技术领域
本发明通常涉及在牙科操作过程中或之后用以改善出血、液体渗出或漏液或者其他形式的液体丢失同时促进愈合的组合物、组件和方法。
背景技术
图1是健康牙齿的解剖视图(anatomic view)。牙齿包括牙冠和牙根。牙根被牙槽(颌)骨内的支持韧带包围。该韧带包括由减震的结缔组织组成的牢固带(tough band),其在物理上将牙根结合于颌骨。由骨内的牙齿占据的洞被称为牙槽。
牙冠暴露于牙龈之上。称为釉质的有光泽的坚硬外表面覆盖牙冠。釉质之下是牙本质,显微镜下其为有孔的坚硬组织。牙齿的中央是髓腔,其容纳由血管和神经组织构成的牙髓。
牙齿可被破坏或腐蚀,例如由于细菌而使牙釉质内的钙腐蚀。这又可以导致牙釉质下的牙本质的腐蚀。随着腐蚀的持续,细菌可通过有孔的牙本质迁移并感染牙髓。随后可发生针对该感染的免疫应答,引起该牙齿周围的血管扩张(enlarge)并压迫进入该牙齿的神经。结果就是牙痛。
在出现这些或者其他影响口腔及其解剖结构的病症时,可进行多种介入牙科操作。这些操作由普通的执业医师、牙科医生、口腔外科医生、颌面外科医师和牙周病医师(peridontistic)常规实施。
例如,在多种情形下,牙髓手术也称根管治疗术可介入以将细菌、神经组织、有机碎片和细菌毒素从腐蚀牙齿内部区域(aspect)内去除。随后,医师填充并封闭该牙齿的内部。目前,每年可实施约16,000,000次根管治疗术。
如果腐蚀发展得太厉害,则可能指示应该拔除该牙齿。目前,每年实施30,000,000次以上的拔牙手术。在简单的拔牙操作过程中,牙医用工具如钳子抓住该牙齿并前后晃动该牙齿。晃动通过破坏将该牙齿维系于恰当位置的韧带而将牙齿从牙槽骨松开。随后将该牙齿从牙槽中拔出,使得牙槽成为开放性的。
当牙齿的存在引起拥挤或咬合不正,或者阻碍另一颗牙(例如智齿)长出,或者正准备做正畸治疗(“支架”)时,也可能指示应该移动或拔除该牙齿。还可能由于进展性牙周(牙龈)疾病指示应该拔牙。有时候,如果选择拔除的牙齿不完全长出于牙龈之上,为 了到达该拔除的牙齿,可能有必要首先去除部分覆盖的牙龈和骨组织。
这种常规牙科操作例如牙髓手术或牙周手术、正畸治疗、拔牙、正颌手术、活检以及其他口腔手术操作的过程中和之后,常发生出血、液体渗出或漏液或其他形式的液体丢失。出血、液体渗出或漏液或其他形式的液体丢失还可由于口腔内组织和结构的损伤或创伤而发生在口腔内。在该方面,每年由于意外可发生约2,000,000颗掉牙。在操作或损伤后的愈合阶段,通常认为肿胀和残留出血会持续。在愈合期,新的牙龈组织将长入由于拔牙而留下的空隙中。
因此,采取措施以止血、封闭、和/或稳定外科手术的位置-或者组织损伤或创伤的位置-而抵抗由于出血、液体渗出或漏液引起的液体丢失,是愈合过程中所需要的。在牙科操作或口腔损伤过程中和之后,需要迅速有效的止血。
例如,拔牙后,非常需要迅速停止出血并在开放牙槽内的伤口上形成凝血块。的确,在拔牙后的整个愈合过程(可能需要一到两周)中,保持有利于止血的条件比较重要,从而使得在牙槽内形成的凝血块不会破坏和/或移动。如果血凝块破坏和/或移动,则形成称为干槽症(也称为牙槽骨炎)的病症。干槽症还可在治疗颌骨囊性空洞缺陷(cystic cavity defect)的过程中由于相同的原因而发生。干槽症可引起疼痛或不适,其仅在该牙槽通过二期愈合过程愈合时才消退。
传统上,通常采用棉花填充物或者成卷或折叠的纱布垫中止牙科操作过程中和之后发生的出血。尽管这些材料的存在可吸收血液和液体,其并不促进或创造有利于迅速而长期止血或愈合的条件。现在仍然需要改善的可在牙科操作过程中或之后应用的止血组合物、组件和方法。
发明内容
本发明提供用于治疗口腔内组织或骨骼或者邻近解剖结构的组件、系统和方法,包括置入亲水性聚合物海绵状结构。
本发明一方面提供一种亲水性聚合物海绵状结构,其成形、确定尺寸并设置成使置入后与口腔内的组织或骨或者邻近的解剖结构相配合,并提供一种用于将亲水性聚合物海绵状结构置入而使其与口腔内的组织或骨骼或者邻近的解剖结构相配合的方法。

本发明另一方面提供用于治疗口腔内组织或牙槽(颌)骨由于意外引起的组织或骨骼的损伤或创伤的系统和方法。根据本发明的该方面,该系统和方法置入亲水性聚合物海绵状结构使与被治疗的组织或骨骼相配合。
利用亲水性聚合物海绵状结构的组件、系统和方法止血、封闭或稳定组织或骨损伤、组织或骨创伤、或者组织或骨手术部位。利用亲水性聚合物海绵状结构还可以构成抗微生物或抗病毒屏障、和/或促进凝血、和/或释放治疗剂(agent)、和/或治疗牙周或骨表面、和/或其组合。
