行的平面上。所述侧壁、底部和隔膜界定了一个容纳流体的空间。所述连接口组件具有一个延伸穿过所述底部、所述隔膜和所述容纳流体的空间的大体上在中心的轴。所述侧壁大体上环绕此轴并与该轴在径向上分隔。

具体实施方式
现在参见图1,图1示出了本发明系统的连接口组件10的简化透视图。示出的连接口组件10处于被可手动操作的注射器3的针头2穿透的状态下。通过将流体推入所述连接口组件10或通过所述连接口组件10抽出流体--如将在下文更详细描述的--所述针头2和注射器3提供了用于使常规胃束带4膨胀和/或缩小的方便手段,从而可调节小孔大小或对患者的胃5的限制水平。在图2中示出了缩小状态下的胃束带3,在图3中示出了膨胀状态下的胃束带3,但不认为该胃束带本身构成本发明的实施方案。
现在参见图4,图4示出了依照本发明一个实施方案的系统20。所述系统20主要包括可植入的连接口组件10和用于将所述连接口组件10固定于身体组织的器件30。所述连接口组件10被如下设置:例如通过流体管线6(见图1)连接至胃束带4的可膨胀部分。
现在参见图5,所述连接口组件10主要包括具有隔膜36的连接口34、腔37(示于图6A)和与所述腔37相连的进口/出口接头38。所述连接口34被构造成能容纳、接收和允许流体在进口/出口接头38和流体管线6之间的流通。
在所示出的实施方案中,所述连接口组件10包括便于与患者缝接的装置,这是在不希望使用所述器件30固定所述连接口组件10时。例如,提供了缝接孔40。还可提供针头间隙区41以便于缝接。
图6和6A详细示出了所述连接口组件10。所述连接口34包括连接口底部42、基本上为圆柱形的连接口侧壁44和针可穿透的隔膜36。所述连接口34还包括通道(passage)例如从腔37伸出的出口倒钩48,其构成可与流体管线6连接的进口/出口接头38的一部分。
进口/出口接头38可包括锁住外壳侧壁56的联结器51的应变释放元件50,并且所述应变释放元件50可保护流体管线6免于在其与所述连接口组件10连接的地方折叠、弯折、转动或扭转。进一步的应变释放可通过柔性套管51a提供。柔性套管51a可由耐穿刺材料制成,并且与应变释放元件50一起保护管线6免遭意外的针头穿刺。
所述隔膜36与所述连接口底部42相分隔,位于与所述底部基本平行的平面上。隔膜36可由任何合适的针可穿透的材料例如自密封的针可穿透材料制成。所述连接口侧壁44、连接口底部42和隔膜36界定了用于容纳流体的腔37或空间。所述连接口底部42和连接口侧壁44可为由生物相容性金属材料例如钛制成的基本上为一体结构的整体部件。出口倒钩48也可由相同的材料制成。
所述连接口组件10具有在图6中以线52示出的并且延伸穿过所述连接口底部42、所述隔膜36和所述腔37的大体上在中心的轴。所述连接口侧壁44大体上环绕所述大体上在中心的轴52并与该轴在径向上分隔。应理解,当所述连接口组件10被植入以用于患者时,所述大体上在中心的轴52大体上垂直于所述连接口组件所固着的组织或肌肉的表面。
所述连接口组件10还包括外壳54,所述外壳54包括基本上环绕所述连接口侧壁44的外壳侧壁56和致动器组件。致动器组件由致动器帽58和可相对所述外壳侧壁56转动的致动器元件62(如致动器帽上的箭头64和致动器元件62所示)构成。所述外壳54还包括锚底座66,所述锚底座66包括用于容纳致动器元件62的轨道68。
所述连接口组件10还包括固定机构70。所述固定机构70被构造为能将所述连接口组件10锚定或固定于所述患者。所述固定机构70可包括例如多个可在未展开位置和展开位置之间移动的可转动锚74。
