技术领域

背景技术
外科手术程序可提出许多挑战,从而可影响到病人的健康以及医务人员的健康。在某些外科手术程序中,例如心脏外科手术及肺切除术,接近器官通常通过胸骨正中切口术来获得,所述胸骨正中切口术是一种用例如锯或其它合适的切割仪器的装置切开胸骨的外科手术程序。在执行胸骨正中切口术之后,必须将胸骨重新接合。医务人员通常使用具有锋利切割点的不锈钢针来闭合胸骨,所述不锈钢针上附装有缝合线,其包括一定长度的相对无挠性的不锈钢丝或者硬不锈钢丝及挠性不锈钢绳的组合。所述丝(或绳与丝的组合)缝合线被手动牵引穿过胸骨的两侧以便使足够长的丝从胸骨的两侧伸出且胸骨下方不再有任何松弛的丝。在将丝从针中拆除之后,医务人员必须用双手或镊子手动地将一条丝的末端绕另一条丝缠绕,并以螺旋或盘旋方式将所述丝的两个末端绕彼此反复地扭在一起。然后医务人员将所述扭在一起的丝切割到需要的长度且使用外科手术工具将所述丝的锋利尖裂的扭在一起的末端埋入到重新接合的胸骨边缘之间的空间中,从而使所述丝的锋利尖裂的扭在一起的末端不会刺到病人皮肤下面。通常,将六到八条之间的金属丝缝合线放置在胸骨中以沿其长度闭合胸骨。

为实现这些及其它优点且根据本文中体现的本发明目的,本发明包含一种用于连接组织(例如骨组织)的设备及方法。根据本文所图解说明的各方面,提供一种可用于(例如)胸骨正中切口重新接合的连接组织的装置,其中包含推杆组合件及套筒。所述推杆组合件具有从驱动枢轴延伸的驱动臂及位于所述驱动臂远端处的驱动机构。所述驱动机构能够以可松开方式啮合具有尖端及钝端的缝合针并使其绕可旋转轴线旋转。所述套筒包含保护外罩及缝合针。所述套筒从套筒支架组合件的远端延伸且以可松开方式附装到所述套筒支架组合件。
根据本发明的进一步方面,缝合针的尖端优选地在缝合针绕旋转轴线的完整旋转之前及之后定位在保护外罩内。此外,如果需要,可在所述装置中提供一种可移动电子模块,所述可移动电子模块由致动器控制,所述致动器以机械方式啮合驱动枢轴以旋转所述驱动枢轴及驱动机构,由此使缝合针绕旋转轴线旋转。

本文中呈现的装置及方法可用于组织闭合。本发明尤其适用于包括骨组织(例如胸骨)闭合的程序;如图中所描绘,所揭示的组织闭合装置或缝合装置是需供电的机动设备。电源可以是电池或标准墙壁电力。本文中揭示的组织关闭装置可有利地防止针意外地刺到医务人员且可轻松容易地驱动针穿过骨组织,从而排除了手动强制针穿过骨头的需要。在胸骨正中切口闭合的环境中,所揭示的装置保护心脏及周围血管及结构在关闭及重复标准胸骨正中切口闭合技术期间不会遭到无意中的针刺。在进行此操作时,所揭示的实施例提供胸腔或其它胸骨内对丝或丝/绳组合更容易及更流畅的操纵,并以时间有效方式进行组织闭合,因而病人不会遭受到与过多次地打开组织段(例如胸骨)相关联的健康风险。

如图31A及图31B中最好地显示,推杆组合件71与套筒支架组合件72之间的O形环57形成分离屏障以确保装置50在操作期间保持已消毒状态。第一O形环57紧靠前端组合件来密封旋转轴线。第二O形环57更接近外罩而密封且紧靠鼻状凸边157来密封前端组合件。所述多个O形环57提供已消毒屏障来将所述装置密封在把手内部之外。或者,如图31C所示,可使用特氟纶密封57A以向流体及其它可能污染物的入口及出口提供更持久的屏障。所述屏障密封排除了使任何未经消毒的微生物、碎片、颗粒等从装置内部流出到已消毒的装置前端外部的手段。
图3B及图3C显示推杆组合件的展开图,所述推杆组合件包含位于尖端的棘爪95(如图3A中所显示),所述棘爪驻留于所述推杆组合件的驱动臂75中的狭槽中且通过枢转销99连接到驱动臂75。所述针(未显示)由棘爪95沿环形路径驱动。
图4显示本发明所揭示实施例的弯曲的组织闭合针110的实施例,其尤其适用于穿透骨材料。针110形成为界定开孔(或裂口)111、锋利的尖端112及相对的钝端113的环形开口环。相对于针110轴向对准的圆柱形镗孔114位于钝端113处。用于组织闭合的缝合材料的前引端插入到镗孔114中且通过机械压接来进行箍固。或者,用于装纳缝合材料的开口可呈“眼”的形式,其中缝合材料的前引端可被穿透以将其附装到针110。为使针110能够穿到所要求的深度,针110优选地具有在约250°与约330°之间的弓形范围。