根据本发明的所有方面,理想地,亲水性聚合物海绵状结构包括壳聚糖生物材料,其在使用前已经通过压缩而致密化到0.6g/cm3 至0.1g/cm3之间的密度。

具体实施方式

I.牙垫组件
图2示出了拔牙之后成为开放性的牙槽。图3A和3B示出了置入拔牙位置的开放牙槽后的牙垫组件10,其体现了本发明的特 征。在图3A和3B中,牙垫组件10经过定形、确定大小并设置成能够插入或“塞入”拔牙后的开放牙槽中。拔牙可涉及到单个牙齿(如图所示)或多个牙齿。
牙垫组件10包括组织敷料基质12。组织敷料基质12包括生物兼容性材料,其在血液、体液或水分存在的情况下发生反应变成强有力的粘合剂或胶。理想地,组织敷料基质还具有其他有利的属性,例如抗细菌和/或抗微生物和/或抗病毒特征,和/或加速或以其他方式增强凝血和机体对损伤的防御反应的特征。
理想地,组织敷料基质12包括亲水聚合物形式,例如聚丙烯酸酯、藻酸盐(酯)、壳聚糖、亲水性多胺、壳聚糖衍生物、多聚赖氨酸、聚乙烯亚胺、黄原胶、角叉藻聚糖、季铵聚合物、硫酸软骨素、淀粉、改性的纤维素聚合物、葡聚糖、透明质酸、或其组合。淀粉可以为淀粉酶、支链淀粉和淀粉酶与支链淀粉的组合物。
在一个优选的具体实施方式中,基质12的生物相容性材料包括非哺乳动物材料,其最优选为聚[β-(1→4)-2-氨基-2-脱氧-D-吡喃葡萄糖],其更常被称为壳聚糖。
由于壳聚糖基质12的特殊性质,牙垫组件10能够在血液、或体液或水分存在的情况下黏着于牙槽内的组织。该牙垫组件10的存在可止血、封闭和/或稳定拔牙位置,同时建立有利于愈合期过程中在伤口处形成并保持血凝块的条件。
图4A示出了代表性牙垫组件10在使用前的状态。牙齿填充垫状组件10的大小、形状和设置可根据其预期用途而变化,这包括考虑待治疗位置的局部解剖和形态以及患者的年龄/状态(例如成年人或儿童)。垫状组件10可以为直线的、伸长的、正方形、圆形、椭圆形、或者是其复合或复杂组合。理想地,垫状组件10的 形状、大小和设置可以在使用过程中或使用前通过切割、弯曲或塑形而具体形成并适合于应用位置的局部解剖和形态。图4B示出了一个或多个相同或不同所需形状、大小和设置的牙垫组件10可从较大的源垫状组件11切割而成。
图3A和3B示出了垫状组件10在拔牙后开放牙槽内的应用。应该理解,垫状组件10可以根据口腔内待治疗组织位置或周围解剖结构的局部解剖和形态而以多种方式进行定形、确定大小和设置。所治疗的靶治疗位置可包括在牙科手术操作过程期间例如拔牙过程中切割或改变或以其他方式受影响的组织。垫状组件10还可以被成形、确定大小并设置以适应其他类型的牙科手术操作,例如牙髓手术、或牙周手术、或正畸治疗、或正颌手术、或活检、或牙龈手术、骨手术、或刮牙术或根面平整术、或牙周病维护、或全部上颌骨或下颌骨托牙、或者全部或部分托牙调整、或托牙换底或重衬、或软组织手术取出、或骨手术取出或安装咬合矫形装置或护牙合器或咬合调整、或涉及到颌骨修复的口腔手术、或骨生长或骨生长促进、或影响口腔内组织、口腔内解剖结构或牙槽(颌)骨的任何其他手术操作或介入。还可由于意外引起口腔内组织或结构或牙槽(颌)骨损伤或创伤而需要应用牙垫组件10。
由垫状组件10治疗的位置可包括由手术仪器或者创伤或损伤或者由于在手术或牙科操作过程中放入金属丝、肘钉、扣钉、或缝线、或者由裂伤、或创伤、或刺伤、或烧伤、或骨折、或挤压伤引起的动脉和/或静脉出血。可将牙垫组件10确定大小且设置成当将其插入或置入时,与口腔内或者邻近解剖结构上或其附近任何类型的组织破坏、创伤或损伤相配合。
无论原因是什么,垫状组件10的基质12的性质可用来减轻出血、液体渗出或漏液或其他形式的液体丢失,同时也促进愈合。
理想地,由于基质12的性质,该牙垫组件10还可在口腔内的组织治疗位置或其周围形成抗细菌和/或抗微生物和/或抗病毒的保护性屏障。
由于壳聚糖基质12的特殊性能,牙垫组件10还可指示用于经历牙科操作或口腔内组织有创伤的个体,其具有不同类型的出血或凝血疾患例如血友病、或特发性血小板减少性紫癜(ITP)(其本身引起牙龈出血)。壳聚糖基质12的存在吸引红细胞膜,其一旦接触则融合于壳聚糖基质12。凝血块可非常迅速地形成而无需通常凝血所需的凝血蛋白(clotting protein)。即使是未患有出血或凝血疾患的个体,壳聚糖基质12的存在可不依赖于凝血级联过程(clotting cascade)而加速该凝血过程。因此,该基质12可与抗凝血剂/血液稀释剂如肝素、氯吡格雷(PlavixTM)、阿司匹林、双嘧达莫(潘生丁)等联合应用,且效力并无损失。