在所示的实施方案中,所述多个可转动锚74包括4个锚74。所述锚74通常被间隔开,例如环绕所述连接口34的圆周基本上等距离间隔开。当处于所述未展开位置时,所述锚74基本上隐藏并收纳于所述致动器元件62与所述外壳侧壁56之间。在展开过程中,所述锚74转动并且通过滑过锚底座66中的孔75从它们被收纳的、基本上隐藏的位置移出到外露的位置。
每个锚74均可由金属丝例如不锈钢丝制成。所述锚74可包括具有大体上为圆形的横截面和尖锐远端尖端76的弯曲金属丝。
所述锚尖端76被构造成可在所述锚74转动进入所述展开位置时穿透并进入身体组织。在某些实施方案中,所述锚尖端76包括一个或多个平面。例如,所述尖端76可具有一个平面,或可具有多个平面。例如,所述尖端76可具有2个或3个或更多个平面。
在一个具体实施方案中,所述锚74是弯曲的不锈钢丝,所述不锈钢丝具有一个弧直径略小于约0.5英寸的大体上的弧形和一个直径约0.023英寸的恒定圆周横截面。
每个锚74均包括啮合于组织的弯曲远端部分80和可旋转地连接于所述口外壳的锚底座66的枢轴近端部分82。在所示实施方案中,所述枢轴近端部分82基本上垂直于所述弯曲或螺旋形远端部分80,或更具体地,基本上垂直于当所述锚74转动到所述展开位置时所述弯曲远端部分移动的平面。
简单地参照图4,所述连接口组件10还可包括可移除的安全帽83以保护医生或医务人员的手和手指免于被锚意外刺伤。所述安全帽83通过按压配合(press-on fit)安装在所述连接口外壳54的底部。所述安全帽83的颜色可容易地与所述口外壳的颜色区分开。
现在回来参见图6,所述连接口组件10可包括一个或多个定位元件84,例如至少一个或两个或更多个在x射线下清晰可见的不透射线标志物86。这些可被固定于所述口外壳54并与所述连接口34分隔开从而不会使所述连接口34的图像盖住所述标志物的图像。在一个具体实施方案中,提供了两个标志物86,每个标志物均具有约.075″(直径)X.200″(长度)的尺寸,与所述连接口34的分隔距离为至少约0.100″英寸。所述标志物可用于例如通过使用标志物86的不同结构来借助所述连接口组件10的X射线图像识别的胃束带的类型或其他有用信息。
如图7所示,所述器件30包括具有大体上为纵向的轴(由线92示出)的柄90和远端部分96,所述远端部分96被构造为可拆除的并与所述连接口组件10功能性地连接。
在所示实施方案中,将所述器件30装配成可使得:当所述器件30与所述连接口组件10连接时,所述柄90的大体上为纵向的轴与所述连接口34大体上在中心的轴52分隔开或不排成一直线。换言之,啮合所述连接口组件10的器件30的传送头96与所述器件的柄90--即所述器件30被其操作者抓握的部分--偏离。在某些实施方案中,所述连接口34的大体上在中心的轴与所述柄的纵向轴偏离一段至少约1英寸到约2英寸或更多的距离。
例如,所述器件30具有大体上为弧形、勺形、L形或类似的“偏离”构造,从而使得所述传送头96位于所述柄90的前方或背离所述柄90延伸。此构造使得所述器件30可用于使用相对较小的切口植入所述连接口组件10,所述相对较小是例如相对于常规置放器(applier)或构造基本上为直线的器件,或者要求器件在手术植入过程中与相似大小的连接口的中心轴基本上完全排成一直线的器件而言。在所述连接口组件10的植入过程中,医生将所述传送头96插入稍微偏离植入位点--即固定所述连接口的目标位置--的切口中。
现在再参见图8A和8B,所述器件30包括致动机构98。致动机构98使得可例如通过由医生使用系统10而使所述固定机构70自动展开,以将所述连接口组件10固定于患者。本文其他部分将更详细地描述致动机构98。