针110进一步包含沿径向后端边缘(即,最接近套筒支架组合件的边缘)的两个对称的切口115a及115b。切口115b朝向针110的锋利尖端112而定位。切口115a朝向针110的钝端113而定位。切口115a与115b彼此相对地定位,每一切口具有垂直的(约90°)扇形体及与所述垂直扇形体成约60°角的有角扇形体。切口115a及115b由套筒支架组合件72中的驱动机构(棘爪)啮合,且使针110能够在受所述驱动机构致动时经历旋转运动,由此致使其穿透并前进通过跨越所述组织扇形体(例如切开的胸骨)的空间。切口116位于针110的径向外部边缘上(“外切口”),所述切口最接近离锋利尖端112更近的切口115b。外切口116与位于套筒支架组合件72中的防旋转杆啮合,由此可防止针110沿与前进方向相反的方向旋转或发生“针倒退”。针外切口116在操作期间的刚性啮合防止了针110在缝合过程期间偏离出序列。
针110被封装在套筒内,因此锋利尖端112并未暴露。所装载的此针位置称为“原始”位置。在原始位置处,针110完全含纳于套筒外罩内以排除对套筒或已装载装置的处理期间的针刺。针套筒中开孔的宽度与针110中裂口的宽度相当且与其对应,因而当针110处于原始位置时,针110不会显著地突出到开孔111中。这一对准致使针110整个地驻留在针套筒内,由此防止了在处理一次性针管以放置在套筒支架组合件上或在使用后处理且同时操作组织闭合装置50期间使锋利尖端112与使用者的手指无意接触。对组织闭合装置50中的针110的此保护防止了意外“针刺”的发生,由此大致降低了由可在丢弃之前使用期间或之后可污染针110的病原菌或病毒导致的感染的风险。可绕缝合装置50的纵向轴线沿针套筒的弯曲轨道旋转针110以使尖针端112前进,从而使针110首先跨越开孔111且然后返回到原始位置。将缝合材料附着到针110,且因此所述缝合材料遵循针110的路径。然后可切割缝合材料并将其扭曲打结并通过适当的方法加以固定。不论是单个间断针还是一连串连续不间断针中的一者,每线圈均可以相同方式放置。因此,组织闭合或缝合装置50可用于插入单个针或插入包括多个连续针的缝合,以作为一种代替更单调及耗时的手动缝合过程的方法。
图5A提供推杆组合件71相对于缝合针110的详图。驱动臂75尖端处的棘爪能够装配到沿针110径向内部边缘而定位的楔形切口。驱动臂75能够在跨越约190°的弧中来回扫动。所述驱动臂旋转大于180°,且所述驱动过度是对针110及移动零件的任何不规则设计及公差的原因,且确保棘爪将一直拾取针切口116。
操作期间针110的前进移动致使沿针110的径向内部边缘的三角形狭槽与驱动臂75中的楔形棘爪对准,由此致使所述棘爪由于施加在销99上的正压力而啮合所述狭槽,且“锁定”到所述狭槽中。因此可控制针110的旋转前进移动以使其在每次致动组织闭合装置50时均经过约190°地依序发生。对每一缝合针来说,针110均旋转过约360°的弧,而驱动臂75每针总共约760°地来回旋转。
图5B及图5C为组织闭合针及推杆组合件的剖面图。如图5B所显示,片弹簧96向棘爪95施加张力以将棘爪95远端推到针110中及切口115b中。当旋转棘爪95时,棘爪95拾取所述切口115b并驱动针110。
图5C显示不啮合在切口115b中且啮合针110的表面的棘爪95。随着棘爪95沿针110的外表面移动,棘爪95遇到切口115b。棘爪尖端啮合切口115b的斜面且片弹簧96将棘爪95推到针切口115b中。
图5C显示棘爪挡块98,其准许棘爪尖端仅将一定量延伸到针110中,从而不会约束针110且致使针110停止。棘爪95具有远离棘爪挡块98成一角度的有角表面。当棘爪95顺时针方向旋转时,棘爪95可仅在有角表面接触到棘爪挡块98时转变方向。接触棘爪挡块98的棘爪95显示于图5B中。棘爪挡块98防止棘爪95进一步旋转且防止棘爪95将针110约束在套筒90中。

由医用级不锈钢或类似材料组成的弓形缝合针110以可滑动方式定位于轨道92中。界定弓形针110的弧的半径近似等于针套筒90在其中的开孔97处的周长。针110通常驻留在轨道92中的“原始”位置,以使得弓形针110中的裂口与套筒90中的开孔97对准。针110的锋利尖端位于外罩开孔97的一侧且整个地位于外罩开孔97的界限之内,因此针110的尖端由套筒外罩91所遮蔽。附着到缝合材料的针110的钝端位于开孔97的相对侧。因此,针110的锋利尖端整个地含纳于套筒90内且不会伸出套筒90的外罩之外。因此,针110的锋利尖端不会暴露给使用者。
图8A显示防旋转及锁定杆100的近视图。如图8B中所显示,防旋转及锁定杆100包含与针套筒90发生接触并将其锁定到适当位置的套筒锁定表面101及接触针110的防旋转表面102。卷簧104啮合套筒支架组合件72且允许防旋转及锁定杆100上下移动以将针套筒90锁定在套筒支架组合件72上的适当位置或解除锁定。