在口腔内牙科操作或偶然创伤的过程中或之后使用牙垫组件时,牙垫组件10还可提供局部应用平台,用于以控释方式将一种或多种治疗性药物递送入血流中。该治疗性药物可合并入亲水性聚合物海绵状结构,例如在冷冻步骤之前或之后,并且在干燥和致密步骤之前,如后面将要阐述的。可合并入亲水性聚合物海绵状结构(例如壳聚糖基质12)的治疗性药物的实例包括但不限于药物或药剂、干细胞、抗体、抗微生物制剂、抗病毒剂、胶原、基因、DNA、以及其他治疗性试剂;止血剂如纤维素;生长因子;骨形态发生蛋白(BMP);以及类似化合物。
壳聚糖基质12的有利性能包括黏着于机体内的粘膜表面例如口腔内所衬粘膜表面。该特征使得可能将壳聚糖基质12并入用来治疗粘膜表面的系统和装置中,其中壳聚糖基质12的黏着封闭特征、和/或加速凝血属性、和/或抗微生物/抗病毒特征,如已阐述的, 提供了优势。这些系统和方法可包括置于口腔内的缝线周围的牙龈修复和封闭。
1.组织敷料基质
组织敷料基质12优选由低模量(modulus)亲水聚合物基质而形成,也就是说本来“未压缩的”组织敷料基质12,其已经通过随后的致密过程而加密,下面将进行阐述。如前所述,组织敷料基质12可包括亲水聚合物形式,其优选包括非哺乳动物材料聚[β-(1→4)-2-氨基-2-脱氧-D-吡喃葡萄糖],其更常被称为壳聚糖。
优选地,选作基质12的壳聚糖具有至少约100kDa的重均分子量,更优选至少约为150kDa。最优选地,壳聚糖具有至少约300kDa的重均分子量。
理想地,在构成基质12时,将壳聚糖置入含酸溶液中,如谷氨酸、乳酸、甲酸、盐酸和/或乙酸。其中,盐酸和乙酸是最优选的,因为壳聚糖乙酸盐和壳聚糖氯化物盐(chitosan chloride salt)在血液中能够抗溶解,而壳聚糖乳酸盐和壳聚糖谷氨酸盐则不能抗溶解。较大分子量(Mw)的阴离子破坏壳聚糖盐的准晶结构,引起该结构中的增塑效应(挠性增强)。不希望地,它们造成这些较大Mw的阴离子盐在血液中迅速溶解。
基质12的一种优选形式包括密度低于0.035g/cm3的“未压缩的”乙酸壳聚糖基质12,其通过乙酸壳聚糖溶液的冷冻和低压冻干而形成,其随后通过压缩而致密化到从0.6至0.25g/cm3的密度,其中最优选密度为约0.20g/cm3。壳聚糖基质12还可以以压缩的亲水性海绵状结构为特征。致密化的壳聚糖基质12表现出上述的认为是所需要的全部特征。其还具有特定的结构和机械的优点,其在 使用过程中为该基质提供坚韧性和持久性,如将在下文更详细阐述的。
壳聚糖基质12表现出强有力、可渗透、高特异性表面区域,带正电荷的表面。该带正电荷表面形成了对红细胞的高反应性表面和血小板相互作用。红细胞膜带负电荷,且其被吸引至该壳聚糖基质12。细胞膜一旦接触即融合于壳聚糖基质12。凝血块可非常迅速地形成,避免对凝血蛋白的即时需要(其为凝血通常所需)。因此,壳聚糖基质12对于正常和抗凝血个体以及患凝血疾患如血友病的人均是有效的。壳聚糖基质12还结合细菌、内毒素和微生物,一旦接触即可杀死细菌、微生物和/或病毒剂。
壳聚糖基质12的结构、组成、生产、以及其他技术特征的进一步细节将在下文阐述。
2.药袋
如图5A和5B所示,理想地,壳聚糖基质12-以预设大小的(presized)牙垫组件10的形式或以较大的源牙垫组件11(预设大小的牙垫组件10可从其切割而来)的形式-在使用前真空包装于密闭热封衬有薄片(foil-lined)的药袋16中,水含量较低,优选5%水分或更少。随后,牙垫组件10或源组件11最终通过使用γ射线照射在药袋16内消毒。如图5B所示,源牙垫组件11(较小的垫组件10可从其切割并确定大小)还可在使用前于无菌条件下真空包装于药袋16中。
将药袋16设置为可由护理师在使用当时撕开。药袋16可沿一端撕开接触组织敷料垫状组件10。抓住药袋16的相对边缘并拉开而暴露组织敷料垫状组件10以备使用。
3.处理牙垫组件
一旦从药袋16中取出(见图6),牙垫组件10即可黏着于靶组织位置。它无需应用前处理以促进黏附。例如,无需撕去保护材料以暴露粘合表面供使用。粘合表面在原位形成,因为一旦与血液、液体或水分接触,壳聚糖基质12本身即表现出强有力的粘合特征。牙垫组件10无需在打开该药袋16后立即用于靶位置。