图9示出了所述器件30的传送头96的侧视图。传送头96包括顶部100和侧壁101。当器件30与连接口组件10连接时,顶部100在连接口34的至少一部分和致动器帽58上方延伸(见图5),并且侧壁101环绕并夹住外壳侧壁56的至少一部分。
现在回来参见图8A和8B,在此示例性实施方案中,所述致动机构98包括连接于触发器机构104的线缆机构102。
所述线缆机构102包括两根由钨或类似材料制成的可反向移动线缆106。所述线缆106从所述触发器机构104沿着所述器件柄90延伸至所述器件30的传送头96。触发器机构104包括手动可压缩触发器104a和触发器释放按钮104b。
通常,每条线缆106均包括沿所述器件30的柄延伸的基本上直的近端部分和连接于所述传送头96的转动元件108的弯曲远端部分(见图5)。提供合适的结构例如缆线锚107将线缆106固定在适当位置。线缆106可在轮132转动时互相以相反方向移动。
为展开钩环(staple)74,操作者按下图8A中示出的触发器104a。触发器104的压缩导致弹簧130的压缩、轮132的转动和线缆106的纵向移动。触发器碰锁134例如通过弹簧(未示出)相对于轮132偏离。一旦触发器104a如图8B所示被完全压缩,触发器碰锁134就会在锁销凹口132a处(在图8A中可更清楚地看到锁销凹口132a)与轮132啮合并锁住触发器机构104。当触发器104a被完全压缩时,触发器释放按钮104b如图8B所示弹出。为使锚74缩回,操作者按压触发器释放按钮104b,这使得触发器碰锁134与锁销凹口132a分开,并且弹簧130上的负荷导致所述轮132反向转动。
再简单地参照图5和6,通过手动按压触发器104a致动的线缆106的纵向移动导致转动元件108的转动和致动器帽58的反向转动。致动器帽58的转动导致致动器元件62的反向转动和锚74的展开。
致动器元件62的转动导致每个锚74均发生转动。
图11、12和13分别示出了锚74从收缩的且未展开的位置,通过转动中的展开中的位置到达致动位置。
现在参见图5和10,通过将连接口组件10插入远端部分30的侧壁101之间而使器件30的远端部分连接于连接口组件10。当器件30啮合于连接口组件10时,转动元件108的突出部112固定在致动器帽58的接收口114中,并且远端部分侧壁101的夹116啮合于外壳侧壁56的切口118。

实施例
所述系统的应用
以下实施例描述了使用本文示出和描述的本发明的系统20将所述连接口组件10连接于胃束带患者的一种方式。
大体上参考所述附图,医生将所述应变释放元件50连接在软管6上,使所述管伸出应变释放元件50的锁定末端约2cm。
然后将所述管连接于倒钩48,直至与口外壳54的外壳侧壁56齐平。然后将应变释放元件50推进并锁在联结器51上。
医生检查所述触发器机构104处于图7中所示的完全打开的位置。
医生通过将所述连接口组件10置于与所述安全帽83接触的平台(图4)上,并在沿着图7的轴52的方向上相对连接口组件10按压传送头96而将所述连接口组件10插入所述器件30的传送头96中。这导致所述连接口组件10扣住传送头96。然后将所述安全帽83从所述连接口组件10中手动移除。在此点上,所述锚74的位置如图11所示。
医生将管从所述胃束带置于腹中。将所述连接口组件10置于套针开口的侧面并为所述连接口组件10制造一个口袋,从而使所述连接口组件离所述套针通路足够远以避免所述管的突然扭结。所述管路的放置情况为:所述管以轻度弧形过渡放入腹中时所述管会形成直线。所述管垂直于患者身体中线放置。
