Abstract

A self-expanding endovascular stent formed of stainless steel wire which is bent into an elongated zigzag pattern (5) and helically wound about a central axis (31) to define a tubular shape (30). Adjacent bends (8) in the helix are interconnected with a filament (32). The stent (30) is capable of being radially compressed such that the straight wire sections (11, 13 and 15), and the bends (8) are tightly packed around a central axis (31). The stent (30) resiliently self-expands to assume a tubular shape when released from the compressed state.

FurtllernloLe, tlle expansile f~rce of converltional zigzag stents generally decreases witll tlle lenyth of tlle stellt. One solu~ion to these drawbacks }las been to modify the conventional zigzag stent by connecting a plurality of shorter s~ents end on en~ to create one lonyer zi~za~ stent assembly. Alt~lough these modifie~ zigzag stents represent an improvement over conventiollal ziyzag stents for certain applications, there exists a need for an elongated self-exparlding stent that irlcludes the advantages oE bo~l conventional and modified zigzag sten~s but wllictl has improved performance charact-elistics over bo~h.
The preser~t invelltion als~ represents an improvemer~t over t~le self-expandillg spiral stents described in U.S. Patent No. ~-5,019,090 to Pincl~ulc. Pinchuk teaches the production of an elongated zigzag pattern by spirally wrappin~ a wire about an oblong mandrel, removin~ the mandrel, and then ~lattenirlg the spiral to produce an elorlgated zigzag band. The elongated zigzag ~a~ld is then spirally wrapped around a c~lindrical mandrel to p~oduce the stent. One drawback to t}le Pinchuk design, however, is that all of tlle elongate melllbers that nlake up the zigzag pattern have equal lengths. I'he resul~
being that a bisector of tlle angle defined by each pair of elongated members is not parallel to the central axi~ of the finislled stent. This causes the sten~ t.o undesirab]y ~ist~rt in shape wl~en radially compre.ssed for tl~e implantati~n procedllr~. What is needed is a spiral zigzag stent in wllicl i a bisector of each pair of elon~ate~ mem~ers is pa~allel to the central axis of t~e fillislled stent. 5ucll a stent cannot be produced by the method for making stents taugllt by E'in~hl~k.