如图7所示,牙垫组件10可现场定形并改造以符合靶位置的局部解剖和形态,靶位置在图示的具体实施方式中是拔牙后的牙槽。医师可获得反映拔牙区域中的牙齿大小和间隔的牙印模,并利用该印模辅助确定牙垫组件10的大小和形状。牙垫组件10可有意识地塑造成多种形状,例如塑造成圆柱形或杯形,以最佳符合治疗位置的特殊局部解剖和形态。如前所述(见图4B),一个或多个牙垫组件10可从较大的源组件11就地切割并定形。
图8示出了壳聚糖牙垫组件10被放置在与靶治疗位置联合的位置,靶位置在图示的具体实施方式中是拔牙位置。如图8所示,将牙垫组件10置入并压进牙槽中(例如,用手或利用镊子13),其中壳聚糖基质12直接抵靠活动性出血的位置或其他需要粘合的位置,因此可直接加压于出血组织(还可参见图9)。理想地,如图9所示,将垫组件10定形、确定大小并设置,从而使垫组件10的一部分从剩余牙齿之间的牙槽伸出,并利用相对牙齿而将其保持于恰当位置。理想地,一旦应用于需要粘合的位置,护理师应该避免重新定位牙垫组件10。
理想地,如图9所示,施强压约2分钟,以允许壳聚糖基质12的天然粘合活性逐渐形成。以这种方式,由朝向拔牙位置的相对牙齿保留的几乎全部压缩力通过垫组件10的几乎全部质量而传递。壳聚糖基质12的粘合强度将随着施加压力的持续(直至约5分钟) 而增加。在该时间过程中(如图9中箭头所示),横跨牙垫组件10所平均施加的压力将提供更均一的黏着和伤口密封。如果需要,压力可在纱布敷布的辅助下施加。患者还可通过向下咬而施加并保持压力。
由于牙槽内形成血凝块,垫组件10的壳聚糖基质12的确更易吸收血液。壳聚糖基质12的粘合强度将其粘合于牙槽内的组织,因此垫组件10的机械性能施加直接压力。此外,壳聚糖基质12的存在吸收红细胞膜,其一旦接触即融合于壳聚糖基质12。凝血块可非常迅速地形成,而不仅仅依赖于血液凝固通常所需的凝血蛋白。壳聚糖基质12的存在可不依赖于凝血级联过程而加速凝结过程。另外,壳聚糖基质12的存在可提供抗细菌和/或抗微生物和/或抗病毒保护效应。在牙科应用中,利用壳聚糖基质12止血作用可在约1分钟内出现,相比于利用传统棉花填充物和成卷或折叠的纱布垫则需要约7分钟。
如前所述,牙垫组件10可现场撕开或切开,以符合拔牙位置的大小。较小的片状垫组件10还可现场切开而确定大小,并安装和黏附于其他已经放入的片状组件,以最接近治疗位置的局部解剖和形态。
理想地,为了预防疼痛并促进迅速愈合,可在愈合过程中将牙垫组件10定位于牙槽内。牙槽内的壳聚糖基质12的存在可提供有利于保留凝血块(从而避免干槽症)以及一般的愈合过程的环境,在该过程中,新骨和牙龈组织长入拔牙留下的间隙内。壳聚糖基质12的物理存在-理想地,壳聚糖基质在其生产过程中特殊致密化以抗溶解-作为骨覆盖性止痛药和生理性支架,以实现(conduction)正常槽骨愈合过程中的成纤维细胞长出、血管形成和拔牙位置的重新骨化。致密化的壳聚糖基质12的增强物理性能进一步由壳聚糖 基质12的粘合强度、其凝血过程的自促作用以及其抗细菌/抗微生物/抗病毒性能而增强。
如前所述,垫组件10可结合药剂或者生理性或药理性试剂,其在机体局部或全身发挥作用,例如酶、有机催化剂、核酶、金属有机化合物、蛋白、糖蛋白、肽、聚氨基酸、抗体、核酸、甾体分子、抗生素、抗真菌药、细胞因子、用于组织和/或骨骼的生长因子、碳水化合物、疏油剂、脂质、细胞外基质和/或单个组分、哺乳动物细胞、干细胞、基因工程细胞、药物和治疗剂。垫组件10提供物理上稳定的、生物相容性、和非细胞毒性环境,促进迅速而无痛的恢复期。
理想地,取出垫组件10,如有指征则在48小时的使用期内替换。垫组件10可撕下,且通常将与在单个完整的敷料内的该治疗位置分开。在某些情况下,可保留残存的壳聚糖凝胶,并且该凝胶可利用生理盐水或水洗涤而除去,如有需要也可利用纱布敷料轻柔擦除。
壳聚糖在机体内是可生物降解的,并分解为葡糖胺,一种良性物质。而且,在最终修复期还应努力从伤口处去除壳聚糖的所有部分。
II.牙垫组件的生产
现在将阐述用于制造组织敷料垫组件10的理想方法。该方法在图10A至10H中以图解的方式示出。当然应该认识到,还可以采用其他方法。
1.壳聚糖溶液的制备
用来制备壳聚糖溶液(在图10步骤A中命名为CS)的壳聚糖优选具有的去乙酰化(脱乙酰作用)的相对程度(fractional degree)大于0.78但小于0.97。最优选地,壳聚糖具有的去乙酰化的相对程度大于0.85但小于0.95。优选地,选作加工成基质的壳聚糖在25℃时在1%(w/w)乙酸(AA)的1%(w/w)溶液中利用轴LVI在30rpm时具有一个粘度,该粘度为约100厘泊至约2000厘泊。更优选的,壳聚糖在25℃时在1%(w/w)乙酸(AA)的1%(w/w)溶液中利用轴LVI在30rpm时具有一个粘度,该粘度为约125厘泊至约1000厘泊。最优选地,壳聚糖在25℃时在1%(w/w)乙酸(AA)的1%(w/w)溶液中利用轴LVI在30rpm时具有一个粘度,该粘度为约400厘泊至约800厘泊。