图14A及14B显示套筒支架组合件72的远端81的放大图。在图14A中,针套筒90处于未锁定状态。为锁定针套筒90,需要将针套筒90逆时针方向旋转。为将针套筒90从套筒支架组合件72的远端81解除锁定,可向下按压防旋转及锁定杆100以啮合所述卷簧,且这样可允许防旋转及锁定杆100在枢转销103处枢转。
图9显示替代套筒锁定机构。锁定销200以可滑动方式设置在套筒支架组合件72内。连接到锁定销200的把手及按钮203允许操作者在近处沿狭槽204将销200从伸出套筒支架组合件72的远端边缘87的位置滑动到完全缩回的位置(未显示)。如图11的剖面图中所显示,由于位于锁定销200的近端上的弹簧201,锁定销200发生偏斜以从套筒支架组合件72的远端边缘87伸出。当针套筒90固定到套筒支架组合件72由此使凹槽94与狭槽82完全啮合时,套筒90的配合表面上的锁定销凹处202与销200对准,从而允许销200伸出并啮合凹处202,由此将针套筒90以可旋转方式锁定到套筒支架组合件72上。形成的锁定销凹处202略大一些,但具有与锁定销200类似的剖面形状。在一个实施例中,锁定销200的剖面形状及锁定销凹处202的形状是环形的。为将套筒从套筒支架组合件松开,使用者可啮合附装到销200的把手与按钮并在近端滑动销200以使其从锁定销凹处202缩回。在缩回锁定销200时,使用者可旋转针套筒90以将凹槽94从狭槽82脱离,且由此将套筒从套筒支架组合件72移出。锁定销凹处202在图12中更清楚地可见,图12为针套筒90的配合或近端表面的一部分的透视图。
防旋转及锁定杆100啮合针110的外切口以防止针110旋转且防止发生“针倒退”,且由此可排除针110偏离出序列。图14B显示同时以可旋转方式且轴向地锁定到适当位置的针套筒90。
当用锁定销200实现套筒锁定功能时可简化防旋转及锁定杆100的构造。可排除通向防旋转及锁定杆100的锁定表面101的臂,从而形成单纯的防旋转杆300,如图13所示。防旋转杆300可包括与针110发生接触的防旋转表面302。防旋转杆300因弹簧304而发生偏斜以将防旋转表面302与针110的外切口116啮合来防止针110的反向旋转。
如图15所示,套筒90可装备有缝合夹持器400以保持缝合材料138不受针110在缝合装置50操作期间的旋转干扰。缝合夹持器400可协助保持缝合材料138远离套筒90的针轨道92。图16中显示针夹持器400的前视图。凸出401协助操作者将缝合材料138引入套筒90的针夹持器凹处402中。如图17所示,针夹持器400的凸爪403致使通向针夹持器凹处402的通道略小于缝合材料138的直径,从而允许操作员将缝合材料138按压装配到针夹持器凹处402中。这在图18中也有所显示,图18图解说明凸爪403如何为缝合材料提供出入针夹持器凹处402的略窄的通道。此特征防止缝合材料138无意地滑出针夹持器400。
针套筒90还可装备有针支撑器500,如图19所示。紧靠套筒90定位针支撑器500可防止在将套筒90装载到套筒支架组合件72上之前处理套筒90及缝合材料138期间针110在套筒90内的无意移动。这可防止针110偏离到轨道内使其偏出用于起始缝合装置50的第一个致动循环的适当序列的位置。针支撑器500具有接触针套筒90的外(远端)表面的凸缘501以将所述针支撑器固定在适当位置,如图20所示。针支撑器500具有本体502,所述本体的外径尺寸符合套筒90的内径尺寸,从而允许针支撑器500的轴肩特征503位于靠近套筒90的内(近端)表面处且与处于套筒90内的针110的衬套或钝端相邻,如图21所示。因此可防止针110沿相反方向在其轨道内旋转。类似地,可防止针110的尖端因针支撑器500的本体502的垂直段504的存在而在其轨道内向前移动,所述本体极为接近地符合套筒90的内径尺寸且阻止针110在其轨道中向前移动。当针支撑器500在适当位置时,个人可操纵套筒/针/缝合组合件而无需担心会无意中在套筒90的轨道内移动针。这一移动可能会在针被放置在套筒支架组合件72上之后将所述针定位在用于激活的适当序列之外。针支撑器500可由合适的刚性医用级一次性金属或塑料材料形成。