医生确认脂肪已被完全清除并且直肌筋膜可见。将连接于连接口组件10的所述器件30的传送头96以一个角度放入一个切口的袋中以便于插入。将所述连接口组件10相对于所述筋膜放平以确保全部锚74会完全啮合于所述筋膜和/或肌肉组织。医生对传送头96的顶部100施加指压以确保所述连接口组件10相对于所述筋膜放平并且所述器件30对于触发稳定。
医生紧压所述触发器机构104直至其完全关闭从而使所述锚74展开进入下面的筋膜中。在此点上,所述致动机构98以如图8B中所示的关闭位置锁定并且所述锚74如图13、15和17中所示完全展开。
为使所述器件30与所述连接口组件10分开,医生例如水平或侧向滑动所述传送头96远离所述连接口组件10,并将所述器件30从缺口中取出。医生通过绕所述连接口组件底座移动手指而确保所述锚74完全啮合在筋膜中。
若要使所述连接口组件10与所述筋膜脱离并复位,则按压所述触发器释放按钮,这将所述碰锁机构从图8B中所示位置处解锁,从而释放所述触发器机构104。一旦所述触发器完全打开,所述不锈钢锚会完全收进所述连接口组件10中。然后可使用如上文所述的器件30使所述连接口组件10在不同的例如更理想的位置重新展开。

一个用于当所述传送头与所述连接口组件这样啮合时使所述固定机构展开的致动机构;
所述传送器件被装配成能使得:在所述传送头与所述连接口组件这样啮合时,所述柄的纵向轴与所述连接口的大体上在中心的轴分隔开。
2.权利要求1的系统,其中所述连接口底部与所述连接口侧壁构成整体。
3.权利要求2的系统,其中所述连接口侧壁和连接口底部为一体结构。
4.权利要求3的系统,其中所述连接口侧壁和连接口底部为钛。
5.权利要求2的系统,其中所述连接口组件还包括一个外壳,所述外壳包括围绕所述连接口侧壁的外壳侧壁;和一个可相对所述外壳侧壁转动的致动器组件。
6.权利要求1的系统,其中所述器件的致动机构包括一个线缆机构。
7.权利要求6的系统,其中所述线缆机构包括含钨材料的线缆。
8.权利要求6的系统,其中所述线缆机构包括沿所述柄沿伸到所述传送头的线缆。
9.权利要求6的系统,其中所述线缆机构的纵向移动导致所述固定机构在所述传送头啮合于所述连接口组件时发生转动。
10.权利要求8的系统,其中所述线缆包括基本上直的近端部分和弯曲的远端部分。
11.权利要求1的系统,其中所述固定机构包括至少一个金属丝锚。
12.权利要求11的系统,其中所述金属丝锚包含一条具有一个多面尖端的弯曲金属丝。
13.权利要求1的系统,其中所述固定机构包括至少一个锚,所述锚具有弯曲的远端部分和基本上垂直于所述弯曲远端部分的枢轴近端部分。
14.权利要求1的系统,其中所述固定机构包括至少一个锚,所述锚具有螺旋形远端部分和基本上垂直于所述螺旋形远端部分的直的近端部分。
15.一种用于将连接口组件固定于身体组织的传送器件,所述连接口组件包括可展开的紧固件和连接口以及大体上在中心的轴,所述器件包括:
一个具有大体上为纵向的轴的柄;
一个位于所述柄远端并被构造成能与具有可展开紧固件的连接口组件啮合的传送器件,和
一个用于当所述传送头与所述连接口组件这样啮合时使所述紧固件展开的致动机构;
所述传送器件被装配成能使得:在所述传送器件与所述连接口组件这样啮合时,所述柄的纵向轴与所述连接口的大体上在中心的轴分隔开。
16.权利要求15的器件,其中所述器件的致动机构包括一个线缆机构。
17.权利要求16的器件,其中所述线缆机构包括含钨材料的线缆。
18.权利要求16的器件,其中所述线缆机构包括沿所述柄延伸到所述传送头的线缆。
19.权利要求16的器件,其中所述线缆机构的纵向移动导致所述固定机构在所述传送头啮合于所述连接口组件时发生转动。
20.权利要求18的器件,其中所述线缆包括基本上直的近端部分和弯曲的远端部分。