有利地,壳聚糖溶液CS可在25℃时通过将水加入固态壳聚糖薄片或粉末中并通过搅动、搅拌或摇动将固体分散于该液体中(见图10步骤A)而制备。一旦将壳聚糖分散于该液体中,即加入酸性组分并通过分散进行混合,以引起壳聚糖固体的溶解。溶解速率将依赖于溶液的温度、壳聚糖的分子量以及搅动水平。优选地,在具有搅拌叶片的闭合槽型反应器或闭合的旋转式容器中实施溶解步骤。这确保壳聚糖的均质溶解而没有积存在该容器的侧壁上的高粘性残留物。优选地,壳聚糖溶液的百分比(w/w)为壳聚糖高于0.5%且低于2.7%。更优选地,壳聚糖溶液的百分比(w/w)为壳聚糖高于1%且低于2.3%。最优选地,壳聚糖溶液的百分比为壳聚糖高于1.5%且低于2.1%。优选地,所采用的酸是乙酸。优选将乙酸加入该溶液中以实现乙酸溶液百分比(w/w)为高于0.8%且低于4%。更优选地,将乙酸加入该溶液中以实现乙酸溶液百分比(w/w)为高于1.5%(w/w)且低于2.5%。
壳聚糖基质12的结构或形状生成步骤通常从溶液进行,且可采用诸如冷冻(引起相分离)、非溶剂模挤出(生成细丝)、电纺丝 (生成细丝)、利用非溶剂的相转化和沉淀(其通常用来生成透析膜和滤膜)或将溶液涂布于预先形成的海绵样或针织产品上等技术而实现。在冷冻的情况中,通过冷冻可形成两个或多个分离相(通常水冷冻成冰,而壳聚糖生物材料则演变为分离的固相),需要另一个步骤除去已冷冻的溶剂(通常是冰),从而生成壳聚糖基质12而不破坏该冷冻结构。这可通过冷冻-干燥和/或冷冻替代步骤而实现。通过非针织性纺织过程,细丝可形成非针织性海绵样网孔。可替代地,通过传统纺丝和编织过程,细丝可生成黏结织物(feltedweave)。其他可用来生产生物材料海绵样产品的过程包括溶解已加入的来自固态壳聚糖基质12的致孔剂或将来自所述基质的材料钻孔。
2.将含水壳聚糖溶液脱气
优选地(见图10步骤B),将壳聚糖生物材料溶液CS脱去一般大气的气体。典型地,脱气是从该壳聚糖生物材料溶液CS充分除去残留气体,从而在进行随后的冷冻操作时,该气体不会逸出而在患者伤口处的敷料产品内形成不需要的大空间或较大的积存气泡。脱气步骤可以通过加热壳聚糖生物材料(通常以溶液CS的形式),随后对其施加真空而进行。例如,脱气可通过将壳聚糖溶液加热至约45℃,随后立即施以约500mTorr的真空约5分钟,同时搅动该溶液。
在一个具体实施方式中,在最初脱气后,可将某些气体加回该溶液中至受控分压。这种气体将包括但不限于氩气、氮气和氦气。该步骤的优点在于含这些气体分压的溶液一旦冷冻即形成微孔。随后,微孔随着冰的前进而穿过海绵状物。这形成了一个非常确定且受控的通道,可辅助海绵状物的孔的相互连接。
3.冷冻含水壳聚糖溶液
接下来(见图10步骤C),壳聚糖生物材料-至此其通常处于酸溶液中且已脱气,如上所述-经历冷冻步骤。优选通过冷却在模具内承载的壳聚糖生物材料溶液并将该溶液的温度从室温降至低于凝固点的最终温度而进行冷冻。更优选地,该冷冻步骤在平板速冻机上进行,从而通过平板冷却表面失热而在模具内的壳聚糖溶液中引入温度梯度。优选地,这种平板冷却表面与模具是良好地热接触的。优选地,在于平板速冻机表面接触之前,该壳聚糖溶液和模具的温度接近室温。在引入模具+溶液之前,优选该平板速冻机表面温度不高于-10℃。优选模具+溶液的热质量低于该平板速冻机支架+热传递液体的热质量。优选模具由(但不限于)金属元素,例如铁、镍、银、铜、铝、铝合金、钛、钛合金、钒、钼、金、铑、钯、铂和/或其组合而形成。模具还可涂布薄的惰性金属涂层例如钛、铬、钨、钒、镍、钼、金和铂,以确保与壳聚糖溶液的酸组分以及该壳聚糖盐基质不发生反应。热绝缘的涂层或元件可与该金属模具联合应用,以控制模具内的热传递。优选地,该模具表面不与冷冻的壳聚糖溶液结合。该模具的内表面优选涂布有薄的、永久结合的、氟化的释放涂层,其由聚四氟乙烯(Teflon)、氟化乙烯聚合物(FEP)或其他氟化的聚合材料而形成。尽管优选已涂布的金属模具,薄壁塑料模具也可用作承载溶液的方便替代品。这种塑料模具将包括但不限于通过注射成型、加工或热成型由聚氯乙烯、聚苯乙烯、丙烯腈-丁二烯-苯乙烯共聚物、聚脂、聚酰胺、聚氨基甲酸酯和聚烯烃制备。金属模具结合局部放置热绝缘元件的一个优势在于它们还为改善对该冷冻海绵状物内的热流及结构的控制提供机会。对热流控制的改善是由于模具内热传导和热绝缘元件配置之间较大的热传导差异。