可通过摩擦“按压”装配或通过沿针支撑器500的外围放置小片来将针支撑器500固定在套筒90的内圆周内,其可与沿套筒90的内圆周的对应下压(未显示)协作。在对针支撑器500的进一步改进中,可构造垂直段504以使其形成图22中所示的小片504A。小片504A可由具有充分的弹性特性且可某种程度地弯曲的材料构造而成。例如,前文提到的Lubricomp塑料材料即具备此特征。当针110的尖端定位在套筒90的轨道的末端时,安装针支撑器500可致使小片504A略微弯曲并搭扣到针112的尖端上方的位置中,由此将针支撑器500锁定在适当位置,如图23所示。使用者可通过施加足以克服小片504a的弹性阻力的力来拆除针支撑器500。
图24为可拆卸电子模块60的分解透视图,所述可拆卸电子模块是一种电池操作的电机组合件。电子模块60包含电池组62及包含齿轮箱59及输出轴58的电动机61以及电子电路。电池62优选地为可再充电的且为电子模块60供电。电池62应最少具有约400次放电循环的电池寿命且能够穿过例如胸骨等组织段执行18次缝合(或总共36次循环)。发动机61提供组织闭合装置50发挥其功能所必要的旋转力。电子板基于使用者控制的电源及致动按钮来控制电子模块60的操作。提供指示器来向使用者提供电子模块60的当前状态的反馈。微处理器及固件控制电动机61的起动、停止及扭矩。所述电子装置还包含电池组62的再充电电路。可未经消毒地提供电子模块60且可通过防腐技术在外科手术前将其附装到前端组合件。可在用于清洁、电池充电及储存的程序之后将电子模块60从所述前端组合件拆除。电子模块60的额外视图显示于图25(A)到25(H)中。
图26A及26B显示组织闭合装置50,其中可拆卸电子模块60已被封装在热塑封闭物150及热塑外罩151中。由夹闩153锁定的门152提供可拆卸电子模块的入口及出口。所述装置的把手160的门152在近端处,可拆卸电子模块60从此处插入。当安装未经消毒的电池组时,需要关闭及密封门152。使用O形环或特氟纶类似于装置的前端来密封门152以防止任何未经消毒的细菌从装置内部流出到已消毒的操作区域中。
为便于将未经消毒的电子模块60放置到已消毒的把手160中,可提供漏斗状物600,所述漏斗状物临时地放置到把手160的近端的开口内,如图27(A-F)所示。漏斗状物600操作为处理未经消毒的电子模块60的人(例如,巡回护士)与处理缝合装置50的已消毒组件的人(例如,手术洗手护士)之间的物理屏障。所述已消毒组件包含:把手160、门152、闩153、鼻状凸边157、轴扇形体74、套筒支架组合件72及套筒90。当插入到把手160中时,漏斗状物600起初是已消毒的。如图28(A-F)所示,漏斗状物600具有近端柱体601及远端柱体602,每一柱体剖面的形状均符合把手160及对应的电子模块60的剖面形状。漏斗状物的近端柱体601及远端柱体602由凸缘603隔开,凸缘603足够宽以在插入电子模块60的人的手与缝合装置50的其余部分之间形成一个屏障。可在所述凸缘的表面上印刷或雕刻箭头604来指导使用者沿适当的垂直定向安装漏斗状物600。凸缘603上的箭头604及其它合适的印刷605可协助使用者区分漏斗状物600的近端柱体601与远端柱体602。远端柱体602可装配到把手160内,如图27B及27E所示。电子模块60可穿过漏斗状物600的空腔,如图27C及27F所示。一旦电子模块已穿过漏斗状物600且已被插入到把手160中,漏斗状物600便不再被视为已消毒且可被拆除。然后可以已消毒方式关闭门152并用闩153将其固定,同时将电子模块60的近端固定在适当位置。缝合装置50的外表面保持已消毒,通过使用漏斗状物600即可便于形成此状态。在替代实施例中,漏斗状物600可具有已消毒帘(优选地为一次性的),所述帘附装到凸缘603的外围以延伸将已消毒组件及人员与未经消毒的组件及人员隔开的屏障。将所述帘附装到凸缘603的模式可包含(例如)可防止在加热、潮湿及消毒气体中分解的高强度粘合剂。
如图26A到26C中所示,硅树脂键盘可囊封到封闭物150中且提供两个按钮,一个用作电源154,且一个用于致动155。图29中显示具有不同的键盘按钮布置的替代电子模块60,其中开/关按钮154A与致动按钮155A由LED指示器156隔开。电子键盘154A与155A及LED指示器156是构成电池/发动机/齿轮箱单元的一部分,且形成电子模块60,如图29所示。开/关按钮154使得电子模块被打开或关闭。可下压致动按钮155以使电子模块运行一次循环。一次循环由通过输出轴58的两次190°旋转来完成的针110的360°旋转组成。电子模块60通过安装到电子模块60的前部的挡板66与封闭物150介接。挡板66具有平坦面及用于啮合到封闭物150中的锥形体。此可提供连接以捕获施加于所述单元内的矩扭,因而操作者就不用必须抵抗缝合操作的扭矩(防旋转特征)。挡板66还将输出轴58的旋转行程限定为约190°。将驱动轮附装到电子模块60的输出轴。驱动轮含有与前端组合件70的驱动板啮合的驱动销。所述驱动销向前端组合件70提供扭矩以驱动针110穿过组织。所述驱动轮还含有两个永久磁铁。这些磁铁优选地与霍尔效应(Hall effect)传感器162结合使用以检测旋转行程范围。鼻状凸边157经由螺钉158或用于机械啮合的某些其它手段将前端组合件70附装到外罩151。
