(PHM), formerly International Health Partners, Inc., is a Canada-based company which provides in-home disease management services for patients in the United States cardiology market. The initial focus is on providing in-home monitoring equipment, supplies and services to patients in the United States who take prescription blood thinners, such as Coumadin (Warfarin). The Company focuses on Medicare-reimbursed and private insurance-reimbursed United States healthcare market for in-home monitoring of patients on blood thinners. On January 3, 2012 the Company completed the asset purchase and management services agreement for the point of care clinic at the Cardiology Associates of New Haven in New Haven, Connecticut. In January, 2012, PHM acquired its anticoagulation clinic operation.