以这种方式冷冻该壳聚糖溶液(形成一种冷冻的壳聚糖溶液,在图10步骤C中命名为FCS),使得能够制备垫组件产品的优选结构。
平板冷冻的温度影响最终壳聚糖基质12的结构和机械性能,该平板冷冻温度优选不高于约-10℃,更优选不高于约-20℃,最优选不高于约-30℃。在-10℃冷冻时,未压缩壳聚糖基质12的结构非常开放且在开放的海绵状结构中均是垂直的。在-25℃冷冻时,未压缩壳聚糖基质12的结构较为闭合,但仍是垂直的。在-40℃冷冻时,未压缩壳聚糖基质12的结构是闭合且不垂直的。壳聚糖基质12反而包含更加强化且互相啮合的结构。已观察到当采用较低的冷冻温度时,壳聚糖基质12的粘着/粘合封闭性能改善。约-40℃的冷冻温度形成的壳聚糖基质12结构具有优良的粘着/粘合性能。
在冷冻步骤的过程中,温度可以在预定的时间段内降低。例如,壳聚糖生物材料溶液的冷冻温度可通过平板冷却应用从室温降至-45℃,平板冷却应用为在约-0.4℃/mm至约-0.8℃/mm之间的恒定温度冷却坡度,持续约90分钟至约160分钟的周期。
4.冷冻干燥壳聚糖/冰基质
理想地,冷冻的壳聚糖/冰基质(FCS)经历从该冷冻材料的间隙内除去水(见图10步骤D)。可不破坏冷冻壳聚糖生物材料FCS的结构完整性而完成该水分去除步骤。可不产生液相而完成该步骤,而液相可破坏最终壳聚糖基质12的结构排列。因此,冷冻壳聚糖生物材料FCS中的冰将从固态冷冻相转化为气相(升华)而不形成过渡的液相。在大大低于该冷冻壳聚糖生物材料的温度下,升华的气体在真空冷凝器室内积存为冰。
优选执行该去除水步骤的方式是通过冷冻-干燥或冻干法。冷冻的壳聚糖生物材料FCS的冷冻-干燥可通过进一步冷却该冷冻的壳聚糖生物材料而实现。通常随后施以真空。接下来,可逐步加热该真空冷冻的壳聚糖材料。
更具体地,该冷冻的壳聚糖生物材料可经历后续的冷冻,优选在约-15℃,更优选在约-25℃,以及最优选在约-45℃,优选周期至少约1小时,更优选至少约2小时,最优选至少约3小时。该步骤之后可通过冷凝器冷却至低于约-45℃的温度,更优选约-60℃,以及最优选约-85℃。随后以优选至多约100mTorr,更优选至多约150mTorr,以及最优选至少约200mTorr的量施以真空。该真空冷冻的壳聚糖材料可优选在约-25℃加热,更优选在约-15℃,以及最优选在约-10℃,优选至少约1小时的时间,更优选至少约5小时,以及最优选至少约10小时。
进一步的冷冻干燥,维持真空压力接近于200mTorr,在支架温度约20℃时进行,更优选约15℃,以及最优选约10℃,优选至少约36小时的时间,更优选至少约42小时,以及最优选至少约48小时。
5.壳聚糖基质的致密化
致密化之前的壳聚糖基质(密度接近0.03g/cm3)将称为“未压缩的壳聚糖基质”。该未压缩的基质在中止出血时并不是理想的,因为其在血液中迅速溶解,且机械性能较差。因此,最好对该壳聚糖生物材料进行压缩(见图10步骤E)。利用热压盘垂直于亲水性基质聚合物表面的压缩负荷可用来压缩该干燥的“未压缩的”壳聚糖基质12,以减小该基质的厚度并增加其密度。该压缩步骤,有时简称为“致密化”,显著增加了壳聚糖基质12的黏着强度、粘合强度以及溶解抵抗性。适当冷冻并以大于阈密度(接近0.1g/cm3)压缩的壳聚糖基质12在37℃的流动血液中不易溶解。
优选该压缩温度不低于约60℃,更优选不低于约75℃且不高于约85℃。
致密化后,基质12的密度在该基质12的基面(“活性面”)(即暴露于组织的表面)可以不同于该基质12的顶面。例如,在典型的基质12中,其中在活性面测得的平均密度为(或接近)最优选密度值0.2g/cm3,而在顶面测得的平均密度可显著较低,例如为0.05g/cm3。如本文所述,所需要的致密化基质12的密度范围倾向存在于基质12的活性侧面或其附近,一旦发生,该处暴露于血液、液体或水分。
接下来,优选通过在烤箱内加热壳聚糖基质12而预处理致密的壳聚糖基质,优选加热直至约75℃的温度,更优选加热直至80℃的温度,以及最优选加热有利地直至约85℃的温度(图16步骤F)。预先准备通常进行直至约0.25小时的时间,优选直至约0.35小时,更优选直至约0.45小时,以及最优选直至约0.50小时。这种预处理步骤用一个很小的代价即对于损失20-30%粘合性能的溶解抵抗性提供了进一步的显著改善。