三个指示器(LED)156显示所述单元的状态:电源(绿)、错误(红)及电池电量低(黄)。外罩151及键盘提供能够支持医院清扫程序但不可在高压灭菌器中消毒的外表面。电子模块软件及操作特征致使在开/关按钮154起初被激活(下压)时针驱动臂自动移动到其原始位置(反向运动极限)。所述单元将在电源接通时被存储的情况下10分钟的未激活之后自行关断。在致动循环期间下压开/关按钮154将会被软件忽略。当电子模块打开时,“开”LED指示器将照亮且保持明亮直到关闭。在一个实施例中,通过下压致动按钮155触发致动循环以驱动针穿过组织将致使装置50在所述循环末端自动断电。一旦打开或关闭,电子模块即停留在所述状态直到刻意地切换到相反状态,除非存在错误模式,电子模块从未激活状态中止,或已发生致动循环。电子模块具有用以指示错误模式的错误LED(红色)。电子模块还具有指示需要充电的内部电池的电池电量低LED(黄色)。必须下压装置50的致动按钮155以驱动组织闭合针110。致动循环在下压致动按钮155时开始。致动按钮155不必一直保持被下压(即,当松开按钮时致动循环即完成)。致动按钮155仅需要一次下压来将针110旋转360°(针驱动臂将两次自动旋转190°以完成使针110完整旋转360°)。只要装置50已打开且尚未进入缝合序列,致动按钮155即是可操作的。必须松开致动按钮155,且在下压致动按钮155之前重新下压打开按钮154将再次开始下一缝合循环。与固件结合的电子霍尔效应(Hall effect)传感器162利用驱动轮中的永久磁铁感测约190度旋转的范围。一旦检测到运动结束,所述固件即修改施加到发动机的扭矩以形成软停止。操作期间可能发生的一些错误模式列于表1中。

图26C显示位于电子板161上用于检测驱动轮67的旋转位置的霍尔效应(Halleffect)传感器162。当旋转接近停止点时,磁铁65靠近霍尔效应(Hall effect)传感器162,向控制电子装置发送针110将很快停止的信号。此时,发动机扭矩降低。发动机继续驱动到降低的扭矩水平下的机械停止点。当驱动销63达到机械停止点时,控制电子装置感测因发动机不再转动及关断发动机的电源而导致的电流尖峰。将至少两个磁铁65定位在驱动轮67中以感测驱动轮67所行进的到达弧两端的停止点的途径。

图32A显示具有附着的组合闭合材料的弯曲缝合针110。针110形成为具有开孔(或裂口)111、锋利尖端112及相对的钝端113的环形开口环。相对于针110轴向对准的圆柱形镗孔114位于钝端113处。由组织闭合缝合材料制成的挠性引线136插入到钝端113中且可通过机械啮合(例如机械压接)来加以箝制。所属技术领域的技术人员将认识到,挠性引线136可通过任何类型的机械啮合来啮合针110,其包含但不限于:焊接、锡焊或激光焊接。例如,包括插入到针的镗孔114中的挠性引线136的组合件可固定在夹钳中,且接合点用YAG激光焊接或其它合适的激光焊接绕圆周焊接。可以类似方式焊接或激光焊接挠性引线与单丝缝合之间的接合点。以此方式焊接的缝合针组合件可在针与挠性引线之间及在挠性引线与单丝缝合之间提供比机械夹持更平滑的转移,从而便于使缝合材料穿过组织。因此可更容易地使缝合线穿过已切开胸骨的第一侧,例如,当操纵装置50以准备将缝合线放置在切开的胸骨的第二侧中时。为使针110穿透到所要求的深度,针110优选地具有介于约250°与约330°之间的精确范围。
对穿过骨头进行缝合来说,可优选地具有如图33A所示的三面(或“切割边缘”)针尖,每一面占据针轴的圆周的120°。如图33B所示,所述面中的一者可经定向以面向针的外曲率,在这种情况下,面向外的面可以比另外两个面更浅的角度被切割,优选地以相对于针轴的长轴大约成21°的角度。如图33C所示所形成的针尖切面将具有更短的切割高度,从而能够更有效地穿透骨头或其它密实组织。或者,当将缝合线放置在胸骨外边缘或横向边缘周围时,针尖可配置为“钝锥形尖”或“球形尖”。针轴的圆形尖及圆锥形前端组件准许较少地外伤性穿透较软组织。其它应用可要求使用具有较锋利尖端的“锥形尖”针或甚至“反向切割边缘”针。