WO9~/l382s PCT/US93/003~5 SUMMARY OF l~E INVENTION

Referring back to FIG. 1, the zigzag pattern is Inade IIE~
of straight wire sections having various lengths which are distributed in a certain pattern to ~etter facilitate the helical structure of tle ~inal stent construction. For ~-instance, in one embodiment, end wire sections 9 col-ld be made to a length of 9 Inm followed by two wil:e sections 11 each being 11 nun in length. Wire sections 11 are followed by two 13 mm wire sections 13, which are in turn followed by two wire sections 15 having a length of 15 mm. Sections 15 are followed by a single wire section 17 having a length of 17 mm. These gradually increasing wire sections at eit~ler end o~ the zigzag pattern enable the fillal ~tent to h~ve well , 25 defined squale ends. Jn other words, the gradually increasing length wire sections on either end of the zi4zag pattern enable the final stent to have a tubular shape in W}liCh tlle ends df the tube are substarltially per~enclicular to the central axis of the stent. Following wire secti-~n 17, there are a plurality of alternating length secl.ions 13 an 15. Short sections 13 being 13 n~m in length and long sections 15 being 15 n~ in length. This alternating sequence is continued for whatever distance is desired to correspond to the desired length of the Ein~l stent. The difference in length between tlle sho~t sections 13 and long sections 15 is WO93/13825 PCT/VS93/0~355 2 127~8 prilllarily depen~e~lt up~n ~he desired slo~e oE tlle ll~lix (see J~ in FIG. ~) and t~le ~esire~ llum~er ~ bends in eacl ~revolu~ioll of the }lelix.
Anottler inlportant aspe~t of tlle Lresent inventivrl is illllstrat~d in FIG. fi, wllich i~ a~l enlarged ~iew oE ~ portion of the stent shown in FIG. 3. The body of stellt 30 inclu~es a series of alternating sholt an~ lony ~eo~tio~ls, 13 and 15 respectively. A bend 8 connects eac~l pair of sllort and lolig sections 13 and 15. Eacll belld 8 defilles an al)gle 2~ which can be ~isected by a ~isector ~0. These sholt and l~ng sections are arranged in such ~ way that bisector 40 is parallel to tlle central axis 31 of the sterlt. This allows the stent to ~e ra~;ally compressed wi~hout unllecessary distortion .
FIG. 2 shows a~l enlaLged view ~f ol-e en~ of tl~e zigzag pattern. End 6 of the wiee i5 bent to form a closed eye portion 20. Eye 20 is preferal)ly kept c~osed t~y the application of the slnall anlount of solder to t~le end 6 of the wire after it has been bent into a small loop. Each of the bends 8 of the zigzag pattern are bent to inclu(le a small eye portion designated as 21 and 23 in FIG. 2, respectively. Eye 21 includes a small amount of solder 22 which renders eye 21 closed. Eye 23 includes no s~lder and is left open. In ~lle preEerred embodin~ellt, all the end ben~s ~6 sllown in E'IG. 3 ,25 are formed and soldered sinlilar to closed eye 21 so that the filalnent which is tied to each eye is less like]y to cscape.
The ~ends 8 wlliCIl define t}le helix call h~ either in the rol~n of a ~losed eye, as in eye 21, or open as in eye 23.
! I ~IG.~ 7 shows arlotller enlbodillle)lt of the preserlt illvel~tion. Sterlt 50 is similar to stent 30 ~iscus~e previously except in this case, end bends 5~ are not intercoJlnected to one an~ther. Stellt 50 is also diffeIellt in the way that ttle respective ends or tlle wire are conl-ecte~ to tbe remainder of the ster~t. In the case oE stent 50, the end sectiolls 5~ are sol~ered to arl adjaceJIt elongate~ sectio WO93/1~25 PCT/US93/00355 60. ~1l the otller halld, e~ds 6 of stellt 3U showl~ F]~. 3 are tied il'ltO t~le helix between two adjacellt ~en~s 8 of the helix. It is to ~e urlderstood that ~he means em~loyed for connec~ing tlle ends o~ tlle wire into the ~ir)ished stent ca be varied without depalti~ from ttle intellde~ sco~e ~E tlle present inventioJl. What is im~ortarlt is that t~le e~
sections of the wire be secured to tlle stent ill sucll a w~y that they are not left to dangle freely so as ~o illterfere with the stents expansion or present a risk oE unnecessary -trauma to the vascular wall of the paLient.
Because of the l~elical pattern of the present inve2ltion, t}le bends of the stent are distribllted along its lellgth rather than being concentrate~ at certai-l locatiolls alorlg the lellgtll of the stent. Thus th~ spiral stellt is able to defille a smaller compressed shape than either conve~ltional zi~zag stents or modified zigzag stents in whicll the bends are all located at a single location along t~le length of the stent.
It is also believ~d that ~lle helical pattern allows the elongated stent to more uniformly accommodate cllrves in the vascular system of a ~atient. T~e helical structllre also produces a more ulliform radial expansile force along the length of the stent. E'inally, the spiral stents o~ tlle present invention are more relia~le in mail~tainin~ eir pOSitiOII within a patierlt.
One variation of t~le presellt inven~iorl migllt illclllde a fle~ible sleeve attaclled to t~le outer or inner surface of t~le ~.
stent sucll that an elonyated stent could be used to Lepair a~
aneurysm. ln this use, the end portions o~ the sleeved stent I wollld contact the wall u~ t~e vefisel, and the body o~ the sleeved s~ent would span the alleurysnl. l'he sleeve could be made from nylon, plastic or any otller biocoml~atible material. If the sleeve were n~ade o~ ~astic, the s~en~
cvuld be attached to t~le sleeve by ell~beddiny the stent in tlle plasl;ic. If made of nylo~ he sleeve could be attached l:o 35 the sterlt by a plurali~y of stitches. It lS to be understo~d wo93J13x2s PCT/US93/00355 7 127~ ~8 tllat ~lle Illeans for attachillg ~l~e sten~ to ~lle sleeve ~oul~ be varied deperlding upon ~he sleéve nlaterial, ~nd other factors, without diverging from the intellded scope of the present invention. Furtherll~o~e, tlle sleeve itself could be used as the nlearls for intercon-lec~:ing adjacell~ bends of the helix.
Ano~her variation of the ~)resel~t illvention corlterllplates varyil~g tlle leny~hs of ~lle wi1e sectio~ls ill tlle zi~3zag pattern. Eor installce, shortening the lon~est ~ection and lengthening the shortest sections could be employed to even out the forces near the ends of the stent as well as ~o make square ends. Another variation would be to arrange the zigzag pattern in such a way that t~e tubular s~lape defined by the s~ellt has a dianleter that varies alol~y the lenytll o~
the stent. In other words, a stent could be made hour-glass in sha~e, or have a steadily tapered inner diameter, or even a combination of the two. This could 1)e accomplished by varying along the lenq~h o~ tlle stellt the nuln~er oE bends in each revolution of the helix. In these variations, the l-elical pattern would be maintained but would ~e irreyularly shaped along the len~th o~ the stent. Still another variation would be to vary the lenqths of the wire sections within the body oE the stent tv create curves or to vary the expansile force along the lengttl of the stent.
While the invention has been illus~rated alld descri~ed in detail in the drawings and foregoing descriptioll, tll~ same is to be considered as illustrative and n~t restrictive in character, it being understood that only the preferred embodiment has been showrl and described and t~lat al1 ch~ncles I and modifications tl~at conle withil~ the spirit of the inventicm are clesired to be protected.