6.置入药袋中
随后,可将组织敷料垫组件10以预先切割过的大小或作为源牙垫组件11包装于药袋16中(见图16步骤F)。理想地,该药袋16用惰性气体例如氩或氮气而净化、抽真空并热封。药袋16用来在较长时间(至少24个月)内保持内容物无菌,且在相同的时间段内还提供对水分和大气渗入的极高屏障。
7.灭菌
装袋后,理想地,将该加工过的组织敷料垫组件10经历灭菌步骤(见图10步骤G)。组织敷料垫组件10可通过多种方法灭菌。例如优选方法是通过照射,如通过γ射线照射,其可进一步增强伤口敷料的血液溶解抵抗性、抗张性能以及黏着性能。照射可在至少 约5kGy的水平进行,更优选至少约10kGy,以及最优选至少约15kGy。
8.改善顺应性和挠性
已经阐述并推荐在安排损伤位置的靶向治疗之前将牙垫组件10弯曲和/或塑形。在亲水性聚合物海绵状结构中,其中垫组件10仅是一个实例,该结构的挠性和顺应性越强,则其越能抵抗撕裂和断裂,因为该结构可制成符合靶治疗位置的形状并实现该海绵状结构与下面(通常)不规则损伤表面的对合。对撕裂和断裂的抵抗性是一个优势,因为其保持伤口封闭及止血效力。
顺应性和挠性改善可通过在生产过程中或之后对任何亲水性聚合物海绵状结构进行机械处理而实现,而不损失有益特征即对溶解抵抗力的坚韧性和持久性。
存在几种方式,其中可在生产过程中或之后实现这种机械处理;例如(i)通过利用滚动、弯曲、扭动、转动、振动、探通、压缩、延伸、摇动和揉捏使亲水性聚合物海绵状结构的下部结构受控微断裂;(ii)利用热压技术,在80℃时使特定亲水性聚合物海绵状结构内受控宏观变形(macro-texturing)(通过形成深浮雕结构);(iii)在海绵状结构制备的冷冻步骤过程中,通过进入特定的亲水性聚合物海绵状结构中的垂直通道的可控形成,或可替代地,在压缩(致密化)步骤的过程中通过海绵状结构穿孔而机械性地获得。
可执行以改善顺应性和挠性的机械处理的进一步细节在2004年12月23日提交、序列号为11/020,365的共同待决的美国专利申请中示出,题目为“从诸如壳聚糖的亲水性聚合物海绵状结构形成的组织敷料组件、系统和方法”,其通过引用结合于此。
III.可替代的具体实施方式
牙垫组件10可以多种可替代的形式而提供。
例如,如图11A和11B中所示,垫组件64可包括纺织或无纺的网状材料片66,其包围于组织敷料基质12的层之间。组织敷料基质12浸透片66。组织敷料片状组件64的大小、形状、和结构可根据其预期用途而变化。该片状垫组件64可以为直线形、伸长的、正方形、圆形、椭圆形、或其复合或复杂组合。优选组织敷料片状组件64较薄(相比于该垫组件10),厚度在约0.5mm至1.5mm之间的范围内。该片状组件64的薄层强化结构的优选形式包括壳聚糖基质12或海绵状物,典型的壳聚糖基质密度为0.10至0.20g/cm3,通过可吸收性绷带厚边如棉纱布而强化,得到的绷带厚度为1.5mm或更小。片66可包括纺织或无纺的网状材料,从例如纤维素源性材料如纱布棉网状物而形成。该片状组件64可利用压力适应在伤口位置中的亲水性聚合物海绵状结构(例如壳聚糖基质12)的分层、压紧和/或滚动-即“填料”(如图11C所示),以进一步强化整体结构而抵抗动脉和静脉出血。通过该片状结构以自身进行填充,如图11C所示,浸入该厚边中的血液与亲水聚合物(如壳聚糖)的相互作用提供了高度粘合性、不可溶且高度顺应性的填塞形式。
该片状组件64的进一步细节可在2004年12月23日提交、序列号为11/020,365的共同未决的美国专利申请中找到,题目为“从诸如壳聚糖的亲水性聚合物海绵状结构形成的组织敷料组件、系统和方法”,其通过引用结合于此。
IV.结论
已证明亲水性聚合物海绵状结构如壳聚糖基质12可轻易进行改造,以使不同大小和结构的敷料或平台-以牙垫形式、以片状形式或以其他顺应性形式与涉及口腔的牙科操作或创伤相配合。
因此,很显然,本发明的上述具体实施方式仅是为了说明其原理而非限制性的。而本发明的范围将由所附的权利要求包括其等价物的范围确定。
Claims (22)
1.一种亲水性聚合物海绵状结构,包括壳聚糖生物材料并被成形、确定尺寸并设置成用于置入拔牙位置内,并且与口腔内的组织或骨骼相配合,并且其中所述亲水性海绵状结构的所有表面一旦与体液接触,则被暴露和粘合。
2.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构在使用前已通过压缩而致密化到0.6g/cm3至0.1g/cm3之间的密度。