针110具有沿径向后边缘(即,最接近套筒支架组合件的边缘)的两个对称切口115a及115b。切口115b朝向针110的锋利尖端112而定位。切口115a朝向针110的钝端113而定位。切口115a与115b彼此相对定位,每一切口具有垂直(约90°)扇形体及与所述垂直扇形体成约60°角的角扇形体。切口115a及115b由套筒支架组合件72中的驱动机构(棘爪)啮合,且使针110能够在致动所述驱动机构时经历旋转移动,从而致使针110穿透并前进通过跨越切开的胸骨或其它组织位置的空间。切口116位于针110的径向外边缘上(“外切口”),所述切口最接近离锋利尖端112更近的切口115b。外切口116与位于套筒支架组合件72中的防旋转及锁定杆啮合,由此可防止针110沿与前进方向相反的方向旋转或发生“针倒退”。针外切口116在操作期间的刚性啮合防止了针110在缝合过程期间偏离出序列。另外,针110的钝端或衬套113还可啮合防旋转杆100(或在装置具有单独的锁定杆的情况下为300),如图39C中可见,以防止针110在横穿针套筒90中的开孔后转变方向。
针110被封装在套筒内,因此锋利尖端112不会暴露出来。所装载的此针位置称为“原始”位置。在原始位置中,针110完全含纳于套筒外罩内以排除处理套筒或所装载装置期间发生的针刺。针套筒中开孔的宽度与针110中裂口的宽度相当且与其对应,因而当针110处于原始位置时,针110不会显著地突出到开孔111中。这一对准致使针110整个地驻留在针套筒内,由此防止了在处理一次性针管以放置在套筒支架组合件上或在使用后处理且同时操作组织闭合装置50期间使锋利尖端112与使用者的手指无意接触。对缝合装置50中的针110的此保护可防止意外的“针刺”发生,由此大致降低了由可在丢弃之前使用期间或之后可污染针110的病原菌或病毒导致的感染的风险。可绕缝合装置50的纵向轴线在针套筒的弯曲轨道中旋转针110以使针尖端112前进,从而使针110首先跨越开孔111且然后返回到原始位置。

在一实施例中,缝合装置50的把手不含有电池,但其由从手术室(OR)消毒区域外部提供的电力来供电。在此外部电力实施例中,所述装置配置有把手中的耐高压加热发动机及附装到所述把手的可消毒电源线。所述装置可通过所述可消毒软线连接到位于手术室消毒区域外的电力供应单元。所述电力单元可插入到墙壁电力中,且能够将所述壁电力(即,120伏或240伏)转换为适于发动机的电压水平。可消毒软线将电力传输到含有发动机及针驱动机构的手持单元。另外,控制电子装置可位于外部电力单元中而不是把手内部。配置上的此变化将降低把手的大小。
本发明所揭示实施例的缝合装置50可用于需要骨组织或密实软组织闭合的任何程序。所选择的组织应具有充分的顺应性以准许由可用于所述装置的各种针尖中的一种来穿透。其它可能的使用可包含(例如)对伤口或切口的修复,所述伤口或切口包括肩袖附着点、四头肌腱、髌腱或跟腱,及侧向胸廓切开术后的肋骨重新接合。在这些情况中的某一些中,可有利地使用附装到与装置50的套筒90、套筒支架组合件72及推杆组合件71相容的针的非金属缝合材料。所述非金属缝合材料可包含(例如):编织或单丝尼龙、聚丙烯纺织纤维或涤纶织物、例如丝绸或肠线的天然材料及例如聚乙醇酸、羟基乳酸聚合物、聚葡萄糖酸酯或聚二乙恶嗪二酮的合成可吸收材料。
使用胸骨正中切口程序来作为实例,在所述程序之前,装置50的把手在医院高压灭菌器中被消毒且以与任何其它一种消毒手术设备相同的方式递送到手术室。已使用充电器对电子模块60的电池组62充电且将电池组带到手术室的非消毒区。作为例行建立手术室中的手术区域的一部分,手术洗手护士将打开含有装置50的已消毒把手的已消毒封装。然后所述洗手护士打开所述把手后面的口并将使用标准惯例防腐技术来向位于已消毒手术区域外的巡回护士提供把手的开口。此时,所述洗手护士可通过凸缘603抓住已消毒漏斗状物600并将其插入到装置50的把手160中。巡回护士将取已充满电的电子模块60并使用标准防腐技术将模块60安装到把手160的后开口中。然后巡回护士可抓住漏斗状物600的凸缘603并将其从把手160中小心地收回以免与装置50的任何其它组件接触。为协助此程序,巡回护士可通过使用已消毒镊子或夹钳抓住凸缘603来收回漏斗状物600。洗手护士将关闭口,从而将电子模块60固定及密封在把手中。可重新使用的装置50现在可准备用于装载一次性缝合套筒90。
在需要闭合胸骨的时候,洗手护士将打开将用于此情况的已消毒的一次性缝合套筒90(典型封装将为6包)。一个缝合套筒90装载到装置50的套筒支架组合件72上。检验装置50以确保电源灯154已关闭,LED灯156中的每一者都不应是亮的。针套筒90上的10点及2点狭槽与套筒支架组合件72上的10点及2点小片对准。针套筒90与套筒支架组合件72对准,被推到套筒支架组合件72上,且然后逆时针方向旋转以将针套筒90锁定在适当位置。锁定销200开始与套筒90的锁定销凹处202对准,在此点处锁定销200可啮合锁定销凹处202。针支撑器500(如果存在)可在固定到套筒支架组合件72之后从套筒90拆除。然后打开电源接通按钮154。绿色LED将打开,从而指示装置50已作好准备。然后按压致动按钮155以在将装置50放置在外科医生手中之前使针110循环一次。装置50应循环两次,从而致使针110旋转一周。装置50现在已准备用于放置第一缝合线。洗手护士将装置50递给将执行缝合放置过程的外科医生。依外科医生的判断从胸骨的顶部到底部来放置缝合线。通常使用约六到八次缝合来闭合所述胸骨。作为增强其操作安全的额外措施,装置50可经编程以使每一致动循环要求操作者在按压致动按钮155之前按压电源接通按钮154。
为开始胸骨正中切口闭合,将胸骨正中切口闭合装置50的任何实施例放置在胸骨正中切口术的位置处以使装置50位于切开的胸骨的第一侧与第二侧之间,通常开始点在如图40所示的胸骨的胸骨柄170(或头部)周围。外科医生将用左手抬高切开的胸骨的一侧。外科医生将针套筒90的开口(开孔)放置并对准在胸骨的切割边缘上方,从而将其所需的缝合穿透点定位在套筒开口顶部下方。外科医生还可使用套筒支架组合件72的下部边缘在缝合期间抬高切开的胸骨,从而消除将其手放置在胸骨下方的需要。针将从病人外部(体外)进入到胸骨。外科医生在定位装置50之前按压接通按钮154,且然后按压致动按钮155且同时将装置50保持在期望的位置。当按压按钮155时发生以下动作:电子控制器起动发动机且齿轮箱顺时针方向旋转。此运动由以下组件链从电子模块内部转移到驱动轴:齿轮箱轴到驱动轮到驱动板到驱动轴到驱动臂。驱动臂持有棘爪。棘爪啮合针的后驱动切口。随着驱动臂旋转经过环形弧的第一半,棘爪推动针通过一次旋转的第一半。这是穿透胸骨的行程。当驱动臂到达行程的终点时,驱动臂停止。然后驱动臂将转变方向并返回到开始位置。一旦驱动臂达到所述开始位置,驱动臂即再次停止。驱动臂现在开始再次顺时针方向旋转。棘爪拾取前端针切口并牵引针通过一个完整的行程。当驱动臂到达此行程终点时,驱动臂停止。然后驱动臂将转变方向并返回到开始位置。一旦驱动臂到达开始位置,其即再次停止,且装置50自动断电。针现在已被完全驱动通过胸骨的一侧。挠性引线已被牵引到针所形成的孔中并穿过所述孔。外科医生现在将顺时针方向旋转装置50把手,从而牵引所述导杆穿过胸骨的一侧。外科医生的左手可用于协助牵引挠性引线穿过胸骨。外科医生现在将抬高胸骨的另一侧。外科医生将针套筒90的开口放置并对准在胸骨的上方,从而将其所需的缝合穿透点定位在针套筒开口底部下方。在外科医生按压致动按钮155后按压接通按钮154之后,针将从病人内部(体内)进入胸骨。重复所述步骤。此时,挠性引线已穿过胸骨的此侧。外科医生可按压位于防旋转及锁定杆上的套筒锁定表面101并顺时针方向旋转针套筒90以将针套筒90从装置50拆除。
装置50可传递给洗手护士以装载第二针套筒90。外科医生现在将使用双手牵引挠性引线并遵循任何胸骨闭合缝合的标准惯例来穿过胸骨两侧进行缝合。使用标准惯例将针、挠性引线及卷边箍从单丝缝合拆除。外科医生现在继续进行第二次缝合。针对外科医生通常选择放置到(例如)剑状软骨过程172附近的区域的所有剩余缝合来继续所述过程。在所有期望的缝合均已进行后,即使用标准手术技术完成了胸骨缝合。装置50可准备用于清洗、消毒及充电。典型的程序可包含:清洁人员将用消毒剂来清扫装置50外部。后面的口将被打开且电子模块将被拆除。电子模块转到未经消毒的设备存储以在专用充电器上充电。装置把手将根据有效的清洗规程来清洗。装置把手将根据有效的消毒规程来消毒。
推杆组合件与套筒支架组合件72之间的特氟纶密封57A及O形环密封57提供消毒屏障,因而当推杆驱动针时,驱动轴将在特氟纶密封57A及O形环57内旋转,从而可防止任何物体从驱动轴下移并到达组织闭合装置50的把手内。