3.根据权利要求1所述的包括壳聚糖生物材料的亲水性聚合物海绵状结构在制造用于在拔牙过程中或者之后使用并与口腔内的组织或骨骼相配合的组件中的应用。
4.根据权利要求3所述的应用,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构在使用前已通过压缩而致密化到0.6g/cm3至0.1g/cm3之间的密度。
5.包括壳聚糖生物材料的亲水性聚合物海绵状结构在制造用于在手术操作过程中或之后使用并与受手术操作影响的组织或骨骼相配合的组件中的应用,所述亲水性聚合物海绵状结构被成形且确定尺寸,并且其中所述亲水性聚合物海绵状结构的所有表面一旦与体液接触,则被暴露和粘合,所述手术操作包括拔牙、牙髓手术、牙周手术、正畸治疗、正颌手术、活检、牙龈手术、骨手术、刮牙术或根面平整术、牙周维护、全部上颌骨或下颌骨托牙、全部或部分托牙调整、托牙换底或重衬、软组织手术取出、骨骼的手术取出、安装咬合矫形装置或护牙合器或咬合调整、涉及到颌骨修复的口腔手术、骨生长或骨生长促进、或影响口腔内组织、口腔内解剖结构或牙槽(颌)骨的任何其他手术操作或介入中的至少一种。
6.包括壳聚糖生物材料的亲水性聚合物海绵状结构在制造用于与被治疗的组织或骨骼有关的应用的组件中的应用,用于治疗口腔内的受损组织或牙槽(颌)骨由于意外所引起的所述组织或骨骼的损伤或创伤,所述亲水性聚合物海绵状结构被成形且确定尺寸,并且其中所述亲水性聚合物海绵状结构的所有表面一旦与体液接触,则被暴露和粘合。
7.根据权利要求5或6所述的应用,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构在使用前已通过压缩而致密化到0.6g/cm3至0.1g/cm3之间的密度。
8.根据权利要求2所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构在使用前已通过压缩而致密化到0.20g/cm3的密度。
9.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构进一步包括谷氨酸、乳酸、甲酸、盐酸和乙酸中的至少一种。
10.根据权利要求9所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构进一步包括盐酸和乙酸中的一种。
11.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构从较大的源垫状组件切割。
12.根据权利要求11所述的结构,其中,切割包括壳聚糖生物材料的所述亲水性聚合物海绵状结构以符合拔牙位置的大小。
13.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构原位形成粘合表面。
14.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构进一步包括在机体局部或全身发挥作用的药剂或者生理性或药理性试剂。
15.根据权利要求14所述的结构,其中,所述药剂或者生理性或药理性试剂是酶、有机催化剂、核酶、金属有机化合物、蛋白、糖蛋白、肽、聚氨基酸、抗体、核酸、甾体分子、细胞因子、用于组织和/或骨骼的生长因子、碳水化合物、疏油剂、脂质、细胞外基质、哺乳动物细胞、干细胞、基因工程细胞、药物和治疗剂中的至少一种。
16.根据权利要求2或8所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构通过热压在60℃到85℃之间的温度下被致密化。
17.根据权利要求2或8所述的结构,其中,包括壳聚糖生物材料的致密化的亲水性聚合物海绵状结构在暴露于组织的基面上具有0.20g/cm3的更大的密度,且在顶面具有0.05g/cm3的更低的密度。
18.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构通过加热至85℃的温度来预处理。
19.根据权利要求1所述的结构,其中,包括壳聚糖生物材料的所述亲水性聚合物海绵状结构被机械处理用于改善顺应性和挠性。
20.根据权利要求1所述的结构,其中,所述体液是血液。
21.根据权利要求15所述的结构,其中,所述药剂包括抗真菌药。
22.根据权利要求15所述的结构,其中,所述治疗剂包括抗生素。