本文中提供一种用于胸骨重新接合的方法。所述方法包含:(a)将具有保护外罩及缝合针的套筒以可松开方式啮合到胸骨正中切口闭合装置的套筒支架组合件;(b)放置具有所述套筒及所述缝合针的所述胸骨正中切口闭合装置以致使套筒中的开孔位于切开的胸骨的第一侧与第二侧之间,其中缝合针的尖端在缝合针绕旋转轴线的完整旋转之前及之后定位于所述保护外罩内;(c)激活耦合到推杆的电子模块,所述推杆以可松开方式啮合缝合针以致使缝合针穿过套筒中的开孔进行旋转移动并使缝合针前进以通过所述切开的胸骨的第一侧;(d)牵引附着到缝合针的缝合材料以穿过所述切开的胸骨的所述第一侧;及(e)对切开的胸骨的第二侧重复步骤(c)与(d)以形成穿过切开的胸骨的第一侧及第二侧的线圈。重复步骤(b)到(e),直到已重新接合了切开的胸骨的第一侧及第二侧的一定长度。

Claims (43)

1、一种组织闭合装置,其包括:
推杆组合件,其具有从驱动轴延伸的驱动臂及位于所述驱动臂远端处的驱动机构,其中所述驱动机构能够以可松开方式啮合具有尖端及钝端的缝合针并绕旋转轴线旋转所述缝合针;
套筒,其具有保护外罩及所述缝合针,所述套筒从套筒支架组合件的远端延伸且以可松开方式附装到所述套筒支架组合件,其中所述缝合针的尖端在所述缝合针绕所述旋转轴线的完整旋转之前及之后定位于所述保护外罩内;及
可拆卸电子模块,其由致动器控制,所述致动器以机械方式啮合所述驱动轴以旋转所述驱动轴及所述驱动机构,因此绕所述旋转轴线旋转所述缝合针。
2、如权利要求1所述的组织闭合装置,其中所述缝合针是弓形的,其具有与所述推杆组合件的所述驱动机构的旋转半径同等的半径,所述驱动机构以可松开方式啮合所述缝合针以绕所述驱动轴的纵向轴线定向所述缝合针。

(e)对所述切开的组织段的所述第二侧重复步骤(c)与(d),从而形成穿过所述切开的组织段的所述第一侧及所述第二侧的线圈。
29、如权利要求28所述的方法,其中重复步骤(b)到(e),直到已重新接合所述切开的组织段的所述第一侧与所述第二侧的一长度。
30、如权利要求28所述的方法,其中所述组织段包含骨材料。
31、如权利要求28所述的方法,其中所述组织段为需要重新接合的切开的胸骨。
32、如权利要求28所述的方法,其中所述组织闭合装置包括:
推杆组合件,其具有从驱动轴延伸的驱动臂及位于所述驱动臂的远端处的驱动机构,其中所述驱动机构能够以可松开方式啮合具有尖端及钝端的缝合针并绕旋转轴线旋转所述缝合针;及
套筒支架组合件,具有所述保护外罩及所述缝合针的所述套筒以可松开方式附装到所述套筒支架组合件。
33、如权利要求28所述的方法,其中所述缝合针是弓形的,其具有与所述推杆组合件的所述驱动机构的旋转半径同等的半径,所述驱动机构以可松开方式啮合所述缝合针以绕所述驱动轴的所述纵向轴线定向所述缝合针。
34、如权利要求28所述的方法,其中所述套筒进一步包括:
弯曲唇缘,其覆盖大于约180°且小于约330°的弧;
至少一个凹槽,其用于啮合所述套筒支架组合件上的防旋转及锁定杆;
位于壁中的轨道,借此所述缝合针在针旋转期间遵循沿所述轨道的弯曲路径;及
位于所述弯曲唇缘中的开孔,其截断所述轨道,借此所述缝合针的所述尖端在所述缝合针沿所述路径的每次旋转期间均横穿所述开孔。
35、如权利要求28所述的方法,其中所述缝合针覆盖大于约180°且小于约330°的弧,且具有邻近所述钝端定位的用于啮合缝合材料的开孔。
36、如权利要求35所述的方法,其中所述缝合材料包含啮合到金属丝缝合线的挠性引线。
37、如权利要求28所述的方法,其中所述电子模块由可手动操作的扳柄、按钮或开关来激活。
38、如权利要求28所述的方法,其中所述电子模块由使用电力电源的自动装置来激活。
39、如权利要求28所述的方法,其中所述电子模块由使用电磁电源的自动装置来激活。
40、如权利要求28所述的方法,其中所述电子模块可从所述胸骨正中切口闭合装置拆除。
41、如权利要求28所述的方法,其中所述电子模块进一步包括:
电源,其用于向所述电子模块提供电力;
发动机及齿轮箱,其提供用以旋转所述驱动轴的旋转力及用以旋转所述缝合针的所述驱动机构;
电子板,其控制所述电子模块的操作;
挡板,其限制所述驱动轴及所述驱动机构的旋转;及
驱动轮,其具有:驱动销,其啮合所述推杆组合件上的驱动板并提供用以驱动所述缝合针的扭矩;及至少两个磁铁;以及用于检测所述驱动轴的旋转进程的霍尔效应传感器。
42、如权利要求41所述的方法,其中所述电源包括电池组。
43、如权利要求41所述的方法,其中所述电源包括通过可消毒缆线连接到把手的外部电力供应。
